[Non-infective endocarditis].

Non-infective endocarditis, also referred to as non-bacterial thrombotic endocarditis, represent a wide range of rare pathologies, often severe. This review gathered the data available in the literature, to decipher the major information collected on the pathophysiology, the diagnosis and the treatment of these heterogeneous diseases, often misdiagnosed. Characteristics of non-infective endocarditis are similar to infective endocarditis in terms of valvular lesions (mostly left-sided, with regurgitations and vegetations), and their complications (embolism). The diagnosis of non-infective endoc

Infective or Non-Infective Endocarditis: A Brief Literature Review Based on a Case Report.

In the present report, we describe a patient presenting in the intensive care unit with fever, respiratory failure, and multiple lesions on cardiac valves. The patient, with a history of multiple myeloma under treatment, was intubated due to ARDS from influenza, and cardiac ultrasonography revealed lesions in the aortic, mitral, and tricuspid valves. There is a step-by-step approach in the case presentation, with clinical questions, while there is a review of the current literature concerning the issue.

Long-term outcome of infective endocarditis in non-intravenous drug users.

OBJECTIVE: To describe postdischarge survival rates and late complications in non-intravenous drug users (non-IVDUs) after treatment of infective endocarditis (IE). PATIENTS AND METHODS: This prospective study consists of consecutive cases of IE in non-IVDUs seen between January 1, 1994, and August 31, 2005. Patient treatment (ie, pharmaceutical and/or surgical) and cardiac valve involved in infection (ie, aortic and/or mitral; whether valve was native or prosthetic) were recorded. Patient follow-up, to March 31, 2007, occurred at 1, 2, 3, and 4 years. Complications, survival, and mortality w

Infective Endocarditis Presenting With Non-ST Elevation Myocardial Infarction: A Case Report.

Infectious endocarditis (IE) is a rare disease characterized by infection of the endocardial surface of the heart. IE predominately involves the left-sided valves; however, right-sided valvular IE has increased in incidence with intravenous drug use. Treatment of IE is centered on targeted antibiotic therapy and management of complications, including septic embolization, which can affect all of the major arterial beds. Acute coronary syndrome secondary to septic embolization can be difficult to identify and carries an increased risk of morbidity and mortality. Care is further complicated by a

Non-toxigenic Corynebacterium diphtheriae infective endocarditis with embolic events: a case report.

BACKGROUND: Corynebacterium diphtheriae (C. diphtheriae) infections, usually related to upper airways involvement, could be highly invasive. Especially in developing countries, non-toxigenic C. diphtheriae strains are now emerging as cause of invasive disease like endocarditis. The present case stands out for reinforcing the high virulence of this pathogen, demonstrated by the multiple systemic embolism and severe valve deterioration. It also emphasizes the importance of a coordinated interdisciplinary work to address all these challenges related to infectious endocarditis. CASE PRESENTATION:

Spontaneous Splenic Rupture Unveiled: A Non-traumatic Case Associated With Infective Endocarditis.

Spontaneous splenic rupture (SSR) is a rare and potentially life-threatening condition often associated with trauma. However, SSR can occur without evident trauma, presenting unique diagnostic challenges. We present a case report of a 32-year-old postpartum female who experienced sudden-onset abdominal pain and was diagnosed with SSR. Despite the absence of trauma, she exhibited hypovolemic shock, requiring rapid intervention. Diagnostic imaging, including CT scans, revealed a substantial splenic laceration, which led to an emergent splenectomy. The patient's postoperative course was complicat

Tricuspid valve infective endocarditis in a non-IVDU patient with atopic dermatitis.

A 29-year-old male, with chronic atopic dermatitis (AD), presented with a 2-week history of fatigue, pyrexia and weight loss. Examination showed eczematous patches with lichenified papules, erosions on the right shin and a new murmur. Blood cultures isolated methicillin-sensitive Staphylococcus aureus. Transthoracic echocardiography showed vegetation on the tricuspid valve (TV) that was adherent to the septal leaflet. He was treated for infective endocarditis, attributed to poorly controlled AD, with intravenous Flucloxacillin. Due to ongoing sepsis and pulmonary septic emboli, Clindamycin was

Rheumatic manifestations of infective endocarditis in non-addicts. A 12-year study.

Infective endocarditis (IE) is due to a microbial infection of the heart valves or of the endocardium in close proximity to either congenital or acquired cardiac defects. This infection is associated with a high risk of complications. Rheumatic manifestations are known to be frequent complications of IE. Controversy, however, frequently exists about the actual incidence of these complications. This may be due to the small number of series describing the frequency and type of rheumatic manifestations, the absence of uniform criteria used for the diagnosis of IE, and the fact that some studies o

Native Valve Infective Endocarditis Presenting With a Non-ST-Segment Elevation Myocardial Infarction.

INTRODUCTION: Septic coronary embolism presenting with an acute coronary syndrome is a rare and unusual manifestation of infective endocarditis and remains challenging to identify in the acute setting. CASE PRESENTATION: A 36-year-old man presented with chest pain. A diagnosis of non-ST-segment elevation myocardial infarction was made, and coronary angiography revealed a filling defect in the left anterior descending artery and first diagonal branch, without evidence of coronary dissection or atherosclerosis. The patient subsequently developed recurrent fever. Blood cultures were positive for

Acute drug-use-related native tricuspid valve infective endocarditis: a non-surgical disease.

As a result of the ongoing opioid epidemic, physicians have encountered increasing rates of drug-use-related native tricuspid valve infective endocarditis (DU-TVIE), a complex multi-faceted disease that is best managed by interdisciplinary teams. Despite the large number of patients with DU-TVIE, there is little data to support the optimal treatment strategy with respect to medical and surgical therapy. The recent introduction of percutaneous mechanical aspiration of tricuspid valve vegetations has added another treatment modality that is also of uncertain benefit. Here we review the literatur

Epidemiology and outcomes of non-HACEK infective endocarditis in the southeast United States.

OBJECTIVES: Infective endocarditis (IE) with non-HACEK Gram-negative (GN) organisms is rare, but associated with poor outcomes. The purpose of this study was to quantify the microbiology, treatment strategies, and frequency of poor outcomes in patients with non-HACEK GN IE. MATERIALS: Retrospective cohort of adults with definite non-HACEK GN IE from 1/11-1/19. The primary endpoint was poor patient outcome, defined as a composite of all-cause death or infection-related readmission within 90-days of index infection. RESULTS: 43 patients were included: 51% patients were men, and the median (IQR

Non-HACEK gram-negative bacilli infective endocarditis: data from a retrospective German cohort study.

PURPOSE: Infective endocarditis caused by non-HACEK gram-negative bacilli (GNB-IE) is rare but associated with significant morbidity and case fatality. Evidence on optimal treatment and management is limited. We aimed to describe the characteristics and management of GNB-IE patients, investigating factors associated with disease acquisition and unfavorable outcomes. METHODS: We conducted a retrospective descriptive single-center study (tertiary care and referral hospital) between 2015 and 2021, including adult patients with definite GNB-IE. We reviewed demographic, clinical and microbiologica

Non-valvular infective endocarditis associated with central venous catheter bloodstream infection: a case report.

Non-valvular Infective endocarditis (IE) is exceedingly rare; however, its incidence has risen in tandem with the increased usage of intracardiac devices and the growing prevalence of risk factors associated with IE. We present a clinical case involving an 18-year-old patient with IE occurring at an atypical location, concomitant with central venous catheter bloodstream infection. The patient underwent targeted antibiotic therapy but ultimately required surgical resection of the vegetation due to multiple risk factors associated with a poor prognosis. This case underscores the importance of ma

A case of infective endocarditis caused by Kocuria rosea in a non-compromised patient.

UNLABELLED: A 79-year-old male with no history of immunodeficiency was transferred to our hospital complaining of shortness of breath and general fatigue. He was diagnosed with recent myocardial infarction and underwent emergent percutaneous coronary intervention. However, the course of congestive heart failure was poor, and he required respiratory support and renal replacement therapy. Kocuria rosea was detected in blood culture obtained on admission, and then a follow-up echocardiogram revealed infective endocarditis. We administered ampicillin-sulbactam and performed urgent operation. The p
