Foraminal stenosis frequently causes radiculopathy in lumbar degenerative spondylosis. Endoscopic transforaminal techniques allow for foraminal access with minimal tissue disruption. However, the effectiveness of foraminal decompression by endoscopic techniques has yet to be studied. We evaluate radiographic outcome of endoscopic transforaminal foraminotomies performed at L3-L4, L4-L5, and L5-S1 on cadaveric specimens. Before and after the procedures, three dimensional CT scans were obtained to measure foraminal height and area. Following the foraminotomies, complete laminectomies and facetect

Unilateral cervical nerve root compression causing radiculopathy, which does not improve with conservative measures, is safely and effectively treated with surgery. Both anterior and posterior approaches have been described. Overall results from either of these broad categories of approaches are equivalent. Posterior approaches target the cervical root compression directly and allow decompression by widening the neural foramen and/or removing a lateral disc fragment. Following success of the open technique, variations of this technique were introduced to minimize approach-related complications

Lumbar radiculopathy is a common problem. Nerve root compression can occur at different places along a nerve root's course including in the foramina. Minimal invasive approaches allow easier exposure of the lateral foramina and decompression of the nerve root in the foramina. This video demonstrates a minimally invasive approach to decompress the lumbar nerve root in the foramina with a lateral to medial decompression. The video can be found here: http://youtu.be/jqa61HSpzIA.

OBJECT: Anterior cervical foraminotomy has been advocated as a procedure that preserves the motion segment while treating radiculopathy due to degenerative cervical disc disease. Because the medical literature contains no long-term follow up or randomized studies related to this procedure, the authors reviewed their results, specifically examining cases of failure to determine the efficacy of the approach.
METHODS: The authors identified 23 patients in whom unilateral cervical radiculopathy due to degenerative cervical disc disease was refractory to conservative therapy and in whom anterior c

The minimally invasive posterior cervical foraminotomy procedure has become a common and successful procedure for the treatment of cervical radiculopathy. Specifically, the minimally invasive approach allows for decreased postoperative pain, blood loss, and length of hospitalization by preserving the surrounding soft tissue as compared with the traditional open approach. This article and accompanying video demonstrates the technique for a primary, single-level minimally invasive posterior cervical foraminotomy as performed through a tubular retractor.

INTRODUCTION: In this study, we present "microscopic mini-open foraminotomy (MicroMOF)" as a new, less invasive method for posterior cervical foraminotomy.
TECHNICAL NOTE: Using surgical microscope guidance, the spinous process is hemi-split, and the posterior muscle is detached from the laminar subperiosteal surface. Bony resection is then performed obliquely from medial posterior to lateral anterior under lateral-tilted surgical field exposure. MicroMOF has been performed successfully for cervical radiculopathy and cervical spondylotic amyotrophy, which achieved postoperative symptom improv

BACKGROUND: Cervical spine pathologies causing nerve-root compression are treated with use of decompression through a variety of techniques. Although several operative management options exist, cervical foraminotomy may be indicated in patients with isolated, unilateral cervical radiculopathy. Biportal endoscopic cervical foraminotomy allows decompression with minimal soft-tissue disturbance and excellent visualization.
DESCRIPTION: The procedure is performed with the patient in a prone position with routine neuromonitoring. Intraoperative fluoroscopy is utilized to identify the operative lev

STUDY DESIGN: Retrospective study.
OBJECTIVE: To compare clinical and radiological outcomes after two surgical procedures.
SUMMARY OF BACKGROUND DATA: Anterior cervical discectomy and fusion is the gold standard treatment for cervical radiculopathy. Cervical foraminotomy is an alternative decompressive treatment option to preserve segmental motion and avoid fusion-related complications. Anterior cervical foraminotomy (ACF) and posterior cervical foraminotmy (PCF) has been introduced to achieve foraminal decompression. The objective of this study was to compare long-term clinical and radiolog

BACKGROUND: Open posterior cervical foraminotomy has been shown to be comparable with anterior cervical discectomy and fusion (ACDF) in the treatment of cervical radiculopathy1,2. More recently, posterior endoscopic cervical foraminotomy was described as an ultra-minimally invasive technique that allows for neural decompression in cervical radiculopathy. This technique has been shown to have excellent clinical outcomes with a short length of hospital stay and low postoperative pain levels3.
DESCRIPTION: The procedure is performed with the patient in the prone position under general anesthesia

BACKGROUND: Posterior cervical foraminotomy is a long utilized and commonly performed procedure, but has been supplanted in many cases by anterior procedures. With the advent of minimally invasive techniques, posterior foraminotomy may again deserve a prominent place in the treatment of cervical foraminal stenosis.
OBJECTIVE: To report in detail a successfully utilized minimally invasive technique and the results in a large series of patients, by a single author.
METHODS: The technique is described and illustrated in detail. A retrospective review of the use of this technique in a large seri

STUDY DESIGN: A clinical series of patients with unilateral radiculopathy treated with the anterior cervical foraminotomy procedure.
OBJECTIVE: To establish procedural techniques and clinical and radiologic outcomes for the anterior cervical foraminotomy procedure.
SUMMARY OF BACKGROUND DATA: Cervical radiculopathy is typically caused by unilateral disc herniation or uncovertebral osteophytes that compress the ventral aspect of the nerve. Direct removal of a cervical lesion causing radicular symptoms without concomitant fusion seems to be an ideal treatment in selected patients. The indicati

PURPOSE: This study investigated clinical outcomes of posterior cervical foraminotomy (PCF) in patients who previously underwent cervical disc arthroplasty (CDA) at the same level. As CDA becomes more widely adopted and indications expand, a subset of patients will inevitably experience postoperative symptoms. PCF may provide symptomatic relief for persistent or recurrent radiculopathy without the need for revision arthroplasty or conversion to fusion.
METHODS: A multicenter retrospective chart review was conducted, identifying a consecutive series of patients (n = 17) who received same-level

BACKGROUND: Radiculopathy caused by lumbar foraminal stenosis in older people has become more common in the aging general population. However, patients aged ≥80 years rarely undergo conventional open surgery under general anesthesia because of the high risk of peri-operative morbidity and adverse events. Therefore, less invasive surgical alternatives are needed for older or medically handicapped patients. Transforaminal endoscopic lumbar foraminotomy (TELF) under local anesthesia may be helpful in at-risk patients, although only limited information is available regarding the clinical outcomes
