The epidural compression secondary to genitourinary malignancies is relatively uncommon. Of 2118 patients who were diagnosed as having a genitourinary malignancy at the Columbia-Presbyterian Medical Center from 1970-1979, 21 are known to have developed an epidural cord compression by June 30, 1985 (16 from prostate, 1 from urinary bladder, and 4 from renal origin). The following items were reviewed in these 21 patients: age of the patient at diagnosis of the primary tumor; primary site of the genitourinary primary; time in months from the date of diagnosis of the genitourinary primary to the d

Back pain is a common symptom in the cancer population. For some patients, the complaint reflects a process independent of the underlying disease, such as myofascial pain, discogenic disease, or osteoporosis. Many patients, however, experience pain as the first indication of a spinal neoplasm, and this possibility increases the diagnostic challenge. This article discusses the presentation, evaluation, and treatment of spinal epidural neoplasm in patients with cancer.Symptomatic epidural neoplasm occurs in approximately 5 to 10% of patients with metastatic cancer.1-4 Without effective treatment

Calcified chondroid mesenchymal neoplasm (CCMN) is a rare chondroid matrix-producing tumour characterized by distinctive histopathology and recurrent FN1-receptor tyrosine kinase gene fusions. Reported cases have predominantly involved the distal extremities and temporomandibular joint, with no previously reported spinal cases. We present the first reported spinal epidural lesion morphologically consistent with CCMN. A 40-year-old male presented with progressive mid-thoracic back pain after a self-reported febrile illness. Imaging demonstrated a T10-T12 posterior/right calcified epidural lesio

Early identification and treatment of epidural neoplasm, before the development of significant neurologic deficits, provides the best opportunity for a favorable outcome. Among the many patients with symptoms, signs, or scintigraphic or radiographic findings suggesting possible epidural disease, a small proportion will have the lesion. The selection of patients for definitive imaging of the epidural space should be based on a determination of the risk of this complication. In this study, the medical records, plain spinal radiographs, bone scintigraphs and myelograms of 43 patients were analyze

An experimental model of spinal cord compression by epidural neoplasms was produced in rabbits by injecting a VX2 tumor-cell suspension anterior to the T-13 vertebral body. With this experimental model, edema and circulatory disturbance of the spinal cord compressed by epidural tumors were studied. The characteristic histopathological findings in the compressed spinal cord were edema and axonal swelling in the white matter. Water content and uptake of intravenously injected 99mTc pertechnetate in the compressed spinal cord were significantly greater than in the spinal cord distant from the tum

Fifteen cases of vertebral-epidural neoplasms of the cervical tract, treated surgically by the technique of longitudinal median somatotomy with inlay arthrodesis via the anterior approach are presented. Compression of the spinal anterior artery is eliminated by the wide anterior longitudinal exposition, which also creates a support for a possible second operation, either laminectomy or foraminotomy.

Herniated discs are often in contact with the intervertebral disc and are located in the intraspinal extradural space. However, in rare cases, disc fragments may migrate into the posterior epidural space or even become intradural, potentially leading to a misdiagnosis of spinal tumors. The authors report a novel case of lateral and posterior epidural migration of a lumbar disc herniation, which mimicked a cauda equina neoplasm. They also review the existing literature to provide insight into the distinction between a herniated disc and a spinal tumor. This case involves a previously healthy 43

BACKGROUND: Spinal epidural hematoma associated with epidural metastasis is very rare. Epidural hematoma is unusual in itself, and metastatic epidural tumors do not commonly occur.
CASE DESCRIPTION: A 76-year-old man with a medical history of untreated stage III rectal cancer and chronic obstructive pulmonary disease underwent emergency spine surgery for acute development of severe quadriplegia due to cervical epidural hematoma after minor indirect trauma. Hemorrhagic lesions, such as yellow ligaments, were accompanied by hematomas that adhered to the dura mater and were confirmed on patholog

The formation of an epidural hematoma from an eosinophilic granuloma of the skull is an exceptional occurrence. A 9-year-old boy presented with severe headache, somnolence and vomiting following a minor head injury. Cranial computerized tomography scan showed a seemingly depressed skull fracture together with an epidural hematoma in evolution. A neoplasm and an epidural hematoma were removed at operation. Histopathological study of the excised mass confirmed the diagnosis of eosinophilic granuloma.

Extranodal involvement in lymphomas is well-documented in the literature. However, certain anatomical sites, such as the epidural space, are rarely affected by lymphoproliferative neoplasms. When lymphomas are confined to the epidural space without other recognisable sites of involvement at diagnosis, they are classified as primary spinal epidural lymphoma. We report the case of a man in his early 30s who presented with lower back pain and progressive quadriparesis. Imaging revealed an extradural mass extending from the sixth to the tenth thoracic vertebrae, with lateral extension into the ext

Pathologies affecting the spinal epidural space (SES) comprise various abnormalities. However, they all have the potential to cause thecal sac narrowing or spinal cord compression. In this review, we group these pathologies into degenerative, infective, neoplastic, vascular, traumatic, and others, focusing on their imaging features. Degenerative pathologies of the SES range from disc to facet disease, with a particular emphasis on the less common degenerative pathologies in this review. Infective pathologies affecting the epidural space include spondylodiscitis and associated epidural phlegmon

BACKGROUND: Chronic subdural hematoma generally occurs in elderly patients, who are also prone to neoplasm. However, the association of chronic subdural hematoma and intracranial neoplasm is rare.
CASE DESCRIPTION: An elderly woman harboring a chronic subdural hematoma underlying an epidural extension of a malignant B cell non-Hodgkin's lymphoma is reported. Infiltration of the tumor and obstruction of the dural veins with stasis and rupture of the capillary net may have been the mechanism that led to formation of the hematoma.
CONCLUSIONS: In elderly patients harboring chronic subdural hema

Spinal hemangiomas are common, benign vascular lesions that involve the bony portion of vertebral bodies and are generally asymptomatic. Rarely, they can become aggressive and present with predominantly epidural extension, mimicking other neoplasms. We present the case of a fifty-one year old woman who presented with myelopathy and was discovered to have a large mass causing epidural spinal cord compression, thought to be due to a peripheral nerve sheath tumor. She underwent surgery for tumor debulking. Intraoperatively, the mass was found to be mostly epidural with minimal bone involvement. F

Plasma Cell neoplasms result from monoclonal proliferation of plasma cells. Solitary extramedullary plasmacytomas (SEMPs) are rare and constitute 5% of all plasma cell disorders. SEMPs most commonly involve upper aerodigestive tract. Isolated spinal epidural space involvement by SEMPs is extremely rare and to best of our knowledge only 7 such cases have been reported previously in available English literature. We hereby present a rare case of thoracic epidural SEMP in a 32-year-old female who presented with thoracic compressive myelopathy and discuss the pertinent literature.

PURPOSE: Spinal epidural hematoma is a rare clinical entity. We present a case of atypical contrast enhancement pattern in a chronic epidural hematoma of the lumbar spine mimicking an extradural tumor.
CASE REPORT: A 76-year-old man on treatment with oral anticoagulants presented with a 1-month history of lower back pain radiating into his right upper thigh accompanied by spinal claudication. Preoperative MRI showed a posterior epidural lesion compressing the cauda equina with almost homogeneous contrast enhancement. Surgery was performed under the presumptive diagnosis of spinal extradural n

Intraspinal schwannoma is a rare neoplasm in pediatric patients; cellular schwannoma is an unusual histological subtype of schwannoma. A six-month-old infant with an epidural dumbbell cellular schwannoma presented with progressive weakness of his arms and legs. A spinal MRI revealed an epidural mass from C5 to T4, and a complete surgical resection was achieved after laminotomy and facetectomy. The patient experienced a gradual neurological improvement and was still healthy without recurrence at the latest follow-up. The diagnosis of cellular schwannoma was confirmed on immunohistological exami

Primitive neuroectodermal tumor (PNET) is a generic term used to describe a group of histologically indistinguishable neoplasms, including cerebellar medulloblastomas, which are located at various sites in the central nervous system. Primary epidural PNETs are rare and few patients have been reported. We report a 15-year-old girl who presented with gradual onset, over 1 month, of upper back pain and bilateral lower leg weakness. A thoracic spine MRI showed a dumbbell-shaped epidural mass at T2-4 with right paraspinal and posterior mediastinal extension. Surgical resection of the epidural tumor
