Disequilibrium and gait disturbance are the most common causes of falls, which are becoming a serious social problem for the increasing geriatric population. Gait disturbance is caused not only by neurological diseases but also by joint and skeletal system abnormalities, and these abnormalities have to be taken into account before evaluating neurological conditions. The final common pathway for gait is formed by primary motor and premotor cortices, corticospinal tracts, anterior horn cells, α-motor nerves, neuromuscular junction, and muscles. The multiple other systems are involved in the cont

The newly developed gait trainer allows wheel-chair-bound subjects the repetitive practice of a gait-like movement without overstressing therapists. The device simulates the phases of gait, supports the subjects according to their abilities, and controls the center of mass (CoM) in the vertical and horizontal directions. The patterns of sagittal lower limb joint kinematics and of muscle activation for a normal subject were similar when using the mechanized trainer and when walking on a treadmill. A non-ambulatory hemiparetic subject required little help from one therapist on the gait trainer,

BACKGROUND AND PURPOSE: Stiff-knee gait, which is a gait abnormality observed after stroke, is characterized by decreased knee flexion angles during the swing phase, and it contributes to a decline in gait ability. This study aimed to identify the immediate effects of pedaling exercises on stiff-knee gait from a kinesiophysiological perspective.
METHODS: Twenty-one patients with chronic post-stroke hemiparesis and stiff-knee gait were randomly assigned to a pedaling group and a walking group. An ergometer was set at a load of 5 Nm and rotation speed of 40 rpm, and gait was performed at a comf

Gait apraxia is most commonly a part of the Hakimov triad (gait apraxia, urinary incontinence, dementia) in normotensive hydrocephalus (NPH), although it may be a symptom of some other conditions. In our case the patient was a long term Parkinson's disease sufferer who developed normotensive hydrocephalus and consequently gait apraxia. Only after a third successive evacuation of the CSF his gait apraxia improved (Fig. 1, Ref. 15). Full Text (Free, PDF) www.bmj.sk.

Tardive dyskinesia (TD) is a hyperkinetic movement disorder induced by dopamine receptor blocking drugs (DRBDs). TD typically presents with stereotypy or dystonia, but may also cause akathisia, myoclonus, or tremor. However, no detailed reports of gait abnormalities have been documented. We report three patients exposed to DRBDs who developed "tardive gait" and conclude that gait dysfunction may be part of the TD spectrum.

Gait analysis, or the study of locomotion, has changed dramatically over the last few decades. Advances in computer technology and data analysis techniques have contributed greatly to the progress of this field. Gait analysis has become a valuable tool in the clinical setting. The ability to objectively quantify motion is essential to our understanding of normal and abnormal movement patterns and the evaluation of treatment effectiveness. This paper will discuss the various experimental and analytical techniques currently used for performing clinical gait analyses at the University of New Brun

BACKGROUND: Disorders of gait and mobility are common neurologic abnormalities of aging and can have disastrous consequences. Etiology is multifactorial, and normal age-related changes are difficult to distinguish from those resulting from the disease.
REVIEW SUMMARY: Gait evaluation with special techniques provides scientific understanding of physiologic mechanisms, but experienced clinicians can make diagnoses based on general medical and neurologic history and physical examination. Details to observe include posture, length of stride, width of base, speed and fluidity of motion, arm swing,

BACKGROUND: Duchenne Muscular Dystrophy (DMD) is an X-linked muscle disorder caused by a mutation or deletion in the dystrophin gene. In boys with DMD, muscle weakness progresses in a proximal to distal pattern, leading to gait abnormalities at all joints, in all planes of motion. Longitudinal studies are imperative to quantify changes in gait function due to DMD and are of particular importance when examining the efficacy of treatment interventions.
RESEARCH QUESTION: The purpose of this study was to examine the sensitivity of the Gait Deviation Index (GDI) and Movement Analysis Profile (Gai

BACKGROUND: Spinocerebellar ataxia (SCA) is a rare, genetic neurodegenerative movement disorder primarily affecting the cerebellum. So far, there is no available cure for SCA. However, evidence suggests that neurorehabilitation can alleviate symptoms. The most optimal training protocol has not been established and mechanisms that channel the effects of these interventions are incompletely understood.
OBJECTIVE: This study investigates the disease-specific effects of a gait adaptability training in a SCA cohort compared to healthy controls and explores underlying cerebral mechanisms.

[Purpose] The present study investigated the effects of gait speed on temporal and spatial gait characteristics of hemiplegic stroke patients. [Subjects and Methods] Twenty post-stroke hemiplegic patients participated in the present study. To enhance the reliability of the analysis of the gait characteristics, the assessments were conducted three days per week at the same time every day. Each subject walked maintaining a comfortable speed for the first minute, and measurement was conducted for 30 seconds at a treadmill speed of 1 km/hour thereafter. Then, the subjects walked at a treadmill spe

Highest level gait disorders are produced by pathology in one or more structures in the cortical-basal ganglia-thalamocortical loop, which plays an important role in producing movements and postural synergies that meet personal desires and environmental constraints. Virtually all patients with dementia have pathology in one or more components of this loop, so highest level gait disorders are common in patients with dementia. The terminology surrounding these gait disorders is unnecessarily complex and too heavily influenced by the controversial concept of gait apraxia. Straightforward descript

A study of clinical features and an analysis of gait with floor reaction forces and EMG of leg flexors and extensors were made in patients with multiple cerebral infarction or patients with Parkinson's disease having frozen gait. A case with occlusion of the bilateral anterior cerebral arteries resulting in massive infarction of the infero-medial part of the frontal lobes was also studied as he showed a unique disorder characterized by apraxia of gait. Frozen gait is characterized by shuffling steps, broad-based stance, positive foot grasp, kinesia paradoxa and disturbance of postural reflexes

BACKGROUND CONTEXT: Current severity assessment methods for cervical spondylotic myelopathy (CSM) overlook asymptomatic gait dysfunction and potential heterogeneous gait impairment. Also, gait alterations for disease progression remain inadequately characterized.
PURPOSE: This study aimed to detect asymptomatic gait dysfunction, achieve accurate detection of CSM, and discover heterogeneous gait impairment in CSM using wearable sensor-based gait evaluation.
STUDY DESIGN: Cross-sectional study.
PATIENT SAMPLE: A total of 45 CSM patients and 39 age-matched healthy controls (HC) were recruited

BACKGROUND: Mobility often is tested under a low challenge condition (ie, over a straight, uncluttered path), which often fails to identify early mobility difficulty. Tests of walking during challenging conditions may uncover mobility difficulty that is not identified with usual gait testing.
OBJECTIVE: The purpose of this study was to determine whether gait during challenging conditions predicts decline in gait speed over 1 year in older people with apparently normal gait (ie, gait speed of ≥1.0 m/s).
DESIGN: This was a prospective cohort study.
METHODS: /b> Seventy-one older adults (mean

INTRODUCTION: Gait integrates sensorimotor and affective processes, serving both locomotor function and emotional expression. This embodiment framework has clinical relevance in neurodegenerative disorders and suggests potential for emotion-based modulation of gait.
OBJECTIVES: This study developed and tested a range of emotion-specific gait videos to assess whether healthy individuals could correctly recognize different emotions, in order to identify standardized stimuli for future research on emotion recognition and embodied simulation in neurological patients.
METHODS: We created a video

Quantitative data assessment on the basis of three-dimensional gait analysis has been routinely used in the evaluation of pathological gait of children with cerebral palsy. However, a similar quantitative methodology has not been applied for spina bifida patients in whom atypical gait patterns are thought to correlate with various levels of neurological paralysis. The purpose of this study is to investigate the differences among gait patterns in spina bifida between different levels of neurological lesions using quantitative methods: Gait profile score (GPS) and gait variable scores (GVS), sco
