We report a patient with pontine infarction who showed non-paralytic pontine exotropia (NPPE) with alternating exotropia. An 81-year old woman developed diplopia and gait disturbance. On the fourth day, she was admitted to our hospital. Oculomotor findings on admission showed that the ocular position of the left eye on forward gaze was fixed at the midline, while the right eye was abducted. When she was instructed to use the right eye for forward gaze, the left eye deviated outward. On right gaze, the left eye was not abducted and monocular nystagmus was noted in the abducted right eye. Intermittent exotropia (IXT) is the most common type of divergent strabismus. It is the consequence of passive mechanisms due to the anatomy of the globes and orbits or due to active innervational mechanisms, resulting in divergence of the visual axes, which is compensated by fusional convergence. Intermittent insufficiency in this compensation gives this form of exotropia its intermittent nature. The most common symptoms of IXT are closure of one eye, asthenopia and diplopia, but they are often absent. The clinical classification of IXT (according to Burian) is based on the difference between

INTRODUCTION: Cyclic strabismus is a rare disorder in which manifest strabismus and controlled latent deviation (heterophoria) alternates on a regular pattern which is usually over a 48-hr period. During the "straight" period the patient has a fully controlled heterophoria for all distances, with very good fusion and stereopsis. During the "squinting" period the patient has a constant manifest strabismus for all distances, with no demonstrable fusion and stereopsis.
METHOD: We describe 2 cases of alternate day exotropia. One patient requires strabismus surgery to obtain constant binocularity.

A 20-year old female was found to have an alternating pattern of right esotropia and right exotropia following surgery for a clivus chordoma three years previously. When esotropic, abduction was markedly limited while adduction was full in the right eye. When exotropic, mild limitation of both abduction and adduction was noted. Horizontal saccadic velocity studies were consistent with the diagnosis of sixth nerve palsy when esotropia was manifest and a Duane's syndrome, type II when exotropia was present. The etiology of these unusual findings is not well understood.

Purpose: To assess the correlation between the contribution rates of fusional convergence from the dominant and non-dominant eye and suppression depth and exotropia control. Study design: Cross-sectional prospective study. Methods: The fusional convergence of 25 participants with intermittent exotropia (mean age 10.8 ± 3.4; range 6-18 years) was measured with an eye-tracking system. The contribution rate was defined based on the amplitude of fusional convergence during refusion relative to the exo-deviation angle. The suppression depth was assessed, and exotropia control was evaluated using th

Intermittent exotropia is a type of strabismus between exophoria and constant exotropia. The onset age is usually one to four years old. Different from other types of strabismus, intermittent exotropia patient can maintain normal binocular vision in a long period of time. So how to choose the optimal timing and effective treatment is a challenge for clinicians. The objective of this article is to review the recent research progress of intermittent exotropia, including the severity classification, non-surgical treatment, timing of surgery and surgical methods, overcorrection and under correctio

Cyclic strabismus is an uncommon disorder in which strabismus alternates with orthotropia in alternating 24-hour periods, although cycles of other lengths have also been described. Alternate-day exotropia is a rare form of cyclic strabismus which, to date, has been reported in few patients. We describe a case of alternate-day exotropia in a child who had previously undergone surgical correction of constant esotropia.

BACKGROUND/AIMS: We evaluate the clinical characteristics of intermittent exotropia with controllability and compare surgical outcomes between patients with and without controllability.
METHODS: We reviewed the medical records of patients aged 6-18 years with intermittent exotropia who underwent surgery between September 2015 and September 2021. Controllability was defined as the patient's subjective awareness of exotropia or diplopia associated with the presence of exotropia and ability to instinctively correct the ocular exodeviation. Surgical outcomes were compared between patients with an

BACKGROUND/OBJECTIVES: To determine the clinical features of different types of intermittent exotropia according to the distance and near angles of exodeviation.
METHODS: This study included 5331 patients with intermittent exotropia. The patients were divided into three groups according to the near-distance differences in their exodeviations: (1) Basic-type: difference between distant and near angles of the exodeviation < 10 prism diopters (PD); (2) Convergence insufficiency (CI)-type: near-distance angle ≥ 10 PD; (3) Divergence excess (DE)-type: distance-near angle ≥ 10 PD. The main outcome

We compare the surgical outcomes of intermittent exotropia of the basic, pseudo-divergence excess (pseudo-DE) and true divergence excess (true DE) types. A study was performed with 342 patients who had undergone surgery for intermittent exotropia of the basic, pseudo-DE or true DE type with a postoperative follow-up period of 6 months or more. The main outcome measures were postoperative angles of deviation at distance and near, and surgical success rates. Surgical success was defined as alignment between exodeviation of 10 PD and esodeviation of 5 PD at distance and near. Additionally, surviv

Patient with exotropia frequently alternate fixation, looking at something with one eye and then switching their attention to acquire a new target with the other eye. Which eye informs the brain about the location of the new target? To address this issue, we presented targets dichoptically to 16 exotropes that were visible to the fixating eye, the deviated eye, or to both eyes. We then compared the subjects' choice of eye for target acquisition with the organization of their suppression scotomas. There was a correspondence between suppression scotoma maps and the eye used to acquire peripheral

BACKGROUND: Diplopia after cataract surgery has been reported by several authors, but diplopia after recovery from vitreous hemorrhage (VH) has not been described.
METHODS: We examined eight patients with manifest exotropia and binocular diplopia after recovery from dense VH by vitreous surgery. VH was bilateral in three patients and unilateral in five, and lasted for an average of 7.7 years.
RESULTS: Visual acuity before vitrectomy ranged from 20/200 to light perception; that after vitrectomy ranged from 20/20 to 20/60. Exotropia was present in all patients after vitrectomy. Additionally, s

PURPOSE: To determine success rates over time for strabismus surgery for sensory exotropia and to determine factors associated with successful outcomes.
METHODS: We retrospectively reviewed medical records of patients with sensory exotropia (best-corrected visual acuity ≤20/200 in the affected eye) who underwent strabismus surgery between May 2009 and December 2019. Patients with paralytic/restrictive exotropia and patients who did not follow up postoperatively were excluded. Surgical success was defined as exotropia of ≤10Δ or esotropia of ≤6Δ. Cox-proportional hazard models were used to eva

PURPOSE: To evaluate a group of infants with this disorder to determine the long-term outcome of surgery and to assess the need for neurologic evaluations.
METHODS: This interventional case series reviewed the records of infants who underwent surgery for the treatment of exotropia with onset during the first year of life. The preoperative ophthalmic and systemic findings, treatment, and developmental and ophthalmic outcomes were reviewed. Surgery was considered successful if the horizontal deviation was less than 10 prism diopters (PD). Developmental assessments were obtained at each visit.

A 67-year-old woman complained of horizontal diplopia shortly following bilateral cataract surgery with intraocular lens implantation performed under retrobulbar anesthesia. Retrobulbar anesthesia was administered at an inferotemporal injection site using 1 cc lidocaine hydrochloride 2% mixed with bupivacaine hydrochloride 0.5%. The initial ophthalmologic evaluation showed a 12-prism diopter (PD) exotropia, and ocular motility evaluation revealed marked limitation of adduction without vertical limitation. One year after cataract surgery, the exodeviation increased up to 60 PD. The patient unde

PURPOSE: Consecutive exotropia is commonly the result of a surgical correction of a primary esotropia but may develop spontaneously. This study examines the clinical characteristics of patients who have developed a spontaneous consecutive exotropia and relates the key features to previously reported findings.
METHODS: Patients were identified by routine clinical practice from May 2000 to December 2003 by retrospective case note analysis. Patients that presented with an esotropia that became an exotropia (without surgery or botulinum toxin A) were included.
RESULTS: This study consisted of 14

A 36-year-old male had been constantly wearing a patch to avoid diplopia since an operation on the right parietal area of the brain. Postsurgical visual field evaluation indicated a complete bilateral left hemianopia with macular splitting. The patient had a constant alternating comitant exotropia with no suppression. By terminating the patching, establishing the appropriate fixing eye, and applying relieving prisms, diplopia was removed. Once binocularity was thus established, it was found (over the course of 7 months) that binocular function was possible with diminishing amounts of prism pow

Twelve exotropes of various types received oculomotor biofeedback therapy at State College of New York (SUNY) University Optometric Center. Feedback of a variable pitch tone which reflected changes in ocular vergence reinforced motor control of eye posture. Patients were trained to achieve and sustain alignment in a variety of viewing situations. The six intermittent exotropes in the study who did not have amblyopia or prior history of unsuccessful surgical or orthoptic therapy achieved the highest recovery rating after training. The amblyope and those who had orthoptic training learned to vol
