[Jugular Foramen, Foramen Magnum Meningiomas].

The jugular foramen, also known as the foramen magnum, is a highly intricate region of the skull base through which numerous critical blood vessels and nerves traverse. Meningiomas, the most common tumors in neurosurgical pathology, can arise at any location where the meninges are present, posing significant challenges. Meningiomas involving the jugular foramen and sublingual neural tube are particularly notable for their potential to extend from intracranial to extracranial sites, necessitating familiarity with extracranial anatomy, which is not typically encountered in clinical practice. Foramen magnum meningiomas.

The foramen magnum meningioma (FMM) is one of most challenging tumors among all the meningiomas because of its distinctive location, clinical course, insidious onset, and the relative large size of the tumor during presentation. These tumors are slow-growing and clinical symptoms vary according to involvement of different structures including the spinal cord, vertebral artery, lower cranial nerves, and medulla oblongata. Controversies regarding appropriate surgical approach for resection of tumor in this unique location continue. Tumors in the posterior or posterolateral region of the foramen

[Proposal for the definition of "foramen magnum syndrome"--foramen magnum tumor and abnormalities].

The clinical appearance of foramen magnum tumor is protean and, even at the stage when serious neurological deficits are present, the lesions are often misdiagnosed as another disease, especially cervical spondylosis and multiple sclerosis, and patients may undergo improper concervative or even surgical treatments. The best guarantee against misdiagnosis, we believed, was to establish a definition of "Foramen Magnum Syndrome" to facilitate the recollection of its peculiar clinical findings. "Foramen Magnum Syndrome" is composed of: 1. Cape distribution of sensory loss; 2. Atrophy of the intrin

The foramen magnum in scaphocephaly.

PURPOSE: The foramen magnum (FM) presents various alterations in craniosynostoses, such as brachycephaly or Crouzon syndrome. However, to date, no study has been devoted to its morphology and morphometry in scaphocephaly, which is the most common of cranial deformities resulting from premature fusion of cranial sutures. METHODS: We assessed the morphology and morphometry of FM using preoperative thin-cut CT scans of 107 children with non-syndromic sagittal craniosynostosis aged 1-12 months (mean age 5.38 months). A series of sagittal and transverse dimensions were taken and the FM area was ca

Foramen magnum metastatic malignant melanoma.

The foramen magnum as a site for brain metastasis is extremely rare. We report the case of a 24-year-old male who presented with features of increased intracranial pressure and lower cranial nerve palsies. Imaging revealed a foramen magnum tumor with extension up to the cerebellar vermis superiorly and into the spinal canal inferiorly (craniospinal mass) with mild obstructive hydrocephalus. A malignant melanoma was completely excised. The patient continued to be tumor-free 24 months later.

Growth of the foramen magnum in achondroplasia.

Foramen magnum growth curves in achondroplasia and in the general population are presented. The achondroplastic foramen magnum is small at birth, and during the first year it has a severely impaired rate of growth especially in the transverse dimension. This markedly diminished growth results not only from abnormal endochondral bone growth but also because of abnormal placement and premature fusion of the synchondroses. Evaluation of the foramen magnum in achondroplasia should address absolute size of the transverse and sagittal dimensions, shape, and growth centers to determine growth potenti

[Tumor in the region of foramen magnum].

Twenty-five patients with tumors in the region of foramen magnum confirmed by pathology and surgery are presented. There were 19 males and 6 females. The ages ranged from 4-56 years. According to the clinical findings, the authors suggest that if a patient has occipitocervical pain or headache accompanied with neurological signs of medulla, cervical spinal cord or lower cranial nerves, tumors in the region of foramen magnum should be considered. The authors emphasize that a total removal of the extramedullary tumor is the best choice. The initial symptoms and signs, cases misdiagnosed and surg

Foramen Magnum Meningioma: Far Lateral Approach.

We present a case of a foramen magnum meningioma in a 42-year-old female who presented with headaches for 2 years, associated with decreased sensation and overall feeling of "heaviness" of the right arm. The tumor posed significant mass effect on the brainstem, and given the location of the tumor in the anterolateral region of the foramen magnum, a right far lateral approach was chosen. The approach incorporates the interfascial dissection technique to safely expose and preserve the vertebral artery in the suboccipital triangle. After drilling the posteromedial portion of the occipital condyle

Classification system of foramen magnum meningiomas.

BACKGROUND: Foramen magnum meningiomas (FMMs) are challenging tumors. We report a classification system based on our experience of 107 tumors. MATERIALS AND METHODS: The three main algorithm criteria included the compartment of development of the tumor, its dural insertion, and its relation to the vertebral artery. RESULTS: The compartment of development was most of the time intradural (101/107, 94.4%) and less frequently extradural (3/107, 2.8%) or both intra-extradural. (3/107, 2.8%). When developed inside the intradural compartment, FMMs were subdivided into posterior (6/104, 5.8%), later

[Anterior and lateral foramen magnum meningiomas].

We report our experience with 11 cases of foramen magnum meningiomas, eight originating inside the posterior fossa and three in the caudal region. The mean age of the patients was 50.8 years and the main complaint was cervical headache for at least 18.6 months and at the neurological examination, tetraparesis and deficit of the lower cranial nerves were very often observed. All patients were submitted to surgical treatment, always with exposition of the vertebral artery at the entry zone in the dura mater of the posterior fossa, with partial removal of the occipital condyle in only three cases

Foramen magnum stenosis in homozygous achondroplasia.

A 4-month-old female with homozygous achondroplasia and daytime apnea was found to have a small foramen magnum by computerized tomography. Following suboccipital craniectomy and C1-C2 laminectomy respiratory problems did not recur during a 7 month post-operative period. We suggest that the respiratory problems in homozygous achondroplasia may be due to brainstem compression from an abnormally small foramen magnum.

Foramen magnum position in bipedal mammals.

The anterior position of the human foramen magnum is often explained as an adaptation for maintaining balance of the head atop the cervical vertebral column during bipedalism and the assumption of orthograde trunk postures. Accordingly, the relative placement of the foramen magnum on the basicranium has been used to infer bipedal locomotion and hominin status for a number of Mio-Pliocene fossil taxa. Nonetheless, previous studies have struggled to validate the functional link between foramen magnum position and bipedal locomotion. Here, we test the hypothesis that an anteriorly positioned fora

Basilar Invagination: A Tilt of the Foramen Magnum.

OBJECTIVE: Congenital basilar invagination (BI) is a craniocervical deformity marked by odontoid prolapse into the skull base. The foramen magnum angle (FMA), which is formed by the Chamberlain's line and McRae's line, has not been fully studied. The study aimed to investigate the FMA and its relationship with other craniocervical parameters. METHODS: Participants were divided into control, type A BI, and type B BI groups. Parameters included Chamberlain line violation, atlantodental interval, clivus height, clivus anteroposterior dimension, FMA, basal angle, clivo-axial angle, head and neck

The foramen magnum in isolated and syndromic brachycephaly.

BACKGROUND: Though the foramen magnum (FM) is often altered in complex craniosynostosis, no study analysed the FM dimensions in patients with brachycephaly specifically. PATIENTS AND METHODS: We measured the FM area, sagittal and transverse diameters on preoperative CT scans in patients with bicoronal synostosis (n = 40) and age-matched control group (n = 18). Our study included 16 children with FGFR3 p.Pro250Arg mutation (mean age 6.1 months), 10 with TWIST-1 mutation (mean age7.6 months) and 14 patients with isolated bicoronal synostosis (mean age 6.1). RESULTS: We observed a significantly

The growth of the foramen magnum in Crouzon syndrome.

BACKGROUND: Though the craniovertebral junction is often abnormal in children with Crouzon's syndrome, no study had measured accurately the size of their foramen magnum (FM). PATIENTS AND METHODS: We compared the FM size (area, diameters) on computed tomography examination in 21 children with a genetically confirmed Crouzon's syndrome prior to any surgery and in 23 control children without craniofacial abnormalities. We extrapolated the growth pattern in both groups. RESULTS: We found a statistically significant smaller FM area (p=0.0228), FM sagittal diameter (p=0.0287), and FM sagittal pos

Meningiomas of the cerebellopontine angle and foramen magnum.

The most commonly occurring meningiomas in the posterior cranial fossa are located along the posterior surface of the temporal bone in the region of the cerebellopontine angle. Topics discussed in this article include clinical features, radiologic findings, surgical techniques, postoperative care, alternative treatments, and meningiomas of the foramen magnum. The author underscores the need for early diagnosis of these lesions so that surgical intervention can result in optimal chances for recovery.
