The effects of extreme cold on sensory nerves.

The effects of extreme cold on sensory nerves are discussed and a clinical application of these effects is proposed. The structural changes observed following the freezing of sensory nerves in the rat are described and correlated with the clinical results in patients with chronic facial pain treated by cryogenic peripheral nerve blockade. It is suggested that this technique offers features which are not shown by any other method for interrupting peripheral pain pathways and provides a useful alternative to existing methods of treatment for chronic pain.

Electricity load forecasting under extreme heat and cold waves.

Extreme heat and cold waves can push electricity demand beyond the conditions for which power systems are routinely planned, increasing the risk of shortages and blackouts. Accurate load forecasts are therefore critical because they give operators the lead time needed to keep supply and demand in balance. Because such events are rare, forecasting models are trained mostly on ordinary weather conditions and often fail when demand patterns shift during extremes. Here we show that Extreme Synthesis-Disentanglement Forecasting, a data-centered approach that generates realistic examples of extreme

Are cold extremities an issue in women's health? Epidemiological evaluation of cold extremities among Japanese women.

BACKGROUND: Unlike traditional East Asian medicine, the necessity of health care services for cold extremities is yet to be acknowledged in Western medicine. In this study, we aimed to conduct an epidemiological evaluation of this unremarkable symptom among women in Japan. MATERIALS AND METHODS: A cross-sectional study was conducted from February 2016 to April 2017, and data of 238 women throughout Japan were analyzed. Questionnaires were used to examine participants' demographics, health-related behaviors, health status, and frequency of subjective symptoms over the past 1 year. The associat

Challenges and limitations for live cell imaging in extreme cold.

Many ecosystems thrive in near-0 °C conditions, and the mechanisms supporting life in these conditions remain understudied due to the challenges in reproducing such environments in laboratory conditions. One such example is polar organisms, that have adapted their entire lifecycle to operate below freezing temperatures through largely unknown cellular adaptations. As rapid polar warming threatens these species, elucidating their survival strategies is increasingly urgent. Fluorescence-based optical microscopy has been central to the understanding of the dynamic processes sustaining life at the

The modified crABCDE treatment algorithm as recommendation in extreme cold.

Trauma care prioritizes life-threatening conditions using the ABCDE algorithm based on the principle "treat first what kills first". As for catastrophic hemorrhage, a leading preventable cause of death in trauma, modifications of this algorithm are necessary in specific cases. In cold climates, life-threatening hypothermia poses additional challenges. Rapid cooling of a patient's core temperature, especially when immobile or poorly insulated, necessitates early prevention. Modified algorithms like the military MhARCH therefore prioritize hypothermia management alongside hemorrhage control in e

Out in the cold: Testing a multi-agency approach to working in extreme cold emergency situations.

Climate change has amplified the frequency and intensity of extreme weather events, including snow, floods, and heatwaves, which pose significant challenges to emergency service provision. While existing literature on the impacts of extreme weather on emergency services is limited, the United Kingdom (UK), with its varying weather patterns, faces considerable risks to infrastructure, health, and service delivery during these events, which are no longer extremely rare occurrences. This study investigates the effectiveness of multiagency, immersive simulation training in preparing emergency resp

Significance of cold intolerance in upper extremity disorders.

After upper extremity injury, pain on exposure to cold (cold sensitivity) is a significant problem. This cross-sectional observational study (1) assesses the incidence and prevalence of cold intolerance, (2) evaluates the relationship between functional status and degree of cold intolerance, and (3) correlates health-related quality of life (HRQL) with symptoms of cold intolerance. Patients in a tertiary care center completed questionnaires to document (1) cold sensitivity, (2) upper extremity pain, symptoms, and function, and (3) HRQL. Cold sensitivity was found to be associated with more fun

Task Analysis Tool to Evaluate Tactical Combat Casualty Care in the Extreme Cold.

Due to the evolving geopolitical strategy of near-peer nations, there has been a growing national interest in the Arctic region. However, Tactical Combat Casualty Care (TCCC) assessments and interventions have been shaped based on input from conflicts such as Iraq and Afghanistan. They do not consider the unique factors seen in extreme cold environments. Naval Medical Research Unit San Antonio has developed a task analysis tool, the Task Step Analysis Tool-Binary (TSAT-B), to evaluate the effects of extreme cold on tasks within the current TCCC protocols. The tool was pilot-tested using a trau

Is there a role for sarcolipin in avian facultative thermogenesis in extreme cold?

Endotherms defend their body temperature in the cold by employing shivering (ST) and/or non-shivering thermogenesis (NST). Although NST is well documented in mammals, its importance to avian heat generation is unclear. Recent work points to a prominent role for the sarco/endoplasmic reticulum Ca2+ ATPase (SERCA) in muscular NST. SERCA's involvement in both ST and NST, however, posits a tradeoff between these two heat-generating mechanisms. To explore this tradeoff, we assayed pectoralis gene expression of adult songbirds exposed to chronic temperature acclimations. Counter to mammal models, we

Thyroid hormone economy in response to extreme cold exposure in healthy factory workers.

The effects of cold exposure on serum total T4 (TT4), total T3 (TT3), free T4 (FT4), free T3 (FT3), rT3, TSH, T4-binding globulin (TBG), and T3 resin uptake were investigated in 82 euthyroid factory workers. Twenty-five workers (group 1) were exposed intermittently (approximately 3.5 h daily) to extreme cold (-40 to -20 C) during the 8-h work shift, and 47 (group 2) were exposed to moderate cold (-10 to 8 C) for the entire 8 h. Ten individuals working at room temperature for the same period also were studied. After cold exposure, serum TT4 decreased in group 1 and did not change in group 2, wh

A 3-D whole-body human thermoregulatory model to simulate extreme cold stress.

Prolonged exposure to Arctic-like conditions can lead to adverse physiological outcomes, significantly affecting the health, performance, and operational readiness of civilians and military personnel. In this study, we extended our previously developed, anatomically realistic, three-dimensional thermoregulatory virtual human model to more comprehensively predict changes in core body temperature and skin temperature of peripheral body regions (dorsum hand, fingertip, face, dorsum foot, and toe-tip) and the cheek under extreme cold-air temperatures down to -40.0 °C and wind speeds ranging from 0

Physiological responses of the human extremities to cold water immersion.

Five subjects immersed their hands and feet twice during 1 hour sessions in a calorimeter bath, filled with water at 25 degrees C. Before immersion the hand (33.0 +/- 2.2 degrees C) was warmer than the foot (30.6 +/- 2.1 degrees C) and so was the blood flow: respectively 5.8 +/- 5.2 versus 0.7 +/- 0.3 ml.min-1.100 mo tissue-1 as determined by strain gauge plethysmography and 10.8 +/- 5.1 versus 6.7 +/- 5.3 by the doppler method. At the end of the immersion period the blood flow had decreased a factor of 2.9 (doppler) and 4.3 (plethysmography). The local heat transfer, determined by heat flux t

Stability of sufentanil sublingual microtablets (Dzuveo) in extreme cold weather conditions: an experimental study.

INTRODUCTION: Extreme cold weather environments are challenging for medication storage and delivery, particularly for injectable analgesics vulnerable to freezing and device failure. These risks may compromise pain management in austere or operational settings where temperature control cannot be maintained. Sufentanil sublingual microtablets (sold in the UK as Dzuveo) provide an oral analgesic option with manufacturer-reported sub-zero stability. Evidence describing the effects of sustained extreme cold exposure and repeated freeze-thaw cycling on chemical stability and delivery system functio

Imaging and Interventional Radiology Capabilities in Extreme Cold Environments: Operational Implications for Military Medicine.

INTRODUCTION: Extreme cold environments impose unique constraints on the delivery of image-guided and interventional care due to hypothermia-associated coagulopathy, altered drug metabolism, impaired operator performance, and temperature-sensitive imaging and device systems. These challenges are particularly relevant to military operations in polar and austere settings, where evacuation may be delayed, and medical capability must be delivered with limited infrastructure. MATERIALS AND METHODS: This work presents an operationally focused review of the physiologic effects of cold exposure, imag

Prolonged Extreme Cold Water Diving and the Acute Stress Response During Military Dive Training.

Introduction: Cold water exposure poses a unique physiological challenge to the human body. Normally, water submersion increases activation of parasympathetic tone to induce bradycardia in order to compensate for hemodynamic shifts and reduce oxygen consumption by peripheral tissues. However, elevated stress, such as that which may occur due to prolonged cold exposure, may shift the sympatho-vagal balance towards sympathetic activation which may potentially negate the dive reflex and impact thermoregulation. Objective: To quantify the acute stress response during prolonged extreme cold water d

Cold-Related Illness In An Era Of Extreme Climate Events: US Trends, 1998-2022.

Cold-related illnesses (CRIs) are preventable yet often deadly. Using twenty-five years of data from the National Inpatient Sample (1998-2022), we assessed nationwide trends in CRI hospitalizations and concomitant alcohol use, substance use, and mental health disorders and housing insecurity. We identified 345,314 (weighted) CRI hospitalizations and found that age- and sex-adjusted rates tripled from 42.0 to 122.5 per 100,000 hospitalizations. CRI inpatients were more likely than others to die during hospitalization, live in high-poverty ZIP codes, be publicly insured or uninsured, and have be

Combat Casualty Care in Arctic or Extreme Cold Environments: Considerations for the Army Health System.

The Department of the Army's 2021 strategy to "Regain Arctic Dominance" highlights the operational necessity of sustaining combat readiness in Arctic or extreme cold environments (AoECE). Presented are five scenarios inspired by casualties that illustrate capability gaps and best practices as the Army Health System applies tactical combat casualty care (TCCC) in austere, frigid climates: (1) adapting TCCC to the AoECE as a unique environment; (2) technological limitations in the prevention and treatment of hypothermia; (3) freezing cold injury (FCI) and non-freezing cold injury (NFCI) treatmen
