Finger joint injuries.

Finger joint dislocations and collateral ligament tears are common athletic hand injuries. Treatment of the athlete requires a focus on safe return to play and maximizing function. Certain dislocations, such as proximal interphalangeal and distal interphalangeal volar dislocations, may be associated with tendon injuries and must be treated accordingly. Treatment of other dislocations is ultimately determined by postreduction stability, with many dislocations amenable to nonoperative treatment (ie, immobilization followed by rehabilitation). Protective splinting does not necessarily preclude at

The climber's finger: imaging of non-pulley finger injuries.

Climbing is a rapidly growing sport that subjects the fingers to intense mechanical stress, predisposing athletes to a wide spectrum of injuries beyond the well-recognized flexor pulley lesions. Non-pulley-related finger injuries, although often underrecognized, can impair performance, cause chronic pain, and lead to long-term complications such as osteoarthritis, cyst formation, or tendon rupture. These injuries involve flexor tendons, collateral ligaments, volar plates, joint capsules, extensor mechanisms, lumbrical muscles, epiphyseal growth plates in adolescents, digital nerves, and palmar

Mallet Finger: Two Different Injuries.

Mallet injuries, either tendinous or bony, are common. They are often studied together and typically treated in the same way with extension splintage for 6 to 8 weeks. Yet the evidence clearly shows there are different injuries that present in the same way. Tendinous mallet injuries present in older patients usually following a low energy injury; they are often painless. The commonly injured fingers are the middle and ring. The injuries are almost always single digit without concomitant injuries. There is an extensor lag of a mean of 310 (range 3°-590) in the patients treated in my unit. In co

Finger Injuries in Ball Sports.

Finger injuries are common in athletes playing in professional ball sports. Understanding the intricate anatomy of the digit is necessary to properly diagnose and manage finger injuries. Unrecognized or poorly managed finger injuries can lead to chronic deformities that can affect an athlete's performance. Multiple factors and treatment options should be considered to provide the best functional outcome and rapid return to play for an athlete. This article discusses the mechanism of injury, diagnosis, treatment, and return-to-play recommendations for common finger injuries in ball sports.

[Finger injuries in ball sports].

Ball sports are the most frequent cause of sport injuries and 60% of all hand injuries involve the fingers. The most common injury is closed rupture of the extensor tendon of the distal interphalangeal joint (mallet finger). Rupture of the deep flexor tendon (jersey finger) occurs particularly in contact ball sports, such as rugby. Injuries of the proximal interphalangeal and metacarpophalangeal joints are of high functional relevance. These injuries frequently represent complex lesions which are demanding both in diagnostics and therapy. Errors in diagnosis or insufficient treatment can lead

Finger-level avulsion-type injuries.

Fifty-nine avulsed fingers were managed between May 1986-December 2004. Of 59 cases, 39 (66%) were treated with microsurgical techniques, and the remaining 20 cases (34%) were treated in the conventional manner. Follow-up time varied between 9-18 years. All cases were classified according to the Kay-Wolff classification. Replantation was done in 26 fingers, and revascularization in 13 fingers. The survival rate was 100% in the revascularization cases, and 80.7% in the replantation cases. The total active range of motion was more than 90 degrees in the interphalangeal joints in 48% of cases, an

The climber's finger : imaging of finger flexor tendon pulley injuries.

INTRODUCTION: Finger flexor tendon (FT) pulleys are delicate anatomical structures whose primary function is to anchor the tendon sheaths to the underlying bone. They maintain the tendons in close proximity to the phalanges, thereby stabilizing them during finger flexion. This arrangement allows for efficient force transmission, preserves an effective lever arm, and prevents bowstringing, ensuring a full range of motion. These structures can be accurately assessed using high-resolution ultrasound (US) or high-field magnetic resonance imaging (MRI). METHODOLOGY: We conducted a structured narra

Imaging of Sport-related Finger Injuries.

Traumatic finger injuries in athletes are widespread and vary widely, from minor sprains to complex fractures or tendon damage, with occurrences differing by sport due to distinct biomechanical demands. Imaging plays a pivotal role in assessing these injuries. Radiography serves as the primary diagnostic tool, crucial for detecting bone-related abnormalities; ultrasound and magnetic resonance imaging are essential for evaluating soft tissue injuries. Prompt and accurate diagnosis is key to ensuring effective treatment and preventing long-term complications, thereby facilitating a quick and saf

An overview of mallet finger injuries.

Mallet finger describes a fingertip deformity where the distal interphalangeal joint (DIPJ) of the affected digit is held in flexion, unable to extend the distal phalanx actively. The deformity is typically a consequence of traumatic disruption to the terminal extensor tendon at its insertion at the proximal portion of the distal phalanx or slightly proximally at the level of the DIPJ. Patients typically present with a history describing the event of injury with a typical mallet deformity. Common mechanisms include sport activities causing a direct blow to the finger, low energy trauma while p

Acute Mallet Finger Injuries-A Review.

Acute disruptions of the terminal extensor tendon are common and can result in significant dysfunction if not recognized and treated appropriately. This article provides a topical review of the contemporary literature concerning acute mallet finger injuries. It also proposes a modification to the Doyle classification to make it more encompassing and less prone to interobserver error.

Imaging Traumatic Hand and Finger Injuries.

The hands and fingers play a vital role in everyday activities, which increases the risk for accidental injury. Imaging these potentially complex injuries is vital to successful diagnosis and treatment. Imaging techniques and best practices vary; multiple modalities might be needed to achieve an accurate diagnosis. This article discusses anatomy and common injuries of the hand and fingers. Methods for successful imaging also are described. A broad understanding of anatomy and imaging techniques enables radiologic technologists to better assist with hand trauma management.

[Management of finger-level avulsion injuries].

BACKGROUND: In avulsion-type injuries of the fingers recovery of blood circulation is one of the major obstacles. The indication for finger reconstruction is discussed controversely, being influenced by the patient's needs, the degree of damage to the soft tissue and the prospects of success of the healing process. In this study we present our results after reconstruction of avulsion-type injuries of the fingers. Indications for finger reconstruction will be assessed in consideration of the expected outcome. PATIENTS AND METHODS: From 1999 to 2006 we treated 18 patients with finger level avul

Interventions for treating mallet finger injuries.

BACKGROUND: Mallet finger, also called drop or baseball finger, is where the end of a finger cannot be actively straightened out due to injury of the extensor tendon mechanism. Treatment commonly involves splintage of the finger for six or more weeks. Less frequently, surgical fixation is used to correct the deformity. OBJECTIVES: To examine the evidence for the relative effectiveness of different methods of treating mallet finger injuries. SEARCH STRATEGY: We searched the Cochrane Musculoskeletal Injuries Group trials register (November 2003), the Cochrane Central Register of Controlled Tri

[Closed injuries of finger bones in children].

Closed dislocations of finger bones is a rare injury in children. Most often complications are localized in the metacarpophalangeal articulation of the first finger. The main method of treatment of fresh dislocations is a one-step closed hand reposition. In case of its failure an open setting of the dislocation should be performed along with liquidation of the cause of interposition. The method of choice of treatment of neglected dislocations of fingers in children is the application of a compression-distraction apparatus.

Non-operative treatment of common finger injuries.

Finger fractures are common injuries with a wide spectrum of presentation. Although a vast majority of these injuries may be treated non-operatively with gentle reduction, appropriate splinting, and careful follow-up, health care providers must recognize injury patterns that require more specialized care. Injuries involving unstable fracture patterns, intra-articular extension, or tendon function tend to have suboptimal outcomes with non-operative treatment. Other injuries including terminal extensor tendon injuries (mallet finger), stable non-articular fractures, and distal phalanx tuft fract

[Closed extensor tendon injuries of the finger].

The extensor apparatus of the fingers is a complex structure. The diagram of the extensor apparatus as seen in figure 1 is in reality not so clearly structured. Closed lesions are the mallet finger, where either an avulsion of the bony insertion of the tendon occurs or where a fracture of the base of the distal phalanx with dislocation of a bone-fragment together with the tendon insertion has happened. Other closed lesions are the closed Boutonnière deformity and the avulsion of a lumbrical muscle. The mallet finger is mostly treated conservatively with a prefabricated splint holding the DIP-j

Four finger amputation injuries. Concept of treatment.

Fourteen patients with an amputation injury of four fingers are reported. Fifty-one of these 56 fingers were replanted with a survival rate of 88%. To achieve optimal results replantation of all fingers should be attempted. Dividing the operative procedure into a macrosurgical and a microsurgical part helps to economize both tourniquet time and total operative time. In case of arterial spasm or other difficulties to reestablish blood flow, a check list can be of great value. To achieve good results in such severe and difficult procedures a special organisation of replantation centers is needed
