BACKGROUND: Prolonged slow expiration (PSE) is a physiotherapy technique often applied in infants to reduce pulmonary obstruction and clear secretions, but there have been few studies of PSE's effects on the respiratory system.
OBJECTIVE: To describe PSE's effects on respiratory mechanics in infants.
METHODS: We conducted a cross-sectional study with 18 infants who had histories of recurrent wheezing. The infants were sedated for lung-function testing, which was followed by PSE. The PSE consisted of 3 sequences of prolonged manual thoraco-abdominal compressions during the expiratory phase. W

BACKGROUND: There has not been a detailed description of expiratory reserve volume (ERV) during slow expiration with glottis open in infralateral decubitus position (ELTGOL, for Expiration Lente Totale Glotte Ouverte en infraLatéral) and its reproducibility. The aim of this study was to determine ERV during ELTGOL and to evaluate ERV intra-observer and inter-observer reliability.
METHODS: In this prospective study, subjects were 30-70 y of age with chronic lung disease. ELTGOL (an active-passive or active physiotherapy technique) was applied in random order by 3 observers: 2 trained physiothe

Obesity, particularly severe central obesity, affects respiratory physiology both at rest and during exercise. Reductions in expiratory reserve volume, functional residual capacity, respiratory system compliance and impaired respiratory system mechanics produce a restrictive ventilatory defect. Low functional residual capacity and reductions in expiratory reserve volume increase the risk of expiratory flow limitation and airway closure during quiet breathing. Consequently, obesity may cause expiratory flow limitation and the development of intrinsic positive end expiratory pressure, especially

PURPOSE OF REVIEW: Obesity is an increasing world-wide public health concern. Obesity both causes respiratory symptoms and contributes to many cardiorespiratory diseases. The effects of obesity on commonly used lung function tests are reviewed.
RECENT FINDINGS: The effects of obesity on lung function are attributed both to mechanical factors and to complex metabolic effects that contribute to a pro-inflammatory state. The effects of obesity on lung function correlate with BMI and correlate even better when the distribution of excess adipose tissue is taken into account, with central obesity a

It is certainly difficult to understand the clinical significance of dyspnea or breathlessness among the elderly, though this symptom is quite routine. Setting a conjugate of the dyspnea index as an indicator of ventilatory reserve ratio, its distribution was tested along an age abscissa upon the twenty senile dyspnea and demonstrated almost all of them below the 30% level. Their arterial blood oxygen tensions were grouped onto the severity rank of dyspnea, clarifying that some senile lived of no complaint by the blood oxygen tensions within the range of pulmonary insufficiency, despite there

Expiratory flow limitation results in dynamic hyperinflation, dyspnoea and premature exercise intolerance. We aimed to measure whether expiratory resistance reduces locomotor power via limiting maximal voluntary motor activity, exacerbating muscle fatigue, or both. Healthy volunteers (n = 14; 23 (3) years) performed a series of very heavy-domain constant power cycling exercise tests with and without an imposed expiratory flow resistance (7 cmH2O/L/s). The decline in maximal evocable isokinetic power at intolerance during each experimental condition was apportioned to: (1) the power equivalent

Measurements of deposition, De, during steady breathing through the mouth of particles between 0.5 and 2.5 micons diameter and density 0.92 g/cm3 show that it is related to the tidal volume, VT, expiratory reserve volume, Vr, and a "normal" volume, Vro, by the formula 100 De= Vt/B+C/FD- E- Vr-Vro/A where B, C, D and E are constants which depend on particle size but do not vary from subject to subject enough to have much influence on corresponding values of De. A and and Vro are independent of particle size but relate to the individual subject, especially Vro. A new method is described of corre

1. The extent to which the pattern of breathing at maximal ventilation in man is affected by the mechanical properties of the respiratory pump has been studied. 2. The maximal effort flow volume (MEFV) loop has been used to calculate the shortest possible inspiratory (TI) and expiratory (TE) durations associated with the highest ventilation for all tidal volumes (VT). These minimal TIS and TES hve been plotted on a VT-TI-TE diagram. 3. Such predicted minimal TIS and TES were compared with observed minimal values from five healthy subjects who tried to reach their maximal ventilations during th

Pneumoperitoneum for laparoscopic surgery is known to stiffen the chest wall and respiratory system, but its effects on resting pleural pressure in humans are unknown. We hypothesized that pneumoperitoneum would raise abdominal pressure, push the diaphragm into the thorax, raise pleural pressure, and squeeze the lung, which would become stiffer at low volumes as in severe obesity. Nineteen predominantly obese laparoscopic patients without pulmonary disease were studied supine (level), under neuromuscular blockade, before and after insufflation of CO2 to a gas pressure of 20 cmH2O. The effect of general anaesthesia on postoperative pulmonary mechanics was studied by comparing pre- and postoperative spirometry in 30 patients undergoing extracorporal shock wave lithotripsy, 15 of whom received a unilateral intercostal nerve block, the remainder a general anaesthetic. Both groups of patients had significant postoperative changes in peak expiratory flow rate, expiratory reserve volume and vital capacity; forced expiratory volume at one second was not significantly changed in either group. There were no significant differences in the pre- or postoperative mechanics between th

PURPOSE: Wheelchair seating configuration may influence respiratory mechanics by altering pelvic alignment, trunk posture, and diaphragmatic excursion. Deep sitting is generally recommended to maintain pelvic neutrality; however, the quantitative impact of sacrum-backrest distance on ventilatory performance remains unclear. This study aimed to investigate the effect of sacrum-backrest distance on respiratory function and thoracic expansion during wheelchair sitting.
MATERIALS AND METHODS: Nineteen healthy young females were assessed under three wheelchair sitting conditions defined by sacrum-

A case of mild hypoventilation associated with an incompletely inflated pneumatic antishock suit in traumatic quadriplegia is presented. Pulmonary function studies were performed on the same patient 23 days later. Base line studies were performed and then repeated with the antishock suit inflated to 40 and then 100 mm Hg. Measurements were made of tidal volume (Vt), inspiratory reserve volume (IRV), expiratory reserve volume (ERV), forced vital capacity (FVC), forced vital capacity in one second (FEV1), and maximum voluntary ventilation (MVV). With increasing suit pressures, all parameters dec

Respiratory insufficiency after acute cervical trauma is well documented, but the relationship between respiratory function and chronic lesions, such as cervical spondylosis, has received scant attention. This clinical study investigated the effect of cervical spondylosis on respiratory function in 12 patients over 65 years of age who underwent expansive laminoplasty. Functional and neurological status were assessed using the Japanese Orthopaedic Association (JOA) scale and Neurosurgical Cervical Spine Scale (NCSS). To assess the effect of laminoplasty on respiratory function in patients with

BACKGROUND: Respiratory disorders result in rehospitalization and premature death of patients with cervical spinal cord injuries (CSCI). Community game-based respiratory muscle training (RMT) programs could reduce secondary complications.
METHODS: We examined the feasibility and preliminary efficacy of RMT as a community-based exercise program. Among the 10 included participants (eight male and two female), four, one, one, and four reported C3, C4, C5, and C6 complete injuries, respectively (eight graded by American Spinal Injury Association impairment scale [ASIA] A and two by ASIA B). OBJECTIVE: Singleton pregnancy causes important changes in respiratory function. The incidence of twin pregnancies is increasing, but it is not known whether affected women suffer greater respiratory compromise. The aim of this study was to determine if changes in respiratory function during pregnancy in healthy women were greater in those with a twin pregnancy compared with those with a singleton pregnancy.
DESIGN: Cross sectional study.
SETTING: London teaching hospital.
POPULATION: Healthy pregnant women attending the antenatal clinic and ultrasound department.
METHODS: A cross sectiona

Highly trained aerobic athletes progressively use most of their breathing reserve with increased exercise intensity during whole-body exercise. Additionally, females typically present proportionally smaller lungs than males. Therefore, sex, exercise intensity, and breathing reserve use likely influence the volume and time in which respiratory parameters vary between consecutive breaths during whole-body exercise. However, breath-by-breath variability has been scarcely investigated during exercise. Accordingly, we sought to investigate breath-by-breath pulmonary ventilation (V̇E), tidal volume

Asthma and chronic obstructive pulmonary diseases (COPD) lead to functional obstruction of airways, identified by increased inspiratory and expiratory resistances. Increased expiratory resistances cause, in turn, a reduction in expiratory flow. The analysis of flow-volume loops shows that, as the disease progresses, the flow generated during expiration of a tidal volume becomes very close to the flow generated during forced maximal expiration. In such condition, where there is little or no reserve of expiratory flow, higher tidal volumes need to be reached in order to increase the expiratory f

INTRODUCTION & PURPOSE: Obesity-related increases of weight on the chest wall are known to decrease lung volume and chest wall compliance, but the effect of this mass loading, independent of other obesity-related complications on the ventilatory response to exercise is unknown. The purpose of this study was to investigate the effect of chest mass loading on the ventilatory response to exercise.
METHODS: External chest loading (CL) was used to simulate the effect of moderate obesity (BMI = 32 kg x m). Eight healthy nonobese subjects performed two incremental exercise tests on a cycle ergometer
