BACKGROUND: Gadolinium-induced encephalopathy is a well documented complication due to the inadvertent entrance of a high dose of gadolinium into the intrathecal compartment. In lab animals, injecting gadolinium into the intrathecal compartment resulted in neurotoxicity and seizures. It is also well recognized that the presence of autologous blood in the intrathecal compartment can cause a broad range of neurological changes that can include seizures and mental status changes. At the time of writing this report, there were no references in the literature of simultaneous injection of gadolinium

OBJECTIVE: Gadolinium neutron capture therapy (GdNCT) is a potential treatment for malignant tumors based on two steps: (1) injection of a tumor-specific (157)Gd compound; (2) tumor irradiation with thermal neutrons. The GdNC reaction can induce cell death provided that Gd is proximate to DNA. Here, we studied the nuclear uptake of Gd by glioblastoma (GBM) tumor cells after treatment with two Gd compounds commonly used for magnetic resonance imaging, to evaluate their potential as GdNCT agents.
METHODS: Using synchrotron X-ray spectromicroscopy, we analyzed the Gd distribution at the subcellu

Purpose To evaluate the speciation of gadolinium-containing species after multiple administrations of the gadolinium-based contrast agents (GBCAs) gadodiamide and gadoteridol and to quantify the amount of intact gadolinium complexes and insoluble gadolinium-containing species. Materials and Methods A total dose of 13.2 mmol per kilogram of body weight of each GBCA was administered in healthy Wistar rats over a period of 8 weeks. Three days after the final administration, rats were sacrificed, and the brains were excised and divided into three portions. Each portion of brain homogenate was divi

Gadolinium-based contrast agents (GBCAs) are commonly used in medical imaging. Most intravenously (IV) administered gadolinium is excreted via the kidneys, and pathological retention in renal failure leading to nephrogenic systemic fibrosis (NSF) is well described. More recently, retention of gadolinium in the body in the absence of renal disease has been identified, with unknown clinical consequences. Many patients are aware of this, either through the media or via comprehensive consent documentation. Some internet sites, without hard evidence, have suggested a constellation of possible sympt

INTRODUCTION: Three-dimensional fluid-attenuated inversion recovery (3D-FLAIR) imaging 24 h after intratympanic gadolinium injection (IT method) or 4 h after intravenous injection (IV method) has been used to visualize endolymphatic hydrops in Ménière's disease. The aims of this study were to evaluate the difference in gadolinium distribution in cochlear perilymph between the two methods by comparing the enhancement of the basal and apical turns and clarify the pharmacokinetics in cochlear perilymph.
METHODS: A total of 24 ears of 22 patients who underwent the IT method (gadolinium-diethylene

The inherent overlap in the T1 and T2 relaxation of benign and malignant breast lesions has hindered progress in the application of MRI to evaluate this organ. Indeed, lesions in a glandular breast may completely blend in with normal breast tissues, a problem unenhanced MRI shares with mammography. However, infusion of intravenous Gd-DPTA accompanied by rapid MR imaging results in successful detection of most invasive breast malignancies. Inspection of the lesion morphology and enhancement profile shows that most malignancies have an irregular border, enhance very rapidly, and display a distin

Experience in three patients (one each with meningioma, pineal tumor, and prominent jugular bulb) illustrates that magnetic resonance (MR) angiography can benefit from the administration of gadolinium diethylenetriaminepentaacetic acid. Data were acquired with a three-dimensional velocity-compensated (fast imaging with steady-state precession) sequence. MR angiograms were obtained with a ray projection algorithm by using maximum intensity values. Portions of the vascular anatomy--particularly venous structures and smaller arteries--were better portrayed on the postcontrast than on the precontr

Gadolinium-based contrast agents have expanded the diagnostic usefulness and capability of magnetic resonance imaging. Despite their highly favorable safety profile, these agents have been associated with nephrogenic systemic fibrosis in a small number of patients who have advanced kidney disease. Recently, trace amounts of gadolinium deposition in the brain and other organs have been reported after contrast exposure, even in patients with normal renal function. In this review, we provide a brief overview of recent updates and discuss typical clinical situations related to the use of gadoliniu

Nephrogenic systemic fibrosis is a progressive, potentially fatal multiorgan system fibrosing disease related to exposure of patients with renal failure to the gadolinium-based contrast agents used in magnetic resonance imaging. Because of this relationship between nephrogenic systemic fibrosis and gadolinium-based contrast agents, the U.S. Food and Drug Administration currently warns against using gadolinium-based contrast agents in patients with a glomerular filtration rate less than 30 mL per minute per 1.73 m2, or any acute renal insufficiency related to the hepatorenal syndrome or periope

Nephrogenic systemic fibrosis (NSF) is characterized by red skin areas or plaques that over several weeks successively develop to painful thickened skin with a 'woody' texture, resembling 'peau d'orange'. Starting at the extremities, it may spread to the trunk, and may progressively inhibit flexion of adjacent joints. In skin biopsies of affected areas, thickened collagen bundles, mucin deposition, and proliferation of fibroblasts and elastic fibers are seen. Originally described as nephrogenic fibrosing dermopathy (NFD) because of its primarily cutaneous manifestation, this entity was then na

QUESTION: A pregnant patient who underwent magnetic resonance imaging (MRI) because of an acute abdomen, was told that MRI contrast agents (ie, gadolinium-based contrast agents) are contraindicated during pregnancy. What is the risk to her baby?
ANSWER: Current radiology practices and recommendations discourage the use of gadolinium-based contrast agents during pregnancy because their safety for the fetus has not yet been proven. In line, however, with the European Society of Radiology guidelines and based on the available evidence, gadolinium-based contrast agents appear to be safe in pregna

A 1:100 solution of 48% gadopentolate in liquid tear solution was used in magnetic resonance dacryocystography to image the canaliculi, nasolacrimal sac, and nasolacrimal duct. It was administered as an eyedrop, one drop every minute for five minutes, immediately before scanning in six normal and five abnormal nasolacrimal outflow systems. In cases of nasolacrimal obstruction, the dilute gadolinium solution was injected through the canaliculus immediately before scanning. A three-inch surface coil enhanced the detail of soft-tissue structures such as the canaliculi and lacrimal sac. In the eva

Acute pancreatitis is a sudden swelling and inflammation of the pancreas. The two most common causes are alcohol use and biliary stones. Drug-induced acute pancreatitis are rare (1.4-2%). In this present study, we present a case of recurrent acute pancreatitis induced by a specific magnetic-resonance-imaging (MRI) contrast agent called gadobenate dimeglumine.

PURPOSE: As imaging delays during MR arthrography can result in suboptimal studies and potential pitfalls in diagnosis, we sought to evaluate the temporal behavior of intraarticular Gd-DTPA.
METHOD: We prospectively studied four shoulders, four knees, and four hips. Two of each group received either an intraarticular injection of 3 mmol/L Gd-DTPA and iodinated contrast medium or 2 mmol/L Gd-DTPA and 0.9% saline. We measured contrast-to-noise ratio (CNR) and joint distention on serial T1-weighted sequences. Retrospectively, we measured CNR and imaging delays in 31 MR arthrography patients (20

Introduction: Gadolinium-based contrast agents are widely used for magnetic resonance imaging and, until recently, had been generally considered to have an excellent safety profile in patients with normal renal function. Nephrogenic systemic fibrosis is a well-established disease process involving fibrosis of the skin and internal organs seen in some patients with severely impaired renal function following exposure to these agents. Following reports that individuals with normal renal function may experience gadolinium deposition within brain and bone tissue, the term "gadolinium deposition dis

Purpose To evaluate whether gadolinium penetrates human cerebrospinal fluid (CSF) after MR imaging (MRI) with a gadolinium-based contrast agent (GBCA). Materials and Methods For this retrospective study, the authors analyzed 60 CSF samples from 57 patients (median age, 50 years; range, 3-92 years) who underwent one contrast material-enhanced MRI examination with gadoterate meglumine within 60 days of CSF extraction between January and December 2016. CSF samples from patients who underwent MRI without contrast material administration (n = 22) or those who underwent contrast-enhanced MRI at leas

AIMS: Late gadolinium enhancement (LGE) quantification by cardiovascular magnetic resonance is central to risk stratification in hypertrophic cardiomyopathy (HCM). Conventional techniques require contour tracing and region of interest (ROI) placement, which may reduce reproducibility and increase analysis time. We developed a novel visual approach, the visual standardized quantification (VISTAQ) of LGE, that does not require myocardial contouring, arbitrary ROI positioning, or dedicated post-processing software.
METHODS AND RESULTS: LGE images from 400 patients (100 prior myocardial infarctio

OBJECTIVES: In view of the mounting evidence for markedly increased cerebellar Gd retention under neuroinflammatory conditions after repeated linear GBCA administration in vivo , we aimed to discriminate between Gd retained within the GBCA complex and forms dissociated from the complex within the CNS and to characterize the chemical environment of the released Gd³⁺. For this purpose, we used electron paramagnetic resonance (EPR) spectroscopy, which enables direct detection of Gd³⁺ release and evaluation of its molecular surroundings in intact cerebellar tissue from inflamed and noninflamed bra
