Mechanical countermeasures to headward fluid shifts.

Head-to-foot gravitationally induced hydrostatic pressure gradients in the upright posture on Earth are absent in weightlessness. This results in a relative headward fluid shift in the vascular and cerebrospinal fluid compartments and may underlie multiple physiological consequences of spaceflight, including the spaceflight-associated neuro-ocular syndrome. Here, we tested three mechanical countermeasures [lower body negative pressure (LBNP), venoconstrictive thigh cuffs (VTC), and impedance threshold device (ITD) resistive inspiratory breathing] individually and in combination to reduce a pos

Space sickness and fluid shifts: a hypothesis.

In a sample of 64 first-time Space Shuttle crew members, 9 preflight variables related to fluid, electrolyte, and cardiovascular status were previously found to be significantly related to space sickness. The nine variables are serum uric acid, red cell count, environmental temperature at the launch site, serum phosphate, urine osmolality, serum thyroxine, sitting systolic blood pressure, calculated blood volume, and serum chloride. Using discriminant analysis, these preflight variables were used to correctly classify the 64 astronauts according to their space sickness incidence (NOTSICK or SI

Dynamic fluid shifts induced by fetal bypass.

OBJECTIVE: Fluid shifts have been suggested to occur with fetal bypass. The degree or mechanisms behind these volume changes (or location) have not been defined. We characterized the preceding and correlated the findings to plasma vasopressin concentrations, the critical peptide of osmoregulation. METHODS: Seventeen ovine fetuses (105-111 days' gestation) were started on bypass and followed 2 hours after bypass. Hemodynamics and volume replacements needed to maintain minimum reservoir volume during bypass and normal physiologic parameters after bypass were recorded. Serial blood samples were

Ubiquitin reduces fluid shifts after traumatic brain injury.

BACKGROUND: Ubiquitin has well-described intracellular properties. Recent data also suggest pleiotropic effects of extracellular ubiquitin, including induction of apoptosis, regulation of immune functions, and therapeutic potential during fluid resuscitation from severe trauma. However, the actions of exogenous ubiquitin after traumatic brain injury (TBI) are unknown. METHODS: Series 1: Thirty-five minutes after TBI and hemorrhage, 1.5 mg ubiquitin/kg (n = 5) or albumin (n = 5) intravenous was followed by fluid resuscitation to maintain mean arterial and cerebral perfusion pressure. Series 2:

Amniotic fluid volume changes in response to frusemide induced maternal fluid shifts.

Frusemide use in pregnancy is generally restricted to patients with cardiac decompensation. In the past, maternal administration of oral frusemide had been tried to identify non-functioning fetal kidneys by utilizing the diuretic effect of frusemide on the fetal kidneys. We report a case of primigravida with severe mitral stenosis presenting in pulmonary edema that received high dose of frusemide. This was associated with the development of transient oligohydramnios. We discuss the clinical significance of this observation and the possible explanations.

Venoconstrictive thigh cuffs impede fluid shifts during simulated microgravity.

BACKGROUND: This study determined the efficacy of venoconstrictive thigh cuffs, inflated to 50 mmHg, on impeding fluid redistributions during simulated microgravity. METHODS: There were 10 healthy male subjects who were exposed to a 2-h tilt protocol which started in the standing position, and was followed by 30 min supine, 30 min standing, 30 min supine, 30 min of -12 degrees head down tilt (HDT, to simulate microgravity), 15 min of HDT with venoconstrictive thigh cuffs inflated, a further 10 min of HDT, 5 min supine, and 10 min standing. To increase the sensitivity of the techniques in an E

Fluid Retention and Rostral Fluid Shift in Sleep-Disordered Breathing.

Sleep-disordered breathing (SDB) is common and adversely affects cardiovascular morbidity and mortality. Despite multifactorial pathogenesis, SDB is prevalent in patients with fluid retention disorders, such as drug-resistant hypertension, end-stage renal disease, and heart failure, suggesting that fluid retention may play a role in the pathogenesis of SDB. During the day, fluid is likely to accumulate in the legs, and upon lying down at night is displaced from the legs. Many data suggest that some of this fluid displaced from the legs may redistribute to the upper body and predispose to SDB.

Cardiovascular adaptations, fluid shifts, and countermeasures related to space flight.

Significant progress has been made related to understanding cardiovascular adaptations to microgravity and development of countermeasures to improve crew re-adaptation to gravity. The primary ongoing issues are orthostatic intolerance after flight, reduced exercise capacity, the effect of vascular-smooth muscle loss on other physiologic systems, development of efficient and low-cost countermeasures to counteract these losses, and an understanding of fluid shift mechanisms. Previous animal studies of cardiovascular adaptations offer evidence that prolonged microgravity remodels walls of blood v

Hydration and vascular fluid shifts during exercise in the heat.

This study examined the effects of hypohydration on plasma volume and red cell volume during rest in a comfortable (20 degrees C, 40% relative humidity) and exercise in a hot-dry (49 degrees C, 20% relative humidity) environment. A group of six male and six female volunteers [matched for maximal O2 uptake (VO2 max)] completed two test sessions following a 10-day heat acclimation program. One test session was completed when subjects were euhydrated and the other when subjects were hypohydrated (-5% from base-line body wt). The test sessions consisted of rest for 30 min in a 20 degrees C antecha

Brain distortion and cerebrospinal fluid shifts in spontaneous intracranial hypotension.

BACKGROUND: Spinal cerebrospinal fluid (CSF) leaks in spontaneous intracranial hypotension (SIH) are assumed to cause intracranial CSF volume changes and brain sagging. The spatial pattern of CSF and brain volume changes, or shifts, remains incompletely characterized. We aimed to quantify CSF and brain shifts before and after surgical closure of spinal CSF leaks. METHODS: We retrospectively analyzed 96 patients with SIH before and after surgical closure of their spinal CSF leaks. Artificial intelligence (AI)-assisted automated diffeomorphic registration and brain region segmentation of pre- a

Glycerol-induced fluid shifts attenuate the vestibulosympathetic reflex in humans.

The glycerol dehydration test (GDT) has been used to test for the presence of Ménière's disease and elicits acute alterations in vestibular reflexes in both normal and pathological states. Activation of the vestibulosympathetic reflex (VSR) increases muscle sympathetic nerve activity (MSNA) and peripheral vascular resistance. We hypothesized that the GDT would attenuate the VSR through fluid shifts of the inner ear. Sixteen male subjects (26 ± 1 yr) were randomly assigned to be administered either glycerol mixed with cranberry juice (97 ± 3 ml glycerol + equal portion of cranberry juice; n = 9

Physiological and behavioral effects of tilt-induced body fluid shifts.

This paper addresses the "fluid shift theory" of space motion sickness. The primary purpose of our research was the development of procedures to assess individual differences in response to rostral body fluid shifts on earth. Experiment I examined inner ear fluid pressure changes during head-down tilt in intact human beings. Tilt produced reliable changes. Differences among subjects and between ears within the same subject were observed. Experiment II examined auditory threshold changes during tilt. Tilt elicited increased auditory thresholds, suggesting that sensory depression may result from

Hemodynamic changes and fluid shifts after large-volume fluid infiltration: results from a porcine model.

While certain parameters such as blood loss and serum lidocaine levels following liposuction have been well studied, fluid shifts between the intravascular and extravascular space have not. With the advent of large volume liposuction, prudent fluid management has become obligatory. Hence, the reason for our study. To test the impact of large volume infiltration on intercompartmental fluid shifts, we measured urine output and hemodynamic changes in 10 anesthetized female Yorkshire pigs weighing between 50 and 85 kg. Eight pigs were infused with 5 to 10 L of tumescent fluid. Two pigs were anesthe

Fluid Shifts Induced by Physical Therapy in Lower Limb Lymphedema Patients.

Complete decongestive therapy (CDT), a physical therapy including manual lymphatic drainage (MLD) and compression bandaging, is aimed at mobilizing fluid and reducing limb volume in lymphedema patients. Details of fluid shifts occurring in response to CDT are currently not well studied. Therefore, we investigated fluid shifts before, during and after CDT. Thirteen patients (3 males and 10 females, aged 57 ± 8.0 years, 167.2 ± 8.3 cm height, 91.0 ± 23.4 kg weight) diagnosed with stage II leg lymphedema participated. Leg volume, limb and whole-body fluid composition (total body water (limbTBW/%T

Estimation of acute fluid shifts using bioelectrical impedance analysis in horses.

BACKGROUND: Multi-frequency bioelectrical impedance analysis (MF-BIA) has been used to evaluate extracellular fluid volume (ECFV), but not fluid fluxes associated with fluid or furosemide administration in horses. If able to detect acute changes in ECFV, MF-BIA would be useful in monitoring fluid therapy in horses. HYPOTHESIS: The purpose of this study was to evaluate the ability of MF-BIA to detect acute fluid compartment changes in horses. We hypothesized that MF-BIA would detect clinically relevant (10-20%) changes in ECFV. ANIMALS: Six healthy mares were used in the study. METHODS: This

Shifting subretinal fluid in rhegmatogenous retinal detachment.

In a consecutive series of 470 cases of rhegmatogenous retinal detachment 25 (5%) were found to have shifting subretinal fluid (SRF) at the preoperative examination. The study showed that the association between SRF and rhegmatogenous retinal detachment is unusual but not rare. Shifting SRF was most often associated with aphakic and longstanding retinal detachment, and found in cases in which the retinal holes were small.

Fluid polarity shifts initiate and amplify preferential flow in clay-rich media.

Preferential flow governs fluid and solute transport across scales from micropores to regional watersheds, yet it is commonly attributed to static pore-structure heterogeneity. Here, we show that fluid polarity can actively reorganize pore networks and amplify preferential flow in kaolinite-rich clay media. In permeation experiments, replacing water with a low-polar hydrofluoroether triggers early breakthrough (~0.4 d) and permeability up to ~62.7× higher than predicted by standard relative permeability functions at only 18.8% low-polar saturation. Multiscale imaging and porosimetry show a tra
