Quantifying rearfoot-forefoot coordination in human walking.

A method is proposed to facilitate the quantification and interpretation of inter-joint/-segment coordination. This technique is illustrated using rearfoot-forefoot kinematic data. We expand existing vector coding techniques and introduce a set of operational terms through which the coordinative patterns between the rearfoot segment and the forefoot segment are summarized: in-phase, anti-phase, rearfoot phase and forefoot phase. The literature on foot mechanics has characterized the stable foot at pushoff by a decreasing medial longitudinal arch angle in the sagittal plane, which is accompanie

Cross-sectional geometry of the human forefoot.

The human forefoot presents an interesting biomechanical problem of clinical importance because loads are distributed unequally across multiple bones. The fact that long bones typically have a several-fold safety factor relative to peak loads suggests that metatarsal strengths should be related to their peak loads. This study is the first to systematically examine the cross-sectional geometric properties of the human forefoot and their relationship to external loads during walking and running. We report midlength cross-sectional geometric properties (CA, Ix, Iy, Imax, Imin, J, Zx, and Zy) of m

Metabolic and mechanical aspects of foot landing type, forefoot and rearfoot strike, in human running.

The study was undertaken to assess the metabolic and the mechanical aspects of two different foot strike patterns in running, i.e. forefoot and rearfoot striking (FFS and RFS), and to understand whether there is some advantage for a runner to use one or the other of the two landing styles. Eight subjects performed two series of runs (FFS and RFS) on a treadmill at an average speed of 2.50, 2.78, 3.06, 3.33, 3.61, 3.89, 4.17 m s-1. Step frequency, oxygen uptake, mechanical work, and its two components, external and internal, were measured. No differences were found for step frequency, mechanica

Vibrotactile Presentation of Musical Notes to the Glabrous Skin for Adults with Normal Hearing or a Hearing Impairment: Thresholds, Dynamic Range and High-Frequency Perception.

Presentation of music as vibration to the skin has the potential to facilitate interaction between musicians with hearing impairments and other musicians during group performance. Vibrotactile thresholds have been determined to assess the potential for vibrotactile presentation of music to the glabrous skin of the fingertip, forefoot and heel. No significant differences were found between the thresholds for sinusoids representing notes between C1 and C6 when presented to the fingertip of participants with normal hearing and with a severe or profound hearing loss. For participants with normal h

Hallux valgus.

Hallux valgus is a complex deformity of the forefoot. Beside the lateral deviation of the first metatarsophalangeal joint angle exceeding 15-20 degrees and intermetatarsalvarus exceeding 8-9 degrees, it is also characterized by extensive changes of the soft tissue arch, sesamoid mechanism and metatarsocuneiform joint. It occurs almost exclusively in humans wearing shoes. Although in some measure it can be treated non-operatively, by corrective separators and inserts, operative treatment is predominant. There are numerous operative procedures aimed at the correction of the deformity and inducin

[Risk factors associated to musculoskeletal pathologies in athletes with cavus foot through baropodometric studies].

INTRODUCTION: The cavus foot is a musculoskeletal pathology with an increase of the medial arch of the concavity of the foot. The etiology of the cavus foot is still enigmatic, it is related with neurologic conditions, Charcot-Marie-Tooth disease, Friedreich's ataxia, and cerebral palsy. The aim of this research was to analyze the plantar pressure distribution of the feet on young athletes with cavus foot. MATERIAL AND METHODS: Eighty-three young athletes between nine and 20 years old, that presented cavus feet pathology were recruited. The mass and height average were 56.9 ± 12.36 kg and 1.6

Minimalist shoes running intervention can alter the plantar loading distribution and deformation of hallux valgus: A pilot study.

BACKGROUND: Hallux valgus (HV) is one common deformity of the human foot. Metatarsal pain, gait deviation and foot function disorder may occur when HV is severe. RESEARCH QUESTION: It was hypothesized that a 12-week minimalist shoes running intervention among mild and moderate hallux valgus males may alter foot morphology and plantar pressure distribution during walking and running. METHODS: The foot morphology and plantar pressure data, in this study, were collected from eleven participants using the Easy-Foot-Scan (EFS) and Novel EMED force plate. RESULTS: Compared to pre-intervention ses

Tuberculosis of the foot as the initial manifestation of acquired immune deficiency syndrome: a report of two cases.

Two cases of tuberculosis of the forefoot are presented to highlight a growing threat in our Western communities. Each represents an initial manifestation of occult infection with the human immunodeficiency virus. Delays in diagnosis and treatment existed because of equivocal and nonspecific clinical, radiographic, and laboratory findings. It is imperative that tubercular infection be considered when evaluating often nonspecific lesions of the foot, particularly in the immunocompromised population.

Forefoot injuries in dancers.

Dancers, particularly ballet dancers, are artists and athletes. In dance, the choreographer acts as a sculptor, using the dancer as a medium of expression. This often entails placing the dancer's body in positions that require extraordinary flexibility and movement, which requires controlled power and endurance. Ballet and other forms of dance can be highly demanding activities, with a lifetime injury incidence of up to 90%. Ballet is stressful particularly on the dancer's forefoot. The en pointe position of maximal plantarflexion through the forefoot, midfoot, and hindfoot requires tremendous

Current concepts in curative surgery for diabetic forefoot ulcers.

Forefoot ulcerations in patients with diabetes are quite common. Underlying mechanical deformities of the foot in combination with neuropathy are the most important risk factors for ulcer development and adequate offloading is the mainstay of management. Most ulcers heal with local wound care, adequate blood supply, and pressure relief. If a foot deformity cannot be accommodated, ulcers will not heal or may recur. In this case, surgical correction of deformity is necessary. This paper reviews the most common procedures supported by medical evidence to heal neuropathic forefoot ulcers.

Forefoot arthroplasty in the arthritic patient.

The forefoot arthroplasty procedure is frequently used in the patient with inflammatory arthritis. There is a multitude of considerations, both local and systemic, in these patients. As with other surgical procedures, it is important that the surgeon evaluate the patient as a whole. Each case should be evaluated individually and modifications made as necessary. There are several modifications of the original technique as described by Hoffman. Ultimately, the surgeon must select which variation works best in his or her hands. This procedure works extremely well when the patients are carefully s

On the relationship between tibia torsional deformation and regional muscle contractions in habitual human exercises in vivo.

The mechanical relationship between bone and muscle has been long recognized. However, it still remains unclear how muscles exactly load on bone. In this study, utilizing an optical segment tracking technique, the in vivo tibia loading regimes in terms of tibia segment deformation in humans were investigated during walking, forefoot and rear foot stair ascent and running and isometric plantar flexion. Results suggested that the proximal tibia primarily bends to the posterior aspect and twists to the external aspect with respect to the distal tibia. During walking, peak posterior bending and pe

Systemic Exophiala equina infection in an Eastern box turtle (Terrapene carolina carolina): a case report and literature review.

Phaeohyphomycosis is an infection caused by melanized fungi. This disease has been reported in several animal species including invertebrates, cold-blooded vertebrates, mammals, and humans. Melanized fungi have similar phenotypical features and confirmation requires culture and molecular diagnostics. To exemplify this we present a case of a 333 g adult of unknown age, free-ranging, male Eastern box turtle (Terrapene carolina carolina) that was referred to the Turtle Rescue Team at North Carolina State University for evaluation of multilobulated masses occupying the entire left orbit and at the

Pediatric forefoot pathology.

Surgical management of the pediatric forefoot often brings challenges to the foot and ankle surgeon. It requires a thorough understanding of the pathologic abnormality and underlying causes involved, which include the contributing genetic conditions. Albeit most of the deformities carry a rare level of incidence, they do however have a significant level of psychological component and stress on the pediatric patient. The goals of managing those pathologic abnormalities are ultimately to improve function while achieving a cosmetically acceptable outcome. The common forefoot pathologic abnormalit

[What's new in forefoot surgery].

New implants and instruments have recently emerged in foot surgery. However, an additional and important development is the technique of minimally invasive surgery. As a result of new surgical approaches some common foot deformities can be corrected in a percutaneous manner. The benefits include improved wound healing, less pain, faster recovery, and less need for outpatient care. The main indications for such a technique are hallux valgus, correction of the lesser metatarsals, hallux rigidus, and some diabetic foot deformities.

The Spring Ligament Complex-Anatomy and Function.

This article provides an overview of the soft tissue contributions to the normal structures that surround the talo-calcaneal-navicular (TCN) joint of the human arch. The TCN joint has a multiplanar range of motion that makes it essential to the kinetic coupling that links the forefoot and hindfoot. The soft tissue connection surrounding this joint is known as the spring ligament complex. More accurate knowledge of the anatomy of this complex will enhance the understanding of its role in the support of the head of the talus and, potentially, its critical interactions with the normal or abnormal
