Ocular fixation with quadriceps tendon allograft.

Management of difficult strabismus, such as strabismus fixus and paralytic strabismus, in order to maintain the alignment is complicated. There are many surgical approaches described in the current literature, together with notes on the materials used to stabilize these deformities. We present a new surgical approach using quadriceps tendon allograft for the correction of difficult strabismus such as strabismus fixus and paralytic strabismus. Our idea for using deep frozen quadriceps tendons developed from the fact that this is the most stable and strongest tendon in the whole body. Six patien

Ocular fixation, vestibular dysfunction, and visual motion hypersensitivity.

BACKGROUND: A subgroup of individuals with vestibular dysfunction and visual motion hypersensitivity (VMH) become dizzy and imbalanced in response to movement of the visual environment. The purpose of this study was to investigate ocular fixational stability during gaze on a target, with and without visual background movement. Binocular vision functions were also examined as possible contributory factors to the dizziness and imbalance. METHODS: Twenty-four individuals with VMH, 20 with vestibular disorders without VMH and 20 healthy subjects were tested. Assignment to the experimental group w

Effect of ocular fixation on positional nystagmus in BPPV patients.

OBJECTIVE: The quantitative suppression rate of positional nystagmus (PN) by ocular fixation (OF) is unknown. This study aims to analyse the effect of OF on the slow phase velocity (SPV) of PN during diagnostic positional manoeuvres (DPMs) with videonystagmography in patients with benign paroxysmal positional vertigo (BPPV). DESIGN: DPMs were performed on 58 patients with BPPV, 33 (56.9%) of whom were women. OF was initiated when PN was most evident, and recording was continued. The mean SPV of three consecutive nystagmus before (F0) and after (F1) OF initiation was calculated. The rate of su

Assessing inter-ocular fixational eye movements throughout the lifespan.

This study aims to assess fixational eye movements (FEMs) obtained under binocular and monocular viewing in normal individuals across different age groups. We recruited 68 healthy participants divided into Group 1 (children, 3-9 years, n = 20), Group 2 (adolescents, 10-19 years, n = 26), and Group 3 (adults, 20-73 years, n = 22). FEMs were collected using a high-resolution video-based tracker under 3 viewing conditions: binocular viewing (BV), monocular viewing right eye (MV_RE), and monocular viewing left eye (MV_LE). We quantified fixation stability, the frequency, amplitude, and disconjugac

Ocular oscillations induced by shifts of the direction and depth of visual fixation.

Shifts of the point of fixation between two targets aligned on one eye that are located near and far (Müller paradigm) stimulates a combined saccadic-vergence movement. In normal subjects, this test paradigm often induces saccadic oscillations of about 0.3 degrees at 20 to 30 Hz. We measured eye movements using the magnetic search coil technique in 2 patients recovering from viral opsoclonus-myoclonus syndrome, comparing saccadic-vergence responses to the Müller paradigm with conjugate saccades between distant targets. Both patients exhibited intermittent conjugate ocular oscillations of about

Effects of fixation and optokinetic stimulation on vestibulo-ocular reflex suppression.

Suppression of the vestibulo-ocular reflex was assessed in normal subjects and patients with neurological disorders to determine the relative effects on suppression of a single fixation target and an optokinetic field. Subjects were rotated sinusoidally in yaw at varying frequencies of up to 0.5 Hz whilst seated in a Barany chair. A comparison was made between eye movements in darkness, those produced during fixation on a central target mounted to the chair, and eye movements during fixation on the target plus an "earth-fixed" or "chair-fixed" visual background. The effect of uncorrected ametropia on ocular torsion induced by changes in fixation.

BACKGROUND AND OBJECTIVE: Ocular torsion, the eye movements to rotating around the line of sight, has not been well investigated regarding the influence of refractive errors. The purpose of this study was to investigate the effect of uncorrected ametropia on ocular torsion induced by fixation distances. METHODS: Seventy-two subjects were classified according to the type of their refractive error, and ocular torsion of the uncorrected eye was compared based on changes induced by different fixation distances. Ocular torsion was measured using a slit-lamp biomicroscope equipped with an ophthalmi

Ocular fixations and presaccadic potentials to explain pareidolias in Parkinson's disease.

In Parkinson's disease, a precursor phenomenon to visual hallucinations presents as 'pareidolias' which make ambiguous forms appear meaningful. To evoke and detect pareidolias in patients, a noise pareidolia test was recently developed, although its task-dependent mechanisms are yet to be revealed. When subjected to this test, we hypothesized that patients exhibiting pareidolias would show altered top-down influence of visual processing allowing us to demonstrate the influence of pareidolic illusionary behaviour in Parkinson's disease patients. To that end, we evaluated eye-movement strategies

Trocar-assisted sutureless intrascleral posterior chamber foldable intra-ocular lens fixation.

PURPOSE: We report a novel technique characterized by sutureless scleral fixation of three-pieces foldable intraocular lens (IOL) using 25-gauge transconjunctival sutureless vitrectomy (TSV) trocars in patients with insufficient posterior capsule support. MATERIALS AND METHODS: We performed this technique on the eight eyes of the seven patients. The scleral tunnels (STs) are prepared by insertion of the 25-gauge TSV microcannulas using the trocars, and anterior vitrectomy is performed through the clear corneal paracentesis with the aid of anterior chamber maintainer (ACM). Finally, the three-

Visual vestibular interaction: vestibulo-ocular reflex suppression with head-fixed target fixation.

In order to maintain clear vision, the images on the retina must remain reasonably stable. Head movements are generally dealt with successfully by counterrotation of the eyes induced by the combined actions of the vestibulo-ocular reflex (VOR) and the opto-kinetic reflex. We have studied how, in humans, the VOR gain (VORG) is modulated to provide appropriate eye movements in two situations: 1. fixation of a stationary object of the visual space while the head moves. This requires a visuo-vestibulo-ocular reaction to induce eye movements opposite in direction, and equal in velocity to head move

Sutureless fixation of amniotic membrane using a soft ring for ocular surface diseases.

PURPOSE: To evaluate the efficacy of sutureless amniotic membrane transplantation using a soft bioavailable ring in the management of ocular surface disorders. METHODS: We present a modified technique using a soft, special designed bioavailable ring set (Amnioring) for sutureless amniotic membrane fixation to the ocular surface. A fresh amniotic membrane with a diameter greater than the outside diameter of the ring was folded inwardly over the ring such that the inner side of the ring was covered with the amniotic membrane. The ring was inserted onto the ocular surface. RESULTS: The ring was

Ocular fixation instabilities in motor neurone disease. A marker of frontal lobe dysfunction?

OBJECTIVE: Eye movements are classically felt to be spared in motor neurone disease (MND). Although a range of ocular motor disorders have been reported, no consistent pattern has been established. Disturbances of ocular fixation have been noted in MND; however, fixation has not yet been formally examined. With the recent characterization of ocular fixation using saccadic intrusion amplitude and fixation periods, we performed a cross-sectional study to examine for abnormalities of ocular fixation in non-dementing patients with MND. METHODS: A total of 44 patients and 45 controls were recruite

Eye-head movement coordination: vestibulo-ocular reflex suppression with head-fixed target fixation.

To maintain clear vision, the images on the retina must remain reasonably stable. Head movements are generally dealt with successfully by counter-rotation of the eyes induced by the combined actions of the vestibulo-ocular reflex (VOR) and the optokinetic reflex. A problem of importance relates to the value of the so-called intrinsic gain of the VOR (VORG) in man, and how this gain is modulated to provide appropriate eye movements. We have studied these problems in two situations: 1. fixation of a stationary object of the visual space while the head moves; 2. fixation of an object moving with

Influence of ocular gaze and head position on 4 m heterophoria and fixation disparity.

Four-meter heterophoria and fixation disparity measurements were taken from a sample of 40 young adults in primary gaze, 33 degrees left and right lateral gaze, and in 2 head/gaze positions related to golf putting. Comparisons using analysis of variance were made between the mean values in each gaze position and between the test-retest range of values in each gaze position. Mean heterophoria was more eso (p less than 0.05) in the two head/gaze positions related to golf putting than in other gaze positions, whereas mean fixation disparity was more eso (p less than 0.05) in rightward than in pri

Ocular lateral deviation with brief removal of visual fixation differentiates central from peripheral vestibular syndrome.

OBJECTIVE: Ocular lateral deviation (OLD) is a conjugate, ipsilesional, horizontal ocular deviation associated with brief (3-5 s) closing of the eyes, commonly linked to the lateral medullary syndrome (LMS). There is limited information regarding OLD in patients with the acute vestibular syndrome (AVS). In one case series 40 years ago OLD was suggested to be a central sign. Recently, horizontal ocular deviation on imaging (RadOLD) was frequently associated with anterior circulation stroke and horizontal gaze palsy. Similarly, RadOLD has been associated with posterior circulation stroke, e.g.,

Clinical Outcomes of Sutureless Scleral-Fixated Intra-ocular Lenses with and without Fibrin Glue.

PURPOSE: To compare the clinical outcomes of sutureless scleral-fixated posterior chamber intraocular lens (SFIOL) implantation performed with fibrin glue versus without fibrin glue in eyes with inadequate posterior capsular support. METHODS: This prospective, randomized, comparative clinical study included 40 eyes of 40 patients requiring secondary intraocular lens implantation. Patients were randomly allocated into two groups: Group A (n = 20) underwent glued sutureless SFIOL implantation using fibrin glue, and Group B (n = 20) underwent non-glued sutureless SFIOL implantation. Formalin Fixation and Cryosectioning Cause Only Minimal Changes in Shape or Size of Ocular Tissues.

Advances in imaging have made it increasingly common to study soft tissues without first embedding them in plastic or paraffin and without using labels or stains. The process, however, usually still involves fixation and cryosectioning, which could deform the tissues. Our goal was to quantify the morphological changes of ocular tissues caused by formalin fixation and cryosectioning. From each of 6 porcine eyes, 4 regions were obtained: cornea, equatorial and posterior sclera, and posterior pole containing the optic nerve head. Samples were imaged using visible light microscopy fresh, 1-minute
