BACKGROUND: Cardiothoracic surgery (CTS) training may coincide with family needs including pregnancy, illness, birth/adoption/fostering of a child, and care for family members. Certifying bodies set requirements and limits on family leave, complicating the navigation of family needs during training. Our goal is to examine policies of medical boards and their alignment with the 2022 Accreditation Council for Graduate Medical Education policy and to develop a practical tool to help trainees and program directors navigate family leave.
METHODS: A literature review was conducted with PubMed of pu

BACKGROUND: A growing number of residents are having babies during residency training. While many businesses are working to improve maternity conditions and benefits for their employees, residency programs are often not prepared to accommodate pregnant residents. This study was conducted to examine the maternity leave experiences of women who delivered infants during their family practice residency training.
METHODS: Program directors from each of the 394 family practice residency programs listed in the 1993 Directory of Family Practice Residency Programs were asked to distribute surveys to f

BACKGROUND: Growing research points to economic policies as protective mechanisms for vulnerable families. Research on pediatric abusive head trauma suggests that paid family leave (PFL) may protect infants in the general population from physical abuse.
OBJECTIVE: To examine the association of state-level paid family leave policies with infant (ages 0-1) maltreatment rates.
PARTICIPANTS AND SETTING: A state-level panel dataset was constructed from the National Child Abuse and Neglect Data System (2002-2019) data on infant maltreatment investigations among four states with PFL (California, Ne

We evaluate changes in elementary school children health outcomes following the introduction of California’s Paid Family Leave (PFL) program, which provided parents with paid time off following the birth of a child. Our health outcomes--overweight, ADHD, and hearing-related problems--are characterized by diagnosis rates that only pick up during early elementary school. Moreover, our health outcomes have been found to be negatively linked with many potential implications of extended maternity leave--increased breastfeeding, prompt medical checkups at infancy, reduced prenatal stress, and reduce

The recently enacted Family and Medical Leave Act requires larger employers to provide eligible employees with up to twelve weeks of unpaid leave for certain medical and family-related reasons. This article addresses who the law applies to; the circumstances when leave must be granted; how the leave period is supposed to be scheduled; various notice, scheduling, medical certification, and reporting requirements; reinstatement and continuation-of-benefits requirements; and other key provisions of the Act. The basic requirements of the Act are not complicated, but certain issues that are not add

The COVID-19 pandemic has laid bare a problem that many people have managed behind the scenes for years: how to balance work and family caregiving responsibilities. For physicians, many of whom were already experiencing burnout prior to the pandemic, the extra burden of COVID-19-related work stress combined with fewer options for childcare and other support has made coping all but untenable. In early 2022, the Accreditation Council for Graduate Medical Education (ACGME) promulgated new paid family and medical leave policy for residents and fellows. This editorial considers the importance of th

Breastfeeding is increasingly recognized as a powerful biologic intervention associated with positive infant and maternal health outcomes. Emerging evidence continues to expand our understanding of the mechanisms through which human milk promotes health, including via immune system development and modulation of antimicrobial resistance. Yet despite the robust scientific evidence supporting breastfeeding as critical to public health, many families in the US face structural barriers to achieving their breastfeeding goals. In the US, paid family leave has been strongly associated with both the in

IMPORTANCE: Acute respiratory tract infections are the leading cause of emergency department visits and hospitalizations in US children, with highest risks in the first 2 months after birth. Out-of-home childcare settings increase the spread of respiratory tract infections. The study team hypothesized that access to state-paid family leave could reduce acute care encounters (hospital admissions or emergency department visits) for respiratory tract infections in young infants by reducing out-of-home childcare transmissions.
OBJECTIVE: To determine if the 2018 introduction of paid family leave

OBJECTIVES: This study analyzes how the Massachusetts Paid Family Medical Leave (PFML) program impacts parental leave utilization and breastfeeding duration and exclusivity. Multiple organizations recommend exclusive breastfeeding for the first six months of life to protect against disease and promote cognitive growth.
METHODS: We used data from the Massachusetts Pregnancy Risk Assessment Monitoring Surveillance System to calculate prevalence and duration of parental leave and breastfeeding. The sample included 3,480 Massachusetts residents who gave birth in state between 2016 and 2022, overs

OBJECTIVES: The United States remains the only high-income nation without a national paid family leave (PFL) policy. It also has persistently high maternal and infant mortality rates. Understanding PFL impacts on perinatal health is critical. We determined the impact of state PFL policies on perinatal health, with additional focus on socioeconomic disparities.
METHODS: We analyzed live singleton births using 2016 to 2022 birth certificate data (N = 11 452 411). We conducted a quasi-experimental analysis using heterogeneity-robust differences in differences, comparing pre-post trends in a comp

Paid Family and Medical Leave (PFML) programs provide material resources and time away from the labor force around the time of a child's birth. Past research indicates that the programs improve maternal and child health and may increase fertility. To date, 13 states and the District of Columbia have passed PFML laws, with varying lengths of leave, eligibility, job protections, and benefit levels. Programs in states other than California remain understudied. We examined the effects of New York State's (NYS) PFML program on birth outcomes using difference-in-differences (DiD) models with data fr

Intimate partner violence (IPV) is a preventable public health problem with documented negative effects on well-being. The perinatal period represents a time of increased stress for partners, which can increase the likelihood of IPV. Economic support policies, like paid family leave (PFL), may reduce family violence, including IPV. Using data from the Pregnancy Risk Assessment Monitoring System, we examine the association between receipt of PFL and IPV for women employed during the perinatal period. Results indicate reduced odds of IPV for employed women who received PFL. PFL may reduce IPV vi

Physician well-being is essential to high-quality patient care, yet the lack of universally available paid family and medical leave (PFML) in the United States undermines this principle. The dynamic interplay between work and personal responsibilities-especially during significant life events such as childbirth, illness, or caregiving-can impact healthcare providers' mental and physical health, ultimately affecting patient outcomes. While the Family and Medical Leave Act (FMLA) provides unpaid, job-protected leave, it excludes many workers and disproportionately impacts women, minorities, and

Postplacement involvement and well-being of 53 families who had placed their child into a residential facility were studied. Parents were interviewed an average of 1, 2, and 3.5 years following placement. Visitation remained moderately high, with some type of visit reported at least monthly at Time 3 by 83% of families. Expressed attachment was high and stable, and most parents reported experiencing less guilt than at the time of placement. Parents were still thinking and talking about the child frequently. They were more likely, over time, to view the placement as permanent, and for about two

INTRODUCTION: Hypertensive disorders in pregnancy, including chronic and pregnancy-induced hypertension, pose significant risks to maternal health. This study evaluated the association of New York State (NYS)'s Paid Family Leave (PFL) law, implemented in 2018, with postpartum healthcare utilization among women with hypertensive disorders in pregnancy.
METHODS: Using commercial claims data (2017-2022) for 312 470 employed women aged 15-45 years with live births, we assessed postpartum outpatient visits, hospital admissions, and medication adherence.
RESULTS: The PFL law was associated with a

BACKGROUND: Sick-leave certification (SLC) is a legal document that provides official work exemption. In Israel, all SLCs are issued by primary care physicians. When providing a SLC, physicians are influenced by medical as well as nonmedical issues, such as the patients' psychosocial state and their workplace characteristics.
OBJECTIVE: To evaluate the explicit and implicit reasons for a SLC in Israeli primary care practices.
METHODS: A case-control prospective study was conducted in 47 urban primary care clinics in Israel. Patients of working age who received a SLC and a control group of co

This article examines the intersection of workers' compensation laws with the Americans with Disabilities Act (ADA) and the Family and Medical Leave Act (FMLA). Much ADA and FMLA litigation stems from work-related injuries or illnesses. Knowledge of the ADA and FMLA may help prevent workers' compensation cases from resulting in costly employment litigation. Employees who are absent from work for a work-related condition often have rights under other laws, besides workers' compensation laws, such as the ADA and FMLA. Employers need to be cognizant of this while addressing these cases. First, th

INTRODUCTION: As of 2024, paid paternity leave was available in 102 countries worldwide. However, the accessibility and methods of taking paternity leave are influenced by cultural background and individual values, leading to variations in uptake rate. Both positive and negative effects of paternity leave have been reported on the health of fathers, partners and children. A comprehensive understanding of this topic is essential for medical professionals supporting fathers undertaking childcare. The aim of this review is to identify and provide an overview of research related to the impact of p
