[Esophageal pH monitoring].

According to westernization of dairy life-style and aging, gastroesophageal reflux disease (GERD) with its physical symptoms such as heartburn is increasing in Japan. Although typical GERD has reflux esophagitis, GERD patients without abnormal endoscopic findings called non-erosive GERD (NERD) have been paid great attention. And more, the association between GERD and chronic respiratory diseases or laryngeal disease has been reported. To catch the more information about clinical questionnaire of GERD cases, esophageal pH monitoring is a useful method because of evaluating the direct condition

Esophageal pH monitoring, indications, and methods.

Symptomatic gastroesophageal reflux disease (GERD) is a common problem that affects a substantial proportion of the American population. It is estimated that the symptoms of GERD may afflict 40% to 45% of Americans each month. The diagnosis of GERD can be difficult, as its symptoms vary from typical symptoms like heartburn to atypical symptoms such as hoarseness, coughing, and chest pain. Most patients present with typical symptoms and are diagnosed with GERD if they respond to empiric trials of acid suppression. Many tests are available to help with diagnosing GERD in patients who either pres

Indications of 24-h esophageal pH monitoring, capsule pH monitoring, combined pH monitoring with multichannel impedance, esophageal manometry, radiology and scintigraphy in gastroesophageal reflux disease?

Ambulatory esophageal pH monitoring is an essential method in patients exhibiting signs of non-erosive reflux disease (NERD) to make an objective diagnosis. Intra-esophageal pH monitoring is important in patients who are non-responsive to medications and in those with extraesophageal symptoms, particularly in NERD, before surgical interventions. With the help of the wireless capsule pH monitoring, measurements can be made under more physiological conditions as well as longer recordings can be performed because the investigation can be better tolerated by patients. Simultaneous Laryngopharyngeal and Conventional Esophageal pH Monitoring.

In addition to typical reflux symptoms, many patients with gastroesophageal reflux disease (GERD) present with extraesophageal symptoms such as cough, hoarseness or asthma, which can be caused by laryngopharyngeal reflux (LPR). Due to their multifactorial origin, those symptoms can be a great diagnostic and therapeutic challenge. Esophageal pH-monitoring is commonly used to determine abnormal esophageal acid exposure and confirm the diagnosis of GERD. However, for better evaluation of acid exposure above the upper esophageal sphincter, a new laryngopharyngeal pH measurement system is now avail

Esophageal Impedance-pH Monitoring and Pharyngeal pH Monitoring in the Diagnosis of Extraesophageal Reflux in Children.

Various clinical symptoms are attributed to extraesophageal reflux disease (EERD). Multichannel intraluminal impedance-pH monitoring (MII-pH) is considered to correlate symptoms with acid and nonacid gastroesophageal reflux (GER) events. Pharyngeal pH monitoring (Dx-pH) is considered to correlate the decrease in the pH level in the oropharynx with reported symptoms and to diagnose supraesophageal reflux. We aimed to assess the correlation between acid reflux episodes recorded by Dx-pH and GER detected via MII-pH in children with suspected EERD. The study enrolled 23 consecutive children (15 bo

Validation of an ambulatory esophageal pH monitoring system.

The present study documents the accuracy of a commercially available ambulatory esophageal pH instrument. The distal esophagus of five subjects with daily heartburn was monitored for 24 h in the laboratory via an antimony pH electrode. The computer output from the ambulatory unit was compared to the on-line recording in terms of all events in which the pH dropped below four. A signal detection model was used, with an on-line pH tracing serving as the criterion response. The events noted as true positives (80%) were an accurate representation by the computer output of the events of pH less than

[24-hour esophageal multichannel intraluminal impedance and pH monitoring].

Monitoring of esophageal impedance is a technique for determining the physical characteris- tics (liquid, gas, or mixed) of refluxate, while impedance monitoring combined with pH recording makes it possible to assess the acidity of reflux(acid or non-acid). Assessment is usually performed by the total acid exposure time (%time pH< 4) and symptom-reflux asso- ciations including acidic and non-acidic reflux. This monitoring is quite useful for evaluation of proton pump inhibitor-resistant gastroesophageal reflux disease or non-erosive reflux disease. When normal acid exposure time and significan

Wireless esophageal pH monitoring: new technique means new questions.

INTRODUCTION: The introduction of wireless pH monitoring has been touted as a significant advance in the diagnosis of gastroesophageal reflux and associated disorders. We prospectively enrolled patients in a research registry to assess the feasibility and safety in clinical use. METHODS: All patients undergoing endoscopy with wireless pH studies (Medtronic Bravo pH system) for a 12-month period starting in April 2004 were prospectively enrolled. Probes were placed 6 cm above the endoscopically localized squamocolumnar junction. Successful completion was defined as at least 24 hours of pH reco

[Gastroesophageal reflux during pregnancy: 24-hour esophageal ph monitoring].

Gastroesophageal reflux (GER) occurs in 30-50% of all pregnancies. The progressive rise in plasma progesterone has been suggested as a possible mediator of GER during pregnancy. Recent advances in technology have made it possible to detect GER through monitoring of esophageal pH for prolonged periods, including sleep. 24-hour pH monitoring is the proper method for diagnosing GER in pregnant women. If 24-hour esophageal pH monitoring is to be a useful diagnostic tool, it must reliably discriminate GER patients despite daily variations in distal esophageal acid exposure. To address this issue, w

A new ambulatory system for extended esophageal pH monitoring.

A new 24-h ambulatory esophageal pH monitoring system (AMBI 24) was evaluated in 72 pediatric patients with suspected gastroesophageal reflux (GER). Forty-seven of these patients were monitored at home and the other 25 in our hospital. This system was employed without complication in infants and a full range of pediatric patients, as well as in older children with severe psychomotor delay. The data obtained were consistent with those reported utilizing standard monitoring equipment, and the results served as the basis for subsequent medical management. The results of this study, therefore, sug

Ambulatory esophageal and hypopharyngeal pH monitoring in patients with hoarseness.

Ten patients referred by an otolaryngologist for ambulatory esophageal pH monitoring for suspected reflux-induced hoarseness were monitored with a dual ambulatory pH system in which probes were simultaneously placed 5 cm above the lower esophageal sphincter and 2 cm above the upper esophageal sphincter (laryngeal inlet). All were nonsmokers. Hypopharyngeal reflux (pH drop less than 4 in upper electrode preceded by pH drop less than 4 in esophageal electrode) was conclusively demonstrated in 7 of 10 patients. Three of these seven patients had normal frequency and duration of esophageal reflux a

The aim of this study is to establish an intra-twin birth weight discordance prediction model. METHODS: We created an intra-twin birth weight discordance prediction model by logistic regression, based on the 1995-1997 register twin birth data of the USA. The twin sets were randomly divided into two groups: group 1 to establish the prediction model and group 2 to validate the prediction model. Dual-channel ambulatory esophageal pH monitoring. A useful diagnostic tool?

Ambulatory pH monitoring of the distal esophagus is the most accurate diagnostic study for patients with suspected gastroesophageal reflux disease (GERD). The measurement of proximal esophageal acid exposure time may be useful in patients with atypical reflux symptoms. The aim of this study is to evaluate if proximal esophageal pH monitoring provides useful information beyond that learned with distal esophageal pH monitoring. We routinely performed dual-channel pH monitoring with pH electrodes positioned at 20 and 5 cm above the manometric lower esophageal sphincter from January 1992 to August
