Femoral Neck Complications in Isolated Femoral Neck Fractures Versus Ipsilateral Femoral Neck and Shaft Fractures.

OBJECTIVES: There are limited data on the difference in outcomes between patients with isolated femoral neck fractures (IsoFN) and those with a femoral neck and ipsilateral femoral shaft fracture (IpsiFNS) in terms of osteonecrosis, nonunion, and revision surgery rates. We hypothesized that displaced IsoFN fractures would have higher rates of femoral neck nonunion, osteonecrosis, and hip revision surgery than displaced IpsiFNS fractures. Comparisons were made to determine the relationship between these outcomes and other variables. METHODS: A retrospective review of patients age 18 to 55 year

[Femoral neck stress fractures].

Femoral neck stress fractures are relatively rare and caused by repetitive high pressure on the bone with insufficient time to recover. These fractures are often seen in fanatic runners or military personnel, who cover great distances. Patients with a femoral neck stress fracture present with mild pain at the front of the thigh or groin. Radiological imaging includes a plain X-ray and/or MRI-scan. Differentiation between a tension- and compression type stress fracture is important, since their treatment strategies differ. Generally, tension type fractures are unstable and demand operative fixa

[Combined fractures of the femoral neck with femoral shaft fractures].

Ipsilateral fractures of the femoral neck and shaft presents diagnostic difficulties and complex choices as to treatment. The surgeon is often faced with a multiply-injured patient with an obvious fracture of the femoral shaft. The accompanying femoral neck fracture can be overlooked (20-50%). Most frequently, the missed fracture is a minimally displaced vertical fracture of the femoral neck. Reviewing the literature on the subject offers little guidance of managing this fracture combination. Our present protocol for this double fracture is treatment with immediate internal fixation: 1. [Femoral neck fracture].

FEMORAL NECK FRACTURE. Femoral neck fractures represent the third most common type of fracture in traumatology. The diagnosis is based on the clinic and radiographs of the pelvis and the hip. The choice of treatment depends on the type of fracture (with displacement in coxa vara, coxa valga, or without displacement), the age of the patient, his autonomy and his co-morbidities. In elderly patients with a displaced fracture in coxa vara, arthroplasty will be preferred, whereas for non-displaced or coxa-valga fractures, as well as in younger patients, osteosynthesis will be performed. The experti

Fractures of the femoral neck.

Fractures of the femoral neck are increasing at an exponential rate as a result of the longevity of the general population. The Garden and Pauwels classifications both are routinely used to describe displacement and stability of femoral neck fractures. Osteonecrosis and nonunion remain problematic because of the compromised blood supply to the femoral head in displaced fractures. Nondisplaced fractures and displaced fractures in patients physiologically younger than 65 years are treated with closed or open reduction and internal fixation. Anatomic reduction is the single most important step in

[Femoral neck fractures in childhood].

Twenty-eight fractures of the neck of the femur occurred in children between 1963 and 1982 were treated and followed up at the Pediatric Surgical Hospital of the University of Mainz. Three children had transepiphyseal fractures (type I), eight children transcervical fractures (typ II), six children cervicobasal fractures (typ III), and eleven children pertrochanteric fractures (typ IV). Surgical intervention was performed in fifteen children, and thirteen children were submitted to conservative treatment. The results were evaluated according to a new system considering troubles, mobility, and

Femoral neck fractures: current management.

Femoral neck fractures are a commonly encountered injury in orthopaedic practice and result in significant morbidity and mortality. It is essential that surgeons are able to recognize specific fracture patterns and patient characteristics that indicate the use of particular implants and methods to effectively manage these injuries. Use of the Garden and Pauwels classification systems has remained the practical mainstay of femoral neck fracture characterization that help dictate appropriate treatment. Operative options include in situ fixation, closed or open reduction and internal fixation, he

[Treatment of femoral neck fractures].

Due to the high incidence (600-900 patients/year >65 years old), the expected increase in frequency by a factor of five by 2050, and the proportionately shrinking capacity in trauma centers, femoral neck fractures are relevant to health care both economically and politically. Surgical treatment within 6 h improves results of osteosynthesis, within 24 h reduces general complications, and within 48 h reduces mortality. The literature displays great regional differences in methods and results. There is however general agreement that the hip joint should be preserved in young, active patients, reg

[Femoral neck fractures in childhood].

In a retrospective multicenter study we followed-up 30 fractures of the neck of the femur in children aged 11 years (1.5-15 years) for 4.8 years (0.5-20.0 years). It is the aim of this study to analyse factors possibly related to outcome, like age at injury, type of fracture, interval between injury and treatment, method of stabilization and postoperative interval until full weight bearing. At follow-up subjective parameters (pain, weather sensitiveness), clinical parameters (range of motion, claudication, leg length discrepancies) and radiological parameters (hip series) were investigated and

Characteristics of Femoral Shaft Fractures That Predict Ipsilateral Femoral Neck Fractures.

INTRODUCTION: This study aims to characterize radiographic features and fracture characteristics in femoral shaft fractures with associated femoral neck fractures, with the goal of establishing predictive indicators for the presence of ipsilateral femoral neck fractures (IFNFs). METHODS: A retrospective cohort was collected from the electronic medical record of three level I trauma centers over a 5-year period (2017 to 2022) by current procedural terminology (CPT) codes. Current CPT codes for combined femoral shaft and IFNFs were identified to generate our study group. CPT codes for isolated

Femoral nerve block in extracapsular femoral neck fractures.

We randomised 50 patients with extracapsular fractures of the femoral neck to receive either a bupivacaine femoral nerve block or systemic analgesia alone. A femoral nerve block was found to be an easy and effective procedure which significantly reduced perioperative analgesic requirements and postoperative morbidity.

A pathological fracture of the femoral neck associated with osteonecrosis of the femoral head and a stress fracture of the contralateral femoral neck.

A pathological fracture of the right femoral neck associated with osteonecrosis of the right femoral head and a stress fracture of the contralateral femoral neck occurred in a 47-year-old man. Osteonecrosis was noted in almost the entire femoral head, and the pathological fracture occurred at the subcapital area. Six months later, a stress fracture was detected in the contralateral femoral neck. The stress fracture of the left femoral neck might have been caused by the incremental repetitive mechanical loading in the left hip as a result of the pathological fracture in the right hip. Therefore

Impact of Intraoperative Femoral Fractures During Cemented Hemiarthroplasty for Femoral Neck Fractures.

BACKGROUND: There have been several studies on intraoperative femoral fractures (IFFs) during primary total hip arthroplasty, but it is not well understood how this complication affects the patient population undergoing cemented hemiarthroplasty. This study aimed to analyze the impact of IFFs sustained during cemented hemiarthroplasty for the treatment of femoral neck fractures. METHODS: A retrospective review was conducted of all patients who were treated for Arbeitsgemeinschaft für Osteosynthesefragen/Orthopaedic Trauma Association 31B fractures with cemented hemiarthroplasty between Januar

[Osteosynthetically-treated intracapsular femoral neck fractures].

AIM OF THE STUDY: Treatment of intracapsular femoral neck fractures, especially in elder patients, has been a topic of ongoing discussion. This study analyzes the results of 97 osteosynthetically-treated femoral neck fractures. It had to be proved whether the Dynamic Martin Screw (DMS) represents an alternative to the already existing implantation devices. Until now no systematic results have been reported on this topic. METHODS: Over a period of seven years, 389 patients with femoral neck fractures were treated. In 101 of these patients operations with preserved femoral heads were carried ou

Ipsilateral femoral neck and shaft fractures.

Ipsilateral femoral neck and shaft fractures are uncommon injuries that present a surgical challenge. Patients are relatively young, are usually victims of high-energy trauma, and have frequently sustained multisystem injuries. A comminuted midshaft femoral fracture secondary to axial loading should alert the treating physician to the possibility of an associated femoral neck fracture. This is important in light of the frequency of unrecognized ipsilateral femoral neck fractures. Several treatment options are described in the literature, but no clear consensus exists regarding the optimal trea

Ipsilateral femoral neck and shaft fractures.

Ipsilateral femoral neck and shaft fractures occur in 2.5% to 6% of femur fractures. The injury results from high energy trauma. Victims are usually young, with multiple associated injuries. The diagnosis of the neck fracture is delayed in 19% to 31% of patients. The neck fracture line is almost vertical and nondisplaced, or minimally displaced in 26% to 59% of cases. The shaft fracture is often midshaft and open, and/or comminuted in 47% to 67% of cases. Ipsilateral knee injuries occur in 20% to 40% of patients. Two major complications, osteonecrosis of the femoral head and nonunion of the ne

[Head endoprostheses in femoral neck fractures].

By clinical, radiographic and scintigraphic means we attempted to examine, whether the implantation of a simple head-endoprosthesis after fractures of the femoral neck still has to be considered as an appropriate operating method or whether on principle, primary endoprosthesis should be used. The results prove that in suitable cases and with correct technique the head-endoprosthesis still has justification, especially as the "protrusio acetabuli", known only as complication of pure head-endoprosthesis, is mostly avoidable. Reference to the choice of cases, operating conditions and appropriate
