Problems in the treatment of injured persons, especially if lesions of larger organs or organ systems exist, are great. Each method of treatment has some deficiency respectively dark side. In such cases a combination of methods of treatment is a necessity and a rule. In combining various methods, special care must be taken to cover these deficiencies up resp. to improve the methods. Regarding the fact that the method of external fixation has in its indications for treatment numerous combinations, it is also more easily combined with other surgical methods. It proved successful in combinations

External fixation has a wide variety of orthopedic applications. Although external fixator frames may have a complex appearance, these constructs are formed from several basic components and can be broadly categorized into unilateral, circular, or hybrid designs. The introduction of computer-aided circular external fixation devices (hexapod frames) has simplified the treatment of multiaxial and especially rotational deformities. Serial radiography plays a central role in the evaluation of callus formation (at the level of treated fracture or nonunion as well as the regenerate and docking site

Hypertension is not a recognized complication of circular external fixation. The aim of this study was to determine the incidence of hypertension following this procedure. A retrospective review of 57 patients was carried out between January 2006 and May 2010. At least three blood pressure readings were used to define hypertension. Five of the 41 qualifying patients (12.2%) developed hypertension. Three of these were found to have a sustained hypertension throughout the frame period, two requiring treatment for visual symptoms and left ventricular hypertrophy. A limb reconstruction surgeon nee

Despite the growing use of hinged external fixators of the elbow, there are no studies regarding the complications associated with their application. The purpose of this study is to report our experience with complications with this procedure. Between 1998 and 2005, we reviewed the records of 100 consecutive patients who were treated with hinged external fixators (including 433 pin sites). Complications specifically related to pin placement were recorded. There were 15 patients with minor complications (15%) involving 21 pins (4.8%) and 10 patients with major complications (10%) involving 29 p

Toxic shock syndrome (TSS), first described by Todd et al. in 1978, can be a life-threatening entity. Familiarity with the pathogenesis and clinical presentation of TSS may help achieve early diagnosis and prompt appropriate intervention. TSS is not a septicemia, but a toxemia. The most extensively described pathogenesis involves a focus of specific Staphylococcus aureus strains capable of producing an exotoxin (TSST-1). We report two patients who developed TSS while in external fixators and describe their initial symptoms, management, and subsequent problems. This report will serve to alert p

Background: External fixation has been traditionally used to treat comminuted or open distal radius fractures that are not amenable to open reduction internal fixation. This procedure is associated with a relatively high complication rate and has been used with decreasing frequency in recent years. However, trends in external fixation utilization for the treatment of distal radius fractures have not been described. Methods: Using the Nationwide Inpatient Sample, patients with a distal radius fracture treated with external fixation from 2003 to 2014 were identified. The annual incidence was rep

Complex foot and ankle wounds present multiple challenges for the reconstructive surgeon. Soft tissue deficits must be closed to protect underlying structures from infection and to provide a stable environment for healing. Surgical options commonly used include healing by secondary intention, local flap closure, skin grafts, pedicled flaps, and free tissue transfer. Despite a surgeon's best operative efforts, these strategies may fail because of postoperative shear forces created by premature joint motion or pressure (either weight bearing or decubitus). In the properly selected patient popula

From 1991 to 1998, 23 humeral fractures has been treated using the Orthofix external fixator. Average age of the patient was 42 y and average follow-up 55.5 m. Initial trauma was: 13 traffic accidents, four falls at home, two devastating farming accidents, two sports accidents, one aggression and one gun accident. AO classification was used and location of the fracture was classified using Hackethal classification modified by de la Caffinière. Majority of fractures were located at the one-third distal humerus and the majority was also comminuted. At follow-up, elbow range of motion was 130 deg

OBJECTIVES: To quantify and evaluate risks of complications attributable to external skeletal fixator (ESF) usage in dogs.
METHODS: A retrospective review of medical records following ESF placement.
RESULTS: Case records of 97 dogs were reviewed; fixator-associated complications occurred in 79/97 dogs. Region of ESF placement was significantly associated with complication development (p = 0.005), not complication type (p = 0.086). Complications developed most frequently in the tarsus (9/10), manus (8/9) and humerus (8/9). Superficial pin-tract infection and implant failure occurred in 38/97

Iatrogenic vascular injuries from external fixation in orthopaedics and traumatology are frequent. Three cases of vascular injuries after the treatment with Ilizarov external fixators were treated at our institution. These include two cases of pseudoaneurysms and one case of acute ischaemia of the lower limb. Two patients became symptomatic only after removal of the fixator. In all cases, the diagnosis was made by color flow duplex sonography. All vascular injuries needed surgical repair.

The use of external fixators is part of the cultural background of orthopedic surgeons in treating numerous clinical conditions. Over the years, fixator design and biomechanical knowledge have led to different solutions and techniques, and bone lengthening and its better understanding come together with the development of external fixators and the application of the biological principle of distraction osteogenesis. The authors conducted a literature review about using external fixators and lengthening systems in pediatric upper limbs. Despite the applications of external fixators in upper limb

New properties are being modified in the external fixator instrumentation that are leading to the more indicated use of this type of fixation especially in children's lower limb fractures. Dynamization, the ability to lengthen the limb and easy wound care are among the new properties of the monoplane external fixator, along with greater stability than older models. These new properties are now making the external fixator an indication rather than a choice in many cases. We have retrospectively studied 21 children with lower limb fractures treated by 28 external fixators. The results of this st

We are describing 21 limb reconstruction procedures performed in 18 patients with the use of external fixators from 1996 to 1998. The average age of patients was 21, ranging from 1 to 50 years old. Indications for surgery included short limb, non-union, pseudoarthrosis and bone or soft tissue deformities. Average length obtained for cases of limb lengthening was 6 cm. All the seven clubfoot deformities in five children were fully corrected. Equinus deformity recurred in one foot and was treated with supramalleolar osteotomy. Out of the seven cases with infected nonunion and bone loss, three fa

PURPOSE: Carrying out osteosynthesis is challenging, and controlling for results and complications is necessary to define the limits of acceptable complications. Within the context of sudden-onset disasters, comparing internal with external osteosynthesis remains controversial.
METHODS: The most recent and significant Médecins Sans Frontières (MSF) experience with osteosynthesis was following the earthquake in Haiti in 2010: 353 external fixators were used in the 12 months following the catastrophe, 62 of which were used in the first month. Carrying out internal osteosynthesis was possible tw

PURPOSE: Nonbridging external fixation is becoming popular for distal radius fractures, although its biomechanical characteristics have not been documented. This study evaluated the biomechanical characteristics of nonbridging external fixators for distal radius fractures.
METHODS: We tested 3 currently available nonbridging fixators (F-Wrist fixator, Hoffman II Compact, and Pennig Dynamic Wrist Fixator) and determined their relative stiffness under axial compression, torsion, and bending moments (dorsal, volar, radial, and ulnar aspects) using a uniform unstable distal radius fracture model.

BACKGROUND: We aimed to evaluate the results of the closed manipulation or transcutaneous reduction and external fixation in the treatment of two-part fractures.
METHODS: Eight patients with two-part proximal humerus fractures who had been treated in our clinic with closed manipulation or transcutaneous reduction and external fixation between 1996-2001 were evaluated for this study. We evaluated the final functional status according to the Neer's classification system.
RESULTS: Mean age of the patients was 42 (21-75) years. The major aetiologic factor was motor vehicle accident (in 5 cases).

BACKGROUND: Extramedullary fixation of hip fractures involves the application of a plate and screws to the lateral side of the proximal femur. In external fixators, the stabilising component is held outside the thigh by pins or screws driven into the bone on either side of the fracture.
OBJECTIVES: To compare different types of extramedullary fixation implants and external fixators for treating extracapsular hip fracture in adults.
SEARCH STRATEGY: We searched the Cochrane Musculoskeletal Injuries Group specialised register and reference lists of relevant articles up to July 2002.
