The anti-reflux barrier at the esophagogastric junction (EGJ) has 3 functional components: (1) the lower esophageal sphincter, (2) the crural diaphragm, and (3) the gastroesophageal flap valve. Using endoscopic measures of length of hiatal herniation, dilatation of the hiatus, and presence of a flap valve, the American Foregut Society (AFS) Hiatus Grade stratifies EGJ integrity from normal (grade I) to increasing degrees of EGJ disruption (grade II-IV). The AFS classification stipulates endoscopic methodology to utilize in making the assessment and the AFS Hiatus Grade correlates well with EGJ

In the Chicago Classification version 4.0 (CCv4), esophagogastric junction outflow obstruction (EGJOO) is manometrically defined as an elevated median integrated relaxation pressure (IRP) and elevated intrabolus pressure (IBP) during supine wet swallows, and persistently elevated median IRP in the upright position. A clinically relevant conclusive diagnosis of EGJOO requires a manometric diagnosis of EGJOO and associated symptoms such as dysphagia and/or chest pain with at least one of the following supportive investigations (pharmacologic provocation, timed barium esophagogram, and/or endofli

In the Western world, there has been an alarming rise in the incidence and prevalence of adenocarcinoma arising at the esophagogastric junction during recent decades. Epidemiological, clinical and pathological data support a sub-classification of adenocarcinomas arising in the vicinity of the esophagogastric junction (AEG) into adenocarcinoma of the distal esophagus (Type I), true carcinoma of the cardia (Type II) and subcardial carcinoma (Type III). While most, if not all, adenocarcinomas of the distal esophagus arise from areas with specialized intestinal metaplasia, which develop as a conse

Adenocarcinoma of the esophagogastric junction (EGJ) has increased rapidly in incidence in the latter half of the twentieth century. The increase in incidence has affected white men between the ages of 40 and 60 disproportionately. Understanding the etiology and improving treatment requires careful classification of EGJ tumors. A recent consensus conference recognized three types of EGJ adenocarcinomas: distal esophageal, cardia, and subcardia gastric. Distal esophageal adenocarcinomas are associated with Barrett's esophagus. Helicobacter pylori infection may play a role in some adenocarcinoma

The esophagogastric junction is a muscle, not a mucosal, junction. The boundary of the esophagus is at the upper esophageal sphincter and lower esophageal sphincter (LES) at the oral and anal ends, respectively. The distal end of the LES is the esophagogastric junction. Fine palisade vessels occur in the mucosa above the LES, and the esophagogastric junction is at the distal end of this palisade zone. The esophagogastric junction is defined as the part changing into the lumen seen in radiographic, examination or in surgically resected specimens. It is extremely difficult to identify the esopha

The definition of cancer at the esophagogastric junction has been discussed for over 30 years. Although at present, two classifications advocated by Nishi or Siewert have been used, these classifications are still controversial from the viewpoints of anatomy, physiology, histology, diagnosis and treatment. We should analyze a large number of patients based on the same definition of cancer at the esophagogastric junction. Adjustment of the Japanese rules for cancer of the esophagus and stomach is necessary, and a treatment strategy should be established for cancer at the esophagogastric junctio

In order to avoid confusion in categorizing malignant lesions for surgical treatment, a definition and criteria of carcinomas significantly involving the esophagogastric junction (ECJ-Ca) and those of gastric cardia less significantly involving the esophagogastric junction (eC-Ca) are proposed, devised as this study was from a practical viewpoint. A comparative analysis of carcinomas of these groups was carried out, the control group being those carcinomas situated mainly in the upper third segment of the stomach but not involving the esophagogastric junction (C-Ca). Carcinomas of these region

BACKGROUND: Allergic rhinitis affects around one quarter of the Western European population. Prophylactic allergen immunotherapy may be useful to reduce the risk of acute symptomatic attacks (hayfever). A five-grass pollen extract sublingual immunotherapy (5GPE-SLIT) has been developed for the treatment of allergic rhinitis to grass pollen. The objective of this study was to describe real-world treatment patterns with 5GPE-SLIT in France with respect to the prescribing information.
METHODS: This prospective cohort study was conducted by 90 community and hospital allergists. Adults and childre

Siewert's classification of adenocarcinoma of the esophagogastric junction (EGJ) classifies tumors anatomically for determining the appropriate surgical technique. According to this classification, a type II tumor, true carcinoma of the cardia, is defined as a cancer within 1 cm proximal to 2 cm distal of the EGJ. Histological analysis indicates that the cardiac gland is present with a high degree of frequency between 1-2 cm to the gastric side and 1-2 cm to the esophageal side of the EGJ, which means that this zone can be considered as neither the stomach nor the esophagus but rather as a thi

BACKGROUND: Teratoma at the esophagogastric junction is extremely rare.
CASE REPORT: A 26-day-old male neonate who presented with vomiting and melena.
OBSERVATION: Investigations revealed a mass at the esophagogastric junction, which was excised and confirmed to be a teratoma.
OUTCOME: A fistula at the lower end of the esophagus and an esophageal hiatal hernia were observed as complications.
MESSAGE: A careful surgical approach is warranted for a teratoma in esophagogastric junction, to avoid postoperative complications.

Hiatus hernia is known to be an important risk factor for developing gastroesophageal reflux disease. We aimed to use the endoscopic functional lumen imaging probe (EndoFLIP) to evaluate the functional properties of the esophagogastric junction. EndoFLIP assessments were made in 30 patients with hiatus hernia and Barrett's esophagus, and in 14 healthy controls. The EndoFLIP was placed straddling the esophagogastric junction and the bag distended stepwise to 50 mL. Cross-sectional areas of the bag and intra-bag pressures were recorded continuously. Measurements were made in the separate sphinct

As cervical metastases in esophagogastric junction cancer are extremely rare, the authors herein report a case. A 63-year-old woman presented with dysphagia since 6 months. Diagnostic endoscopy showed that the tumor was located at the esophagogastric junction and histopathological diagnosis of adenocarcinoma was offered. A subtotal gastrectomy was performed. Histopathological diagnosis was moderately differentiated adenocarcinoma, invading upto the serosa with metastases to perigastric nodes. The patient received chemotherapy of cisplatin and fluorouracil for one cycle and oral capecitabine fo

Surgical resection with lymphadenectomy is the mainstay of treatment for all resectable esophagogastric junction tumors, prior to systemic generalization of the disease. This makes accurate pre-treatment staging and classification of the tumors most demanding. A well-established and internationally accepted classification for adenocarcinomas of the esophagogastric junction (AEG) helps to choose the appropriate surgical approach and to make results from different institutions comparable. Distal esophageal adenocarcinomas (AEG I) are distinguished from true cardia carcinomas (AEG II) and subcard

The diagnosis of esophagogastric junction outflow obstruction (EGJOO) is currently based on high-resolution esophageal manometry and is characterized by impaired EGJ relaxation with preserved esophageal peristalsis. This condition has been defined by the Chicago Classification as a major esophageal motility disorder, although its clinical significance is controversial since heterogeneous and irrelevant presentations have been reported. EGJOO commonly has a benign clinical course, with spontaneous resolution, but it can also be associated with opioid usage, early achalasia, and mechanical obstr

BACKGROUND AND AIM: Esophagogastric junction outflow obstruction (EGJOO) may be due to anatomical abnormalities, but it is unclear how to evaluate them after high-resolution manometry. We aimed to determine (i) clinical and high-resolution manometry parameters differentiating anatomical EGJOO from functional EGJOO, (ii) investigations chosen and yield for anatomical EGJOO, and (iii) clinical outcomes of functional EGJOO.
METHODS: Medical records of consecutive patients with symptomatic EGJOO from February 2012 to December 2015 were reviewed. EGJOO was defined as anatomical if investigations i

The current 7th version of the TNM classification newly defines esophagogastric junction cancer as cancer invading the junction, of which the epicenter is located within 5cm proximal or distal to the junction. Progression of tumors is newly staged in this version of the TNM classification by separating adenocarcinoma and squamous cell carcinoma. The background to this revision was to resolve the confusion that occurred when classification of cancer of the esophagus or stomach was left to the judgment of the physician in the 6th version, to acknowledge that the prognosis of gastric cancer patie
