Home or foster home care versus institutional long-term care for functionally dependent older people.

BACKGROUND: Changing population demographics have led to an increasing number of functionally dependent older people who require care and medical treatment. In many countries, government policy aims to shift resources into the community from institutional care settings with the expectation that this will reduce costs and improve the quality of care compared. OBJECTIVES: To assess the effects of long-term home or foster home care versus institutional care for functionally dependent older people. SEARCH METHODS: We searched the Cochrane Central Register of Controlled Trials (CENTRAL) via the C

The VA Medical Foster Home Program, Ambulatory Care Sensitive Conditions, and Avoidable Hospitalizations.

This quality control study analyzes whether the Veterans Administration Medical Foster Home (VA MFH) program has been successful in improving access and effectiveness of ambulatory care. Individuals hospitalized for one or more of 22 adult ambulatory care sensitive conditions were identified. Pre and post comparisons of a specified population of participants in the program were conducted to determine rates of avoidable hospitalizations for 6 months prior to and following MFH enrollment. The overall rate of avoidable hospitalizations declined from 18.5 to 14.9 per 100 enrollees following enroll

Quality care for ill children: AIDS-specialized foster family homes.

Children with AIDS and related complexes present a new and serious challenge to the child welfare field's foster care system. At this stage of the world's knowledge of AIDS and treatment capacity for its victims, the children are stigmatized as plague bearers, their viability is unknown, and few, if any, will ever have a home to return to. This article describes a special foster care demonstration in behalf of AIDS infants.

Characterizing the Use of Home-Based Behavioral Health Services Among Children in Foster Care.

BACKGROUND: Children in foster care (FC) experience disproportionately high rates of mental health (MH) needs and frequently use behavioral health services. They are overrepresented in restrictive MH settings and are more likely to be prescribed psychotropic medications. Home-based behavioral health services (HBHS) offer a less restrictive alternative for addressing the complex needs of children in FC, but their use and effectiveness remain understudied. METHODS: Using Medicaid data from 28 states, we examined MH service utilization among children aged 3-18 with primary MH diagnoses across th

CAPABLE Care + Connect: A Qualitative Study to Adapt CAPABLE for Home-Based Primary Care and Foster Social Connection.

Fostering social connection among homebound older adults and their care partners presents a distinct challenge. Evidence on strategies to address social isolation and functional impairment for homebound individuals is limited. This study aims to inform adaptation and implementation of the CAPABLE program within a home-based primary care practice. We conducted interviews with older adults, care partners, CAPABLE clinicians, and home-based primary care staff (n = 12). We explored factors influencing social connection and program implementation for a home-based primary care population. We identif

The prevalence of child sexual abuse in out-of-home care: a comparison between abuse in residential and in foster care.

We investigated the 2010 year prevalence of child sexual abuse (CSA) in residential and foster care and compared it with prevalence rates in the general population. We used two approaches to estimate the prevalence of CSA. First, 264 professionals working in residential or foster care (sentinels) reported CSA for the children they worked with (N = 6,281). Second, 329 adolescents staying in residential or foster care reported on their own experiences with CSA. Sentinels and adolescents were randomly selected from 82 Dutch out-of-home care facilities. We found that 3.5 per 1,000 children had bee

Pilot testing the feasibility of the fostering positive care interactions for residents with dementia in pain (FPCI) intervention in nursing homes.

PURPOSE/AIMS: Pain is common in nursing home (NH) residents with dementia, and those in pain are unfortunately more likely to receive more negative interactions from NH staff. The purpose of this study was to pilot test the feasibility of the Fostering Positive Care Interactions for Residents with Dementia in Pain (FPCI) intervention which is focused on engaging NH staff in positively interacting with residents with dementia in pain. A research nurse facilitator implemented the FPCI, which consisted of the following steps: (1) assessments of the nursing home environment and policies around fac

[Family development conditions of autistic children before foster home care: a comparison with families of hyperkinetic, emotionally disturbed and antisocial children].

With reference to psychogenetic autism models a retrospective study investigates, if autistic children more frequently than children with other disturbances grow up in a family-environment, which is characterized (a) by deficits in emotional warmth and (b) by a relatively high educational and occupational status of parents. The parallel group study is based upon a sample of 112 children, who had been referred to a residential facility for psychiatric care. The hypothesis concerning deficits in emotional warmth is not confirmed. As to occupational status of parents however, comparative findings

[From an attractive training site to a dream workplace? Fostering potential and retaining staff - Insights from a cross-sectional descriptive survey study in home-based long-term care].

INTRODUCTION: The growing demand for home care due to demographic changes and the preference of older people to remain at home presents new challenges for nursing professionals. Despite positive practical experiences, only a few trainees currently choose to start their professional career in the home care sector, indicating structural deficits and specific demands in this setting. The aim of the study was to identify factors influencing entry and retention of nursing professionals in ambulatory care, and to highlight actionable features for increasing attachment to the home care sector. Foster children avoidance-related interactive behaviors with caregivers.

BACKGROUND: A lot of the research concerning foster children - often children who have suffered maltreatment in the family home - has focused on internalized and externalized symptoms. Few studies, however, have looked at the interactions between such children and caregivers. PURPOSE: The purpose of this study is to explore the Emotion Regulation Strategies (ERS) of children in foster care and to highlight those most commonly employed in family or placement contexts. The parents' and foster carers' ERS are also analyzed in order to understand the co-regulatory processes at work. METHOD: An i

Sleep in children in foster care: A scoping review.

BACKGROUND AND OBJECTIVE: Although the focus on sleep disturbances in children who have experienced adversity has increased, little is known about sleep among children in foster care (CFC). This study aimed to review what research tells us about the relationship between foster care experience and sleep disturbances and to identify knowledge gaps. METHOD: This study used Arksey and O'Malley's (2005) 5-part methodology for conducting scoping reviews and the Reporting Checklist for Scoping Reviews (PRISMA-ScR) guidelines. 14 studies met inclusion criteria. Data were synthesized using the 6-phase

Fetal Alcohol Spectrum Disorders: Characteristics, Complications, and Treatment.

Fetal Alcohol Spectrum Disorders (FASD) includes a continuum of disorders that occur in children as a result of their mothers' consumption of alcohol during pregnancy. The most severe of these disorders is Fetal Alcohol Syndrome (FAS). FASD presents differently in every child, but all children with FASD have intellectual and/or behavioral impairments. There is no cure for FASD, but research shows that early intervention and life-long support help those born with FASD to manage the difficulties that come with it. This paper examines the characteristics, complications, and treatment for FASD.

Influence of placement stability on developmental outcomes of children and young people in out-of-home care: Findings from the Pathways of Care Longitudinal Study.

BACKGROUND: Placement stability is an important indicator of the functioning of an out-of-home care system. Previous research suggests that frequent placement changes have a negative impact on the outcomes for children and young people in out-of-home care. OBJECTIVE: This paper examines the association between placement stability in out-of-home care and children's socio-emotional, cognitive and physical health outcomes. PARTICIPANT AND SETTING: The Pathways of Care Longitudinal Study (POCLS) is the first large-scale prospective longitudinal study of children in out-of-home care in Australia.
