The Australian Incident Monitoring Study. Equipment failure: an analysis of 2000 incident reports.

Of the first 2000 incidents reported to the Australian Incident Monitoring Study, 177 (9%) were due to "pure" equipment failure according to pre-defined criteria. Of these 107 (60%) involved anaesthetic equipment, 42 (24%) involved monitors, 17 (10%) other theatre equipment and 11 (6%) the gas or electricity supply. Ninety-seven (55% of the 177) were potentially life-threatening; of these two-thirds would be detected by the array of monitors recommended by the Australian and New Zealand College of Anaesthetists and all but 9 of the remainder would be handled by application of the crisis manage

Detecting the Failures Nobody Sees: Applying Pareto Analysis and Failure Mode and Effects Analysis to Equipment Reliability in a Healthcare Simulation Institute.

Background Hospitals have used failure mode and effects analysis (FMEA) for two decades to examine clinical processes before they fail, and healthcare simulation has served as a useful instrument for carrying out that analysis. The equipment inside the simulation center has attracted far less scrutiny. Audiovisual (AV) and manikin failures interrupt teaching, and when video capture is affected, they destroy the recording on which objective structured clinical examination (OSCE) grading and debriefing depend. Methods We analyzed a 24-month log of reported equipment incidents (N = 22) from the s

Operational reliability assessment of radiotherapy equipment failures through integrated Pareto and FMECA analysis.

INTRODUCTION/BACKGROUND: Unplanned equipment and infrastructure interruptions are a persistent source of radiotherapy service loss, yet prioritization is often based on either downtime burden or risk criticality alone. This study analyzed five years of interruption logs to integrate a standardized failure taxonomy with downtime-based Pareto analysis and FMECA criticality scoring. METHODS: A harmonized event log captured asset class, subsystem, cause category, operational severity, downtime, and maintainability indicators, including response and repair times. Event burden and downtime distribu

System reliability analysis of CRTS III track slab considering multiple failure modes.

Under train loads, temperature loads, and subgrade deformation, CRTS III track slabs may experience various failure modes such as longitudinal bending, transverse bending, steel yielding, and fatigue damage. To explore the system reliability of CRTS III track slabs under various failure modes, this study is based on analytical expressions and finite-element techniques. The load effects on track slabs were calculated, and the limit state functions corresponding to different failure modes of track slabs were analyzed. To perform series system analysis of various failure modes, and establish dime

A novel sensor concept for optimization of loosening diagnostics in total hip replacement.

The main reason for the revision of total hip replacements is aseptic loosening, caused by stress shielding and wear particle induced osteolysis. In order to detect an implant loosening early, the osseointegration of endoprosthetic implants must be measured exactly. Currently applied diagnostic methods, such as standard radiographs and clinical symptomatology, often result in an imprecise diagnosis. A novel radiation-free method to improve the diagnostic investigation of implant loosening is presented. The osseointegration of an implant can be identified using mechanical magnetic sensors (osci

Radiation tolerance of contemporary implantable cardioverter-defibrillators.

BACKGROUND: Implantable cardioverter-defibrillators (ICDs) are complex instruments using integrated circuit technology. Previous studies suggested risk to the device when exposed to a radiation environment. Little data is available on contemporary ICD systems. OBJECTIVES: The purpose of the present study was to assess the ability of contemporary ICD designs to resist the damaging effects of direct exposure to therapeutic doses of radiation. METHODS: Four contemporary ICDs and four legacy ICDs devices were exposed to escalating doses of photon irradiation (XRT) from a 6-MV linear accelerator.

PURPOSE: Increasingly, autoidentification technology is being utilized in healthcare settings to make delivery of care cheaper, more efficient, and safer. However, the risk of electromagnetic interference (EMI) when using these autoidentification systems is not entirely clear. Thus, we sough to evaluate the likelihood of electromagnetic interference (EMI) in high technology healthcare areas with a low frequency, magnetic-field based wireless autoidentification protocol. METHODS: Thirty-two devices were tested for EMI with an autoidentification technology whose maximal magnetic field output ra

Pressure drop and service life predictions for respirator canisters.

Respirator canister designers and manufacturers have a responsibility to provide canisters with increasingly higher capacity and longer service life due to rising safety standards in the workplace. Optimizing the service life of conventional canisters is not about simply increasing the depth of adsorbent present in the canister because this will lead to an increased resistance to airflow, and a greater physiological burden on the user. To optimize canister design it is necessary to establish how the physical dimensions of the adsorbent bed affect breathing resistance and adsorption performance

Development of the STAR Evaluation System for Assessing Bicycle Helmet Protective Performance.

Cycling is a leading cause of mild traumatic brain injury in the US. While bicycle helmets help protect cyclists who crash, limited biomechanical data exist differentiating helmet protective capabilities. This paper describes the development of a bicycle helmet evaluation scheme based in real-world cyclist accidents and brain injury mechanisms. Thirty helmet models were subjected to oblique impacts at six helmet locations and two impact velocities. The summation of tests for the analysis of risk (STAR) equation, which condenses helmet performance from a range of tests into a single value, was

Redefining flow measurement in centrifugal pump: the case for RPM and power consumption-based estimation.

Mechanical circulatory support (MCS) devices, particularly those utilizing centrifugal pumps (CP), are crucial in sustaining patients with severe cardiac conditions. Traditional flowmeters, integral to these systems, present considerable challenges, including substantial physical bulk, complex wiring, and a dependency on robust backup systems. These challenges become acute in portable applications, where compact, reliable, and discrete solutions are essential for enhancing patient mobility and quality of life. This paper proposes a strategic shift towards integrating RPM (Revolutions Per Minut

[Descemet's stripping automated endothelial keratoplasty (DSAEK)].

Penetrating keratoplasty has been the gold standard for the surgical treatment of corneal endothelial pathologies, but tremendous progress has been made in recent years in improving the technology of posterior lamellar keratoplasty techniques such as Descemet's stripping automated endothelial keratoplasty (DSAEK). This progress is shown by a literature review using PubMed sources and our own clinical and experimental data. Posterior lamellar keratoplasty using a microkeratome is a reliable surgical technique for Fuchs' endothelial dystrophy and pseudophakic bullous keratopathy. Visual rehabili

Safety of stationary grinding machines - impact resistance of work zone enclosures.

Guards on machine tools are intended to protect persons from being injured by parts ejected with high kinetic energy from the work zone of the machine. Stationary grinding machines are a typical example. Generally such machines are provided with abrasive product guards closely enveloping the grinding wheel. However, many machining tasks do not allow the use of abrasive product guards. In such cases, the work zone enclosure has to be dimensioned so that, in case of failure, grinding wheel fragments remain inside the machine's working zone. To obtain data for the dimensioning of work zone enclos

Simple and rapid determination of ferulic acid levels in food and cosmetic samples using paper-based platforms.

Ferulic acid is an important phenolic antioxidant found in or added to diet supplements, beverages, and cosmetic creams. Two designs of paper-based platforms for the fast, simple and inexpensive evaluation of ferulic acid contents in food and pharmaceutical cosmetics were evaluated. The first, a paper-based electrochemical device, was developed for ferulic acid detection in uncomplicated matrix samples and was created by the photolithographic method. The second, a paper-based colorimetric device was preceded by thin layer chromatography (TLC) for the separation and detection of ferulic acid in
