[International classification of epileptic seizures, epilepsies, and epileptic syndromes].

Epilepsy is one of the common diseases in neurology. Its correct diagnosis and classification are important for choosing the most appropriate treatment. Currently, Classification of Epileptic Seizures in 1981 and Classification of Epilepsies and Epileptic Syndromes in 1989, both were proposed by International League Against Epilepsy (ILAE), are available. As for Classification of Epilepsies and Epileptic Syndromes, two divisions continue to be widely used to shape the major classes: The first separates epilepsies with generalized seizures (generalized epilepsy) from epilepsies with partial or

[Genes Responsible for Epileptic Syndromes].

The first causative gene for epileptic syndrome was revealed 20 years ago. Since then, many genes responsible for epileptic syndrome, particularly sporadic epileptic encephalopathies, such as Ohtahara syndrome, West syndrome, and focal cortical dysplasia, have been identified. Although epilepsy was recognized as a channelopathy in the beginning stages of gene discovery, other molecular mechanisms for epileptic syndromes, such as interneuronopathy, synaptic vesicle release, and mTOR signal transduction, are emerging. A new technique for gene analysis using the next-generation sequencer is now a

Idiopathic epileptic syndromes and cognition.

Epilepsy is frequently associated with cognitive impairments which result from various interacting factors. The present paper deals with the contribution of neuropsychology to the characterization of the type of epilepsy and the possible mechanisms underlying idiopathic epileptic syndromes. The non-lesional, so-called idiopathic epilepsies, constitute an interesting model for assessing the relationship between epileptiform EEG discharges and cognition. Among the idiopathic generalized epilepsies, disorders of social integration and personality have been frequently reported in juvenile myocloni

Drop attacks and epileptic syndromes.

1. In addition to 4 types of drop attacks previously described by Ikeno et al., we could ascertain the falls of the myoclonic-tonic type in genuine drop attacks. The genuine drop attacks were seen in Lennox-Gastaut syndrome, epilepsy with myoclonic-astatic seizures and other symptomatic generalized epilepsies, and never in partial epilepsies. 2. None of the patients with partial epilepsy revealed genuine drop attacks as defined. Their drop attacks were slower (i.e. the falls were completed in more than 1 sec.). 3. In the 15 patients with Lennox-Gastaut syndrome, the drop attacks disappeared in

Classifications of epileptic syndromes: advantages and limitations for evaluation of childhood epileptic syndromes in clinical practice.

The advantages and limitations of the two most recent International League Against Epilepsy classifications of the epilepsies and epileptic syndromes have been assessed after examining the clinical records of 645 consecutive outpatients aged 1 month to 15 years followed at the Children's Epilepsy Center of the University of Milan, Italy, from 1977 through 1985. The percentage of cases that could be classified according to the 1970 and 1985 proposals for classification were 94.1 and 98.1%, respectively. According to the 1985 proposal, partial epilepsies (PE) and generalized epilepsies (GE) were

Neonatal seizures and neonatal epileptic syndromes.

The neonatal period is defined as the first 28 days of life of a term infant; for premature infants the limit of this period is 44 completed weeks of the infant's conceptional age (CA)-defined as the chronological age plus gestational age (GA) at birth. The clinical and electroencephalographic (EEG) manifestations of seizures during this period are determined primarily by the development features of the immature brain at the time of seizure onset, but are also related to the type and diversity of etiologies and risk-factors for seizures neonates may face early in life. Neonatal seizures may be

[Epileptic syndromes in ascariasis and enterobiasis].

The report contains some results of a study of 18 patients where in 17 cases the authors believe that the convulsive syndrome was due to an infestation by ascarides, soatworms or by a combination of helminths with other enteral helminthosis. In one case intestinal parasitism aggravated the development of frontal epilepsy. It was demonstrated that the removal of helminths leads to an arrest of convulsive seizures, while a new infestation--to their appearance. The authors have attemped to give characteristics of the epileptical syndrome, provocated by ascarides and seat-worms.

Infantile epileptic syndromes and metabolic etiologies.

Inherited metabolic disorders can cause onset of epilepsy in the first year of life. Epilepsy rarely dominates the clinical presentation, which is more frequently associated with other neurologic symptoms, such as mental retardation, hypotonia and/or dystonia, or vigilance disturbances. The pathogenesis of seizures is multifaceted; inherited metabolic disorder can affect the balance between excitatory and inhibitory chemical mediators, eliminate an energetic substrate at the cerebral level, cause in utero brain malformation, or provoke acute brain lesions. Some clinical disorders that strongly

Relation of photosensitivity to epileptic syndromes.

Photosensitivity is the most common mode of seizure precipitation. It is age-related, more frequent in females, and most often found in generalised epilepsies. Little is known about its relation to individual epileptic syndromes. This study on 1062 epileptic patients who had 4007 split screen video EEG investigations revealed that the relation to generalised epilepsy is even more close than generally believed. Versive seizures with visual hallucinations was the only focal seizure type related to photosensitivity. Of the syndromes of generalised epilepsy, only childhood absence epilepsy, juveni

[P300 latencies in epileptic children: the differences among various epileptic syndromes].

Auditory event-related potentials (ERPs) were recorded from 129 epileptic children of 5 to 20 years of age. To clarify the developmental changes in each of the epileptic syndromes, P300 latencies of ERPs were examined. ERPs were elicited with the auditory oddball paradigm. P300 detection was occasionally difficult in epileptic children. Mean P300 latency in epileptic children (375 +/- 39.7 msec) was significantly prolonged compared with that in normal control children. (355 +/- 39.1 msec). P300 latencies in the children with symptomatic partial epilepsies were prolonged and were not recognized

Vagus nerve stimulation in pediatric epileptic syndromes.

Vagal nerve stimulation (VNS) has shown promising results in various cohorts of non-surgical refractory epilepsy in adults and children. However studies report a significant delay between implantation and clinical response. We describe a cohort of 28 children and adolescents prospectively followed, classified by epileptic syndromes and treated with VNS using a 6-week rapid ramping protocol between January 2000 and March 2005. Our cohort showed favorable outcome within 6 months which was sustained at 24 months: 68% (19/28) showing >or=50% reduction in seizure frequency, including 14% (4/28) who

Classification criteria of epileptic seizures and syndromes.

Care must be exercised not to intermingle with classification of seizures and classification of epilepsies in an inconsistent fashion. Criteria for each class must be defined as clearly as possible, and these criteria must be those which are necessary for classifying any given case. The international classification first proposed by the ILAE in 1970 was an attempt to distinguish seizures from epilepsies; the seizure types defined in the 2001 diagnostic scheme are conceptually akin to the syndromes in the 1989 classification in the sense that they imply etiological, therapeutic, and prognostic

West syndrome and its related epileptic syndromes.

The purpose of this project was to study the relationship between West syndrome (WS) and its related epileptic syndromes, and reconsider the nosological limits of WS. The electroclinical features of 45 patients who experienced spasms in series were investigated, as well as some features not common in patients with WS. All patients were mentally retarded. The patients were divided into three groups: Group 1 consisted of 12 patients with refractory epilepsies with onset in early infancy, group 2 consisted of 5 patients with symptomatic localization-related epilepsies associated with spasms in se

Epileptic syndromes in children with somatosensory evoked spikes.

We studied 186 children with frequent EEG paroxysms evoked by tapping of the feet or hands (ES). Seventy-five (40.3%) patients had a history of epileptic seizures. Febrile convulsions alone occurred in 31 (16.7%) of the children and nonfebrile in 44 cases (23.7%). Non-febrile seizures occurred in a greater proportion among patients with ES than in a control group with normal EEG. Spontaneous epileptiform activity was recorded in 39.6% of the patients without epileptic seizures but in a great percentage of the children with seizures (85.3%). The non-febrile seizures were partial in 26 cases and

Classifying epileptic syndromes: problems and a neurobiologic solution.

Because the effectiveness of an antiepileptic drug in controlling a particular seizure type and the prognosis for remission may differ between epileptic syndromes, accurate syndrome classification is crucial. However, a lack of universally accepted diagnostic criteria for some syndromes makes precise classification impossible in many patients. When syndromic classification is not possible, it is often more useful to consider genetic, clinical, electroencephalographic, neuropsychologic, radiologic, and other laboratory factors in characterizing the epileptic condition and selecting the optimal

Relationship among eye condition sensitivities, photosensitivity and epileptic syndromes.

BACKGROUND: Electroencephalogram (EEG) activity in normal subjects and epileptic patients is often closely related to the eye's status such as eye opened (EO), eye closure (ECL) and eyes closed (EC). ECL is the period immediately after closing of the eyes and only lasts for less than 3 seconds if the eyes remain closed. EC is the period as long as the eyes are closed. Epileptiform changes on EEG induced by ECL or EC are called the changes of ECL sensitivity (ECLS) or EC sensitivity (ECS). ECLS occurs mainly but not exclusively in photosensitive patients and ECS has been seen rarely in photosen

[Relation between spina bifida occulta and various epileptic syndromes].

Radiographs of the lumbar vertebral column and sacral region obtained for 182 epileptic patients after spina bifida occulta were studied. Spina bifida occulta was twice as common in patients with idiopathic epilepsy as in those with symptomatic epilepsy and twice as common as in the normal population. A relationship between spina bifida occulta and idiopathic epilepsy is discussed.
