Physiological, psychophysical, and psychological responses of firefighters to firefighting training drills.

This study was designed to describe the physiological, psychophysical, and psychological responses of firefighters to firefighting drills in a training structure containing live fires. Fifteen male firefighters, wearing standard turnout gear which resulted in full encapsulation, performed two firefighting tasks (advancing fire hose, chopping wood) while inside the training structure. Measurements of heart rate, tympanic membrane temperature, blood lactate, perceptions of respiration, mood, perceived exertion, and thermal sensation were obtained after 8 min of advancing fire hose, and again aft

Environmental study of firefighters.

A study of firefighter exposures was undertaken at the request of the U.S. Fire Administration. This work was part of a larger study which included field evaluation of the performance of the self-contained breathing apparatus (SCBA) worn by firefighters during structural firefighting. Measurements were made for a variety of contaminants including CO, CO2, benzene, HCN, HCl, H2SO4, HF, acrolein, CH4, formaldehyde and PNAs. Many of the analyses were performed by collection of bag samples followed by Fourier transform infrared spectroscopy using a field mobile spectrometer. Measurements were also

Firefighting Induces Acute Inflammatory Responses that are not Relieved by Aspirin in Older Firefighters.

OBJECTIVE: Sudden cardiac events account for 40% to 50% of firefighter line-of-duty deaths. Inflammatory proteins are strong biomarkers of cardiovascular inflammation. The present study investigated the effects of aspirin supplementation on inflammatory biomarkers following firefighting. METHODS: Using a randomized, placebo-controlled, double-blind crossover design, 24 male firefighters (48.2 ± 5.9 years) were allocated into four conditions: acute (81 mg; single-dose) aspirin and placebo supplementation, and chronic (81 mg; 14 days) aspirin and placebo supplementation. Inflammatory proteins [

Stage-resolved zinc isotope dynamics in firefighters' hair after firefighting.

Firefighting exposes firefighters to combustion-derived particulate matter enriched in hazardous metals, yet biomarkers capable of integrating repeated fire events and recovery under real exposure conditions remain limited. This study quantified hair metal concentrations and δ66Zn in 53 hair samples collected from 15 Beijing firefighters across five stage-defined periods spanning pre-mission, active firefighting, and recovery. Hair δ66Zn ranged from -0.79‰ to -0.13‰ (mean -0.44‰) and displayed a non-linear incident-recovery trajectory, enriched in the heavier Zn isotopes after repeated mission

Firefighters' Expertise in Locating Sounds.

BACKGROUND: Firefighters search for survivors in low-visibility environments and have documented expertise in spatial orientation and wayfinding. No studies so far have investigated their ability to make use of auditory spatial cues. METHODS: The performance of 41 professional firefighters and 24 control subjects to localise sounds played in a dark environment at 9 azimuthal positions was assessed using (i) meaningless single sounds and (ii) meaningful sounds presented with two simultaneous distractors. Localisation errors were analysed with a 3-way ANOVA Task × Position × Group. RESULTS: Th

Urinary biohazard markers in firefighters.

Firefighters are the professional force at high risk of suffering potential health consequences due to their chronic exposure to numerous hazardous pollutants during firefighting activities. Unfortunately, determination of fire emission exposure is very challenging. As such, the identification and development of appropriate biomarkers is critical in meeting this need. This chapter presents a critical review of current information related with the use of different urinary biomarkers of effect and exposure in occupationally exposed firefighters over the last 25 years. Evidence suggests that urin

Air contaminants encountered by firefighters.

The concentrations of eight air contaminants suspected of causing acute and chronic health problems for firefighters were measured in over 200 fires in the City of Boston using a personal air sampler. Threatening concentrations of both carbon monoxide and acrolein were found in a small proportion of the fires. Less hazardous levels of hydrogen chloride, hydrogen cyanide, nitrogen dioxide and carbon dioxide were also noted. Benzene was found in most fires, but at concentrations well below those expected to cause acute injury. The air sampling data have application in treatment of smoke inhalati

Occupational Contact Dermatitis in Firefighters.

Occupational contact dermatitis (OCD) emerges as a salient concern within the context of firefighters, a professional cohort routinely exposed to an array of hazardous substances as an intrinsic facet of their occupational responsibilities. This continual skin exposure to a spectrum of allergenic and irritant agents engenders an elevated predisposition to OCD among firefighters. Notably, the ramifications of OCD in the domain of occupational health assume substantial import, contributing significantly to the prevalence of work-related dermatological maladies and consequential productivity decr

Daytime sleepiness among Midwestern firefighters.

Excessive daytime sleepiness (EDS) is the most common symptom of sleep-disorders. Although high levels of EDS have been reported among firefighters, there is a limited understanding on EDS in firefighters. A cross-sectional survey was conducted in full-time firefighters (n = 275). The survey included the Epworth Sleepiness Scale to assess EDS, as well as other questions related to job characteristics, health history, and demographics. Firefighters reported inadequate sleep hours, extremely short sleep latencies, and high EDS higher than the general population. A majority of firefighters indica

Mercury toxicity in wildland firefighters.

OBJECTIVE: Recent studies have shown that significant levels of mercury are being released into the atmosphere by wildland fires throughout the United States. Eagle populations in the western United States are experiencing mercury toxicity in areas where many of these massive fires are occurring. We hypothesized that wildland firefighters exposed to smoke and other contaminates from these extensive fires would also reflect elevated blood levels of mercury. METHODS: Wildland firefighters in the western United States were studied during the summers of 2007, 2008, and 2009. Pre- and post-fire se

The Development of a Sleep Intervention for Firefighters: The FIT-IN (Firefighter's Therapy for Insomnia and Nightmares) Study.

Background: Firefighters are vulnerable to irregular sleep patterns and sleep disturbance due to work characteristics such as shift work and frequent dispatch. However, there are few studies investigating intervention targeting sleep for firefighters. This preliminary study aimed to develop and test a sleep intervention, namely FIT-IN (Firefighter's Therapy for Insomnia and Nightmares), which was based on existing evidence-based treatment tailored to firefighters in consideration of their occupational characteristics. Methods: This study implemented a single-group pre-post study design, utiliz

Perceived Health Risks Among Firefighters; The New Jersey Firefighter Health Survey.

OBJECTIVE: There is a growing literature on the risk of chronic disease among firefighters, including cardiovascular disease (CVD) and cancer. However there is little information on firefighter's perception thereof. METHODS: Firefighters attending a union convention in New Jersey completed a survey with four domains: firefighting experience; perceived additional risk for chronic diseases (six-point Likert scale); cancer screening history; demographics, and risk behaviors. RESULTS: Among 167 enrolled firefighters, all were men and 86.6% active career. Median perceived risk ranged from high ri

Effects of simulated firefighting and asymmetric load carriage on firefighter obstacle crossing performance.

Slips, trips, and falls (STF) of firefighters may occur while traversing stationary obstacles. STF risk may be amplified by fatigue from firefighting and carrying an asymmetric load. Vertical and horizontal clearances of the lead (VCL, HCL) and trailing (VCT, HCT) foot and contact with a 30 cm obstacle were examined in 24 firefighters. We examined the impact on obstacle crossing performance due to three exercise protocols (treadmill walking or simulated firefighting in an environmental chamber, and simulated firefighting in a live-fire burn building) and carrying a hose load on the right shoul

Accuracy of Tympanic Temperature Measurement in Firefighters Completing a Simulated Structural Firefighting Task.

INTRODUCTION: In the course of their duties, firefighters risk heat stroke and other medical conditions due to exertion in high-temperature environments. Infrared tympanic temperature measurement (TTym) is often used by Emergency Medical Services (EMS) to assess the core body temperature of firefighters. The accuracy of TTym in this setting has been called into question. Hypothesis/Problem This study aimed to examine the accuracy of TTym for core body temperature assessment at emergency firefighting events compared with gastrointestinal temperature measurement (TGI) as measured by ingestible t

Relationship between burnout and PTSD symptoms in firefighters: the moderating effects of a sense of calling to firefighting.

PURPOSE: Firefighting has been reported to lead to burnout and posttraumatic stress disorder (PTSD). However, burnout and PTSD symptoms may vary depending on personal characteristics, such as having a sense of calling. This study examined the role of calling in the association between burnout and PTSD symptoms. We hypothesized that burnout would be associated with more severe PTSD symptoms and calling would buffer the relationship between burnout and PTSD symptoms. METHODS: The Korean version of the Maslach Burnout Inventory-General Survey, Sense of Calling Subscale of the Professionalism Sca

Assessing gait changes in firefighters after firefighting activities and while carrying asymmetric loads.

One of the most common causes of injuries among firefighters is slips, trips, and falls on the fireground. Acute fatigue from firefighting activities and/or carrying asymmetric loads might impact gait characteristics increasing slip, trip, and fall risk. The purpose of this study was to examine the effects of fatigue from simulated firefighting activities and carrying asymmetric loads (fire hose over one shoulder) on firefighters' gait behavior. Both firefighting activities and asymmetric hoseload carriage led to shorter step lengths, stride lengths, single leg support time, and longer double

Serum PFAS levels in UK firefighters.

Per- and polyfluoroalkyl substances (PFAS) represent an emerging concern due to their persistence, bioaccumulation, and emerging associations with different cancers and other diseases. Biomonitoring studies worldwide report elevated PFAS serum concentrations in firefighters compared with other occupations and the general population. The aim of this study was to quantify PFAS serum concentration in current and former firefighters and a general population comparison group in the UK. PFAS serum concentrations were analysed in a total sample of 1224 participants, including 942 firefighters from ei

Cancer incidence and mortality among firefighters.

Firefighters are exposed to both known and suspected carcinogens. This study aims to systematically review the literature on the association of firefighting occupation and cancer incidence and mortality, overall and for specific cancer sites. A systematic review using PubMed, Embase, and Web of Science was performed up to January 1, 2018. We extracted risk estimates of cancers and calculated summary incidence risk estimates (SIRE), summary mortality risk estimates (SMRE), and their 95% confidence intervals (CI). Publication bias and risk of bias in individual studies were assessed using Begg's
