Postless Hip Arthroscopy: A Safer Alternative for Treatment of Femoracetabular Impingement Syndrome.

BACKGROUND: Hip arthroscopy for femoroacetabular impingement (FAI) syndrome continues to gain popularity and indications for its use are expanding. Though low complication rates have previously been reported, there are iatrogenic complications specific to the use of the perineal post, such as pudendal nerve injuries, and possible pressure skin necrosis that warrant concern in the healthy young patient. The risk of these complications are increased during simultaneous bilateral hip arthroscopy. INDICATIONS: We describe a new technique, which will prevent such problems by using a postless pink

Biomechanical comparison of two squatting protocols in adolescents and young adults with femoracetabular impingement syndrome.

INTRODUCTION: Squatting is an important activity of daily living which can be particularly useful in the management of Femoroacetabular Impingement Syndrome (FAIS). However, double-leg squat protocols can vary significantly, potentially impacting biomechanical outcomes. A squat with a "hold" before the ascent phase represents one variation which increases muscular endurance demands and may elucidate such impacts. The purpose of this study was to identify biomechanical differences between a traditional squat (TSquat) and squat with a hold (HSquat) in adolescents and young adults with pre-operat

Arthroscopic Treatment of Tenosynovial Giant Cell Tumor of the Hip With Concomitant Femoracetabular Impingement Syndrome.

BACKGROUND: Tenosynovial giant cell tumor (TGCT) is a benign but locally aggressive proliferative disorder of the synovium. TGCT of the hip is destructive to the joint and is a risk factor for early total hip arthroplasty (THA). Open synovectomy has previously been the gold-standard treatment. However, hip arthroscopy has emerged as a less invasive approach for the management of TGCT of the hip. INDICATIONS: Arthroscopic management of TGCT of the hip is indicated when there is either confirmed TCGT on biopsy or strong suspicion for TCGT based on clinical and radiologic findings, minimal-to-no

Femoral neck-shaft angle can predict the anterior capsular thickness in patients with femoracetabular impingement syndrome.

PURPOSE: To measure the femoral neck-shaft angle (NSA) on computed tomography (CT) images in femoracetabular impingement syndrome (FAIS) patients and explore its relationship with the anterior capsular thickness (ACT). METHODS: A retrospective review of prospectively collected data from 2022 was performed. Inclusion criteria included: primary hip surgery, 18 to 55 years of age, and CT imaging of the hips. Exclusion criteria included: revision hip surgery, mild or borderline hip dysplasia, hip synovitis, and incomplete radiographs and medical records. NSA was measured on CT imaging. ACT was me

Editorial Commentary: Hip Borderline Dysplasia Patients May Have Acetabular Undercoverage and Larger Labra.

The compensatory labrum needs to considered in patients with mechanical hip pain. It is no longer adequate to broadly characterize patients with femoracetabular impingement as either cam or pincher patients. Effective treatment of the syndrome requires in-depth assessment version, head-neck offset, subspine, and capsule-labral morphology, especially in patients with borderline dysplasia. A larger acetabular labrum is associated with hip dysplasia, and labral length correlates with lateral center-edge angle and acetabular roof obliquity. Symptomatic hips show larger labra. Labral size and aceta

Editorial Commentary: Physical Therapy Alone May Not Successfully Treat Patients With Hip Pain and Large α Angles or Decreased Femoral Anteversion Combined With Femoroacetabular Impingement Syndrome.

Understanding the relation between spinopelvic (lumbopelvic) tilt and femoracetabular impingement syndrome (FAIS) is complex, and determining the optimal patient parameters that lead to successful nonoperative management is vital. Physical therapy (PT) focusing on core and posterior chain strengthening is often successful. PT can change the posterior tilt of the pelvis by 5° to 10°, allowing increased range of motion (ROM) and decreased impingement of the hip. However, PT does not change cam anatomy. Thus, PT alone may not sufficiently increase ROM in patients with cam-type impingement and lar

Femoroacetabular impingement in a professional soccer player.

UNLABELLED: Presented is the case of a 25-year-old professional soccer player with a long-standing history of hip injuries, including a hamstring injury, adductor partial tearing with surgical release and labral tearing in the hip joint. The patient was eventually found to have a mixed type femoracetabular impingement and adaptive bony changes of the hip. The patient was treated with an arthroscopic acetabuloplasty of the pincer lesion, femoroplasty for the treatment of the cam lesion and labral repair along with open proximal adductor repair to restore the native biomechanics of the hip. The multifaceted etiology of acetabular labral tears.

Acetabular labral tears have been the focus of much attention in recent years. With the increased use of hip arthroscopy and magnetic resonance arthrography, the infrequent labral tear has proved to be more prevalent than previously thought. The majority of labral tears occur due to an underlying anatomic abnormality. Anatomic abnormalities that can lead to labral tears include femoracetabular impingement, acetabular retroversion, abnormal femoral head, Legg Calvé Perthes, slipped capital epiphysis, capsular laxity, and dysplasia of the hip. Tears in the labrum may lead to the advancement of o

Editorial Commentary: Hip Arthroscopists Can Reduce Postoperative Opioid Use.

Despite the harrowing opioid crisis in the United States, the use of opioids to combat musculoskeletal pain continues to be widespread. In the setting of hip arthroscopy, approximately one-third of patients are on opioids while awaiting surgery to address the pain that results from femoracetabular impingement syndrome. In addition, the use of opioids to address pain postoperatively is common practice. With the rapid rise of hip arthroscopy in the United States, it is paramount that other modes of pain relief are promoted by surgeons in conjunction with allied health professionals, such as phys

Editorial Commentary: Two Hips, One Patient: Insights From Staged Bilateral Hip Arthroscopy.

Bilateral femoroacetabular impingement syndrome is commonly encountered in clinical practice, and determining the optimal approach and timing for surgical management of both hips remains an important consideration. I have performed both simultaneous and staged bilateral hip arthroscopies; however, my preference has been to stage procedures to allow for appropriate recovery, functional assessment, and patient tolerance following the initial surgery. Managing bilateral pathology requires balancing surgical efficiency with patient safety, rehabilitation demands, and expectations. Thoughtful consi

Editorial Commentary: Can We Derive Any Practical Information From a Study of "The Top 50 Most Influential Papers in Hip Arthroscopy," or Are We Tooting Our Own Horns?

Orthopaedic literature on hip arthroscopy has become more robust primarily from the advent and transformation in the treatment of femoroacetabular impingement syndrome from an open surgical dislocation to an ambulatory arthroscopic procedure. Numerous studies have reviewed the frequency of authors and subjects in particular areas of arthroscopy, but until this article, none had been performed on the most influential in hip arthroscopy. The question to be answered is, Do such studies contribute significantly to our knowledge by suggesting future topics to clinicians and training programs?

Editorial Commentary: The Hip Capsule: To Close or Not to Close? Is That Still the Question?

The question of capsule closure or no closure after hip arthroscopy remains controversial as we try to decipher best practice and which patients should and should not have a repair. Closure seems of particular importance in younger patients and with larger capsulotomies. In my practice, I routinely repair the capsule after hip arthroscopy, except in patients with significant stiffness. Capsule repair may not be vital in some patients, as a smaller capsulotomy could sometimes heal on its own, but my patients and I certainly do not want to learn the hard way.

Hip arthroscopy versus open surgical dislocation for femoroacetabular impingement: A systematic review and meta-analysis.

BACKGROUND: This meta-analysis aims to evaluate the efficacy and safety of hip arthroscopy versus open surgical dislocation for treating femoroacetabular impingement (FAI) through published clinical trials. METHODS: We conducted a comprehensive literature search using PUBMED, EMBASE, and the Cochrane Central Register of Controlled Trials databases for relevant studies on hip arthroscopy and open surgical dislocation as treatment options for FAI. RESULTS: Compared with open surgical dislocation, hip arthroscopy resulted in significantly higher Nonarthritic Hip Scores (NAHS) at 3- and 12-month

[Femoroacetabular impingement related to Legg-Calvé-Perthes disease].

Femoroacetabular impingement is an undesirable contact between acetabular rim and femoral neck and presents abnormality of proximal femoral part and acetabulum, as well. Two forms may appear: cam impingement and pincer impingement. Femoroacetabular impingement related to Legg-Calvé-Perthes disease may be caused by various reasons, as the consequence of the disease itself, and as the consequence of its treatment. Coxa magna deformity (large femoral head and neck) and coxa brevis deformity (shortened femoral neck) may produce cam femoroacetabular impingement during hip flexion. After the disease

Editorial Commentary: The First Operation Determines the Ceiling: Revision Hip Arthroscopy in Adolescents Offers Gains but Not Full Restoration.

Revision hip arthroscopy in adolescents has become increasingly common as more young athletes undergo primary surgery for femoroacetabular impingement and labral pathology. In my experience, revision procedures can provide meaningful pain relief and functional gains, but the hip rarely behaves like a "primary" hip once again. Clinical data consistently show lower absolute outcome scores, lower clinical benefit rates, and a higher risk of rerevision after adolescent revision compared with primary arthroscopy. For me, the central message is simple: the first operation matters profoundly. Precise

Hip Labral Reconstruction With a Synthetic Graft: Development and Preclinical Validation.

OBJECTIVES: Hip Osteoarthritis (OA) affects a significant component of the adult population, placing itself among the most important causes of disability and need for total hip replacement. Femoroacetabular impingement (FAI) is an anatomic alteration of the proximal femur and/or acetabulum leading to chondro-labral damage and playing a prominent role in OA pathogenesis. Thus, treating FAI is fundamental to relieve hip pain and further joint tissue deterioration. Labral reconstruction is considered the treatment of choice, in particular using tendon allografts or autografts which, however, have
