Cardiovascular frailty - an emerging frailty phenotype.

With an ageing population worldwide, there is a corresponding increase in the prevalence of frailty. It is well established that there is a bi-directional relationship between frailty and cardiovascular morbidity and mortality. Different models have been developed to assess frailty in clinical practice exploring the physical phenotypes, cumulative deficits, social, vascular and cognitive aspects of frailty. However, no model as yet integrates the role of cardiovascular frailty in this context. This narrative review presents the concept of cardiovascular frailty, examining existing literature o

Frailty and frailty syndromes in persons with hemophilia: a review.

Advances in treatment for persons with hemophilia (PWH) have improved life expectancy, but PWH face new challenges, including frailty. Frailty is a health state of increased functional vulnerability. It is associated with aging and linked to adverse outcomes such as falls, hospitalization, and institutionalization. Although well-characterized in the general population, its prevalence and impact on PWH remain underexplored. Emerging evidence suggests PWH currently have increased frailty, particularly those with severe disease and joint damage from inadequate early prophylaxis. This review exami

Social frailty dimensions and frailty models over time.

INTRODUCTION: Social frailty is a complex concept and there is still no consensus on the criteria that best define it, nor on the role that social dimensions play in well-established frailty models. AIM: To analyse the predictive value of social frailty dimensions on distinct frailty models. METHOD: A non-probabilistic sample of 193 community-dwelling adults aged 65 years and over was recruited in 2016 and followed for three years. Frailty was assessed by the Tilburg Frailty Indicator, the Groningen Frailty Indicator, and the Fried Phenotype criteria. Questions about living alone, social net

Redefining Frailty: Planning Care Through the Frailty Trajectory Model.

Figure 1. The frailty cycle begins when an individual is already frail and thus vulnerable to decline, designated as baseline frailty. An acute health event or medical intervention initiates the cycle. Despite usual care, there is deterioration in cognition, function, mobility, and/or symptoms, resulting in incomplete recovery. This stepwise decline defines the frailty cycle: Each event establishes a new, higher baseline of dependency and frailty, further increasing vulnerability to future adverse events. Figure adapted from www.pathclinic.ca.

[Aging-related frailty and sarcopenia. Frailty, sarcopenia and dysphagia.].

Frailty and sarcopenia occur not only in whole-body but also oral and swallowing function. Presbyphagia is age-related decline of swallowing function in older adults. Oral frailty is age-related decline of oral function in older adults. Presbyphagia and oral frailty are not dysphagia but frailty of oral and swallowing function. Oral sarcopenia is defined as sarcopenia in muscles of the oral cavity. Sarcopenic dysphagia is characterized by swallowing difficulty resulting from a loss of mass and function in whole-body skeletal and swallowing muscles. Sarcopenic dysphagia is diagnosed using a 5-s

Frailty Stratification Using the Modified 5-Item Frailty Index: Significant Variation within Frailty Patients in Spine Surgery.

BACKGROUND: Frailty status has been associated with higher rates of complications after spine surgery. However, frailty patients constitute a heterogeneous group based on the combinations of comorbidities. The objective of this study is to compare the combinations of variables that compose the modified 5-factor frailty index score (mFI-5) based on the number of comorbidities in terms of complications, reoperation, readmission, and mortality after spine surgery. METHODS: The American College of Surgeons - National Surgical Quality Improvement Program (ACS-NSQIP) Database from 2009-2019 was use

Frailty in anesthesia.

PURPOSE OF REVIEW: Due to demographic change, the number of geriatric patients is increasing in the surgical field. This poses a major challenge in perioperative medicine. In order to improve patient safety, frailty has become a key element for risk assessment in the perioperative setting. This review aims to summarize the current state of anesthesia in frail patients. RECENT FINDINGS: Frailty is an independent risk factor for intraoperative and postoperative complications, mortality, and length of hospital stay. The Clinical Frailty Scale has been shown to be a feasible, easy-to-use tool for

Frailty and Mobility.

Frailty represents a state of heightened vulnerability. Mobility impairment contributes to the construct of frailty and channels adverse events. While mobility disorder is universal at a high burden of frailty, neither mobility nor balance dysfunction is sufficient to fully define frailty. Frailty represents proximity to complex system failure, with higher-order disturbance, such as mobility and balance disturbance, as a consequence. Impairment of mobility and balance is a common manifestation of illness in the frail individual and is therefore a sensitive marker of acute disease, putatively a

Frailty for neurologists: perspectives on how frailty influences care planning.

The concept of frailty, now being adopted in most medical disciplines, is attracting growing interest in neurology. Every day, most neurologists care for patients with varying degrees of frailty, from very mild to very severe. Frailty exacerbates patients' health needs, complicates clinical decision making, and negatively affects their health outcomes. Increasing evidence suggests that frailty affects the risk, clinical presentation, and course of common age-related neurological disorders, including dementia, Parkinson's disease, stroke, and multiple sclerosis. Most neurologists should become

Association of Frailty and Frailty Trajectory with Risk for Respiratory Infectious Diseases.

We explored the association between frailty and respiratory infectious diseases (RIDs) through a large cohort of 423,691 participants in the UK Biobank. Participants without baseline RIDs were assessed by physical frailty and frailty index. A total of 16,848 participants had repeated assessments. We divided participants into nonfrail, prefrail, and frail groups and categorized frailty changes as alleviation, maintenance, or aggravation. We estimated risk for RIDs, including influenza, pneumonia, and other acute lower respiratory infections. Compared with nonfrailty, prefrailty and frailty incr

Frailty in pulmonary hypertension: Establishing the frailty index and impact on survival.

BACKGROUND: Patients with pulmonary hypertension (PH) experience reduced physical capacity, which affects daily life functionality. Frailty signifies increased vulnerability due to diminished physiological reserves and is common in the elderly and those with chronic diseases, but has not been investigated in PH. This study aimed to create a frailty index for PH, to assess the prevalence of frailty, to determine frailty severity and progression over time and to establish a potential association between frailty and mortality in patients with PH. METHODS: This retrospective cohort study included

The Haemato-Oncology Frailty (HOF) score to assess frailty in lymphoma.

Morbidity and mortality during chemotherapy in older adults with haematological malignancy can be unpredictable. The Haemato-Oncology Frailty (HOF) score was previously found to predict outcomes in a cohort of patients with plasma cell myeloma. In this study, we assess its utility in assessing frailty in patients with lymphoma, and compare its performance to that of two other frailty scores. The HOF score was able to predict progression-free survival in this population, and was also shown to have potential in assessing the dynamism of frailty during chemotherapy. It performed well when compare

[Aging-related frailty and sarcopenia. Kampo medicine for frailty and sarcopenia.].

In Japan, which is entering a super-aged society, the prevention of long-term care and bedridden caused by frailty and sarcopenia have been recognized as an urgent need. The pathology of frailty and sarcopenia includes broad problems of senility. A traditional herbal medicine is known to improve symptoms across the organs, many herbal medicines administered by examining the balance of mind and body are effective for frailty and sarcopenia. "The Safety Drug Therapy Guideline 2015 for the Elderly" was fully revised in 2015. In this guideline, traditional herbal medicines, which are suggested to

Understanding social frailty.

Social frailty, along with physical and psychological frailties, is one of the components of frailty that has been reported to increase the incidence of adverse health outcomes. Although the prevalence of social frailty is relatively high, consensus on how to define and measure it is insufficient. In considering social frailty, living alone, interaction with neighbors, and social participation are important indicators, and interaction with neighbors and social participation, which are modifiable factors, are important countermeasures. The promotion of these social activities will contribute to

[The role of physical activity in the transition from pre‑frailty to frailty.].

The aim of the study was to assess the role of physical activity in the transition from pre-frailty to frailty in older adults under hypomobility conditions. The study included 143 elderly patients aged 65-74 years. A comprehensive geriatric assessment was performed using specialized software at baseline, after 1 month, and after 6 months. A decrease in physical activity, both short-term and prolonged, was associated with deterioration of muscle function in patients with pre-frailty. Significant positive correlations were found between handgrip strength dynamics and changes in the maximal torq

The characterization of frailty in spine surgery: a systematic review of frailty indices.

PURPOSE: Frailty is a key predictor of postoperative outcomes in spine surgery, yet its definition varies widely across studies. Most research relies on comorbidity-based indices like the modified frailty index (mFI), overlooking the multidimensional nature of frailty. The purpose of this systematic review is to identify the frailty indices in use, assess their prevalence, and evaluate their association with clinical outcomes. METHODS: Following PRISMA guidelines, a comprehensive search of PubMed, Scopus, and Web of Science identified 34 relevant cohort studies on elective spine procedures us

Psychological well-being in relation to frailty: a frailty identity crisis?

BACKGROUND: Frailty can be defined as the presence of multiple, interacting medical and functional problems. Frailty is associated with psychiatric conditions but its relation to psychological well-being is unclear. A "frailty identity crisis" has been proposed as a maladaptive response to the sense of self as health deficits accumulate. We evaluated this so-called identity crisis by investigating associations between well-being, frailty, and mortality in community-dwelling older Canadians. METHODS: In this secondary analysis of the Canadian Study of Health and Aging (N = 5,703; age 70+), fra

Identification of Frailty Using a Claims-Based Frailty Index in the CoreValve Studies: Findings from the EXTEND-FRAILTY Study.

Background In aortic valve disease, the relationship between claims-based frailty indices (CFIs) and validated measures of frailty constructed from in-person assessments is unclear but may be relevant for retrospective ascertainment of frailty status when otherwise unmeasured. Methods and Results We linked adults aged ≥65 years in the US CoreValve Studies (linkage rate, 67%; mean age, 82.7±6.2 years, 43.1% women), to Medicare inpatient claims, 2011 to 2015. [Association between frailty awareness and frailty among community-dwelling older adults].

AIM: This study investigated the association between frailty awareness-defined as understanding the term "frailty" and its meaning-and actual frailty status as well as oral frailty among community-dwelling older adults. METHODS: This cross-sectional study was conducted among adults ≥65 years old residing in the community. Data were collected via a postal survey assessing frailty awareness, status (using a health assessment questionnaire for the national screening program for older adults in Japan), and oral frailty (using the Oral Frail Index-8).
