Existential concerns are common in serious illness and may include confronting questions about meaning, purpose, and mortality. The salience of these issues can vary between patients and throughout illness, and can oscillate between existential suffering, demoralization, normative processing, and even existential growth. Through analysis of a clinical composite case, this article illustrates dimensions and cues of existential suffering during serious illness. Recognizing the sources and manifestations of existential distress can help expand therapeutic imagination in serious illness care. Guid

Current paradigms for understanding Narcissistic Personality Disorder (NPD)-including psychoanalytic, cognitive-behavioral, and neuroscientific approaches-offer valuable insights but remain fragmented. They largely fail to explain the deep motivational structure of narcissism, particularly the co-existence of grandiose and vulnerable subtypes and the disorder's profound resistance to treatment. This paper proposes an integrative meta-theoretical framework, the Existential Fracture model, to re-conceptualize NPD not merely as a cluster of symptoms but as a fundamental crisis of meaning arising

The hypothesis is explored that today's ideas of self-realization usually remain superficial and do not meet the real expectations of the individual in all their interests if they do not include the concepts of transience and finiteness of life and the possibility of coping with them through e.g., the dialogical power of love. People with mental disorders in particular need reliable concepts and stable relationships with fellow human beings, both of which can be experienced and learned in current forms of existential psychotherapy of May and Yalom in confrontation with basic conflicts such as

As life expectancy in the West increases and companies can no longer promise lifelong security, many businesspeople will need to make major changes during middle age, embarking on a second life and a second career. They must start by getting beyond two pervasive and opposing myths. The first is that midlife marks the onset of decline. Problems do arise during middle age--concerns about health and finances, for instance--but one's life force does not expire at 65, nor do possibilities vanish. In fact, by middle age, most executives have gained a freedom that only self-knowledge can impart, and

"Ask not for whom the bells toll; they toll for thee." John Donne's admonition, though written 350 years ago, endures with astonishing freshness; it speaks to something self-evident, to a truth that is well known to many who have experienced bereavement--that the death of a significant other has the potential to hurl the survivor into a confrontation with his/her own death. A confrontation with death--should we seek it? There is evidence in the clinical literature that in terminally ill patients such a confrontation may lead to pronounced positive psychological changes. Research (Yalom 1980) h

The purpose of this paper is to clarify some existing misunderstandings in the area of psychotherapy, existential approach and the Eastern Tao (Zen Buddhism, Confucianism, Lao-tzu, Chuang-tzu) by showing the common elements between Eastern Tao and psychoanalysis, and existential thought. The author compared the goal of Tao practice, namely, Zen Buddhism, Confucianism etc. with that of Western psychoanalysis, humanistic psychology and transpersonal psychotherapy. He concludes that these goals are the same and that the names are different. He also compared the procedures and processes of psychoa

Existential isolation (EI) has been linked to depressive symptoms, yet evidence suggests it is not always distressing and can be meaningful for some individuals by satisfying their desire for uniqueness. To explore the mechanism between EI and depressive symptoms, the present study examined whether existential loneliness (EL) mediates the association, and whether need for uniqueness (NU) moderates the EI-EL link. Using a cross-sectional questionnaire design, data were collected from a representative sample of 379 UK adults aged 18 to 87 (M = 46.84, SD = 15.57). Correlation analysis, mediation,

"Existential pain" is a widely used but ill-defined concept. Therefore the aim of this study was to let hospital chaplains (n=173), physicians in palliative care (n=115), and pain specialists (n=113) respond to the question: "How would you define the concept existential pain?" A combined qualitative and quantitative content analysis of the answers was conducted. In many cases, existential pain was described as suffering with no clear connection to physical pain. Chaplains stressed significantly more often the guilt issues, as well as various religious questions (P<0.001). Palliative physicians

In this review essay I outline how the current renaissance of psychedelics in mainstream discourses about medicine is spearheaded first and foremost by profit-oriented technomedical initiatives focusing on individual wellness, and how this very situation reinforces a problematic line of demarcation between the psychopharmaceutical approach to mental health, and the more classical approach that relies on intersubjective talk therapy. It will lead me to formulate brief remarks about the unique challenges that this context presents for the field of medical humanities-specifically regarding the st

In this article we offer an existential theory of well-being that is guided by Heidegger's later writings on "homecoming". We approach the question of what it is about the essence of well-being that makes all kinds of well-being possible. Consistent with a phenomenological approach, well-being is both a way of being-in-the-world, as well as a felt sense of what this is like as an experience. Drawing on Heidegger's notion of Gegnet (abiding expanse), we characterise the deepest possibility of existential well-being as "dwelling-mobility". This term indicates both the "adventure" of being called

AIM: The aim of this paper is to present a hermeneutic phenomenological study illuminating patients' existential situation prior to colorectal surgery. The intention was also to explore the value of the encounter between patient and nurse.
BACKGROUND: Patients waiting for major surgery experience multifarious reactions. Emotions of anxiety, fears of the unknown, anaesthesia, cancer diagnosis and death can arise. Several earlier studies have reported the importance of information, coping strategies and the need to reduce anxiety and stress in relation to surgery. However, there is a lack of st

AIM: To assess the psycho-existential distress of patients and their caregivers in a specific setting, like hospice.
METHODS: Patients consecutively admitted to two hospices for a period of 8 months were enrolled. At admission (T0), patients were assessed by a routine data recording: age; gender; Edmonton Symptom Assessment Scale (ESAS); Memorial Delirium Assessment Scale (MDAS); Cut down, Annoy, Guilt, Eye-opener (CAGE); Karnofsky level; primary diagnosis; education; religiosity and comorbidities. Psycho-existential distress was assessed at T0 by the Psycho-existential Symptom Assessment Sca

CONTEXT: Depression and existential distress are highly prevalent among patients with advanced cancer, and there is growing evidence for ketamine as a rapidly acting antidepressant in the population, but few studies have examined existential distress outcomes with ketamine treatment. The INKeD-PC trial demonstrated efficacy of intranasal racemic ketamine for depression in patients with cancer receiving palliative care.
OBJECTIVES: We now report on secondary trial outcomes, including those related to existential distress.
METHODS: This analysis is based on 15 trial participants who received 3

Spiritual beliefs and practices have long served as a critical component of treating or managing serious illness. There is evidence to suggest that patients would like healthcare professionals to address their existential and spiritual needs. The CASH Assessment Tool focuses on four key areas of spiritual concern: Care, Assistance/Help, Stress, and Hopes/Fears. In this QI Project, a palliative care team within an urban, safety net, tertiary-care, and academic healthcare setting sought to elicit themes from the CASH assessment used by chaplains caring for patients with serious illnesses. BACKGROUND: Although interest in existential perspectives on health is growing, little is known about how such perspectives are experienced by older adults in primary care. The aim of this study was to understand how older adults who took part in a primary care-based existential group treatment describe the role of existential challenges in relation to their health and in primary care.
METHODS: Seventeen patients (75+) with psychological distress who had completed a 7-week existential group treatment took part in individual interviews. Transcripts were analyzed using thematic analysis.

Existential obsessions-persistent, intrusive doubts about unanswerable questions such as the nature of reality, identity, free will, and death-are an understudied and underrecognized presentation of obsessive-compulsive disorder (OCD). Although these symptoms align with core OCD features, their abstract and philosophical content distinguishes them from more commonly studied presentations. This article provides a conceptualization of existential obsessions, outlining their phenomenology, cognitive-behavioral underpinnings, and implications for assessment and treatment. We identify four primary

The clinical presentation of obsessive-compulsive patients is characterized by unwanted, intrusive, nonsensical, self-related, and recurrent ideas, thoughts, images, or impulses associated with active compulsive compensations. Under the operational diagnostic criteria adopted by the biological- and cognitive-oriented neopositivist medical paradigm, it is known as "obsessive-compulsive disorder." However, this paradigm has been criticized for its controversial assumptions, limited methodologies, theoretic biases, and inconsistency in producing practical outcomes. To bypass some of these issues,
