HISTORICAL ASPECTS: The idea behind this work is to have an ethical examination of the institutional psychotherapy movement which has long influenced French public psychiatry and which has progressively, since the 80s, been subject to growing doubts. In the first part, institutional psychotherapy is presented. It is a model for theoretical development and practice in psychiatric care. It came into being just following the end of the Second World War at the same time as modern medical ethics. Its principles come on the one hand, from recognition of asylums' pathogenic effects--which led to the

Institutional ethics committees (IECs) are designed to support complex clinical and organizational decision making through interdisciplinary collaboration. Despite the central role of medications in many ethically challenging scenarios, pharmacists are often absent from formal IEC membership. This omission represents a structural gap in ethical deliberation within many modern health systems. Pharmacists routinely navigate decisions involving medication safety, access, cost, allocation, and appropriateness across both inpatient and outpatient settings, many of which carry significant ethical im

National guidelines recommend the opportunity to reconsider existing Do-Not-Resuscitate (DNR) orders periprocedurally rather than automatic suspension of DNR orders. We retrospectively reviewed records for oncology patients with active DNR orders presenting for anesthesia between 2023 and 2025. Twenty patients (mean age 12 years; 60% male) underwent 41 procedures: radiation (43%), imaging (22%), interventional radiology (20%), and surgery (15%). DNR orders were suspended in 85.3% of procedures and maintained in 14.7%. No significant association existed between procedure type and DNR status (p

BACKGROUND: This is a study involving three HIV clinics in the Canadian provinces of Newfoundland and Labrador, and Manitoba. We sought to identify ethical issues involving health care providers and clinic clients in these settings, and to gain an understanding of how different ethical issues are managed by these groups.
METHODS: We used an institutional ethnographic method to investigate ethical issues in HIV clinics. Our researcher conducted in-depth semi-structured interviews, compiled participant observation notes, and studied health records in order to document ethical issues in the clin

Artificial intelligence (AI) is increasingly integrated into health research, creating ethical questions that extend beyond privacy, consent, bias, transparency, accountability, and safety. Existing bioethical principles and AI governance frameworks provide essential foundations but complex health-data ecosystems also raise institutional questions concerning reviewer capability, stewardship, reciprocity, authority, and downstream value. This Perspective proposes a stewardship-centered extension organized around four non-exhaustive governance domains: (1) IRB capability asymmetry; (2) data stew

Biological-age scores translate biomarkers and other health data into an age, age gap, organ age, or pace-of-aging estimate. Consumer testing is expanding, and actuarial literature has begun to examine these scores, although widespread discriminatory use has not been documented. The ethical concern is anticipatory. Healthcare institutions can give biological-age scores clinical and social authority by interpreting them, entering them into records, and permitting secondary use. These scores warrant focused attention because they express a complex prediction through age, a familiar and legally s

BACKGROUND: Israel's Patients' Rights Act (IPRA, 1996) established a unique statutory model for ethics committees (ECs), with legally binding authority, differing from the advisory nature of ECs in most countries. Following the 2015 State Comptroller's report highlighting widespread under-implementation, the Ministry of Health (MOH) issued a directive in 2018 aimed at standardizing EC structure, procedures, and training. However, the extent of subsequent implementation and operational activity has remained unclear.
METHODS: This secondary analysis of the 2022‑2023 MOH national survey examined

PURPOSE: Live case demonstrations have become a common occurrence at surgical meetings around the world. These demonstrations are meant to serve as an educational medium for teaching techniques, promote discussion, improve interventions and outcomes. Despite the valuable educational benefits, many authors still question the ethics of this approach. We present our 8-year experience in live surgery, discuss the ethical issues, and provide recommendations.
MATERIALS AND METHODS: We reviewed records of patients who underwent live robotic surgery during broadcasting events. Procedures performed we

This article presents an in-depth study of one institute's efforts to recover from effects of ethical violations by two senior members. Qualitative data analysis from voluntary member interviews details the damage that spread throughout the institute, demonstrating that a violation of one is a violation of many. Members at all levels reported feeling disturbed in ways that affected their emotional equilibrium, their thinking processes, and their social and professional relationships. The aggregated interview data were reported to the institute community in large and small group meetings design

PURPOSE: Survival of neonatal and pediatric patients undergoing extracorporeal membrane oxygenation (ECMO) ≥ 21 days has not been well described. We hypothesized that patients would have poor survival and increased long-term complications.
METHODS: Retrospective, single center, review and case analysis. Tertiary-care university children's hospital including neonatal, pediatric and cardiac intensive care units. After institutional review board approval, the charts of all patients < 18 years of age undergoing ECMO for ≥ 21 continuous days were performed, and they were compared to comparative pa

Donation after cardiac death (DCD) remains controversial in some pediatric institutions. An evidence-based, consensus-building approach to setting institutional policy about DCD can address the controversy openly and identify common ground. To resolve an extended internal debate regarding DCD policy at Children's Hospital Boston, a multidisciplinary task force was commissioned to engage in fact finding and deliberations about clinical and ethical issues in pediatric DCD, and attempt to reach consensus regarding the development of a protocol for pediatric DCD. Issues examined included values an

BACKGROUND: While health care needs assessments have been conducted among juveniles or adolescents by researchers in developed countries, assessments using an ethics framework particularly in developing countries are lacking. We analysed the health care needs among adolescents at the Nakambala Correctional Institution in Zambia, using the Beauchamp and Childress ethics framework. The ethics approach facilitated analysis of moral injustices or dilemmas triggered by health care needs at the individual (adolescent) level.
METHODS: The research team utilized 35 in-depth interviews with juveniles,

Health care worker (HCW) influenza vaccination rates are modest. This paper provides a detailed ethical analysis of the major options to increase HCW vaccination rates, comparing how major ethical theories would address the options. The main categories of interventions to raise rates include education, incentives, easy access, competition with rewards, assessment and feedback, declination, mandates with alternative infection control measures, and mandates with administrative action as consequences. The aforementioned interventions, except mandates, arouse little ethical controversy. However, t

PURPOSE OF REVIEW: This review highlights the barriers and facilitators that implementation science studies face in the institutional review board (IRB) approval pathway and provides recommendations on how to potentially streamline this process.
RECENT FINDINGS: Significant gaps persist between evidence-based recommendations and real-world clinical practice. Implementation science projects investigate how to promote uptake of evidence-based and guideline-recommended practices. Regulatory ambiguity may arise because implementation science involves aspects of traditional research and quality im

This case reports an ethical dilemma in which a mother who had recently delivered a baby through cesarean section after sustaining life-threatening injuries in a car accident did not have documented wishes whether she wanted to breastfeed. Her medical condition rendered her temporarily mentally incapacitated and critically ill, and the lactation medicine team was consulted about whether lactation choice should be preserved by pumping. Complicating considerations in this case were (1) lack of family or designated decision-makers available at the time of injury and emergent delivery, (2) lack of
