Root canal treatment in a population-based adult sample: differences in patient factors and types of teeth treated between endodontists and general dentists.

OBJECTIVE: The purpose of this study was to identify in a population-based study the differences between general dentists and endodontists with regard to types of teeth treated, fees, and patient characteristics. BASIC RESEARCH DESIGN: The "Florida Dental Care Study" was a prospective cohort study using a representative baseline sample of 873 dentate adults. In-person interviews and clinical examinations were conducted at baseline, 24 months, and 48 months, with 6-monthly telephone interviews between those times. Dental record information was abstracted afterward. RESULTS: A total of 100 roo

Adult Medicaid Coverage for Periodontal Treatment: A State-to-State Comparison.

Dental care remains an optional benefit for adults with Medicaid, with coverage varying across states. Recent expansions increased periodontal service coverage, but the scope of coverage and the policies that govern that scope remain unknown. This study analyzed coverage policies and fees for four periodontal services across 43 Medicaid programs between January and March 2024. Frequency limitations were the most common coverage policies identified, followed by prior authorization requirements, clinical requirements, and quadrant limitations. Veteran Health Administration fees were roughly thre

Frequency of examination of children by dentists working under different payment systems in the United Kingdom.

The aims of this study were, first, to determine the frequency of dental examination of child patients by general dental practitioners working under a fee-for-service or capitation payment system in eight areas of the United Kingdom and, secondly, to consider how the frequency of examination varied by dentist payment system, by area, by age of the child and by dentist. The number of examinations carried out by 198 general dental practitioners over a one-year period for 17,389 children aged 0-15 years was recorded. The mean number of examinations per year for children treated by dentists workin

Patient debt management and student academic progress.

A survey of the patient debt management policies of all U.S.-accredited dental and dental hygiene educational programs was taken to assess institutional patient debt management procedures and their relationship to student academic progress. The policies were evaluated to determine the level of compliance with existing standards and to analyze them in light of their legal implications relative to student rights. The results illustrated a vast breadth of policies for both dental and dental hygiene programs, ranging from no relationship between debt management and student progress, to an unspecif

Specialist practice for minor oral surgery: a comparative audit of third molar surgery.

Cost efficiency of removing third molars in a practice specialising in minor oral surgery was compared to that of a hospital oral surgery department. A total of 100 patients treated in each locality were prospectively audited during the financial year 1989-90. Surgical complexity, waiting time, complications and patient satisfaction were compared. The hospital cases were costed individually and compared to fees received for the patients treated in the practice. The practice fees were also compared to average expenses as assessed by the Dental Rates Study Group Inland Revenue Enquiry. Patients

A survey of patient perceptions of dental charges.

The aim of this study was to examine patients' perceptions of charges for dental treatment. A sample of 96 patients attending the University Dental Hospital of Manchester completed two short questionnaires on the monetary value of various dental treatments (a dental examination, a scaling, an amalgam restoration, a crown, an extraction, a denture, a root filling and a radiograph). The first questionnaire gave no details of the procedures and also asked patients which of the items of treatment they had experienced. The second questionnaire asked about the monetary value of the same items but ga

The economic cost of illness revisited.

In and out of government, determining the cost of illness is a major concern. The allocation of health care resources and the evaluation of current research and program efforts depend in large measure on such information. This article updates the 1963 benchmark study of the cost of illness. For the 16 major diagnostic categories of illnesses, the cost is presented in terms of the direct costs for prevention, detection, and treatment; the morbidity losses due to disability; and the mortality losses resulting from premature death. The method of calculating the cost of any illness is described, an

Dentists' practice behaviors and perceived barriers regarding oral-systemic evidence: implications for education.

Observational studies consistently support a relationship between poor oral health and systemic diseases like cardiovascular disease and diabetes mellitus. The purpose of this study was to identify current practices and perceived barriers among North Carolina dentists regarding the incorporation of oral-systemic evidence into the delivery of patient care. A survey questionnaire was developed, pilot tested, revised, and mailed to 1,350 licensed dentists in North Carolina. The response rate was 49 percent. Bivariate analysis was used to compare practice behaviors and barriers among age, gender,

Dental care for aging populations in Denmark, Sweden, Norway, United kingdom, and Germany.

This article reviews access to and financing of dental care for aging populations in selected nations in Europe. Old age per se does not seem to be a major factor in determining the use of dental services. Dentition status, on the other hand, is a major determinant of dental attendance. In addition to perceived need, a variety of social and behavioral factors as well as general health factors have been identified as determinants of dental service use. Frail and functionally dependent elderly have special difficulties in accessing dental care; private dental practitioners are hesitant to provid

The effect of different methods of remuneration on the behaviour of primary care dentists.

BACKGROUND: Methods of remuneration have been linked with the professional behaviour of primary care physicians. In dentistry, this can be exacerbated as clinicians operate their practices as businesses and take the full financial risk of the provision of services. The main methods for remunerating primary care dentists include fee-for-service, fixed salary and capitation payments. The aim of this review was to determine the impact that these remuneration mechanisms have upon primary care dentists' behaviour. OBJECTIVES: To evaluate the effects of different methods of remuneration on the leve

Prevention and the demand for dental care in an international perspective.

Organizational patterns of preventive services aiming at stimulating dental health education and hygiene in the young are differently realized in various countries. The priority allotted to dental care within total health services differs in each society. Differences in allocation of responsibility for dental health are reflected in the proportion of co-payment costs borne by the individual. Low perceptions of need by individuals may cause different backlogs of professionally determined need which influence the supply of care and ensuing costs. This is reflected in various levels of expenditur

A framework for costing diagnostic methods in oral health care: an application comparing a new imaging technology with the conventional approach for maxillary canines with eruption disturbances.

OBJECTIVES: The aims were (i) to propose a framework for costing diagnostic methods in oral health care and (ii) to illustrate the application of the framework to the radiographic examination of maxillary canines with eruption disturbances. METHODS: The framework for costing, following Drummond et al. (2005), includes three elements: (i) identification of different resources used in producing and delivering the service, (ii) measurement of the amount of each resource required and (iii) valuation of the resources in monetary terms. Four data collection instruments were designed - a protocol fo

Reframing Periodontal Care: Communication and Fee Strategies for Effective Patient Engagement.

Periodontal therapy remains a cornerstone of dental practice. However, effectively managing periodontitis patients throughout the continuum of care can be challenging. This article proposes a communication framework and an integrated fee model designed to support evidence-based comprehensive treatment protocols while enhancing patient education, motivation, and long-term adherence. By emphasizing prevention and patient-centered care, the proposed approach contrasts with the dominant reactive model, often focused on surgical interventions and the extraction of salvageable teeth. Instead, this a

Dental service utilization and the COVID-19 pandemic, a micro-data analysis.

BACKGROUND: Global crises and disease pandemics, such as COVID-19, negatively affect dental care utilization by several factors, such as infection anxiety, disrupted supply chains, economic contraction, and household income reduction. Exploring the pattern of this effect can help policy makers to be prepared for future crises. The present study aimed to investigate the financial impact of COVID-19 disruptions on dental service utilization. METHODS: Data on the number of dental services offered in Dental School Clinics of Tehran University of Medical Sciences was collected over a period of two
