Infection after fracture fixation.

Infection after fracture fixation is a feared complication in orthopaedic surgery leading to poor bone healing and loss of function.Early detection is essential and interdisciplinary care is mandatory.Eradication of infection is only possible through combined surgical and antibiotic treatment.Intraoperative tissue samples must be taken and are effective for guidance of the antibiotic regimen.Infection after fracture fixation is different from prosthetic joint infection (PJI) and needs a specific strategy. In this review, we define infection after fracture fixation, and outline the clinical, rad

Arthroscopy-assisted fracture fixation.

PURPOSE: the purpose of this article was to systematically analyze the results of published studies in the literature which evaluated the use of arthroscopically assisted techniques in intra-articular fracture fixation. METHODS: published investigations to date were analyzed by classifying them according to joints that were involved with intra-articular fractures including: knee, ankle, hip, shoulder, elbow, and wrist joints. The results were studied to assess the feasibility, efficiency, and outcomes of arthroscopy-assisted fracture fixation. RESULTS: arthroscopy-assisted techniques have be

Timing of Fracture Fixation in Ankle Fracture-Dislocations.

Ankle fracture-dislocations may require delayed internal fixation. Our aim was to compare acute open reduction and internal fixation (ORIF) with delayed ORIF, using external fixation or cast splint in ankle fracture-dislocations. Factors that affect the rates of re-operation and Surgical site infection (SSI) were identified. In this retrospective cohort study, patients were included with open and closed ankle fracture-dislocations treated with ORIF from two large peripheral hospitals and one academic center in the Netherlands. This study included 447 patients with an ankle fracture-dislocation

Arthrofibrosis After Periarticular Fracture Fixation.

Arthrofibrosis after periarticular fractures can create clinically significant impairments in both the upper and lower extremities. The shoulder, elbow, and knee are particularly susceptible to the condition. Many risk factors for the development of arthrofibrosis cannot be controlled by the patient or surgeon. Early postoperative motion should be promoted whenever possible. Manipulations under anesthesia are effective for a period of time in certain fracture patterns, and open or arthroscopic surgical debridements should be reserved for the patient for whom nonoperative modalities fail and wh

Bilateral clavicle fracture external fixation.

Fractures of the middle third of the clavicle are frequent and their conservative treatment ends in bone union in nearly 95% of cases. Surgical treatment is unanimously indicated with open fractures or in cases of cutaneous damage, neurovascular complications, and impaction of the shoulder stump syndromes. We report herein a case of bilateral fractures of the clavicle that required double stabilization with an external fixator following major cutaneous damage appearing after the initial conservative management. The intraoperative discovery of Propionibacterium acnes infection and bone union ob

[Percalcar Steinmann's pins fixation of intratrochaner fixation fracture. Clinical observation].

80 cases of intratrochaner fractures were treated by fixation of 4 Steinmann's pins which were 3.5 mm in diameter and inserted percutaneously through the calcar and compression trabeculae and distension trabeculae respectively. They were compared with the cases treated by skeletal traction or fixed by Nail-plate or angle plate. The rate of bony union in fixed position in the percalcar Steinmann's pins group was 83.7%. Normal neck-shaft angle accounted to 53.1% in the traction group. Varus deformity occurred in 16.3% cases of the Steinmann's pins group, and 35.9% cases of the traction group. Th

Timing of fracture fixation from an Intensive Care Unit perspective: the obstacles to early fracture fixation.

Trauma is one of the leading causes of death worldwide, with road traffic accidents being the leading cause of death in the age group of 15-29 years However, with modern advances in management and the introduction of specialised trauma centres, more and more are surviving severe and life-threatening trauma. The ideal timing of fracture fixation has been the subject of debate for a number of decades. There is evidence to suggest that fracture fixation in the patient with polytrauma is best achieved early on to reduce the incidence of morbidity and mortality, with damage control surgery in the m

Can a generic fracture fixation assessment tool be used to assess quality of distal radius fracture fixation, and predict fixation failure?

INTRODUCTION: Failure following volar locking plate fixation of distal radial fractures (DRFs) is a common cause of revision surgery. The Fracture Fixation Assessment Tool score (FFAT) is a validated evaluation tool for appraising long bone fracture fixations. It comprises of four domains: reduction, stability, implant choice and overall impression. This study evaluates FFAT as an educational tool for assessing DRF fixation quality. A secondary aim is to analyse its ability to predict fixation failure. METHODS: A pool of 63 DRF fixations, including 22 failures, were scored by 27 orthopaedic s

Subcapital fracture complicating fixation of an intertrochanteric fracture from osteomyelitis.

Subcapital fractures after open reduction and internal fixation of intertrochanteric hip fractures have not previously been attributed to osteomyelitis. We report a unique case of a 63-year-old man with isolated subclinical osteomyelitis of the femoral neck causing ischemic necrosis and subcapital fracture after sliding screw plate fixation of an ipsilateral intertrochanteric fracture.

Cow-hitch fixation in fracture hemiarthroplasty.

BACKGROUND: The treatment of complex proximal humerus fractures with hemiarthroplasty is associated with a high failure rate due to secondary displacement of the tuberosities. It was the aim of this in-vitro study to compare the mechanical stability of tuberosity reattachment obtained with the so-called "Cow-Hitch" (CH) cerclage compared with conventional tuberosity reattachment. METHODS: A 4-part proximal humerus fracture was created in 10 fresh-frozen, human cadaveric shoulders. The greater and lesser tuberosity were reattached to the hemiarthroplasty stem with in total 4 CH Cerclages in th

Column-Specific Distal Radius Fracture Fixation.

Despite the popularity and success of volar fixed angle plating, variations in distal radius fracture presentation underscore the importance of understanding different surgical treatment options to maximize patient outcomes. The 3-column theory of wrist mechanics provided the foundation for using a column-specific fixation approach. Implant placement within the ulnar column can be challenging because of anatomical constraints. Some have described "safe zones" for implant position to decrease the potential for tendon impingement. A thorough understanding of the surrounding anatomy can allow fix

Rib Fracture Fixation: Indications and Outcomes.

Rib fractures are a frequently identified injury in the trauma population. Not only are multiple rib fractures painful, but they are associated with an increased risk of adverse outcomes. Pneumonia in particular can be devastating, especially to an elderly patient, but other complications such as prolonged ventilation and increased intensive care and hospital durations of stay have a negative impact on the patient. Computed tomography scan is the best modality to diagnosis rib fractures but the treatment of fractures is still evolving. Currently patient care involves a multidisciplinary approa

Stress Modulation of Fracture Fixation Implants.

Stress modulation is the concept of manipulating bridge plate variables to provide a flexible fixation construct that allows callus formation through uneventful secondary bone healing. Obtaining absolute stability through the anatomic reduction of all fracture fragments comes at the expense of fracture biology, whereas intramedullary nailing, which is more advantageous for diaphyseal fractures of the lower extremity, is technically demanding and often may not be possible when stabilizing many metaphyseal fractures. Overly stiff plating constructs are associated with asymmetric callus formation

Periprosthetic fracture fixation in osteoporotic bone.

Fixation techniques of periprosthetic fractures are far from ideal although the number of this entity is rising. The presence of an intramedullary implant generates its own fracture characteristics since stiffness is altered along the bone shaft and certain implant combinations affect load resistance of the bone. Influencing factors are cement fixation of the implant, intramedullary locking and extramedullary or intramedullary localization of the implant and the cortical thickness of the surrounding bone. Cerclage wires are ideally suited to fix radially displaced fragments around an intramedu

Fixation of tendo Achilles avulsion fracture.

Achilles tendon ruptures occur commonly in the midsubstance of the tendon, usually 2-6 cm proximal to the insertion to the calcaneus. Ruptures near its insertion into the calcaneus are less common and mostly found in hyperpronators with a heel spur (Haglund's deformity). Avulsion of the bony insertion of the Achilles tendon at the calcaneus is infrequent and is diagnosed by radiography. Open reduction and internal fixation is indicated to achieve bone to bone healing and restoration of the function and continuity of the triceps surae mechanism. Screw fixation is not effective to resist the pul

Timing of fracture fixation: a review.

The timing of fracture stabilization has received little attention in the current orthopaedic literature. The existing data on fracture fixation will be reviewed specifically with regard to early versus delayed stabilization. Timing relative to the multiple trauma patient, open fractures, periarticular fractures, and hip fractures requires special consideration.

Operative fixation of rib fracture nonunions.

INTRODUCTION: Rib fractures are common injuries in trauma patients that often heal without intervention. Infrequently, symptomatic rib fracture nonunions are a complication after rib fractures. There is a paucity of literature on the surgical treatment of rib fracture nonunion. The purpose of this study was to describe the efficacy of rib fracture nonunion operative fixation with particular focus on surgical technique, healing rates, and complications. MATERIALS AND METHODS: Patients aged ≥ 18 years with symptomatic rib fracture nonunions treated with open reduction and internal fixation (ORI

Patient Outcomes Following Ankle Fracture Fixation.

BACKGROUND: Several studies have reported on potential negative predictive factors of functional outcomes after ankle fracture fixation. However, there is minimal patient-reported data on long-term outcomes. This study aimed to evaluate potential risk factors leading to a poor patient-reported functional outcome at 2 and 5 years following ankle fracture fixation. METHODS: We conducted a prospective cohort study over a 5-year period on patients undergoing open reduction and internal fixation for unstable ankle fractures.
