Factors influencing treatment outcomes in mandibular Class II furcation defects.

BACKGROUND: Factors influencing treatment outcomes in furcation defects remain to be studied. Therefore, the aim of the study was to evaluate the association between factors and clinical parameters that may influence treatment outcomes in mandibular Class II furcation defects. METHODS: Twenty-seven systemically healthy subjects with a Class II buccal or lingual furcation defect in lower molars were treated. Clinical measurements (probing depth [PD], clinical attachment level [CAL], recession, mobility, plaque index [PI], and bleeding on probing [BOP]) and defect (vertical and horizontal defec

Regeneration of class II furcation defects: determinants of increased success.

One of the most important indications for guided tissue regeneration (GTR) treatment is class II furcation lesion. However, periodontal regeneration of this type of defect, although possible, is not considered totally predictable, especially in terms of complete bone fill. Many factors may account for variability in the response to regenerative therapy in class II furcation. The purpose of this review is to assess the prognostic significance of factors related to the patient (smoking, stress, diabetes mellitus, acquired immunodeficiency syndrome and other acute and debilitating diseases, and t

Bone formation in furcation defects. An experimental study in the dog.

The aim of the present investigation was to study bone formation in an experimentally-produced furcation defect in the dog. 15 foxhound dogs (group A) and 4 large mongrel dogs (group B) were used. The 2nd and 4th mandibular premolars were extracted and the 3rd lower premolars (3P3) were assigned as experimental teeth. "Through and through" furcation defects, about 4 mm high and 3 mm wide, were first produced in the experimental teeth of the dogs in group A. Reconstructive surgery was subsequently performed in group A using a GTR technique. The dogs of group A were scheduled for biopsy 2 weeks

Periodontal Regeneration - Furcation Defects: Practical Applications From the AAP Regeneration Workshop.

Focused Clinical Question: How should periodontal furcation defects be managed via periodontal regenerative therapy, and what parameters should be used for treatment selection? Summary: The treatment of furcation defects can vary based on the type and location of the furcation involvement. Attaining predictable regenerative outcomes is dependent on the control of local and systemic factors. A combined treatment approach (barrier and bone replacement graft with or without biologic) generally offers the better therapeutic outcome over monotherapy. Class I furcation defects can be managed via con

Long-term stability of conventional non-regenerative periodontal treatment for furcation defects.

BACKGROUND: Furcation involvement (FI) in multi-rooted teeth poses significant challenges in periodontal therapy due to complex anatomy and difficulty in achieving effective maintenance. FI is associated with increased periodontal destruction, especially in sites with narrow furcation entrances and deep root concavities. These features hinder effective debridement and compromise the efficacy of periodontal treatments. MATERIALS AND METHODS: This narrative review examines non-regenerative treatment approaches for managing FI, focusing on clinical outcomes and failure rates associated with flap

Evaluation of an absorbable collagen membrane in treating Class II furcation defects.

Recent research has focused upon the utilization of an absorbable collagen membrane in guided tissue regeneration (GTR). Concern exists as to whether this type of membrane is beneficial in the treatment of periodontal defects. The purpose of this study was to evaluate the effect of a type I bovine collagen membrane on treatment of Class II furcation defects. Twelve systemically healthy patients (six male and six female, ages 32 to 68) were treated. Each had bilateral mandibular furcation defects with attachment loss > or = 6 mm. Prior to surgery all patients completed initial therapy including

Efficacy of combined regenerative treatments in human mandibular class II furcation defects.

BACKGROUND: The treatment of molar furcation defects remains a considerable challenge in clinical practice. The degree of success in the management of furcation involvement is highly variable and inversely related to initial probing depth (PD) measurements in these lesions. The identification of clinical measurements influential to the treatment outcomes is critical to optimize the results of surgical periodontal therapy. Therefore, the objective of this study was to evaluate the clinical response of mandibular buccal Class II furcation lesions to a combined regenerative treatment modality. M

Periodontal wound healing following reciprocal autologous root transplantation in class III furcation defects.

PURPOSE: Furcation involvement in the molars is difficult to treat, and has been recognized as a risk factor for tooth loss. Although periodontal regenerative therapies, including guided tissue regeneration and various types of bone grafts, have been applied to furcation defects, the effects of these treatments are limited, especially in large class III furcation defects. The purpose of this pilot study was to investigate the effect of reciprocal autologous root transplantation on periodontal wound healing and regeneration in class III furcation defects in dogs. METHODS: Furcation defects (7

Regeneration of degree ii furcation defects with a 4% doxycycline hyclate bioabsorbable barrier.

BACKGROUND: Bacterial contamination of a healing regenerative site has been shown to affect the response to therapy adversely. Doxycycline possesses antibacterial properties as well as other biologic actions that may result in an increased production and maintenance of collagen and bone. The purpose of this study was to assess if the sustained release of 4% doxycycline through a bioabsorbable barrier would enhance the regenerative outcomes of healing furcation sites. METHODS: Individuals with a single degree II furcation defect in a mandibular molar participated. They were assigned randomly t

Effect of crown-attached sutures on healing of experimental furcation defects in dogs.

This study evaluates whether suturing of replaced flaps with crown-attached sutures, following reconstructive surgery, counteracts postoperative recession of the gingival margins and facilitates new attachment. Through-and-through furcation defects were created in the mandibular premolars of 5 beagle dogs. Bone was surgically removed from the furcation and around each root to a level 3 to 4 mm below the cemento-enamel junction. Steel wires were placed in the furcations for 6 weeks to enhance plaque formation. After 12 weeks, reconstructive surgery that included acid conditioning of root surfac
