Safety of gadolinium-DTPA: extended clinical experience.

This review reports data on the general and renal tolerance of the paramagnetic contrast agent Gd-DTPA after intravenous administration. Gd-DTPA was administered usually at a dose level of 0.1-0.2 mmol/kg body wt (range: 0.005-0.25 mmol/kg body wt) in cranial, spinal and body MR indications. In phase I-IIIa studies a total of 2154 healthy volunteers and patients were investigated, usually at a dose of 0.1 mmol/kg body wt. From the obtained results it was concluded that adverse events (AEs) may be expected in the order of magnitude of 1%. In phase IIIb-IV studies 13,439 patients were investigat

Detailed toxicity studies of liposomal gadolinium-DTPA.

Acute, subacute, and delayed toxicity testing was assessed in mice for liposomal gadolinium-DTPA (Gd-DTPA), blank liposomes, and nonliposomal Gd-DTPA. In the subacute experiments mice were injected intravenously (IV) with 0.3 mmol/kg Gd-DTPA per day for 30 days in the form of either free Gd-DTPA, liposomal Gd-DTPA, or an equivalent amount of lipid in blank liposomes without Gd-DTPA. The interpolated acute LD50 of liposomal and nonliposomal Gd-DTPA, estimated as a means of identifying the approximate level, was similar (LD50 = 5.7 mmol/kg Gd-DTPA). In subacute toxicity testing, prolonged high d

Gadolinium-DTPA-enhanced MRI of intraocular tumors.

The value of gadolinium-enhanced MRI in 30 patients with intraocular lesions has been evaluated. Seventeen patients had a uveal melanoma, two a ciliary body melanoma, three had uveal metastases, one lymphoma, four had senile macula degenerations, and three uveal nevi. Twelve of 17 patients with melanoma were followed up by MRI after ruthenium plaque therapy on 2-4 occasions. Melanomas showed high precontrast signal intensities and only a slight enhancement after intravenous Gd-DTPA was given. After ruthenium plaque therapy precontrast signal intensities (SI) decreased while a moderate signal i

Gadolinium-DTPA-enhanced MRI in the evaluation of osteonecrosis.

We report the MR findings in three cases of osteonecrosis. Two cases were post-traumatic and the third idiopathic. The diagnosis, localization and characterization of osteonecrosis was improved in all patients by the use of Gadolinium-DTPA-enhanced, fat-saturated, T1-weighted spin-echo imaging. We recommend the use of Gadolinium-DTPA-enhanced MRI in selective cases of osteonecrosis as it can better define the stage of disease and suitability for surgery.

MR imaging of the breast using gadolinium-DTPA.

In a preliminary study 20 patients underwent breast examinations by magnetic resonance (MR) imaging without and with Gd-DTPA as contrast medium. All carcinomas enhanced, whereas dysplastic tissue enhanced slightly or not at all. Significant additional diagnostic information was available on the Gd-DTPA examinations in at least four of 20 cases compared with MR without contrast medium and X-ray mammography. Our preliminary results indicate that MR imaging of breast using Gd-DTPA may be helpful for the evaluation of dense breasts and the differentiation of dysplasia and scar tissue from carcinom

Echoplanar diffusion-weighted MRI with intravenous gadolinium-DTPA.

It is occasionally necessary to repeat diffusion weighted imaging (DWI) after giving intravenous contrast medium (CM). However, the effects of CM on DWI and apparent diffusion coefficients (ADC) have not been fully examined. The aim of this prospective study was to investigate whether there are any diagnostically significant differences between echo-planar imaging (EPI)-DWI before and after intravenous CM. EPI-DWI was acquired twice in 203 consecutive patients before and after i.v. CM. Three blinded readers rated the diagnostic image quality. Quantitative ADC calculations were performed before

Gadolinium-DTPA-enhanced magnetic resonance imaging in optic neuropathies.

Magnetic resonance imaging (MRI), after intravenous administration of the paramagnetic agent gadolinium-diethylenetriaminepentaacetic acid (Gd-DTPA), showed enhancement of the optic nerve in 7 of 13 patients with acute optic neuritis. Four of these patients had Gd-DTPA enhancement of the intracranial optic nerve and two had involvement of the optic nerve at the orbital apex, ipsilateral to the eye with visual loss. Gadolinium-DTPA enhancement of both intracranial optic nerves was seen in one of two patients with bilateral retrobulbar neuritis. Two patients with acute radiation-induced optic ne

Leiomyosarcoma of abdominal veins: value of MRI with gadolinium DTPA.

Primary leiomyosarcomas of the retroperitoneum are rare tumors, the vast majority of which are malignant. Prognosis is largely dependent upon adequacy of surgical resection. Magnetic resonance imaging (MRI) is ideally suited to the evaluation of these neoplasms because of multiplanar imaging capability, inherent signal intensity differences of tissues, and flow-void, flow enhancement techniques to assess vessel patency. Recent advances which include fat suppression, suspended respiration sequences, and Gadolinium enhancement, have further improved image quality. Accurate pre-operative assessme

Gadolinium-DTPA enhancement of dural structures on MRI after surgery.

Gadolinium-DTPA enhanced MRI was examined before and after surgery in 14 patients with particular attention to the enhancement of falx cerebri, tentorium cerebelli and dura mater. A marked enhancement was observed in 2 falx and 2 tentorium before surgery, whereas it was observed in 7 falx, 9 tentorium and 11 dura after surgery. Among 6 falx, 7 tentorium and 12 dura showing increased enhancement after surgery, 2 falx, 3 tentorium and 8 dura showed marked enhancement which was not observed before surgery. A small amount of subdural haemorrhage during surgery is known to heal as subdural neomembr

Sequential gadolinium-DTPA enhanced MRI studies in neuro-Behçet's disease.

Sequential gadolinium-DTPA (Gd-DTPA) enhanced MR images were obtained before and after steroid therapy in a case of neuro-Behçet's disease. Multiple scattered lesions, which could not be detected on pre- and post-contrast CT, were demonstrated mainly in the white matter of the pons and/or the cerebrum with both T1- and T2-weighted images. Some of these lesions, however, were not enhanced at all by infusion of Gd-DTPA. The Gd-DTPA infusion study demonstrated marked enhancement in the white matter of the pons and cerebrum. Some lesions not seen with T2-weighted images were also strongly enhanced

[Gadolinium DTPA in magnetic resonance. Clinical experience in intracranial pathology].

Sixty-seven patients with various intracranial pathologies were studied using Gd-DTPA as a contrast medium applied to MRI. This paramagnetic substance was well tolerated by all patients, and proved particularly useful in improving the diagnosis of extra-axial tumors, in which a rich enhancement was always present. Gd-DTPA allowed a better definition of the tumor outlines, as well as its differentiation from edema, and the demonstration of its relationship to brain parenchyma and cerebral vessels. This was particularly true for meningiomas, which often display poor contrast enhancement at MRI.

Magnetic resonance ventriculography with gadolinium DTPA: report of two cases.

We report intrathecal use of gadolinium DTPA for MRI of the cerebrospinal fluid (CSF). In two patients with leptomeningeal carcinomatosis, we injected 0.01 mmol gadolinium DTPA into the lateral ventricle via an Ommaya device. Coronal T1-weighted images of the head were obtained at 0.2 T prior to and after injection. There was pronounced enhancement of CSF close to the injection site, allowing good delineation of CSF and surrounding brain tissue. No side effects occurred. MRI with intrathecal administration of highly diluted gadolinium DTPA may be a promising alternative to conventional investi

Gadolinium-DTPA-enhanced magnetic resonance imaging in experimental optic neuritis.

Magnetic resonance imaging (MRI) with gadolinium-diethylenetriaminepentaacetic acid (Gd-DTPA) was used to investigate disruption of the blood-optic nerve barrier associated with acute autoimmune demyelination. Leakage of Gd-DTPA was seen in the optic nerves and optic chiasm of adult guinea pigs sensitized for acute experimental allergic encephalomyelitis, but not in normal unsensitized animals. This finding occurred as early as 5 to 8 days after antigenic sensitization with the myelin emulsion and before the onset of paralysis or ataxia. Pathologic examination at this early stage of experiment

Contrast-enhanced NMR imaging: animal studies using gadolinium-DTPA complex.

Gadolinium (Gd)-DTPA complex was assessed as a nuclear magnetic resonance (NMR) contrast-enhancing agent by experimentally imaging normal and diseased animals. After intravenous injection, Gd-DTPA, a strongly paramagnetic complex by virtue of unpaired electrons, was rapidly excreted into the urine of rats, producing an easily observable contrast enhancement on NMR images in kidney parenchyma and urine. Spin-echo intensity of urine within the renal pelvis increased from 2263 to 4414 units; intensity of renal parenchyma increased from 2901 to 3893 after administration of 0.1 mmol/kg Gd-DTPA. Ste

MR imaging of meningiomas including studies with and without gadolinium-DTPA.

The magnetic resonance (MR) appearances in 12 successive cases of histologically verified meningioma are described. In six of these cases intravenous gadolinium-diethylenetriamine pentaacetic acid (Gd-DTPA) (0.1 mmol/kg) was administered. Each of the eight tumours for which T1 was measured displayed an increase in T1 above that of white matter although the T2 was in the normal range for brain in two of four cases. Inversion recovery was the most useful sequence for demonstration of the tumour prior to administration of intravenous Gd-DTPA and also displayed the highest level of contrast enhanc

Chelating DTPA amphiphiles: ion-tunable self-assembly structures and gadolinium complexes.

A series of chelating amphiphiles and their gadolinium (Gd(III)) metal complexes have been synthesized and studied with respect to their neat and lyotropic liquid crystalline phase behavior. These amphiphiles have the ability to form ion-tunable self-assembly nanostructures and their associated Gd(III) complexes have potential as magnetic resonance imaging (MRI) contrast enhancement agents. The amphiphiles are composed of diethylenetriaminepentaacetic acid (DTPA) chelates conjugated to one or two oleyl chain(s) (DTPA-MO and DTPA-BO), or isoprenoid-type chain(s) of phytanyl (DTPA-MP and DTPA-BP

Magnetic resonance with gadolinium DTPA for the investigation of temporal bone tumours.

The membranous labyrinth and 7th and 8th cranial nerves are well shown by magnetic resonance scanning but the lack of bone detail makes MRI secondary to CT for the investigation of lesions of the petrous temporal bone. Twenty-two intrinsic temporal bone tumours were examined by magnetic resonance in this study. Seven glomus tumours, one facial neuroma and one carcinoid tumor received intravenous gadolinium DTPA. All showed signal enhancement although the practical value of this enhancement was variable. The ability of MRI to demonstrate the jugular bulb separate from a glomus tumour confined t

[Gadolinium DTPA in a MR study of expansive lesions of the cranial base].

Twenty-five patients were investigated by MR imaging in order to evaluate the diagnostic value of Gadolinium (Gd)-DTPA in skull-base tumors. The patients were studied with standard acquisition techniques (T1, mixed and T2-weighted images) without contrast medium. The images obtained were then compared with the T1-weighted images obtained after intravenous injection of Gd-DTPA. The contrastographic results in the different types of acquisition were evaluated. Thanks to the extraordinary increase in contrast resolution it provides, Gd-DTPA allowed the precise evaluation of the lesion and of its

Gadolinium-DTPA enhancement of symptomatic nerve roots in MRI of the lumbar spine.

Disc prolapse presenting with sciatica may be associated with enhancement of the symptomatic nerve root following magnetic resonance imaging (MRI) with intravenous gadolinium (Gd)-DTPA. Previous studies have shown, however, that this does not occur in all cases. The aim of this study was to assess the incidence of nerve root enhancement in patients with sciatica and disc prolapse and to try to identify any specific features that might be associated with the phenomenon. A total of 227 patients presenting with low back pain and/or sciatica underwent a MRI study of the lumbar spine with intraveno
