Finger joint dislocations and collateral ligament tears are common athletic hand injuries. Treatment of the athlete requires a focus on safe return to play and maximizing function. Certain dislocations, such as proximal interphalangeal and distal interphalangeal volar dislocations, may be associated with tendon injuries and must be treated accordingly. Treatment of other dislocations is ultimately determined by postreduction stability, with many dislocations amenable to nonoperative treatment (ie, immobilization followed by rehabilitation). Protective splinting does not necessarily preclude at

In a retrospective clinical study 16 vascularized joint transfers to the hand with an average follow-up of 8.2 (3 - 15) years were evaluated. The finger joint defect was caused by trauma in 12 patients, tumour in 2 patients and infection and congenital deformity in 1 patient each. There were 14 men and 2 women. The mean age range was 26 (2 - 42) years. In 6 cases a partial vascularized joint transfer was carried out, with the transplant being harvested in two cases from non-replantable finger according to the "tissue bank concept" according to Chase and in the other two cases from the PIP-join

Extension of the fingers is a complex act. Boutonnière deformity is defined by flexion at the proximal inter-phalangeal (PIP) joint and hyperextension at the distal interphalangeal (DIP) joint due to disruption of the central slip of the extensor tendon. Swan neck deformity is defined by hyperextension at the PIP joint and flexion at the DIP joint, and the pathology is divided into intrinsic, extrinsic, and articular. The deformities are a result of imbalance of the tendons and ligaments in the fingers. Treatment is depending on the underlying cause and includes surgery and non-operative treat

Finger dislocations are common injuries that are often managed by emergency nurse practitioners. A systematic physical examination following these injuries is imperative to avoid complications. Radiographic views, including the anteroposterior, lateral, and oblique views, are imperative to evaluate these finger dislocations. A dorsal dislocation of the proximal interphalangeal (PIP) joint is the most common finger dislocation type often easily reduced. A volar PIP dislocation can often be difficult to reduce and may result in finger deformity. Finger dislocations should be reduced promptly. Re

Infections in the joints of the hand and wrist carry the risk of significant morbidity. Common presenting symptoms include joint redness, swelling, and pseudoparalysis that occurs several days following a penetrating trauma. Diagnostic workup should be expedited, including a laboratory evaluation and arthrocentesis. Imaging, including radiographs, ultrasound, computed tomography, and/or MRI, are helpful tools in diagnosis. Once infection is identified, prompt surgical debridement and antibiotics are required. Once the infection has been managed, hand therapy is initiated to decrease the risk o

OBJECTIVES: We report a retrospective series of 27 cases of composite island homodigital distal interphalangeal joint to replace proximal destroyed interphalangeal joint. Results of our series are compared with other procedures available in order to specify the place of pedicled articular transfers mainly for the proximal interphalangeal joint which is recognized as the most important joint in the finger.
METHODS: The series included 27 cases of destroyed proximal interphalangeal joints with preservation of the distal interphalangeal joint, the flexor tendon and vascular pedicles. The mean ag

BACKGROUND AND PURPOSE: Septic arthritis of finger joints is rare and its management not standardized. The outcome of all consecutive patients with finger joint arthritis was analyzed in terms of risk factors, surgical technique, antimicrobial therapy and hand therapy.
PATIENTS AND METHODS: Data of 31 patients with surgical treatment between 1993 and 2005 were screened and those from 29 patients with > 1 year of follow-up were retrospectively analyzed. Surgical techniques were debridement, primary, secondary arthrodesis or temporary joint distraction with external fixation. In addition, all p

Degenerative arthritis of the finger joints is a very common disease in middle-aged and elderly patients, in females more frequently than in males. Concerning differential diagnosis rheumatoid arthritis and related conditions, gout, calcinosis, tumors and tumor-like lesions must be considered. Therapy is not necessary in every case. Usually treatment is required if the disease causes pains, joint instability or disabling deformity. Conservative treatment such as diet, physiotherapy, orthotics, local physical and antiphlogistic drug treatment or systemic application of non-steroid antiphlogisti

Twenty-one patients who had received treatment for a locked metacarpophalangeal joint of an index finger were reviewed. Five cases were successfully treated with manipulation after an intra-articular injection of a local anesthetic, while the remaining 16 required open reduction. In the vast majority (15 of the 16 operated patients), the locking was attributed to catching of the radial accessory collateral ligament on a bony prominence of the metacarpal head of the finger. All patients who underwent manipulation, and 14 of the 16 patients who were operated on, regained full range of motion of

Calcium deposits close to finger joints are seen very often and are common in systemic diseases. There are also calcium deposits with no relation to other symptoms and therefore diagnosis is difficult. Between 1984-1988 we have analysed twelve such cases and explained the differential diagnosis and therapy. It seems important that these cases with typical clinical and radiologic findings are self-limiting and restitutio ad integrum is common without any therapy.

Selection of the reconstructive technique for the traumatized joint requires a careful consideration of the condition of the injured and adjacent joints, the needs and desires of the patient, and an understanding of the advantages and disadvantages of the available options. The MP joint is the key to a useful arc of motion, providing 77 per cent of the total arc of flexion. Every effort should be made to preserve its maximum pain-free movement. PIP joint motion, although important in maintaining grip strength, can more readily be sacrificed to provide stability when MP joint motion is normal.

Prompt recognition of the magnitude of articular damage is essential for optimal management of digital joint injuries. Basic to this concept is the need to differentiate injuries that can be successfully treated by closed techniques from those that require surgery. Following an accurate restoration of the damaged joint, recovery of function depends on careful immobilization in a protective position and, whenever possible, early joint mobilization. Precise primary care is the key to the prevention of disabling joint contractures.

Injuries of the finger joints with damage to the capsule and ligaments are quite common. Nevertheless early diagnosis may be missed since patients look for relief only after a period of longstanding pain. A careful clinical and X-ray diagnosis is the prerequisite for appropriate treatment. Surgery is still indicated for most of the injuries of the ulnar ligament of the MP-joint of the thumb. Sprains or dislocations of the middle joint of fingers are quite usual. In most instances they may be treated by immobilisation or functional treatment. Persistent subluxation after reduction of the joint

Injuries to the carpometacarpal joints are uncommon and are sometimes overlooked on initial radiographs. Their recognition depends on a careful physical and radiographic examination that may require trispiral tomograms and CT scans. Most injuries can be treated successfully by closed reduction and percutaneous fixation of the joint(s) using Kirschner wires. Chronic joint injuries can also be effectively treated, and if surgery is required, an arthrodesis will restore stability and eliminate discomfort.

Closed injuries affecting the metacarpophalangeal and interphalangeal joints and their stabilizers in the thumb and fingers occur very commonly in athletes, possibly leading to finger joint instability and long-standing or permanent disability. This article reviews the spectrum of joint injuries of the thumb and fingers that are common in the athletic population with a main focus on the ultrasound features of collateral ligament tears, palmar plate injuries, and thumb sesamoid fractures. A thorough understanding of the complex anatomy, mechanism of injury, soft tissue abnormalities, and imagin

Two patients with open complex dislocation of a metacarpophalangeal joint were treated by cleansing of the wound, open reduction, primary wound closure, and early motion. One dislocation occurred in the index finger, the other in the long finger which is only the second such case to be described. Antibiotics were given before and continued for 5 days after operation. There were no infections, and both patients regained an excellent range of motion without pain.

Metacarpophalangeal joint arthritis of the index finger is a debilitating disease often caused by osteoarthritis or inflammatory arthritides such as rheumatoid arthritis. Treatment options include nonsurgical management with nonsteroidal anti-inflammatory drugs, splinting, occupational therapy, corticosteroid injections, and disease-modifying antirheumatic drugs. Operative management options include arthrodesis and arthroplasty, which can be further broken down into silicone implants and 2 component resurfacing implants. The article summarizes the current literature for each of the treatment o
