Aspects of forensic entomology in forensic medicine.

ntomology, as a vast scientific discipline of zoology, deals with the study of insects, which are an integral part of the ecosystem of our planet and are closely linked to the activity and life cycle of all organisms, including humans. As a result of this natural connection, insects often become a direct “living witness” of a crime and thus an invaluable part of the evidence in a comprehensive forensic investigation, which in such cases necessarily includes a specialized field of science - forensic entomology. The goals of forensic entomology include comprehensive analysis of entom

The role of the doctor is to: (see also: Child abuse: management of physical abuse) 1. Take a full medical history without detail about the CSA (role of social worker/police). 2. do a complete physical examination of the child. 3. collect medico-legal evidence using a crime kit if abuse is recent (<72 hours). 4. Do further investigations, e.g. special investigations for physical abuse, pregnancy test if pubertal, and syphilis and HIV serology. 5. Explain the findings to parent(s) and the child if the latter is old enough. 6. Mobilise the child abuse management team. 7. Write detailed notes, ne

Clinical Forensic Medicine: Essentials for Emergency Medicine Trainees.

This edition of Trainee Focus is the first to introduce the topic of Clinical Forensic Medicine (CFM). Despite its inclusion in the ACEM curriculum, forensic medicine and the associated medicolegal frameworks remain an unfamiliar and poorly understood area of medicine for most trainees. Laws vary between jurisdictions, limiting the generalisability of training. Interactions with Police officers and the criminal justice system can be nerve-wracking, whether on the floor during a shift or when tasked to give evidence at a later date. Misunderstandings about legal process are common among clinici

Neonatal asphyxia and forensic medicine.

In the last decades, the scientific literature addressing neonatal encephalopathy has grown in a logarithmic way and malpractice claims in obstetrics and neonatology have become a major threat to the health service. At the moment, scientific evidence are insufficient to clearly identify in each single case whether the hypoxic insult has developed in the course of labor or in the first few hours after the birth or, otherwise, whether the damage has to recognize a remote and long-lasting cause acting during pregnancy. Several authors feel that this scientific uncertainty leads to a higher percen

[Erroneous paths of forensic medicine].

In the history of the progress of forensic medicine, apart from the successes, false theories appeared to happen, many of which were discarded only after many years. Among them, the greatest importance was gained by the theory of status thymo-lymphaticus (said to explain the causes of sudden deaths of young people) and the theory of blood's fluidity as a symptom indicating asphyxia as the cause of death. Of lesser popularity were the theories about the so called "watery shot", a homicide committed by adding powdered glass to the victim's meal and the theory of recreating the image of the kille

Violence in clinical forensic medicine.

OBJECTIVE: to investigate the levels of physical and verbal violence experienced in the preceding year by doctors working in clinical forensic medicine. DESIGN AND SUBJECTS: anonymised questionnaire sent to all full members of the Association of Police Surgeons. RESULTS: 517 eligible questionnaires were returned; 18.2 per cent of respondents had experienced physical violence, a total of 150 incidents. Of those incidents 'warning signs' of violence had been present in only 25 per cent. A total of 54 working days were lost. Injuries included a fractured wrist and corneal scarring. Of the respo

[Taste disorders in forensic medicine].

Taste disorders can a rise from lesions of peripheral receptors, taste control pathways or cortical area involvement. Among peripheral lesions, trauma of the tongue and oropharynx are the most common. Iatrogenic lesions of facial and glossopharingeal nerves are very important in Forensic Medicine, while there are different opinions about taste alterations due to head injuries; hypogeusia associated to smell disorders are found in 0.4-0.5% of patient after head trauma with good prognosis (90% healing) while qualitative disorders are more common (30%). The Authors describe clinical methodologies

Applicability of trichoscopy in forensic medicine.

Hair is a resilient forensic element capable of preserving morphological and biochemical information and may provide relevant evidence in cases of mechanical violence involving forcible traction or avulsion. This report describes the application of trichoscopy as a complementary tool in forensic medicine for the evaluation and documentation of traumatic hair-pulling injuries. A 66-year-old woman underwent forensic medical examination after reporting a single episode of hair traction injury resulting from interpersonal violence. Clinical assessment was performed, followed by trichoscopic examin

Humanitarian forensic medicine: a systematic review.

Various concerns relating to international humanitarian law and human rights were risen by natural catastrophes (tsunamis, floods, fires), the Covid-19 pandemic, the epidemic breakouts of Ebola, as well as the significant migrant wave observed in the Mediterranean region. Forensic Medicine has direct interactions with both domestic and international law, and thus is frequently asked to provide solutions for these issues. The term "Humanitarian Forensic Action" (HFA), which refers to the application of forensic science to serve humanitarian endeavors, was created. The management of crises inclu

Molecular pathology in forensic medicine--Introduction.

Techniques of molecular biology have improved diagnostic sensitivity, accuracy and validity in forensic medicine very much, especially in the field of identification (paternity testing, stain analysis). Since more than 10 years these techniques - meanwhile well established in clinical disciplines - are used also for other applications in forensic medicine: determination of cause and manner of death, tissue identification by mRNA and microRNA, examination of gene expression levels (survival time, time since death, cause of death), toxicogenetics.

An unusual pedestrian road trauma: from forensic pathology to forensic veterinary medicine.

Traffic accidents have increased in the last decade, pedestrians being the most affected group. At autopsy, it is evident that the most common cause of pedestrian death is central nervous system injury, followed by skull base fractures, internal bleeding, lower limb haemorrhage, skull vault fractures, cervical spinal cord injury and airway compromise. The attribution of accident responsibility can be realised through reconstruction of road accident dynamics, investigation of the scene, survey of the vehicle involved and examination of the victim(s). A case study concerning a car accident where

Emergency medicine techniques and the forensic autopsy.

Emergency medicine measures often have to be carried out under suboptimal conditions in emergency situations and require invasive patient treatment. In the case of a fatal outcome these measures have to be evaluated at autopsy, regarding indications, correct implementation and possible complications. As well, alongside the more familiar procedures--such as endotracheal intubation, insertion of chest drains, external cardiac massage and cannulation of central and peripheral veins--there are alternative techniques being increasingly applied, that include new tools for the management of hemorrhag

Sharp force injuries in "clinical" forensic medicine.

In general, the forensic evaluation of sharp force injuries in living and dead individuals follows the same morphologic principles. Still, there are some special features of sharp force injuries in the clinical context, which have to be considered as examination findings on the living are interpreted to differentiate between accidental origin, self-infliction or homicidal assault. These include the frequency and localization of defence injuries, injuries of the perpetrator, and artificial injuries, especially those inflicted for the purpose of insurance fraud. Characteristics and differential

[Self-mutilation from a forensic medicine viewpoint].

The frequency of the diagnosis of self-induced injuries is rising, especially in patients with borderline personality disorder. The number of unreported cases is estimated to be high. Self-mutilation typically affects the patient's skin and therefore often presents to dermatologists or general practitioners. Such injuries are often clinically typical; a knowledge of these features is essential for the correct diagnosis and treatment. Multidisciplinary care is essential because of the tendency for overlap with other psychiatric diseases and the risk of suicide. Self-induced injuries are also us

[Bioterrorism--also a challenge for forensic medicine?].

In view of current events the question is discussed to what extent the risk of bioterrorism may be an issue relevant for forensic medicine also in Germany, although at present there seems to be no concrete threat. The cases which became known so far were either false alarms or foolish pranks (copycats), which have to be, and are indeed, prosecuted by the state (section 126 German Criminal Code). Reference is also made to the measures of disinfection recommended by the Robert Koch Institute.

Artefact in forensic medicine. Fetal congenital abnormality.

A case of congenital defect in an abortus is described and the forensic implications of unawareness of childhood artefactual abnormalities which may appear to have a cryptogenic criminal etiology are reiterated. This is apparently the first case reported in which gastroschisis had been misconceived to be criminal abortion.
