Progressive Myoclonic Epilepsies.

The treatment of progressive myoclonus epilepsy (PME) remains a major therapeutic challenge in neurology. Generalized convulsive seizures are often well controlled through classic antiepileptic drugs (AEDs) like valproate and clonazepam, whereas myoclonus, the main symptom that is affecting patients most in their daily life, is usually refractory to standard AEDs. Alternative therapy concepts have been and still are investigated. Among the new drugs, zonisamide and piracetam have shown the most promising results as add-on treatments. Other therapeutic approaches, like the use of antioxidants,

Progressive myoclonic epilepsies.

The progressive myoclonic epilepsies are a rare but extremely debilitating group of disorders that are difficult to diagnose and even harder to treat. They represent a heterogeneous subgroup of those with secondary generalized epilepsy. Efficacy of treatment is often measured in terms of slowing a patient's inevitable decline. Reviewed here are the classification of progressive myoclonic epilepsies, features of myoclonic seizures, the five most prevalent progressive myoclonic epilepsy syndromes-Unverricht-Lundborg disease, myoclonus epilepsy with ragged red fibers (MERRF) mitochondrial disease

[Myoclonic epilepsies in infancy].

INTRODUCTION: The presence of myoclonus in a patient has different meanings: there exist myoclonus without encephalopathy or epilepsy (sleep myoclonus), encephalopathies with non-epileptic myoclonus (Kinsbourne's syndrome), encephalopathies with epileptic myoclonus (progressive or not), epileptic encephalopathies with myoclonic seizures (the classic West and Lennox-Gastaut syndromes) and myoclonic epilepsies. DEVELOPMENT: Main types of myoclonic epilepsies (benign childhood myoclonic epilepsy, severe-polymorphic-myoclonic epilepsy, juvenile myoclonic epilepsy, childhood familial myoclonic epi

The progressive myoclonic epilepsies.

Progressive myoclonic epilepsies are a group of disorders characterised by a relentlessly progressive disease course until death; treatment-resistant epilepsy is just a part of the phenotype. This umbrella term encompasses many diverse conditions, ranging from Lafora body disease to Gaucher's disease. These diseases as a group are important because of a generally poor response to antiepileptic medication, an overall poor prognosis and inheritance risks to siblings or offspring (where there is a proven genetic cause). A correct diagnosis also helps patients and their families to accept and unde

Familial benign nonprogressive myoclonic epilepsies.

Work on the classification of epileptic syndromes is ongoing, and many syndromes are still under discussion. In particular, special difficulty still persists in correctly classifying epilepsies with myoclonic seizures. The existence of special familial epileptic syndromes primarily showing myoclonic features has been recently suggested on the basis of a clear pattern of inheritance or on the identification of new chromosomal genetic loci linked to the disease. These forms in development include familial infantile myoclonic epilepsy (FIME), benign adult familial myoclonic epilepsy (BAFME), or a

Classification of the myoclonic epilepsies.

Background: Preeclampsia (PE) is a hypertensive disorder of pregnancy characterized by widespread maternal endothelial dysfunction. Although clinical signs subside following delivery, long-term risks associated with PE include hypertension, stroke, and cardiovascular disease. MicroRNAs (miRNAs) are emerging as critical regulators of biological function, and while alterations to the miRNAs have been described in the context of pregnancy and PE, the postpartum implications of PE on miRNA expression are unknown. In the present study, we aimed to determine the clinical performance of miR-296 in PE

Progressive Myoclonic Epilepsies - A Pragmatic Review.

INTRODUCTION: Progressive Myoclonus Epilepsy (PME) is a rare and complex group of inherited neurodegenerative disorders characterized by progressively worsening myoclonus, cognitive impairment, tonic-clonic seizures and ataxia. The clinical features and genetic underpinnings of PME are diverse, with approximately 80% of individuals now able to receive a molecular diagnosis. This review outlines the clinical phenotypes, genotypes, and management strategies for PME. Literature search for publications on PME in the preceding 20 years, with emphasis for the past one decade, performed using Medline

Blink reflex in progressive myoclonic epilepsies.

PURPOSE: Progressive myoclonic epilepsies (PME) include a heterogeneous group of disorders. The brainstem is involved in these disorders, as demonstrated by neuroimaging and autopsy studies. The blink reflex (BR) is characteristically elicited after supraorbital electrical stimulation. The BR has two components, an ipsilateral R1 and bilateral R2 (R2 and R2c). The central generator of the BR is the brainstem. In this study, we aimed to investigate the functional status of the brainstem using the BR in PME cases with different etiological factors. METHODS: We prospectively included 17 patients

Myoclonic jerks occur in a number of different syndromes. There is many classifications of myoclonus. It is preferred the Fejerman classification, slightly modified that present the following five groups: 1. Myoclonus without encephalopathy and without epilepsy, which includes physiological myoclonus; 2. Encephalopathies with non epileptic myoclonus, which includes Kinsbourne syndrome and certain types of hyperekplexia which pose differential diagnosis problems with reflex myoclonic epilepsy; 3. [Myoclonic epilepsies in adolescent and adult patients].

We review the electroclinical characteristics of the main myoclonic epileptic syndromes occurring in adolescents and adults, excepting the progressive myoclonic epilepsies. In the discussion we include some epilepsies which are not currently classified as generalized, but which develop myoclonic crises such as the myoclonic variant of epilepsy when reading. We emphasize strict criteria for diagnosis of juvenile myoclonic epilepsy and discuss the syndromes related to it. Other myoclonic epilepsies, such as those which may occur in adults with Down's syndrome and in Alzheimer's disease, are cons

Progressive myoclonic epilepsies: definitive and still undetermined causes.

OBJECTIVE: To define the clinical spectrum and etiology of progressive myoclonic epilepsies (PMEs) in Italy using a database developed by the Genetics Commission of the Italian League against Epilepsy. METHODS: We collected clinical and laboratory data from patients referred to 25 Italian epilepsy centers regardless of whether a positive causative factor was identified. PMEs of undetermined origins were grouped using 2-step cluster analysis. RESULTS: We collected clinical data from 204 patients, including 77 with a diagnosis of Unverricht-Lundborg disease and 37 with a diagnosis of Lafora bo

Epileptic encephalopathies with myoclonic seizures in infants and children (severe myoclonic epilepsy and myoclonic-astatic epilepsy).

Myoclonic attacks are not characteristic of a specific syndrome. In infancy and early childhood, they are often observed in the context of syndromes that are associated with other types of seizures and with cognitive impairment but no obvious brain lesion. Characterization of the associated seizures and age of expression allows inclusion of a number of cases in two main subgroups: severe myoclonic epilepsy (SME, or Dravet syndrome) and myoclonic-astatic epilepsy (MAE). Severe myoclonic epilepsy is an epileptic encephalopathy with invariably poor outcome in which myoclonic seizures, though freq

Photosensitivity in Lafora and Unverricht-Lundborg progressive myoclonic epilepsies.

OBJECTIVE: Lafora disease (LD) and Unverricht-Lundborg disease (EPM1A) are the most common forms of progressive myoclonic epilepsy and are frequently associated with photosensitivity and photic reflex myoclonus (PRM). This study aimed to characterize the photoparoxysmal response (PPR), evaluate the effect of blue lenses, and explore the neural networks underlying PPR and PRM in both conditions. METHODS: Twenty-six patients with LD and 51 with EPM1A were included. PPR was assessed on the first available electroencephalogram (EEG), including background activity and the presence and distribution

Influence of cholinergic system on myoclonus in myoclonic epilepsies.

The effect of two drugs upon multifocal myoclonic jerks was evaluated. The drugs influence the central cholinergic system in opposite ways. Eight patients with progressive and nonprogressive myoclonic epilepsy were tested. The single blind test was used. The number of myoclonic jerks after intravenous physostigmine (mean dose 0.02 mg/kg) and that after atropine (0.04 mg/kg) was compared to number of myoclonic jerks in the drug-free periods and with placebo. Placebo was without an effect. Physostigmine slightly increased the number of jerks. Atropine decreased the number significantly. In most

Myoclonic status epilepticus in juvenile myoclonic epilepsy.

BACKGROUND: Myoclonic status epilepticus (MSE) is rarely found in juvenile myoclonic epilepsy (JME) and its clinical features are not well described. We aimed to analyze MSE incidence, precipitating factors and clinical course by studying patients with JME from a large outpatient epilepsy clinic. METHODS: We retrospectively screened all patients with JME treated at the Department of Neurology, Medical University of Innsbruck, Austria between 1970 and 2007 for a history of MSE. We analyzed age, sex, age at seizure onset, seizure types, EEG, MRI/CT findings and response to antiepileptic drugs.

[Juvenile myoclonic epilepsy].

INTRODUCTION: In this paper we review the history of juvenile myoclonic epilepsy (JME) since the description by Herpin in 1852 and 1857 of the seizures of some epileptic patients which he called impulsions . Credit is due to Janz and Christian who in 1957 gave a detailed description of the condition which they called impulsive Petit mal and we know today as JME. Delgado-Escueta and his group in Los Angeles discovered the genetic basis of JME in some patients. DEVELOPMENT AND CONCLUSIONS: The homogeneity of the clinical and neurophysiological characteristics of series of patients with JME repo

[Juvenile myoclonic epilepsy].

CONCLUSION: We conclude that despite inevitable variability the clinical picture of JME is characteristic. It is easy to diagnose JME if one thinks of it while the history should be thoroughly analyzed. An EEG recording during sleep confirms the diagnosis. An early diagnosis of JME permits adequate prognosis of the subsequent course of epilepsy, and adequate therapy brings remission in most of the patients. If treatment starts following the large number of severe GTC seizures, the response to therapy is incomplete. The persistency of the illness throughout the life, the need for continuous med

Myoclonic-astatic epilepsy.

Myoclonic-astatic epilepsy (MAE) belongs to the group of epilepsies with primarily generalized seizures as absence epilepsies, and juvenile myoclonic epilepsy, as well as infantile and juvenile idiopathic epilepsy with generalized tonic-clonic seizures. Like these types of epilepsy, MAE is polygenically determined with little non-genetic variability. The disease is characterized by the following criteria: genetic predisposition (high incidence of seizures and/or genetic EEG patterns in relatives); mostly normal development and no neurological deficits before onset; primarily generalized myoclo
