Histopathological Definitions of Extranodal Extension: A Systematic Review.

Extranodal extension (ENE) is a very strong prognostic factor in head and neck squamous cell carcinoma. However, significant variance in reported incidence of ENE suggests discordance in perception of ENE among pathologists. This study aims to map the different definitions of histopathological ENE used in the literature. A systematic review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Statement. Guided by the research question: "How is histopathological ENE defined?" the databases Medline, Embase, and Cochrane were systematically searched. A

Significance of Preoperative Imaging-Detected Extranodal Extension in Pathological Extranodal Extension-Positive Head and Neck Squamous Cell Carcinoma Treated With Postoperative Chemoradiotherapy.

BACKGROUND: The Head and Neck Cancer International Group (HNCIG) recently proposed standardized classifications for imaging-detected extranodal extension (iENE) and pathological ENE (pENE). We evaluated the prognostic value of iENE in pENE-positive head and neck squamous cell carcinoma (HNSCC) treated with postoperative chemoradiotherapy (CRT). METHODS: Patients with pENE-positive HNSCC who underwent postoperative CRT were retrospectively analyzed. iENE and pENE were re-classified using HNCIG criteria. Recurrence-free survival (RFS) and overall survival (OS) were analyzed using Kaplan-Meier a

Radiologic extranodal extension for nodal staging in nasopharyngeal carcinoma.

PURPOSE: Extranodal extension (ENE) has the potential to add value to the current nodal staging system (N8th) for predicting outcome in nasopharyngeal carcinoma (NPC). This study aimed to incorporate ENE, as well as cervical nodal necrosis (CNN) to the current stage N3 and evaluated their impact on outcome prediction. The findings were validated on an external cohort. METHODS & MATERIALS: Pre-treatment MRI of 750 patients from the internal cohort were retrospectively reviewed. Predictive values of six modified nodal staging systems that incorporated four patterns of ENE and two patterns of CN

The Effect of Extranodal Extension on Survival in Laryngeal Carcinoma.

It is known that lymph node metastasis lowers the survival rates in laryngeal carcinoma. This study aimed to investigate the effect of extranodal extension in lymph node metastasis on survival. The demographic characteristics and histopathologic results of 81 patients who underwent total laryngectomy and neck dissection due to advanced larynx squamous cell carcinoma between 2011 and 2018 were reviewed retrospectively. The patients were divided into 3 groups by lymph node metastasis status as reactive lymph node (group 1), lymph node metastasis without detected extranodal extension (group 2), a

Intrathyroidal tumors presenting with extranodal extension: what are we missing?

Extranodal extension (ENE) is an indicator of poor prognosis in well-differentiated thyroid cancer (WDTC). We have demonstrated that extrathyroidal extension (ETE) predicts ENE in patients with positive lymph nodes, indicating concordance between primary tumor and lymph node biology. In an effort to determine if there were other histologic features of the primary tumors that indicated an aggressive biology, we examined a subset of patients with intrathyroidal (T1/T2) disease whose lymph nodes had ENE. A review was conducted from January 2004 to March 2013. The histologic features of ETE-negati

Axillary Extranodal Extension in Breast Cancer: Imaging Features With Histopathology Correlation.

Extranodal extension (ENE) is defined as the spread of tumor cells beyond the lymph node capsule. It is an independent prognostic factor in breast cancer associated with increased nodal disease burden, influencing subsequent treatment strategies. Extranodal extension has been reported in approximately 20% to 50% of patients with metastases to axillary lymph nodes. Despite its clinical relevance, ENE remains challenging to detect. Key imaging features indicative of ENE include irregular nodal contours, nodal matting, and perinodal edema on US and MRI. However, these findings are often not detec

Ultrasound findings suggestive of microscopic extranodal extension in papillary thyroid carcinoma.

PURPOSE: Extranodal extension (ENE) of metastatic carcinoma in patients with papillary thyroid carcinoma (PTC) has been associated with an increased risk of recurrent disease, persistent disease, and disease-specific mortality; however, ultrasound findings suggestive of ENE have not been well established. In this study, we aimed to identify ultrasound findings suggestive of microscopic ENE and validate them histologically. METHODS: We retrospectively examined the ultrasound and histological findings of 21 PTC patients with microscopic ENE and 46 without ENE. RESULTS: Node matting, irregular

Extranodal extension in oral squamous cell carcinoma: clinical and histopathological analysis.

OBJECTIVE: Extranodal extension (ENE) is an important prognostic factor in oral squamous cell carcinoma (OSCC). This study aimed to evaluate the clinical significance of stratifying minor or major ENE in OSCC. STUDY DESIGN: This retrospective cohort study included 75 patients who had undergone neck dissection for OSCC and were classified as pN+. ENE was measured using hematoxylin-eosin-stained specimens and stratified into major (ENEma, >2 mm) and minor (ENEmi, ≤2 mm) by distance. Their association with survival, locoregional relapse, and distant metastases were assessed. RESULTS: Of 49 pati

Survival in Patients With Sentinel Node-Positive Melanoma With Extranodal Extension.

BACKGROUND: Prognostic parameters in sentinel node (SN)-positive melanoma are important indicators to identify patients at high risk of recurrence who should be candidates for adjuvant therapy. We aimed to evaluate the presence of melanoma cells beyond the SN capsule-extranodal extension (ENE)-as a prognostic factor in patients with positive SNs. METHODS: Data from 1,047 patients with melanoma and positive SNs treated from 2001 to 2020 at the Istituto Nazionale dei Tumori in Milano, Italy, were retrospectively investigated. Revisiting the prognostic role of extranodal extension in oral cavity squamous cell carcinoma: minor versus major extension matters.

Extranodal extension (ENE) is a well-known adverse feature in head and neck cancer, with a substantial effect on prognosis. However, the extent of ENE (minor vs major), is not a part of the decision-making process for the treatment of head and neck cancers. The aim of this study was to investigate its effect on prognosis in oral cavity squamous cell carcinoma (OCSCC). A retrospective analysis was conducted of adult patients treated surgically for OCSCC at a a university-affiliated tertiary care centre between the years 2002 and 2022, who had ENE on final pathology. Major ENE beyond the lymph n

Clinicopathological Significance of Extranodal Extension in Hypopharyngeal and Laryngeal Squamous Cell Carcinoma.

BACKGROUND: The AJCC cutoff value of 2 mm for the extranodal extension (ENE) distance was determined from an analysis of patients with or without adjuvant therapy. The purpose of this study was to find out the ENE distance that reflects prognosis only in patients with head and neck squamous cell carcinoma (SCC) who received adjuvant therapy. METHODS: The ENE distance was defined for 109 patients who underwent surgery for SCC of larynx or hypopharynx as a primary tumor. RESULTS: To standardize patient conditions, only 26 patients who received additional postoperative treatment were analyzed.

Prognostic value of extranodal extension in axillary lymph node-positive breast cancer.

Several studies have demonstrated that extranodal extension (ENE) is associated with prognosis in breast cancer. Whether this association should be described in pathological reports warrants further investigation. In this research, we evaluated the predictive value of ENE in axillary lymph nodes (ALNs) in invasive breast cancer and explored the feasibility of employing ENE to predict clinicopathological features, nodal burden, disease recurrence-free survival (DRFS) and overall survival (OS) in clinical practice. In addition, the cutoff values of perpendicular diameter ENE (PD-ENE) and circumf
