Historically, quality improvement efforts have focused primarily on preventing complications from occurring. In contrast, failure to rescue is an emerging quality metric that follows patients after a complication occurs and describes a system's ability to prevent subsequent mortality and rescue them from death. Future efforts to implement the rescue framework in thoracic surgery should consider selective care consolidation, standardized pathways for detection of clinical deterioration and subsequent escalation of care, distribution of best practices, and evaluation of patient factors that cont

Failure to rescue (FTR), defined as mortality after a postoperative complication, is now a central quality metric in cardiac surgery, where high-acuity patients and resource-intensive care increase vulnerability to deterioration. Although complications are common, survival depends on timely recognition, effective escalation, and coordinated multidisciplinary management. This article synthesizes current evidence on the epidemiology, mechanisms, and system-level drivers of FTR, highlighting how patient, provider, and institutional factors influence rescue success. We outline practical, evidence-

BACKGROUND: The role of failure to rescue (FTR) in solid organ transplantation (SOT) is less clear. This scoping review aimed to evaluate the incidence, complications, and risk factors for FTR in SOT.
METHODS: We systematically searched PubMed, Scopus, and Web of Science for studies reporting on FTR in SOT. Data on incidence, complications, risk factors, and definitions were extracted and synthesized narratively due to heterogeneity across studies.
RESULTS: Seven studies were identified: four in liver transplantation, two in lung transplantation, and one in heart transplantation; none addres

BACKGROUND: Although "Failure to Rescue" (FTR) has been widely studied in general healthcare contexts, and a few clinical specialties, its definition and implications remain underexplored within maternal health, particularly given the heightened risks for marginalized women. The quality measure was retired as a national quality metric before formal adoption into obstetric care, leaving significant gaps in maternal patient safety. High rates of preventable maternal morbidity and mortality, highlight the urgent need to explore and define this concept specifically within maternal health.

BACKGROUND: The clinical response to postoperative complications after lung transplantation (LTx) may contribute to mortality differences among transplantation centers. The ability to avoid mortality after a complication-failure to rescue (FTR)-may be an effective quality metric in LTx.
METHODS: The United Network for Organ Sharing database was queried for adult, first-time, lung-only transplantations from May 2005 to December 2015. Transplantation centers were stratified into equal-sized terciles on the basis of observed operative mortality rates. Several postoperative complications were ide

BACKGROUND: The mortality rate of surgical patients in Africa is twice the global average. Pan-African studies have identified that this excess mortality is driven by deaths arising from postoperative complications. Preventable deaths can be reduced with enhanced monitoring in the postoperative period. This study reports on the first of a two-phase Quality Improvement (QI) programme, '5 Rs to Rescue', designed to reduce failure to rescue for surgical patients in Africa.
METHODS: Using a QI approach, we co-produced a failure to rescue intervention with stakeholders across four African countrie

BACKGROUND: Failure to rescue (FTR), defined as death following major complications, has become an important quality metric. While liver transplantation (LT) carries high risks for postoperative complications, the relevance and determinants of FTR in LT remain poorly characterized. This systematic review aimed to identify the incidence and risk factors for FTR in adult liver transplant recipients.
METHODS: Following PRISMA guidelines, a systematic literature search was performed across four databases: MEDLINE Ovid, Web of Science, Cochrane CENTRAL, and Scopus. Studies reporting FTR rates and

AIM: To determine the current status of surgical rescue (SR) and examine the predictors of failure to rescue (FTR). In acute care surgery, predictors of FTR have only been reported in patients with trauma, not in SR. METHODS: This retrospective study included 142 patients who underwent emergency surgery at our institution between April 2019 and March 2023. The primary outcome was the identification of the FTR predictors using logistic regression analysis. The secondary outcome was the development of a Surgical Rescue Severity Score (SRSS) using these SR predictors and long-term outcomes. RESUL

INTRODUCTION: The differentiators of centers performing at the highest level of quality and patient safety are likely both structural and cultural. We aimed to combine five indicators representing established domains of trauma quality and to identify and describe the structural characteristics of consistently performing centers.
METHODS: Using American College of Surgeons Trauma Quality Improvement Program data from 2017 to 2020, we evaluated five quality measures across several care domains for adult patients in levels I and II trauma centers: (1) time to operating room for patients with abd

IMPORTANCE: Failure to rescue (FTR)-death following a potentially manageable complication-has become a key quality indicator. Although clinical and systemic contributors to FTR are well described, how surgeons make sense of these events and how this sense-making shapes communication, escalation, and learning remains underexplored.
OBJECTIVE: To explore surgeon interpretations of FTR events and perceptions of factors that contribute to FTR and to identify insights to inform improvements in surgical education and culture.

Although it is widely acknowledged that health care delivery systems are complex adaptive systems, there are gaps in understanding the application of systems engineering approaches to systems analysis and redesign in the health care domain. Commonly employed methods, such as statistical analysis of risk factors and outcomes, are simply not adequate to robustly characterize all system requirements and facilitate reliable design of complex care delivery systems. This is especially apparent in institutional-level systems, such as patient safety programs that must mitigate the risk of infections a

INTRODUCTION: Intraoperative massive hemorrhage (iMH) is a rare but life-threatening surgical event associated with high mortality. Although failure-to-rescue (FTR) is widely used to evaluate surgical quality, FTR during iMH has not been well-characterized. This study aimed to describe clinical patterns of iMH and identify factors associated with FTR.
METHODS: We analyzed that iMH cases reported to Japan's nationwide incident reporting system from 2010 to 2023. The inclusion criteria were cases in which bleeding during noncardiac surgery required a change in surgical approach or was associate

INTRODUCTION: Understanding the relationship between patient risk factors, postoperative complications, and morbidity and mortality is important when considering elective endovascular aortic aneurysm repair (E-EVAR) performed to prevent aneurysm rupture mortality. We aimed to stratify complications in E-EVAR and explore their relationship with postoperative death.
METHODS: E-EVAR cases from 2012 NSQIP were identified. 30-day complications were categorized as major (MAJCX) or minor (MINCX) using the Clavien-Dindo classification. Failure to rescue (FTR) was defined as death following a complica

BACKGROUND: Pulmonary embolism and deep vein thrombosis are common clinical entities, and the related malpractice suits affect all medical subspecialties. Claims from malpractice litigation were analyzed to understand the demographics of these lawsuits and the common reasons for pursuing litigation.
METHODS: Cases entered into the Westlaw database from March 5, 1987, to May 31, 2018, were reviewed. Search terms included "pulmonary embolism" and "deep vein thrombosis."
RESULTS: A total of 277 cases were identified. The most frequently identified defendant was an internist (including family pr

BACKGROUND: Failure to rescue is an established surgical quality metric. The Centers for Medicare and Medicaid Services will replace Patient Safety Indicator-04 with a new failure-to-rescue measure in 2027. However, no standardized failure-to-rescue definition exists for emergency general surgery, and evidence supporting complication inclusion is lacking. We sought to examine whether failure-to-rescue definitions vary across emergency general surgery studies, how these definitions align with national frameworks, and characterize complication-mortality associations.
METHODS: A scoping review w

BACKGROUND AND OBJECTIVES: Failure to rescue (FTR) occurs when mortality happens within 30 days after a major postoperative complication. The surgical literature contains varying FTR rates based on various patient characteristics and postoperative complication rates. The impact of frailty on FTR has yet to be described within the context of brain tumor resection (BTR). Our study identified patient characteristics and postoperative complications predictive of FTR in this cohort.
METHODS: BTR patients were identified from the American College of Surgeons-National Surgical Quality Improvement Pr

The concept of failure to rescue has been used to measure the quality of care for complications developed following surgery. The concept of failure to rescue has been poorly studied in patients with primary medical diseases, such as sepsis or acute liver failure (ALF). We performed a retrospective multicenter cohort study including consecutive patients with ALF within the United States ALF Study Group (USALFSG) prospective registry from 2010-2016. The failure to rescue rate for 12 medical complications in the registry was calculated as the mortality events up to 21 days after inclusion divided
