Feeding and eating disorders in childhood.

OBJECTIVE: To review the literature related to the current DSM-IV-TR diagnostic criteria for feeding disorder of infancy or early childhood; pica; rumination disorder; and other childhood presentations that are characterized by avoidance of food or restricted food intake, with the purpose of informing options for DSM-V. METHOD: Articles were identified by computerized and manual searches and reviewed to evaluate the evidence supporting possible options for revision of criteria. RESULTS: The study of childhood feeding and eating disturbances has been hampered by inconsistencies in classificat

EATING AND FEEDING DISORDERS IN THE FIRST FIVE YEARS OF LIFE: REVISING THE DC:0-3R DIAGNOSTIC CLASSIFICATION OF MENTAL HEALTH AND DEVELOPMENTAL DISORDERS OF INFANCY AND EARLY CHILDHOOD AND RATIONALE FOR THE NEW DC:0-5 PROPOSED CRITERIA.

Problems of eating and feeding are one of the most common reasons of referral to pediatric and infant mental health clinics. This article is drawn from work done by the ZERO TO THREE Task Force developing the DC:0-5 Diagnostic Classification of Mental Health and Developmental Disorders of Infancy and Early Childhood, specifically dealing with eating disorders in the first 5 years of life. The proposed changes come from both reviewing major studies and reviews published in the last 10 years and reports from clinicians collected through surveys commissioned by the Task Force. The main changes th

A comparison of re-presentation and modified chin prompt to treat different topographies of liquid expulsion.

Treatment of one behavior in the chain of consumption might be associated with the emergence of other problematic behaviors. For example, some children with feeding disorders expel liquid. Moreover, the form in which children expel liquid might vary and influence whether a treatment to reduce liquid expulsion will result in clinically meaningful outcomes. In the current investigation, we first identified topographies of liquid expulsion (e.g., forceful, run out) for each child. We then compared and evaluated the effects of 2 procedures, a modified chin prompt and re-presentation, on the liquid

Mitigating Feeding Disorders and Malnutrition in Children with Developmental Disabilities: A Narrative Review.

CONTEXT: Pediatric feeding disorder (PFD) is characterized by age-inappropriate oral intake associated with dysfunction in feeding skills, medical, nutritional, or psychosocial domains. Children with developmental disabilities (DD), such as cerebral palsy (CP), often meet the criteria for PFD due to underlying neurological, motor, and sensory impairments, compounded by psychosocial stressors. These challenges contribute to undernutrition, poor growth, and adverse developmental outcomes, but remain undiagnosed. This review evaluates the burden and spectrum of feeding problems in children with d

Psychological Treatment for Pediatric Feeding Disorder (PFD) and Avoidant/Restrictive Food Intake Disorder (ARFID).

OBJECTIVE: To produce a consensus statement on the psychological treatment of feeding/eating aversions seen in pediatric feeding disorder (PFD) and avoidant/restrictive food intake disorder (ARFID), diagnoses that share common symptoms and psychological mechanisms but have historically been addressed separately in the literature. METHOD: To help bridge the gaps between ARFID and PFD treatment and research, we convened a group of experts to clarify and describe the psychological interventions for treating learned food avoidance. The current manuscript summarizes the conclusions of the meeting.

The relationship between rumination and eating disorders.

OBJECTIVE: Rumination has been described as a symptom in association with eating disorders, but also as a separate disorder. METHOD: A series of 8 female volunteers who reported rumination were systematically screened for the presence of eating disorders and other psychopathology. RESULTS: Seven met current or prior criteria for an Axis I disorder and five had a current or prior eating disorder. DISCUSSION: The importance of screening for eating disorders among women who ruminate, and for screening for rumination among eating disorder patients, is stressed.

Adult picky eating and associations with childhood picky eating, maternal feeding, aversive sensory responsiveness, disgust and obsessive-compulsive symptoms.

BACKGROUND: Adult picky eating (PE) can cause distress, malfunction, and malnutrition. The purpose of this study was to examine adult PE, its relationship to childhood PE and to symptoms of obsessive-compulsive disorder (OCD), sensory processing disorder, food and general disgust, and maternal child feeding practices. METHODS: Adult participants (N = 772; 636 women) self-reported on measures of adult PE (Adult Picky Eating Questionnaire (APEQ)), obsessive-compulsive symptoms (Obsessive-Compulsive Inventory-Revised (OCI-R)), sensory processing difficulties (Sensory Responsiveness Questionnaire

Avoidant/restrictive food intake disorder: A diagnosis at the intersection of feeding and eating disorders necessitating subtype differentiation.

Avoidant/restrictive food intake disorder (ARFID) is a diagnosis that sits squarely at the cross roads of feeding disorders and eating disorders. It is historically tied to feeding disorders as a replacement of the DSM-IV diagnosis of feeding disorder of infancy or early childhood. The revision process, however, extended the diagnostic umbrella by removing its predecessor's weight loss requirement and age of onset restriction (i.e., 6 years). Implications of this extension include capturing an older cohort of patients with ARFID accessing care at eating disorders clinics, as well as providing

Eating disorders.

Twenty years have passed from the International Classification of Diseases, Tenth Revision (ICD-10) to the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5) and, in the meanwhile, a lot of research data about eating disorders has been published. This article reviews the main modifications to the classification of eating disorders reported in the "Feeding and Eating Disorders" chapter of the DSM-5, and compares them with the ICD-10 diagnostic guidelines. Particularly, we will show that DSM-5 criteria widened the diagnoses of anorexia and bulimia nervosa to less severe

Validity Evidence of Screening Tools for Pediatric Feeding Disorders: A Systematic Review.

CONTEXT: Pediatric feeding disorders (PFDs) present challenges involving food intake, feeding skills, and psychosocial or clinical dysfunctions affecting children and caregivers. Early identification and accurate evaluation are essential for improving outcomes. Despite the availability of various instruments, evidence for their reliability and validity in clinical practice and research remains limited. OBJECTIVE: The aim for this systematic review was to assess the evidence of the validity of instruments used to evaluate PFDs. DATA SOURCES: Articles were searched across Medline, Embase, APA

Eating Disorders in Infancy: Assessment and Interventions.

This article aims to integrate the physical and emotional dimensions of eating problems in infancy into the assessment process and the intervention approach. We emphasize the distinction between the child's own eating pattern and the parent-child feeding relationship. We describe the classification of eating behavior disorders in infancy and provide 2 short clinical vignettes that illustrate using the DC:0 to 5 classification framework. The assessment section includes the recommended physical/medical workup. Finally, we describe 2 clinical vignettes that illustrate the integrated multidiscipli

Eating Disorders in Late-life.

Eating disorders are a heterogeneous group of complex psychiatric disorders characterized by abnormal eating behaviours that lead to a high rate of morbidity, or even death, if underestimated and untreated. The main disorders enlisted in the chapter of the Diagnostic and Statistic Manual of Mental Disorders-5 dedicated to "Feeding and Eating Disorders" are: anorexia nervosa, bulimia nervosa and binge eating disorder. Even though these abnormal behaviours are mostly diagnosed during childhood, interesting cases of late-life eating disorders have been reported in literature. In this review, thes

[Eating disorder in a nineteen months-old girl and associated mother child relationship disturbance].

Feeding and eating problems and weight faltering are common causes for parents contact to health services in infancy and early childhood, and 0.15% of the child population aged 0-3 years are diagnosed at hospital with an ICD-10 childhood eating disorder. We describe the clinical presentation of an early feeding and eating disorder in a child of basically normal health and development. The case illustrates the significance of the mother child relationship both in the putative risk mechanisms and as a key factor in the treatment. The importance of a close cooperation between paediatric and child

An Important Springboard Toward Establishing Diagnostic Connection Between Avoidant Restrictive Food Intake Disorder and Pediatric Feeding Disorder.

Diagnostic ambiguity has surrounded the two primary diagnoses for dysfunctional feeding and eating in pediatric populations-that is, pediatric feeding disorder (PFD) and avoidant restrictive food intake disorder (ARFID). Recent expert consensus emphasized the need for diagnostic refinement to enhance understanding and improve clinical and research activities for both conditions. With the proposed revision of ARFID, Zickgraf and colleagues provide a research grounded springboard for beginning the revision process. The proposal centers upon a new Criterion A that emphasizes ARFID as a disorder d

Eating disorders of infancy and childhood: definition, symptomatology, epidemiology, and comorbidity.

This article describes a range of problem feeding and eating presentations seen in infants and children. In diagnostic terms some fall under the category of "feeding disorder," whereas others are childhood presentations of the eating disorders "anorexia nervosa," "bulimia nervosa," and atypical forms of these. Several other commonly occurring presentations that are difficult to fit into existing diagnostic categories are additionally described here, including "selective eating," "food avoidance emotional disorder," "food phobias," "functional dysphagia," and "food refusal."

From DSM-IV to DSM-5: changes to eating disorder diagnoses.

PURPOSE OF REVIEW: This article reviews the modifications to eating disorders that appear in the 'Feeding and Eating Disorders' chapter of the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5). These modifications include the addition of three disorders (avoidant/restrictive food intake disorder, rumination disorder, and pica) previously described in the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, Text Revision (DSM-IV-TR) section 'Feeding and Eating Disorders of Infancy or Early Childhood'; clarifications and modifications to anorexia nerv

Recollections of pressure to eat during childhood, but not picky eating, predict young adult eating behavior.

Picky eating is a childhood behavior that vexes many parents and is a symptom in the newer diagnosis of Avoidant/Restrictive Food Intake Disorder (ARFID) in adults. Pressure to eat, a parental controlling feeding practice aimed at encouraging a child to eat more, is associated with picky eating and a number of other childhood eating concerns. Low intuitive eating, an insensitivity to internal hunger and satiety cues, is also associated with a number of problem eating behaviors in adulthood. Whether picky eating and pressure to eat are predictive of young adult eating behavior is relatively uns
