A 75-year-old male, known to have achalasia, was admitted to the intensive care unit with massive upper gastrointestinal bleeding and sepsis. He had a history of purulent pericarditis 18 months earlier. He also presented with atrial fibrillation associated with a cerebral transient ischemic accident two months earlier. A contrast computed tomography scan showed an atrio-esophageal fistula with active extravasation of contrast. He was operated on via a median sternotomy, and the defects in the atrial wall, inferior vena cava and diaphragm were closed using pericardial patches. An esophagectomy

A precise diagnosis is key to the successful treatment of achalasia. Barium swallow, upper endoscopy and high-resolution manometry provide the necessary information about a patient's anatomy, absence of other diseases, and type of achalasia (I, II, III). High-resolution manometry also has prognostic value, the best results of treatment being obtained in type II achalasia according to the Chicago classification. Abdominal CT scanning and endoscopic ultrasound might be warranted if an underlying malignancy is suspected.

PURPOSE OF REVIEW: Although not a common disease, the last several years have had exciting breakthroughs in better defining the pathophysiology of achalasia, classifying the disease with high-resolution manometry and potentially exciting new treatments.
RECENT FINDINGS: The introduction of high-resolution manometry with detailed assessment of lower esophageal sphincter function and peristalsis has made it possible to classify achalasia into three subtypes. This becomes clinically important as patients with type II achalasia do best with both pneumatic dilation and surgical myotomy, whereas ty

OBJECTIVE: Manometric findings in achalasia are pathognomic already in early clinical stages of the disease. The authors evaluate the importance of manometry in the diagnosis and therapy, in particular in early stages of the disease.
METHOD: During manometry we obtain data on the function of the lower oesophageal sphincter and the body of the oesophagus. Impaired relaxation of the sphincter is evaluated, as well as increased tonus of the sphincter and tertiary contraction of the body of the oesophagus. In patients with achalasia we implement laparoscopically extramucous myotomy.
RESULTS: In

During the past years the authors performed at the First Surgical Clinic, Faculty Hospital Olomouc laparoscopic cardiomyotomy with Dor's plastic operation in 17 patients with achalasia of the oesophagus. From the above number 15 patients were relieved of their complaints, two patients were reoperated. The cause of failure of the operation was in one case previous treatment with botulotoxin, in the other case achalasia grade IV, refractory to this type of treatment. The authors consider laparoscopic oesophago-cardiomyotomy a safe procedure, effective in particular in the earlier stages of the d

The achalasia is an uncommon pathology in childhood but the disturbances in growth, development and the severity of pulmonary symptoms are more severe than adults. In a period of twenty five years we have diagnosed three children of sixteen months, 4,5 and twelve years of achalasia and treating them with a modified Heller procedure as primary therapy adding anti-reflux procedure in the youngest. Literature review indicates excellent results following modified Heller's operation in more than 85 per 100 of cases. When an anti-reflux procedure is not performed the incidence of gastro-oesophageal

Over the past decade, per-oral endoscopic myotomy has been shown to be a durable minimally invasive approach to the treatment of esophageal achalasia. Patients with suspected achalasia should undergo upper endoscopy, timed barium esophagram, and high-resolution manometry to confirm the diagnosis. The procedure includes several key steps including mucosotomy, submucosal tunneling, selective myotomy of the circular muscle layer, and mucosotomy closure. Specialized endoscopic dissection tools are used during the procedure to access the submucosal space. Common procedural challenges include minor …

Achalasia is a primary esophageal motility disorder involving the body of the esophagus and lower esophageal sphincter. Esophageal manometry is the gold standard to diagnose achalasia. Two cardinal features are encountered in achalasia: incomplete LES relaxation and absent peristalsis in the lower esophagus. In a group of 94 patients with dysphagia we looked for the manometric changes, trying to identify clinical and manometric particularities of the cases with achalasia. 35 cases had manometric changes compatible with the diagnostic of achalasia. A number of 41 patients had other esophageal m

BACKGROUND: The diagnosis and pathological evaluation of esophageal achalasia have been improved dramatically by the development of high-resolution manometry. It is currently known to be divided into three subtypes. However, the differences between subtypes in terms of esophageal clearance remain unclear.
AIMS: To compare the pathology of subtypes in patients with esophageal achalasia from the perspective of esophageal clearance.
METHODS: We classified the patients diagnosed with esophageal achalasia into three subtypes based on the high-resolution manometry findings and compared the patient…

Achalasia is a motility disorder of the esophagus characterized by dysphagia, regurgitation of undigested food, chest pain, weight loss and respiratory symptoms. The most common form of achalasia is the idiopathic one. Diagnosis largely relies upon endoscopy, barium swallow study, and high resolution esophageal manometry (HRM). Barium swallow and manometry after treatment are also good predictors of success of treatment as it is the residue symptomatology. Short term improvement in the symptomatology of achalasia can be achieved with medical therapy with calcium channel blockers or endoscopic

Laparoscopic Heller myotomy and Dor fundoplication (LHD) were superior to pneumatic dilatation in terms of the treatment of esophageal achalasia according to 2 prospective randomized studies. Therefore, LHD is recommended. Some key points should be considered when performing LHD. First, the short gastric arteries and veins should be carefully divided so as not to injure the spleen. Second, during the Heller myotomy, the mucosa of the esophagus and stomach should not be injured because an inappropriate myotomy will not alleviate dysphagia, and intensive myotomy is recommended with care. When th…

A laparoscopic Heller cardiomyotomy technique was used on five patients whose esophageal achalasia was diagnosed clinically, radiologically manometrically. The physiological principles and operational steps are the same as in open surgery. with the patient in an anti-Trendelenburg 30 degrees position and the surgeon between the patient's legs, a CO2 pneumoperitoneum was produced. Five trocars were used. the esophagus was freed by blunt dissection and an 8 cm longitudinal myotomy was made on the anterior surface of the thoracic esophagus starting a few centimetres above the cardias and parallel…

Esophageal achalasia is a rare disorder that may mimic nausea and vomiting of pregnancy. Presentation or persistence of these symptoms in the latter half of pregnancy is unusual. We describe a case of third-trimester weight loss due to the nausea and vomiting of achalasia. Pneumatic dilation of the esophagus is often necessary to improve oral intake. However, advances in nutritional supplementation may prevent malnutrition and allow treatment and its complications to be delayed until after delivery.
