Custom Ankle-Foot Orthosis in Foot Drop: Influence on Body Joints Kinematics.

Ankle Foot Orthoses (AFOs) are external supports typically prescribed in clinical practice to address foot drop deficits. Lower limb orthoses have been shown to have positive effects on spatio-temporal gait metrics, while the impact on body joint kinematics is less clear and varies in the literature. The objective of this study is to investigate the presence of a common movement pattern in body joints that compensates for the foot drop deficit. The study focuses on the trunk, knee, and hip joints. Eight patients with unilateral foot drop participated in walking trials on an instrumented treadm

Kinetic coupling in distal foot joints during walking.

BACKGROUND: Kinematic coupling between the first metatarsophalangeal (MTP) and midtarsal joints is evident during gait and other movement tasks, however kinetic foot coupling during walking has not been examined. Furthermore, contributing factors to foot coupling are still unclear. Therefore, the purpose of this study was to investigate kinematic and kinetic coupling within the foot by restricting MTP motion during overground walking. We hypothesized that when the MTP joint was prevented from fully extending, the midtarsal joint would achieve less peak motion and generate less positive work co

Effect of static foot posture on the dynamic stiffness of foot joints during walking.

BACKGROUND: The static foot posture has been related to the development of lower limb injuries. RESEARCH QUESTION: This study aimed to investigate the dynamic stiffness of foot joints during gait in the sagittal plane to understand the role of the static foot posture in the development of injuries. METHODS: Seventy healthy adult male subjects with different static postures, assessed by the Foot Posture Index (FPI) (30 normal, 20 highly pronated and 20 highly supinated), were recruited. Kinematic and kinetic data were recorded using an optical motion capture system and a pressure platform, an

Deformable foot orthoses shift the functional roles of foot and ankle joints during walking gait.

Carbon fiber deformable foot orthoses (DFOs) with customizable stiffness may benefit clinical populations by allowing an individual approach to enhancing gait energetics. Prior research suggests that increasing DFO stiffness can decrease energy loss while enhancing energy return at the metatarsophalangeal (MTP) joint, making it more spring-like. However, it remains unclear whether increasing DFO stiffness enhances the mechanics of the midtarsal and ankle joints or only the MTP joint, especially during walking. This study investigated whether walking with increasingly stiff DFOs impacted mechan

Small Joint Arthroscopy of the Foot.

Recently, there have been new advances in the arthroscopic approach to small joints of the foot. This is directly related to the improvement of surgical equipment, new techniques, and publications. These improvements led to broadening indications as well as minimizing complications. Several articles recently presented the uses of arthroscopic surgery in the small joints of the foot; however, the use is still relatively limited. Arthroscopic evaluation of the small joints of the foot includes the first metatarsophalangeal, lesser metatarsophalangeal, tarsometatarsal, talonavicular, and calcaneo

Total joint arthroplasty. The foot.

A prosthesis has been designed with which to resurface the painful, degenerated first metatarsophalangeal joint. Clinical evaluation thus far has indicated satisfactory results in 16 of 18 instances. This procedure will probably give the best results in the older patient with hallux rigidus, but the length of follow-up is as yet too short to allow firm opinions to be formed.

Differences in the coordination and its variability among foot joints during running in neutral foot and flatfoot.

Flatfoot is a well-known foot deformity, with a prevalence of 11.2%-29.0% among adults. Running injuries can occur in individuals with flatfoot; however, the underlying mechanism remains unknown. We investigated the coordination pattern and variability among foot joints while running by comparing participants with neutral foot and with flatfoot. Participants with neutral foot (n = 15) and flatfoot (n = 15) were asked to run at their preferred speed. Using the modified vector coding technique, the coupling angle between the foot joints, representing interjoint coordination, was calculated and c

A Bio-Inspired Arched Foot with Individual Toe Joints and Plantar Fascia.

This paper presents the design and testing of an arched foot with several biomimetic features, including five individual MTP (toe) joints, four individual midfoot joints, and plantar fascia. The creation of a triple-arched foot represents a step further in bio-inspired design compared to other published designs. The arched structure creates flexibility that is similar to human feet with a vertical deflection of up to 12 mm. The individual toe joints enable abduction-adduction in the forefoot and therefore a natural pronation motion. Adult female bone data was obtained and converted into a CAD

Variability of the Dynamic Stiffness of Foot Joints: Effect of Gait Speed.

BACKGROUND: Comparison of dynamic stiffness of foot joints was previously proposed to investigate pathologic situations with changes in the properties of muscle and passive structures. Samples must be controlled to reduce the variability within groups being compared, which may arise from different sources, such as gait speed or Foot Posture Index (FPI). METHODS: Variability in the measurement of the dynamic stiffness of ankle, midtarsal, and metatarsophalangeal joints was studied in a controlled sample of healthy men with normal FPI, and the effect of gait speed was analyzed. In experiment 1,

The diabetic foot: Charcot joint and osteomyelitis.

Foot problems are common causes of disability in diabetic patients with as many as 25% expected to develop severe foot or leg problems during their lifetimes. Although skin ulceration is the most frequent problem, bones may also be involved in two different clinical conditions: osteomyelitis and Charcot osteoarthropathy. Osteomyelitis causes complications in up to one third of diabetic foot infections and is due to direct contamination from a soft-tissue ulcer. Osteoarthropathy Charcot foot is a chronic and progressive disease of the bone and joints. Both osteomyelitis and Charcot joint are co

[Autoplastic foot-to-hand joint transplantations].

Our surgical experience in the field of total and partial foot-to-hand joint transplants has evolved over the past 9 years (1973). In the case of serious traumatic injury of finger joints of the hand, we don't suggest any surgical solution other than that of arthrodesis or arthroplasty of the damaged joint. In our experience joint transplantation is a reconstructive operation of the whole functionality of the injured finger. This operation partially restores the active function of the finger involved and gives both the patient and the surgeon the possibility of an alternative to arthrodesis, a

Surgical Treatment of Gout in Interphalangeal Joints of the Foot: A Case Series.

Gout is a common inflammatory arthritic condition, commonly affecting the first metatarsophalangeal joint (MPJ) in the foot. As reported in literature, only 6.8% of patients with pedal gout have the interphalangeal joints affected. Surgical treatment of gouty tophi in the foot is at times necessary in addition to diet modification and oral medications. We report on 7 cases of gout affecting interphalangeal joints that necessitated surgical treatment. Six out of 7 of the patients had no complications after surgery, and no reoccurrence, with an average follow-up time of 26 months.

Obesity Impacts Swelling of Ankle and Foot Joints in Early Rheumatoid Arthritis Patients.

OBJECTIVE: The evaluation of disease activity in obese rheumatoid arthritis (RA) patients presents challenges particularly in the clinical assessment of swollen joints. This study examines the effect of obesity on the American College of Rheumatology (ACR) core set measures used in assessing RA disease activity with specific focus on the swollen joint count (SJC). METHODS: We examined a cross-sectional cohort of 323 early seropositive RA patients (symptom duration ≤15 months). Patients were biologic-naive with equal to or more than 6/44 SJC and equal to or more than 9/44 tender joint count. T

[Bilateral arthrodesis of the medial foot joints in a patient with rheumatoid arthritis].

Patients with rheumatoid arthritis (RA) often have foot problems. The subtalar and particularly talonavicular joints are affected most frequently. The posterior tibial tendon has an important role in mid-foot stability. In RA patients, chronic inflammation of this tendon or talonavicular joint arthritis can results in posterior tibial tendon rupture. This leads to a collapsed talonavicular joint and forefoot instability, first with talonavicular and later Chopart's joint involvement. This shows as a planovalgus foot, with the forefoot in pronation and the heel in valgus deviation. In a 61-year

Osteoarthritis in other joints (hip, elbow, foot, ankle, toes, wrist) after sports injuries.

Osteoarthritis of nonknee joints, although less common than knee osteoarthritis, remains a significant and disabling condition for many present and former athletes. These injuries can be caused by repeated loads or following a specific traumatic event. The resulting pain and loss of motion can limit function and ability. Arthroscopic techniques in many cases enable surgeons to symptomatically treat limitations of range of motion and pain, prolonging active careers. Joint replacement remains the ultimate solution for hip osteoarthritis, and may be a viable option in ankle osteoarthritis.
