Open Repair of a Complicated Late Endoleak Induced by Another Endoleak.

A 66-year-old man had an enlarging aortic aneurysm sac after an endovascular aortic replacement procedure that had been performed at another institution 4 years previously; it was without any endoleak but was complicated by the occlusion of the left limb, requiring cross-femoral bypass. Current computed tomography revealed dilatation of the proximal neck and the right common iliac artery. A type Ib endoleak was found from the distal end of the right limb of the endograft, possibly secondary to the dilatation of the artery around it; it then pressurized and caused the dilatation of the juxtaren

Type 2 Endoleak Management.

Type 2 endoleaks are the most common endoleak type following endovascular aneurysm repair. The natural history of these endoleaks can vary, with some demonstrating a self-limited or indolent course, while others can contribute to aneurysm sac enlargement and rupture. A variety of embolization techniques, including transarterial catheterization and direct sac puncture techniques, have been developed for the treatment of type 2 endoleaks. In this article, the authors review the indications, techniques, and outcomes of current treatment strategies for type 2 endoleaks.

Late Type 3b Endoleak Mimicking Type 2 Endoleak after Endovascular Aortic Aneurysm Repair.

Endovascular abdominal aortic aneurysm (AAA) repair has been widely used for the treatment of AAA as a safe and efficient method, but endoleaks causing persistent expansion of aneurysm sac may cause aneurysmal rupture and death. Type 3 endoleak is rare but a predominant cause of late rupture. Type 3b endoleak can be misdiagnosed as type 2 endoleak, which is more frequent. Here we report two cases of type 3b endoleak mimicking type 2 endoleak, which were successfully treated by open surgery of partial explantation of the stent-graft and endoaneurysmal interposition graft replacement.

Thoracic endoleak embolization using a direct percutaneous puncture of the endoleak through lung parenchyma.

An 84-year-old patient presented with an endoleak after endovascular repair of a ruptured thoracic aortic aneurysm. The endoleak was thought to be a type II endoleak on computed tomographic (CT) angiography and was not visualized on angiography. The endoleak was accessed via direct puncture of the endoleak sac through the pleura and lung parenchyma with use of fluoroscopic and C-arm CT guidance. The endoleak was embolized with platinum coils and n-butyl cyanoacrylate with no complications. Follow-up imaging with CT angiography at 43 days revealed no recurrent endoleak.

Aortic stent-grafts: Endoleak surveillance.

Endoleaks have been referred to as the "Achilles heel" of endovascular aortic aneurysm repair (EVAR) and are the most common complication of this procedure. An endoleak can maintain a high systemic blood pressure within the aneurysm sac, potentially leading to rupture. Follow-up is therefore mandatory to detect and classify possible endoleaks. Computed tomography (CT) remains the gold standard for follow-up, but provides no hemodynamic information on endoleaks and has the disadvantages of exposing patients to iodine contrast and X-ray radiation. Exposure to radiation could be reduced in variou

Imaging modalities for endoleak surveillance.

As the global population ages, the issue of abdominal aortic aneurysm continues to grow. With the evolution of new devices and refined operative technique, aneurysm treatment via endovascular aortic repair is becoming increasingly favourable. This, however, is not without drawbacks, where regular surveillance is paramount to long-term success and detection of post-procedure complications. Of these complications, endoleak is the most notable and poses the greatest risk of potential future aortic rupture. The purpose of this review paper is to discuss the armada of imaging modalities used in the

Endoleak type IIIb not always recognized.

Endovascular aneurysm repair (EVAR) is a standard treatment for abdominal aortic aneurysms but is associated with late graft-related complications. Type IIIb endoleaks caused by graft fabric failure are rare and potentially fatal. We report two cases of late type IIIb endoleaks occurring 60 and 48 months after endovascular aneurysm repair using Endurant (Medtronic, Inc.) and GORE EXCLUDER (W. L. Gore & Associates, Inc.) stent grafts, respectively. Diagnosis was challenging because of limited imaging sensitivity. Both cases highlight the persistent risk of late graft failure and underscore the

Review of Type III Endoleaks.

Endoleak remains a significant challenge to endovascular aneurysm repair, particularly as evolving techniques and devices have allowed treatment of increasingly complex aneurysm anatomy with increasing number of device components. Intervention is recommended for both type I and III endoleaks due to their risk of rupture, and endovascular techniques are the favored modality with placement of a bridging endograft over the endoleak defect. Conversion to open surgical repair remains the definitive option in cases where less invasive methods have failed or are precluded. In this article, the author

[Endoleaks - when is treatment necessary?].

Currently the majority of infrarenal abdominal aortic aneurysm repairs are endovascular procedures using a stent graft. This method continues to be questioned due to an up to 50 % incidence of endoleaks, i.e. the postinterventional persistence of blood flow outside the graft and within the aneurysm sac, potentially bearing the risk of a further increase of the aneurysm diameter and aneurysm rupture. Currently a total of five different endoleak types can be distinguished. Multiphase computed tomography (CT) is the standard imaging method for the detection and classification of endoleaks or alte

What is the significance of endoleaks and endotension.

Endovascular repair has been used over a decade as a treatment of abdominal aortic aneurysm, and has become a widely accepted treatment method with a low rate of perioperative complications. Endoleak, perigraft blood flow outside endograft but within aneurysmsac, has been intensively studied during the last 10 years of endovascular aneurysm repair (EVR). The natural history of aneurysms with endoleak and the true clinical significance of various types of endoleaks remains unclear. Type I/III endoleak has been found to be associated with aneurysm rupture, while the risk of rupture of aneurysms

State-of-the-art of imaging detecting endoleaks post-EVAR with special focus on low-flow endoleaks.

Endovascular aortic repair (EVAR) necessitates lifelong surveillance for the patient, in order to detect complications timely. Endoleaks (ELs) are among the most common complications of EVAR. Especially type II ELs can have a very unpredictable clinical course and this can range from spontaneous sealing to aortic rupture. Subgroups of this type of EL need to be identified in order to make a proper risk stratification. Aim of this review is to describe the existing imaging techniques, including their advantages and disadvantages in the context of post-EVAR surveillance with a particular emphasi

Late post-endovascular abdominal aortic repair rupture due solely to type II endoleak without other types of endoleak.

Rupture of abdominal aortic aneurysm (AAA) due to an isolated type II endoleak (TIIEL) is rarely reported, accounting for less than 1% of all TIIELs; typically, rupture associated with TIIEL is accompanied by type I or type III endoleaks. We report a case of ruptured AAA secondary to TIIEL without any other types of endoleaks, occurring late after endovascular abdominal aortic repair (EVAR). A 77-year-old man with a history of EVAR 11 years earlier presented with abdominal pain. Computed tomography revealed a ruptured AAA, likely due to TIIEL from the lumbar artery. He was on warfarin for atri

Secondary Endoleak Management Following TEVAR and EVAR.

Endovascular abdominal and thoracic aortic aneurysm repair and are widely used to treat increasingly complex aneurysms. Secondary endoleaks, defined as those detected more than 30 days after the procedure and after previous negative imaging, remain a challenge for aortic specialists, conferring a need for long-term surveillance and reintervention. Endoleaks are classified on the basis of their anatomic site and aetiology. Type 1 and type 2 endoleaks (EL1 and EL2) are the most common endoleaks necessitating intervention. The management of these requires an understanding of their mechanics, and

Type II endoleaks: challenges and solutions.

Type II endoleaks are the most common endovascular complications of endovascular abdominal aortic aneurysm repair (EVAR); however, there has been a divided opinion regarding their significance in EVAR. Some advocate a conservative approach unless there is clear evidence of sac expansion, while others maintain early intervention is best to prevent adverse late outcomes such as rupture. There is a lack of level-one evidence in this challenging group of patients, and due to a low event rate of complications, large numbers of patients would be required in well-designed trials to fully understand t

Type II endoleak: a problem to be solved.

Type II endoleak is a common phenomenon after endovascular aortic aneurysm repair (EVAR). The majority of type II endoleaks are considered benign, since approximately one third of them resolve spontaneously and they have no influence on mortality and rupture rate after EVAR. Thus, type II endoleak without sac expansion is recommended to be observed conservatively. Treatment for type II endoleak with sac expansion is still controversial. It has been reported that a certain type II endoleak causes sac expansion and late aneurysm rupture. Type II endoleak is often treated with solid agents as coi

Traumatic Endograft Fracture With Subsequent Type IIIb Endoleak.

Abdominal aortic aneurysm (AAA) repair is commonly treated via endovascular aneurysm repair (EVAR). A known complication of EVAR is an endoleak: types I to IV. Type III endoleaks have been shown to have a relative risk of 8.95 for late rupture requiring prompt treatment. These endoleaks have been found to be caused by over dilation of the main body causing graft fabric distortion, fractures of metallic struts, fabric wear from chronic micromotion, fabric tears, and suture breakage. We present a case of traumatic EVAR fracture with subsequent type IIIb endoleak after blunt abdominal trauma. Thi

Intercostal artery's access for type II endoleak embolization.

Endoleaks represent a main issue of endovascular approach of thoracic aorta diseases and their treatment continue to be challenging. According to some authors, type II endoleaks sustained by intercostal arteries should not be treated because of the technical difficulties. However, the persistence of a pressurized aneurysmal may confer an ongoing risk of enlargement and/or aortic rupture. We describe the successful treatment of type II endoleak in 2 patients with an intercostal artery's access. In both cases, the endoleak was discovered during follow-up and was treated with its direct coil embo

Complex endoleak treatment after failed endovascular aortic repair.

BACKGROUND: Endovascular aneurysm repair (EVAR) has created new possibilities for patients with abdominal aortic aneurysms (AAAs), and in recent years it has become tremendously popular. Use of EVAR in selected groups of patients allows mortality and morbidity to be reduced in comparison to open repair. However, complications such as endoleaks (ELs) can be of great concern and warrant urgent therapy to prevent sac rupture. CASE PRESENTATION: The case report presents urgent endovascular treatment of a high-risk type IA EL in a polymorbid 68-year-old patient 7 years after primary EVAR. The prin
