Revisiting the Evidence for the Reuse of Enteral Feeding Equipment in Ambulatory Patients: A Systematic Review.

BACKGROUND: The reuse of enteral tube feeding (ETF) equipment is not recommended due to the risk of microbial contamination and subsequent risk of infection; however, this practice continues in many ambulatory settings. A systematic review of the literature was undertaken to review the evidence underpinning the cleaning and reuse of ETF equipment. METHODS: Studies that investigated the reuse, decontamination, and/or cleaning of ETF equipment were considered for inclusion. Electronic databases were searched (no limits were placed on date of publication, age, or duration of reuse). Extracted da

Stable Solid Peracetic Acid Precursor for Effective Decontamination and Reuse of Personal Protective Equipment.

The COVID-19 pandemic exposed critical vulnerabilities in the global supply chain for personal protective equipment (PPE), necessitating effective decontamination methods to enable the safe reuse of scarce resources like N95 respirators and Tyvek suits. While peracetic acid (PAA) is a potent disinfectant, its liquid form presents significant logistical challenges. This study introduces acetylperoxyborate (APB), a stable solid PAA precursor powder, as a novel solution for point-of-use PPE decontamination. APB was synthesized and characterized, demonstrating excellent thermal stability (decompos

An Experimental Analysis of Five Household Equipment-Based Methods for Decontamination and Reuse of Surgical Masks.

The current coronavirus pandemic has increased worldwide consumption of individual protective devices. Single-use surgical masks are one of the most used devices to prevent the transmission of the COVID-19 virus. Nevertheless, the improper management of such protective equipment threatens our environment with a new form of plastic pollution. With the intention of contributing to a responsible policy of recycling, in the present work, five decontamination methods for used surgical masks that can be easily replicated with common household equipment are described. The decontamination procedures w

Reuse of Personal Protective Equipment: Results of a Human Factors Study Using Fluorescence to Identify Self-Contamination During Donning and Doffing.

BACKGROUND: At least 115,000 health and care workers (HCWs) are estimated to have lost their lives to COVID-19, according to the the chief of the World Health Organization (WHO). Personal protective equipment (PPE) is the first line of defense for HCWs against infectious diseases. At the height of the pandemic, PPE supplies became scarce, necessitating reuse, which increased the occupational COVID-19 risks to HCWs. Currently, there are few robust studies addressing PPE reuse and practice variability, leaving HCWs vulnerable to accidental contamination and harm. OBJECTIVE: The objective of thi

Reusing personal protective equipment (PPE) did not increase surgical site infection in trauma surgical patients during the COVID-19 pandemic: A retrospective cohort study in Michigan Trauma Centers.

BACKGROUND: Reuse of personal protective equipment (PPE), masks more specifically, during the COVID-19 pandemic was common. The primary objective of this study was to compare pre-pandemic surgical site infection (SSI) rates prior to reuse of PPE, to pandemic SSI rates after reuse of PPE in trauma surgical patients. METHODS: A retrospective cohort analysis collected from the Michigan Trauma Quality Improvement Program database was performed. The pre-COVID cohort was from March 1, 2019 to December 31, 2019 and post-COVID cohort was March 1, 2020 to December 31,2020. Descriptive statistics were

Challenges and future opportunities to unlock the critical supply chain of personal and protective equipment (PPE) encompassing decontamination and reuse under emergency use authorization (EUA) conditions during the COVID-19 pandemic: Through a reflective circularity and sustainability lens.

Severe acute respiratory syndrome Coronavirus-2 (SARS-CoV-2), and the resulting coronavirus disease (COVID-19), was declared a public health emergency of global concern by the World Health Organization (WHO) in the early months of 2020. There was a marked lack of knowledge to inform national pandemic response plans encompassing appropriate disease mitigation and preparation strategies to constrain and manage COVID-19. For example, the top 16 "most cited" papers published at the start of the pandemic on core knowledge gaps collectively constitute a staggering 29,393 citations. Albeit complex, a

Reuse of angioplasty catheters and risk of Creutzfeldt-Jakob disease.

BACKGROUND: In 1996, the Quebec Ministry of Health and Social Services, concerned that reuse of devices contaminated with blood or blood products could cause the transmission of Creutzfeldt-Jakob disease (CJD), discontinued its practice of reusing angioplasty catheters despite the significant cost savings reuse had afforded the health care system for several years. The objective of this study was to establish whether the medical literature provides documentation of any cases in which CJD was transmitted by reused percutaneous transluminal coronary angioplasty (PTCA) equipment. Worldwide variations in purchase and per-procedure costs of single-use flexible ureteroscopes and flexible navigable access sheaths: the economic impact of reuse strategies on procedural outcomes from a 40-country YAU-EAU endourology international collaboration.

Endourological practice increasingly relies on single-use devices, the costs of which vary considerably across regions and healthcare systems. Flexible single-use ureteroscopes (FURS) and flexible and navigable suction ureteral access sheaths (FANS) represent a substantial proportion of ureteroscopy-related expenditure. To mitigate these costs, some centres have adopted reuse strategies aimed at reducing per-procedure expenses. The aim of this study was to evaluate international variation in the purchase prices and real-world per-procedure costs of FURS and FANS, and to assess the economic imp

Unmet need for multiuse phacoemulsification machine products: multisociety position paper.

Nondegradable microplastic waste and carbon emissions from the manufacture, use, and disposal of surgical supplies pose threats to the environment and to public health. Current mandates and manufacturer labeling for single use of phacoemulsification machine supplies contribute to unnecessary waste. These practices have evolved from liability concerns over hypothetical risks. Single use of phacoemulsification supplies became the industry default in many countries partly because of stringent regulations requiring manufacturers to pursue costly studies to prove the safety of reuse. Despite this b

[Critical assessment of single-use endoscopes].

BACKGROUND: Single-use endoscopes have become widely established in urology. They are advocated due to the rapid integration of technological advances, the possibility of bypassing reprocessing requirements, and their potential to address gaps in availability. At the same time, their ecological impact in the context of sustainability is receiving increasing attention. OBJECTIVE: This article critically compares the advantages and disadvantages of single-use endoscopes versus reusable systems, with a particular focus on sustainability and resource consumption. [Conversion from single use to reusable in nephrology care].

Replacement of disposable by reusable care materiel is a mandatory question in sustainability management in nephrology. Recommandations have already been edited for individual protection in the operating room and the DGOS (Direction générale de l’offre de soins) will soon launch an experimentation in the reuse of some catheters and endoscopes. The literature is unanimous on the advantage, in the fields studied, of reuse over disposable material regarding green house gaz emissions. However, this advantage can be lost when considering water consumption that partly results from washing and steril

Reprocessing Effectiveness for Flexible Ureteroscopes: A Critical Look at the Evidence.

Flexible ureteroscopes are essential devices, and reusable models require meticulous cleaning and sterilization between procedures. Reprocessing is complex, time-consuming, and difficult due to the instrument's small size, fragility, and internal channel. The Food and Drug Administration recently raised concerns about the effectiveness of ureteroscope reprocessing. Due to inadequate reprocessing and maintenance, contaminated and damaged ureteroscopes pose risks to patient safety. This review describes ureteroscope reprocessing methods and summarizes evidence on reprocessing effectiveness and d

Disinfection of outpatient ophthalmic devices: a critique of "semicritical" designation.

Ophthalmic devices used in outpatient clinics are undergoing scrutiny by regulatory agencies and hospital infection control groups. Applying a nearly century-old classification for disinfection and sterilization of reusable devices and instruments, they are faulting ophthalmologists for not using high-level disinfection (HLD) listed in newer manufacturer instructions for use. By this classification, A-scan and B-scan ultrasound probes, fundus and laser contact lenses, and gonioscopy lenses are grouped with other "semicritical" devices, such as gastrointestinal endoscopes, laryngoscope blades,

Processing Reusable Medical Devices and End-of-Life Investigation.

Reusable medical devices are intended to be subjected to multiple cycles of clinical processing throughout the life of the device in accordance with manufacturers' instructions for use (IFUs). IFUs should include practical information on the service life of the medical device. This laboratory-based study investigated an approach to support an indefinite lifetime for reusable devices, where the end of life depends on the visual inspection and functional verification provided in the IFU. To evaluate the design features over a broad range of surgical instruments, worst-case, representative device

Guidance on reuse of cardio-vascular catheters and devices in India: A consensus document.

Reuse of medical device is accepted worldwide. Benefits of reuse include not only cost saving but a favorable impact on environment. However, certain requirements should be met for reuse to be safe and effective. The devices, which can be reused, should be clearly defined, a meticulous process for dis-infection and sterilization followed and its functionality ascertained before use. Further, an appropriate consent should be obtained where necessary and the cost saving entailed should be directly passed on to the patient.
