[Motor Evoked Potentials and Aortic Surgery].

Motor dysfunction following descending and thoracoabdominal aortic surgery including thoracic endovascular aortic repair remains as one of their devastating complications, although several interventions have tried to preserve spinal cord blood flow during and after aortic aneurysm surgery for the purpose of reducing the incidence of postoperative paraplegia. One of the main limitations of protective strategies is the inability to assess the adequacy of spinal cord perfusion and spinal cord function intraoperatively. Advances in stimulation technique using short train pulses could make intraope

Reliability of Motor Evoked Potentials Induced by Transcranial Magnetic Stimulation: The Effects of Initial Motor Evoked Potentials Removal.

INTRODUCTION: Transcranial magnetic stimulation (TMS) is a useful tool for assessment of corticospinal excitability (CSE) changes in both healthy individuals and patients with brain disorders. The usefulness of TMS-elicited motor evoked potentials (MEPs) for the assessment of CSE in a clinical context depends on their intra-and inter-session reliability. This study aimed to evaluate if removal of initial MEPs elicited by using two types of TMS techniques influences the reliability scores and whether this effect is different in blocks with variable number of MEPs. METHODS: Twenty-three healthy

[Motor evoked potentials in thoracoabdominal aortic surgery].

Thoracoabdominal aortic disease (aneurysm or dissection) has increased in recent decades. Surgery is the curative treatment but is associated to high perioperative morbidity and mortality risks. Paraplegia is one of the most severe complications, whose incidence has decreased significantly with the implementation of spinal cord protection strategies. No single method or combination of methods has proven to be fully effective in preventing paraplegia. This review is intended to analyse the scientific evidence available on the role of intraoperative monitoring with motor evoked potentials in the

Lateralized asymmetry of facial motor evoked potentials.

BACKGROUND: There is considerable evidence that emotions are expressed more intensely on the left side of the face. This asymmetry could reflect a right hemisphere advantage in processing emotional material or an asymmetry in corticobulbar motor systems. Transcranial magnetic stimulation (TMS) was used to test for lateralized asymmetry in the cortical control of muscles of facial expression in the lower face. METHODS: We administered TMS to the motor cortex of 50 subjects during contraction of bilateral orbicularis oris muscles. We analyzed motor evoked potentials (MEPs) with a repeated measu

The diagnostic value of motor evoked potentials.

OBJECTIVE: To assess the diagnostic usefulness of motor evoked potentials (MEPs) and to identify the optimal method for calculating the central conduction time. The test results were evaluated in a prospective study of 1023 neurological patients. METHODS: We evaluated the correlation between clinical and electrophysiological findings, the accuracy, the sensitivity, the percentage of subclinical abnormalities and the false negative rates of MEPs in different neurological disorders. In patients with lower motor neuron involvement, we compared the central conduction time calculated as the differ

Trans-spinal myogenic evoked potentials: a novel intraoperative technique for localizing motor deficits following loss of transcranial motor evoked potentials.

Trans-spinal motor evoked potentials (TsMEP) represent a novel intraoperative neuromonitoring technique designed to localize motor deficits when transcranial motor evoked potentials (TcMEP) are lost or unreliable. This technique involves direct electrical stimulation of the spinal cord through pedicle screws using a train of biphasic square-wave pulses, with myogenic responses recorded from limb muscles. In the presented case of kyphoscoliosis correction, TsMEP was employed after TcMEP signals failed to recover despite corrective measures. Stimulation at various vertebral levels allowed TsMEP

[Exploration of intraspinal tumors using evoked motor potentials (EMP): correlations with data of evoked somatosensory potentials].

MEPs to transcutaneous magnetic stimulation have been recorded in 43 patients with an intraspinal tumor documented by MRI. The tumor was extramedullary in 18 patients and intramedullary In 25. MEPs were abnormal in 62.8% of patients. There were no significant differences in the rate of MEP abnormalities according to the lesion site or the histological findings, except for meningiomas which showed abnormal MEPs in all eight patients included in this series. The percentage of patients with abnormal MEPs was 9.3% despite the absence of any clinical symptom of central motor pathway dysfunction. In

[Motor Evoked Potential].

Motor evoked potential(MEP)is the most widely used intraoperative neurophysiological monitoring measure. It includes cortical direct stimulation MEP(dMEP), which directly stimulates the primary motor cortex of the frontal lobe identified by short-latency somatosensory evoked potentials, and transcranial MEP(tcMEP), which involves high-current or high-voltage transcranial stimulation using cork-screw electrodes installed in the scalp. dMEP is performed in brain tumor surgery close to the motor area. tcMEP is simple, safe, and widely used in spinal and cerebral aneurysm surgeries. The increase i

"Quadripolar" Transcranial Electrical Stimulation for Motor Evoked Potentials.

PURPOSE: To determine if transcranial electrical stimulation (TES)-induced motor evoked potentials (MEPs) are of higher amplitude when using two electrodes as anodes and two as cathodes, known as "quadripolar stimulation." METHODS: Patients who underwent TES MEP monitoring in which control, bipolar stimulation and four variations of quadripolar stimulation were used were evaluated. The bipolar stimulation montage was C3-C4 (C3 was used as anode for stimulation first, then the polarity was switched to stimulate the contralateral side). Four quadripolar montages were used: C3/C1-C4/C2 (step 1),

Intraoperative Monitoring: Recent Advances in Motor Evoked Potentials.

Advances in electrophysiological monitoring have improved the ability of surgeons to make decisions and minimize the risks of complications during surgery and interventional procedures when the central nervous system (CNS) is at risk. Individual techniques have become important for identifying or mapping the location and pathway of critical neural structures. These techniques are also used to monitor the progress of procedures to augment surgical and physiologic management so as to reduce the risk of CNS injury. Advances in motor evoked potentials have facilitated mapping and monitoring of the

Motor evoked potentials in thoracoabdominal aortic surgery: PRO.

Paraplegia is one of the most severe complications of the repair of open descending thoracic aortic aneurysms and thoracoabdominal aortic aneurysms. To reduce these complications, a comprehensive strategy for spinal cord protection is mandatory. Motor evoked potentials provide the surgeon with important information about spinal cord integrity throughout the operation. Neuroprotective measures include extracorporeal circulation, cerebrospinal fluid drainage, hypothermia, and selective segmental artery revascularization.

Pectoralis major motor evoked potentials in cervical spondylosis.

Myelopathy is a severe complication of cervical spondylosis (CS). We studied 27 consecutive patients with CS referred for evaluation for possible myelopathy using transcranial magnetic stimulation. The findings were compared with those from 20 normal controls. Magnetic resonance imaging was utilized to assess the degree of cord compromise. Central motor conduction time (CMCT) abnormalities showed equivalent diagnostic yield with pectoralis major (PM) recordings, as compared with combined first dorsal interossei and abductor hallucis recordings. Our findings show that CMCT measurement with PM r

Lower extremity motor evoked potentials in multiple sclerosis.

Transcranial magnetic stimulation was performed on 25 patients with definite multiple sclerosis. Motor evoked potentials were recorded from the anterior tibial muscle. Central motor conduction time was calculated using the equation (F + M-1)/2 by stimulation of the common peroneal nerve. Motor evoked potentials were capable of detecting subclinical pyramidal tract lesions in multiple sclerosis. In patients with multiple sclerosis, the incidence of abnormality of motor and somatosensory evoked potentials was similar. Central motor conduction time was correlated with overall and pyramidal tract

Motor-evoked potentials of facial musculature in dogs.

Motor responses of limb musculature have been elicited by transcranial brain stimulation using electrical and magnetic stimulation. Recording from muscles innervated by cranial nerves has not previously been reported. A unilateral hemispheric craniotomy was performed in 24 dogs. An electric stimulus was delivered directly to the exposed motor cortex via surface electrodes. Evoked electromyographic responses to motor cortex stimulation were recorded from the contralateral orbicularis oculi and orbicularis oris muscles. Preliminary investigation into the use of magnetic stimulation of the motor

Motor-evoked potentials of tongue musculature in dogs.

Transcranial brain stimulation by either electrical or noninvasive magnetic stimulation has produced motor responses of limb musculature. However, recordings from muscles innervated by cranial nerves have not previously been reported. A right-sided craniotomy was performed in 18 dogs. An electric stimulus was delivered to the motor cortex via surface electrodes and evoked EMG responses were recorded from the tongue musculature to determine conduction time. Magnetic stimulation of the motor cortex yielded EMG latencies similar to those obtained by electrical direct cortical stimulation, thus de

Transcranial motor-evoked potentials disappear with pons transection.

Motor-evoked potential (MEP) monitoring by transcranial electrical stimulation (TES) is important for intraoperative motor function assessment in neurosurgery; however, false-negative results sometimes occur, and these findings should be interpreted with caution. Herein, we report an interesting MEP change resulting from a pons transection. The patient was a boy aged 5 years and 2 months. He underwent multiple craniotomies for cerebellar anaplastic ependymoma and was already paralyzed in the right upper and lower limbs. Therefore, we decided to remove the recurrent lesion from the left anterio

[Cerebral Blood Flow Monitoring and Motor Evoked Potentials].

Near infra-red spectroscopy (NIRS) and motor evoked potentials (MEP) have been used to monitor brain and spinal cord ischemia. NIRS calculates oxygen saturation of hemoglobin, based on the modified Beer-Lambert law. It correlates with the change in regional tissue blood flow. However, the technology is not matured enough for the measured value to be used as an index of tissue oxygenation, so that relative change should be carefully followed. Myogenic MEP has widely been used to monitor spinal cord ischemia, since the introduction of pulse train transcranial electrical stimulation. It evaluates
