Atrioventricular interval optimization and exercise tolerance.

Modern pacemakers offer many programming options regarding the AV interval including the ability to vary AV intervals depending on whether atrial activity is paced or spontaneous and to shorten AV intervals with increasing rates. To determine if optimization of these features improves exercise tolerance, 14 patients with intact sinus node function and AV block treated with dual chamber pacemakers were enrolled in a randomized double-blind crossover trial. Doppler echocardiographic measurements of cardiac index and mitral flow were assessed over a range of programmable AV intervals at rest to d

Exercise tolerance and daily life in McArdle's disease.

McArdle's disease is a common disorder of muscle metabolism and is due to myophosphorylase deficiency. The major complaint of patients with this disease is effort intolerance. Although the clinical features of affected patients are well known, their daily lifestyle is not well documented. The main objective of this work was to assess their mean daily energy expenditure (DEE) and compare it with control subjects. Thirty patients and 87 control subjects completed a questionnaire. A 3-day self-record of daily physical activities was used to estimate the mean DEE for patients and control subjects.

Cardiovascular hemodynamics and exercise tolerance in thyroid disease.

The heart is an organ sensitive to the action of thyroid hormone, and measurable changes in cardiovascular performance are detected with small variations in thyroid hormone serum concentrations. Most patients with thyroid disease experience cardiovascular manifestations, and the most serious complications of thyroid dysfunction occur as a result of cardiac involvement. The increased metabolic state and oxygen consumption that occur in hyperthyroid patients require an increased supply of oxygen and removal of metabolic products from the periphery. This is accomplished by increasing the cardiac

[Exercise tolerance after correction of tetralogy of Fallot].

Twenty-six postoperative patients with tetralogy of Fallot (TF) were evaluated by exercise stress test with an upright cycle ergometer. Oxygen uptake was assessed at the anaerobic threshold and the peak achieved work load. Oxygen uptake at the anaerobic threshold (VO2AT) was in the normal range regardless of the existence of residual stenosis (PS) or pulmonary regurgitation (PR). However, oxygen uptake at the peak achieved workload (VO2max) was subnormal in patients with PS or PR. Maximal heart rate in TF patients was lower than normal. Patients with PR showed significantly reduced VO2max as c

Is exercise tolerance limited by the heart or the lungs?

This 55-year-old man with known rheumatic mitral valve disease is modestly disabled achieving a VO2max of 72% and a maximal power output of 76% of the predicted normal. His capacity to exercise is limited by dyspnea due to a reduction in his capacity to breathe. Ipratropium bromide was initiated to maximize his expiratory flow and improve his ventilatory capacity. A trial of inhaled steroids produced no improvement. He was referred for rehabilitation and smoking cessation. A decision was made to continue surveillance, postponing mitral valve replacement.

Relation of a Maximal Exercise Test to Change in Exercise Tolerance During Cardiac Rehabilitation.

The purpose of this study was to test the hypothesis that an individualized exercise training target heart rate (HR) based on a maximal graded exercise test (GXT) is associated with greater improvements in exercise tolerance during cardiac rehabilitation (CR) compared with no GXT. In this retrospective study, we identified patients who completed 9 to 36 visits of CR between 2001 and 2016, with a length of stay ≤18 weeks and a visit frequency of 1 to 3 days per week. Patients were grouped based on whether their exercise was guided by a target HR determined from a GXT. To assess the relation bet

Exercise prescription improves exercise tolerance in young children with CHD: a randomised clinical trial.

OBJECTIVE: The main objective of this study was to ascertain if a structured intervention programme can improve the biophysical health of young children with congenital heart disease (CHD). The primary end point was an increase in measureable physical activity levels following the intervention. METHODS: Patients aged 5-10 years with CHD were identified and invited to participate. Participants completed a baseline biophysical assessment, including a formal exercise stress test and daily activity monitoring using an accelerometer. Following randomisation, the intervention group attended a 1 day

Cold Stress Effects on Exposure Tolerance and Exercise Performance.

Cold weather can have deleterious effects on health, tolerance, and performance. This paper will review the physiological responses and external factors that impact cold tolerance and physical performance. Tolerance is defined as the ability to withstand cold stress with minimal changes in physiological strain. Physiological and pathophysiological responses to short-term (cold shock) and long-term cold water and air exposure are presented. Factors (habituation, anthropometry, sex, race, and fitness) that influence cold tolerance are also reviewed. The impact of cold exposure on physical perfor

Cardiorespiratory exercise tolerance in asymptomatic children with Ebstein's anomaly.

The aim of the study was to evaluate cardiorespiratory exercise tolerance in asymptomatic children with Ebstein's anomaly. Eleven children with a mean age of 9.6 years were prospectively studied by spirometry, cardiopulmonary exercise testing (bicycle ergometer n = 8, treadmill test n = 3), and contrast echocardiography. A right-to-left atrial shunt was detected by contrast echocardiography in 7 children (group 1), whereas no shunt was found in 4 (group 2). VO2 max was decreased [84.5 (SD = 16.8)] and was strongly correlated to oxygen saturation in group 1 (p < 0.0001). Oxygen saturation at pe

Sustained improvement in exercise tolerance following physiological cardiac pacing.

Fifty-three patients have received 'physiological' pacemakers, 37 with atrioventricular (AV) block having atrial synchronous units (VAT or VDD) implanted and the remaining 16 patients with both AV block and sick sinus syndrome having 'universal' (DDD) pacemakers. Effort tolerance was assessed by serial bicycle ergometry and in 16 patients direct comparisons between ventricular pacing and atrial synchronous pacing could be made acutely. Physiological pacemakers were found to increase maximum effort tolerance by 43% compared to pre-pacing values (P less than 0.01). The increase was sustained ove

SMART System in the Assessment of Exercise Tolerance in Adults.

Health-oriented physical activity should meet two key criteria: safety and an optimal level of exercise. The system of monitoring and rationalization of training (SMART) was designed to meet them. SMART integrates a custom-configured inertial measurement unit (IMU) and a sensor with real-time heart rate measurement (HR) using a proprietary computer application. SMART was used to evaluate the safety and exercise load with 115 study participants: 51 women (44.35%) and 64 men (55.65%) aged 19 to 65 years. The exercise test was the 6MWT test. In 35% of the participants, the mean HR exceeded the re

High-intensity exercise improves pulmonary function and exercise tolerance in a patient with TSC-LAM.

INTRODUCTION: While exercise has been shown to improve respiratory symptoms, exercise tolerance, and bone mineral density in many populations, no supervised exercise training interventions have been undertaken in patients with lymphangioleiomyo-matosis (LAM). MATERIAL AND METHODS: One patient with TSC-LAM (tuberous sclerosis complex lymphangioleiomyomatosis) participated in two weekly sessions (50-60 min) of supervised aerobic exercise at 80-85% heart rate max for one year. Treadmill ergometry (VO2peak), spirometry (FEV1, FVC, FEV1/FVC, peak flow), and bone mineral density testing were perfor

Iron deficiency and reduced exercise tolerance in adult Fontan patients.

BACKGROUND: The Fontan procedure allows patients with complex congenital heart defects unsuitable for biventricular repair to reach adulthood; however, it results in a non-physiological circulation characterized by chronically elevated systemic venous pressure, impaired preload, and multiple long-term complications. Iron deficiency (ID), a well-recognized determinant of reduced exercise capacity and adverse prognosis in heart failure, may also affect adults with Fontan circulation, yet data remain scarce. This study aimed to evaluate the prevalence, hematologic profile, and functional conseque

Relationship between exercise capacity and glucose tolerance in cystic fibrosis.

BACKGROUND: Improved exercise capacity (EC) and normal glucose tolerance (NGT) are independently associated with favorable outcomes in CF, however, little information on this relationship exists in patients with CF. METHODS: Cardiopulmonary exercise tests, oral glucose tolerance tests (OGTT), and HbA1c values measured within a 12-month period were evaluated on 83 pediatric patients diagnosed with CF. Patients were categorized as having NGT, abnormal glucose tolerance (AGT), or CF-related diabetes (CFRD). RESULTS: EC decreased as severity of glucose intolerance increased across NGT, AGT, and
