Facial paralysis: traumatic neuromas vs. facial nerve neoplasms.

Traumatic neuromas (TN) are benign proliferations of neural tissue that may occur without disruption of the facial nerve. The clinical presentation, as well as the radiographic appearance, may suggest neoplastic involvement of the facial nerve. Histologically, they may closely resemble neurilemomas (Schwannomas) or neurofibromas. Three cases of TN of the facial nerve associated with facial paralysis are presented here. Unlike previously reported cases, these tumors were not associated with chronic inflammatory middle ear disease. TN must be considered in the differential diagnosis and treatmen

[Radiotherapy of neoplasms of the facial skin].

Radiotherapy--predominantly with soft x-rays--is, besides plastic surgery and cryosurgery, suitable for the treatment of facial malignancies for which operation would be undesirable in consideration of tumour size and the patient's general health, age and internal diseases. Operation should be preferred for smaller tumours that can be operated on with primary wound closure; tumours that grow into the mouth angle or into the cartilaginous folds of the ear; for patients under 50 years of age; for facial malignancies that have already caused a defect; for patients with nevoid basal cell epithelio

Black and Brown Oro-facial Mucocutaneous Neoplasms.

Black and brown-colored mucocutaneous lesions present a differential diagnostic challenge, with malignant melanoma being the primary clinical concern. The vast majority of pigmented lesions in the head and neck region are the result of benign, reactive factors such as post-inflammatory melanosis. However, it is not uncommon to discover a range of muco-cutaneous black and brown neoplasms in the oro-facial area. The majority of black/brown pigmented neoplasms are melanocytic in origin; these are neoplasms of neural crest derivation. Melanocytic nevi are a diverse group of benign neoplasms that a

Facial paralysis caused by malignant skull base neoplasms.

OBJECT: Bell palsy remains the most common cause of facial paralysis. Unfortunately, this term is often erroneously applied to all cases of facial paralysis. METHODS: The authors performed a retrospective review of data obtained in 11 patients who were treated at a university-based referral practice between July 1988 and September 2001 and who presented with acute facial nerve paralysis mimicking Bell palsy. All patients were subsequently found to harbor an occult skull base neoplasm. A delay in diagnosis was demonstrated in all cases. Seven patients died of their disease, and four patients a

Facial nerve paralysis secondary to occult malignant neoplasms.

OBJECTIVE: This study reviewed patients with unilateral facial paralysis and normal clinical and imaging findings who underwent diagnostic facial nerve exploration. Study design and setting Fifteen patients with facial paralysis and normal findings were seen in the Mayo Clinic Department of Otorhinolaryngology. RESULTS: Eleven patients were misdiagnosed as having Bell palsy or idiopathic paralysis. Progressive facial paralysis with sequential involvement of adjacent facial nerve branches occurred in all 15 patients. Seven patients had a history of regional skin squamous cell carcinoma, 13 pat

Parotid neoplasms: diagnosis, treatment, and intraparotid facial nerve anatomy.

The demographics of parotid neoplasms in different populations have been reported by various centres. In this investigation, we reviewed retrospectively all the in-patient and out-patient charts and records of 108 patients who were diagnosed with parotid neoplasms and received parotidectomies in our department from 1 January 1993 to 15 April 2000. Patient age, gender, tumour pathology, fine-needle aspiration cytology results, and the intraparotid anatomy of the facial nerve were noted. We showed that despite the difference between our Taiwanese and previously studied patient populations, both

Intraparotid Facial Nerve Schwannoma Mimicking Primary Parotid Neoplasm.

The following report is of a 36-year-old male who presented with a growing mass in the region of the parotid gland. Initial fine needle aspiration biopsy suggested a primary neoplasm of the parotid gland, but subsequent frozen section analysis intraoperatively demonstrated a schwannoma of the facial nerve. An intracapsular enucleation of the schwannoma was performed in order to preserve the fibers of the motor nerve.

BACKGROUND: Facial nerve dysfunction can be a devastating trouble for post-parotidectomy patients. OBJECTIVE: To assess rehabilitation outcomes concerning patients with post-parotidectomy facial nerve dysfunction, comparing benign versus malignant neoplasms. METHODS: Prospective study enrolling adults who underwent parotidectomy with facial nerve sparing between 2016 and 2020. The Modified Sunnybrook System (mS-FGS) was used for facial assessments. Physiotherapy began on the first post-operative day with a tailored program of facial exercises based on Neuromuscular Retraining, to be performe

Trans-mastoid facial nerve localisation for malignant neoplasms confined to the parotid gland.

Up to 32% of parotid neoplasms are malignant, and treatment for resectable parotid carcinomas remains primarily surgical. Resection is centred round the identification and preservation (where possible) of the main trunk of the facial nerve. Limited dissection of the temporal bone and identification of the facial nerve proximally in the mastoid portion can facilitate resection for all parotid carcinomas that occur in the preauricular portion of the gland. Dissection in the narrow space between the ear or base of the skull and the tumour reduces the risk of tumour spillage, and the margin for re

Reflectance confocal microscopy of facial neoplasms: Follicular involvement as a clue to diagnosis.

BACKGROUND: Facial skin is characterized by high density of follicles. Facial neoplasms may present overlapping clinical and dermoscopic findings. Our goal was to evaluate and compare, via reflectance confocal microscopy (RCM), follicular involvement in facial neoplasms. METHODS: We retrospectively searched our image database, between January 2008 and December 2020, for all facial lesions with (1) a standardized set of clinical, dermoscopic, and RCM images, and (2) a biopsy-proven diagnosis of lentigo maligna/lentigo maligna melanoma (LM/LMM, n = 39), basal cell carcinoma (BCC, n = 51), squam

Temporomandibular joint dysfunction and facial pain caused by neoplasms. Report of three cases.

The most frequent causes of painful limitation of temporomandibular joint function are myofascial pain dysfunction or anterior disk displacement. A potential problem in dealing with those patients is the risk of misdiagnosing the rare patient who has a neoplastic disease, primary or metastatic to the epipharyngeal region, parotid gland, jaws, or temporomandibular joint. Three cases are presented in which progressively worsening limitation of mandibular movement and increasing pain led to the diagnosis of a neoplasm in the temporomandibular joint region. The article illustrates the necessity of

Outcomes of immediate facial nerve reanimation with nerve transfer for facial nerve neoplasm-induced paralysis: a retrospective review.

BACKGROUND: The purpose of this study was to evaluate the outcomes of our polyneural, zone-based reanimation approach for patients with neoplasm-induced facial paralysis. METHODS: A retrospective review of consecutive patients who underwent facial reanimation surgery using multiple donor nerve transfers was undertaken. In each case, the selection of donor nerves was based on the availability of donor nerve and the viability of the motor endplate on the affected side. Sources of the neural inputs utilized included the remnant facial nerve stump, masseteric nerve, partial hypoglossal nerve, and

Reliability of the 'M-Line' in the Prediction of the Facial Nerve Position in Patients with Parotid Neoplasms.

Traumatic shock is a complex phenomenon that represents the culminating element of a series of events. It is, in fact, the outcome of an imbalance-decompensation of the organism's defence mechanisms, in which the oxygen supply to the mitochondria is hampered by a macro and/or microcirculation failure. Basically, it is a form of hypovolemic shock in which further factors have a role, including the activation of inflammation mediators. It should also be stressed that part of the cellular damage is caused by tissue reperfusion. Good hemodynamic compensation is maintained with loss of up to 30% of

The facial nerve in the presence of a head and neck neoplasm: assessment and outcome after surgical management.

PURPOSE OF REVIEW: The face is the mirror of personality. Facial expression is the most important part of verbal and nonverbal communication. Patients with head and neck neoplasm and facial palsy are more stigmatized by the latter than by the tumor itself. Facial nerve reconstruction in such a patient is a great challenge. This review gives an overview of the assessment of facial palsy, surgical reconstruction, and postoperative treatment. RECENT FINDINGS: MRI, CT, and electromyography are indispensable tools in the assessment of preoperative facial palsy in patients with head and neck neopla

Functional outcomes after extracapsular dissection with partial facial nerve dissection for small and large parotid neoplasms.

PURPOSE: Multiple surgical options exist for benign parotid tumors without agreement upon a single, best approach. We evaluated the short-term outcomes and rate of complications using the ECD-FND technique for small and large parotid neoplasms involving the superficial and deep lobes of the parotid gland using a categorical approach. MATERIAL AND METHODS: A single surgeon retrospective cohort study with analysis of patient demographics, outcomes, and complication rates was conducted of patients undergoing the ECD-FND for benign parotid neoplasm. Cases from May 2014 to May 2020 with at least 6
