[Combined fractures of the femoral neck with femoral shaft fractures].

Ipsilateral fractures of the femoral neck and shaft presents diagnostic difficulties and complex choices as to treatment. The surgeon is often faced with a multiply-injured patient with an obvious fracture of the femoral shaft. The accompanying femoral neck fracture can be overlooked (20-50%). Most frequently, the missed fracture is a minimally displaced vertical fracture of the femoral neck. Reviewing the literature on the subject offers little guidance of managing this fracture combination. Our present protocol for this double fracture is treatment with immediate internal fixation: 1. [Distal femoral fractures].

CLINICAL ISSUE: Fractures of the distal femur represent rare but serious injuries with a high 1‑year mortality. An increasingly older patient population requires adapted treatment concepts. STANDARD TREATMENT PROCEDURE: Surgical treatment using angular stable plating and retrograde nailing is the standard procedure. Conservative treatment is only indicated in cases of low demands and high perioperative risks of the patient. TREATMENT INNOVATIONS: Primary double plate osteosynthesis and primary implantation of a distal femoral replacement represent new treatment procedures and show promising

Interimplant femoral fractures.

The number of hip or knee arthroplasties, and internal fixations of the proximal and distal femur, is increasing in proportion to the growing and ageing population, whose life expectancy is lengthening. Thus, fractures of the femur between proximal and distal implants, although rare, are becoming more frequent. Women over the age of 70, with fragile bones and whose ends of the two implants are close to each other ("kissing implants") are particularly vulnerable to them. Reliable and reproducible fracture classifications exist when it comes to 2 prostheses, but they are less well established in

[Femoral shaft fractures].

Femoral shaft fractures after completion of growth predominantly affect young people with healthy bones. The causes are mostly high-velocity traffic accidents, crushing or running over mechanisms and falls from a great height. Gunshot wounds are relatively rare in Germany but have a certain importance internationally and in military medicine. Accompanying injuries in local or other regions are frequent. The predominant fracture types are transverse, wedge, segment and comminuted fractures. Spiral fractures are a sign of indirect force and are therefore frequently found in older patients with o

[Trochanteric femoral fractures].

At the present time proximal femoral fractures account for 30% of all fractures referred to hospitals for treatment. Our population is ageing, the proportion of patients with post-menopausal or senile osteoporosis is increasing and therefore the number of proximal femoral fractures requiring urgent treatment is growing too. In the age category of 50 years and older, the incidence of these fractures has increased exponentially. Our department serves as a trauma centre for half of Prague and part of the Central Bohemia Region with a population of 1 150 000. Prague in particular has a high number

Interprosthetic femoral fractures.

OBJECTIVE: Interprosthetic femoral fractures, ones occurring between ipsilateral total hip and total knee arthroplasties, are an increasingly common and challenging problem for orthopaedic surgeons. The purpose of this study was to report specific fracture locations and treatment outcomes associated with a protocol of femoral plate fixation that spanned the interprosthetic zone applied with modern soft tissue preserving reduction techniques without adjuvant bone grafts. DESIGN: Retrospective cohort study. SETTING: One Level I and one Level II trauma center. PATIENTS/PARTICIPANTS: A consecut

Subtrochanteric femoral fractures.

Subtrochanteric femoral fractures are complicated injuries that may be associated with other life-threatening conditions. Patients should be carefully evaluated and appropriately treated for hypovolemic shock. These fractures can be effectively stabilized with 95 degrees plates, femoral reconstruction nails, or trochanteric femoral nails with interlocking options. Nails produce very stable constructs and consistently can be placed with the patient in the lateral position on the radiolucent table or in the supine position on the fracture table. Standard antegrade femoral nails may be indicated

Proximal femoral fractures.

The absolute number of hip fractures is large, increasing, and related to osteoporosis as well as other factors. By 65 years of age, 50% of women will exhibit bone mineral below the fracture threshold; by age 85, 100%. Trabecular bone loss, which occurs before cortical bone loss, is characteristic of postmenopausal osteoporosis, while cortical bone loss, which increases with age (especially after the age of 70), is characteristic of senile osteoporosis. Estrogen treatment is reasonable for early postmenopausal osteoporosis, less so for the senile variety. The elderly often lose the renal abili

Periprosthetic femoral fractures.

The incidence of periprosthetic femoral fractures is rising in the UK, because of an ageing population and an increasing number of hip arthroplasty operations being performed. They can occur intra- or postoperatively, and usually follow low energy trauma. They present with pain, swelling over the thigh, and an inability to weight bear. Periprosthetic femoral fractures are usually classified as per the unified classification system. Their management usually is dependent on their classification, with type A (fracture at level of greater or lesser trochanter) managed non-operatively with protecte

[Subtrochanteric femoral fractures].

Subtrochanteric femoral fractures are proximal femoral fractures which are located between the trochanter minor and an area of 3 cm below the minor trochanter on the femoral shaft. About 10-15% of all proximal femoral fractures correspond to this fracture site. Elderly or geriatric patients are generally affected and the injury is often the result of a fall in the home, while high-energy trauma is the cause in a small group of generally younger patients. Clinical evaluation of the affected extremity shows disability of axial weight-bearing and pain during compression and rotation of the hip jo

[Distal femoral fractures].

Fractures of the distal femur still represent injuries that are difficult to treat as they either affect younger patients after a high-energy trauma with soft tissue damage and osseous comminution or elderly people with impaired local vascularity and a poor bone stock. However, exactly these fractures profit from new, biological principles of treatment, which help to diminish additional surgical trauma by indirect fracture reduction and insertion of stabilizing implants via mini-incisions. Basically, these techniques are represented by retrograde intramedullary nails and submuscularilly insert

Periprosthetic femoral fractures.

Periprosthetic fractures of the femur are increasing both in the absolute number of fractures per year and in frequency (proportion of fractures among patients with hip arthroplasty). As a result, orthopaedic surgeons will face these challenging injuries more often. The optimal treatment must be individualized, taking into consideration the fracture location relative to the arthroplasty component, the implant stability, the underlying quality of the bone, and the medical and functional status of the patient. Based on these factors, successful treatment usually involves some combination of open

Distal femoral fractures.

Distal femoral fractures largely occur secondary to high-energy trauma in the younger population and as osteoporotic fractures in the elderly population, including periprosthetic fractures above a TKA. Attempts to gain satisfactory axial alignment, articular congruity, and knee range of motion with conservative treatment have been largely disappointing. Operative fixation options include open reduction and internal fixation, intramedullary nailing, and bridge plating or percutaneous submuscular plating techniques. As with any fracture, treatment choice must be individualized according to the n

Characteristics of Femoral Shaft Fractures That Predict Ipsilateral Femoral Neck Fractures.

INTRODUCTION: This study aims to characterize radiographic features and fracture characteristics in femoral shaft fractures with associated femoral neck fractures, with the goal of establishing predictive indicators for the presence of ipsilateral femoral neck fractures (IFNFs). METHODS: A retrospective cohort was collected from the electronic medical record of three level I trauma centers over a 5-year period (2017 to 2022) by current procedural terminology (CPT) codes. Current CPT codes for combined femoral shaft and IFNFs were identified to generate our study group. CPT codes for isolated

Fat embolism: special situations bilateral femoral fractures and pathologic femoral fractures.

Few data are available in the literature regarding fat embolism in cases of bilateral femoral and pathological femoral fractures. The incidence of bilateral femoral fractures ranges from 2-9.5% of the total number of patients with femoral fractures, and they usually occur in high energy trauma and multi-trauma patients. Although injury severity scores tend to underestimate the severity of these injuries, fat embolism seems to occur in increased frequency ranging from 4.8-7.5%. Intramedullary nailing, which is the preferred surgical treatment, triggers a systemic inflammatory response that pose

Femoral Neck Complications in Isolated Femoral Neck Fractures Versus Ipsilateral Femoral Neck and Shaft Fractures.

OBJECTIVES: There are limited data on the difference in outcomes between patients with isolated femoral neck fractures (IsoFN) and those with a femoral neck and ipsilateral femoral shaft fracture (IpsiFNS) in terms of osteonecrosis, nonunion, and revision surgery rates. We hypothesized that displaced IsoFN fractures would have higher rates of femoral neck nonunion, osteonecrosis, and hip revision surgery than displaced IpsiFNS fractures. Comparisons were made to determine the relationship between these outcomes and other variables. METHODS: A retrospective review of patients age 18 to 55 year

Proximal femoral nail in intertrochanteric femoral fractures.

INTRODUCTION: Proximal femoral nails (PFN) have been introduced recently but have begun to compete the traditional dynamic hip screw. The mechanical strength of the nail and less invasive procedure has made the procedure preferable. This is a short retrospective review of 26 cases operated in the last two years. METHODS: A retrospective review was conducted of 26 cases operated during two years of study which had completed at least a year of follow up. All had been treated using a proximal femoral nail for unstable intertrochanteric fractures of femur. A radiological assessment was made with

The definition of atypical femoral fractures should include periprosthetic femoral fractures (PAFFs).

UNLABELLED: Periprosthetic hip fractures may have features of atypical femoral fractures. PURPOSE: Atypical femoral fracture (AFF) is a rare complication of treatment with bisphosphonates (BPs) or denosumab. The American Society for Bone and Mineral Research (ASBMR) Task Force definition for AFFs excludes periprosthetic fractures. The purpose of this study was to determine prodromal symptoms, frequency, treatment, and outcomes of periprosthetic AFFs (PAFFs) in persons prescribed a BP or denosumab for osteoporosis and later diagnosed with a periprosthetic hip fracture. METHODS: Participants w

A Periprosthetic Femoral Fracture with Characteristics of Atypical Femoral Fracture.

Although the definition of atypical femoral fracture (AFF) excludes periprosthetic femoral fracture (PFF), the number of reports about PFF with characteristics of AFF is increasing. We present the case of such a fracture in this report. An 87-year-old woman who underwent bipolar hip arthroplasty for a femoral neck fracture 38 months prior reported left thigh pain with no history of trauma. Radiographs showed a simple transverse fracture at the level of the stem distal end with features of AFF: periosteal thickening of the lateral cortex, a medial spike, and a noncomminuted fracture.
