Malunited fractures in the hand.

Posttraumatic deformity of a tubular bone in the hand after malunion can impact function due to alteration in mobility, strength, or associated pain. Surgical intervention is often indicated, with the surgical options based on both the type and location of the deformity, as well as any associated articular, tendon, or soft tissue constraints. This article provides a management approach based on the deformity classification, location, and any associated conditions.

Extension osteotomy in malunited clavicular fractures.

The association of a malunited clavicular fracture with a pattern of disability that includes not only pain but also impairment of shoulder function is rare. But in cases where such an association exists, correction of the clavicular deformity should be considered. We report on 4 patients with a malunited fracture of the clavicle after nonoperative treatment. In all 4 patients fracture union had occurred with shortening associated with pain and ipsilateral glenohumeral dysfunction. The deformity was not associated with neurovascular dysfunction. On preoperative radiographs the shortening of th

Corrective Osteotomies for Malunited Malleolar Fractures.

Ankle fractures are common and may be treated nonoperatively or by reduction and internal fixation. Both treatments may lead to nonanatomic healing, called malunions. Malunions lead to joint instability and pathologic load transfer. Arthritic joint destruction may be the consequence. Corrective osteotomies for malunited malleolar fractures have to restore anatomic conditions. Osteotomies of the medial malleolus, lateral malleolus, posterior malleolus (Volkmann's triangle), and the posteromedial corner or combinations thereof described. Corrective osteotomies for malleolar malunions show good r

Fibular lengthening for malunited ankle fractures.

BACKGROUND: Malunited ankle fractures are uncommon. They produce symptoms of persistent pain, joint effusion, limitation of dorsiflexion and eventually lead to ankle arthritis. We feel that correction of the ankle alignment can improve the final outcome and present our results. MATERIALS AND METHOD: From May 2004 to April 2006, seven patients with a malunited fibular fracture aged 25 to 62 years (male:female ratio, 5:2) were treated in our institute. All patients were referred for persistent pain. The delay between injury and the operative intervention was 3 to 16 months. All patients were as

[Malunited supracondylar and condylar humeral fractures].

We present three important types of posttraumatic deformity of the distal humerus: varus deformity, valgus deformity and complex deformity. Their primary causes are discussed. Slight varus deformities are the result of purely lateral slip of supracondylar fractures or radial overgrowth after a lateral condylar fracture. Severe varus deformities occur after ulnar slip of the distal fragment following supracondylar fracture caused by a rotational deformity. The results after correlation of varus deformities with a rotational deformity are poor. The only way of obviating this problem is to recogn

Corrective osteotomy after malunited distal radius fractures.

Posttraumatic malunion or secondary dislocation can cause wrist joint incongruency. Uncorrected malalignment increases the risk of secondary degenerative changes and chronic pain. Therefore, early correction using the available fixed-angle devices, cancellous bone grafting only becomes necessary in larger bony defects. Premounting the plate through a palmar approach with regard to the desired correction angles leads to predictable results by precise correction. In case of posttraumatic growth arrest with larger discrepancy of the radius and the ulna, a two-staged procedure is advisable.

[Malunited juvenile fractures in the foot region].

Between 1971 and 1990 we treated 121 juvenile patients up to 14 years of age with fractures of the ankle joint and foot in the department of trauma surgery at Hannover Medical School. A total of 128 fractures were treated in these patients, excluding toe fractures. The distribution of fractures as referred to the anatomical-functional planes was as follows: Tibia-talar joint 69; talus 8; calcaneus 6; Chopart joint 5; Lisfranc joint 4 and metatarsal area 36. Clinical and radiological follow-up investigations were possible a mean of 8.7 years post trauma for all talar and calcaneal fractures, fo

Lateral decompression for malunited os calcis fractures.

Nineteen heels in 19 patients underwent a lateral decompression for hindfoot pain secondary to a malunited os calcis fracture with subtalar intra-articular involvement. The operative procedure consisted of a lateral calcaneal bony prominence removal (ostectomy) and sural nerve release or transection. In addition, the peroneal tendons were released, relocated, and lengthened as necessary, with repair or reconstruction of their retinaculum. Eight patients (group I) had previously undergone a late subtalar fusion without relief of lateral symptoms. Eleven patients (group II) underwent a lateral d

[Problems of malunited fractures of the distal radius].

The essential problematic of the distal fracture of the radius lies in the quality of its reduction and retention. 35 patients with distal fractures of the radius and strain fractures of the styloid process of the ulna were followed up with regard to their functional and radiological results and their subjective symptoms. Patients with intra-articular fractures were found to have significantly poorer functional results compared to those with extra-articular fractures. 29 out of 35 patients developed a nonunion of the styloid process of the ulna in the form of a compensatory false position with

Managing Severely Malunited Calcaneal Fractures and Fracture-Dislocations.

Severe calcaneal malunions are debilitating conditions owing to substantial hindfoot deformity with subtalar arthritis and soft tissue imbalance. Type III malunions are best treated with a subtalar distraction bone block fusion. Additional osteotomies may be required for severe varus or superior displacement of the calcaneal tuberosity. Type IV malunions result from malunited calcaneal fracture-dislocations and require a 3-dimensional corrective osteotomy. Type V malunions warrant additional ankle debridement and reconstruction of the calcaneal shape to provide support for the talus in the ank

Corrective Osteotomies for Malunited Extra-Articular Calcaneal Fractures.

The most effective way to treat calcaneal malunions is avoidance. With any articular fracture, progressive arthrosis and dysfunction are common. By restoring the anatomy initially through reduction, late reconstructive options become less complicated. Numerous studies have shown that restoration of the anatomic alignment either through percutaneous or open techniques is effective. In patients with no or minimal articular degeneration, extrarticular joint-sparing procedures can be performed. This represents a small select group who may benefit from simple osteotomy procedures with associated so

Corrective Osteotomies for Malunited Tongue-Type Calcaneal Fractures.

Displaced tongue-type fractures of the calcaneus can lead to severe pain and disability if not treated appropriately. Failure to reduce articular displacement may require subtalar joint arthrodesis with subsequent loss of function. The subtalar joint is crucial for normal foot and ankle function. In selected cases, if the malunited joint is still in good condition, it is preserved by corrective osteotomy. A joint-preserving osteotomy with axial realignment is a treatment option for malunited tongue-type calcaneal fractures encountered early on, before the development of subtalar arthrosis in c

[Surgical treatment of malunited or nonunited talus fractures].

OBJECTIVE: To explore the operative methods of malunited or nonunited talus fractures. METHODS: Twenty-two patients of malunions or nonunions after displaced talar fractures were treated from January 2000 to January 2008. There were 17 males and 5 females with an average age of 34 years (ranged from 15 to 52 years). According to classification of posttraumatic talar deformities (Zwipp 2003), there were 10 cases of type I (malunion and/or joint displacement), 8 cases of type II (nonunion with joint displacement), 4 cases of type III (type I/II with partial AVN). The surgical treatments include

[Malunited juvenile fractures in pelvic and hip area].

Fractures of the hip and pelvis are rare in the growing skeleton. Conversely, they are complicated by morphological and functional abnormalities in a particularly high percentage of cases. We present a review of posttraumatic structural abnormalities and their consequences. In addition to the common presentations, less well-known abnormalities and their possible treatments are discussed.

Operative Treatment of the Malunited Fibula Fracture.

UNLABELLED: Failure to anatomically reduce and stabilize the fractured distal fibula can result in malunion of the fibula and malreduction of the ankle mortise. Fibular malunion results in altered ankle joint biomechanics which often leads to the development of pain, stiffness, and premature joint degeneration. Fortunately, many fibular malunions can be successfully salvaged using osteotomy techniques to restore anatomic fibular length and rotation. Different osteotomy techniques are indicated depending on the location and characteristics of the malunion. In this review, the oblique fibular os

[The malunited juvenile fracture--Monteggia defect].

Late reconstruction after neglected Monteggia lesions in children is discussed. On the basis of the literature (20 cases) and 2 personal cases it is deduced that open reduction of the radial head should be combined with osteotomy of the ulna according to the pathophysiology of the lesion. Details of the operative procedure are given. Reconstruction of the annular ligament is the least important part. Reconstruction is indicated in children under the age of 10-12 years even if the dislocation has been present for years. Unfavorable conditions for operative reconstruction are long-lasting disloc

[Correction of malunited medial and lateral malleolar fractures].

Anatomical reconstruction following malunited fractures of the lateral and medial malleolus represents a valuable treatment option with a good long-term prognosis when ankle joint function is still present and should be considered as an alternative to joint replacement or arthrodesis. If revision surgery is not performed ankle osteoarthritis is almost inevitable and joint preservation will eventually no longer be possible. Corrective osteotomy should therefore be performed as early as possible (< 6 months) but is often still beneficial even after several years. Prerequisites for successful rev
