Gait and balance disorders are a leading cause of morbidity, mortality, and disability in central and peripheral neurologic disorders. Neurologic gait disorders are classically evaluated with a clinical examination and visual pattern recognition. Gait patterns such as a parkinsonian or ataxic gait may have distinct etiologies and are assessed using the neurologic physical examination and validated clinical scales. Technological advances have made gait analysis more accessible, allowing precise objective measurement of gait and balance deficits. Gait analysis may be more sensitive at detecting

The prevalence of gait disorders among neurological inpatients is unknown, although disturbed gait is a common symptom. Gait disorders often lead to loss of independence with restraints for the patients and caregivers and costs for the health system. We designed a prospective study and investigated all patients admitted to a neurological hospital during a 100-day period for the presence of a gait disorder. Clinical investigation and several disease-specific rating scales were carried out for 493 patients. In 60% of the patients, a disturbance of gait was diagnosed. Most frequent diagnoses were

BACKGROUND: Neurological disorders lead to walking disabilities, which are often treated using robot-assisted gait training (RAGT) devices such as the driven gait-orthosis Lokomat. A novel integrated biofeedback system was developed to facilitate therapeutically desirable activities during walking. The aim of this study was to evaluate the feasibility to detect changes during RAGT by using this novel biofeedback approach in a clinical setting for patients with central neurological disorders.
METHODS: 84 subjects (50 men and 34 women, mean age of 58 ± 13 years) were followed over 8 RAGT sessio

Gait and mobility impairments are common and relevant in patients with chronic neurological disorders. It reduces the health-related quality of life and induces falls with morbidity. Symptomatic treatment options are therefore necessary in order to improve the health status of patients with neurological disorders.By means of a selective literature research focusing on studies with specific gait-related outcome measures. We discuss the differential treatment options for (1) hypokinetic gait disorders (Parkinson´s disease, Normal pressure hydrocephalus, vascular encephalopathy), (2) gait unstead

BACKGROUND: Neurological gait disorders are mainly classified based on clinical observation, and therefore difficult to objectify or quantify. Movement analysis systems provide objective parameters, which may increase diagnostic accuracy and may aid in monitoring the disease course. Despite the increasing wealth of kinematic movement and balance parameter data, the discriminative value for the differentiation of neurological gait disorders is still unclear. We hypothesized that kinematic motion and balance parameter metrics would be differently altered across neurological gait disorders when c

The aim of this study was to objectively assess and compare gait capacity and gait performance in rehabilitation inpatients with stroke or incomplete spinal cord injury (iSCI) using inertial measurement units (IMUs). We investigated how gait capacity (what someone can do) is related to gait performance (what someone does). Twenty-two inpatients (11 strokes, 11 iSCI) wore ankle positioned IMUs during the daytime to assess gait. Participants completed two circuits to assess gait capacity. These were videotaped to certify the validity of the IMU algorithm. Regression analyses were used to investi

Gait abnormalities are frequent in children and can be caused by different pathologies, such as cerebral palsy, neuromuscular disease, toe walker syndrome, etc. Analysis of the "gait pattern" (i.e., the way the person walks) using 3D analysis provides highly relevant clinical information. This information is used to guide therapeutic choices; however, it is underused in diagnostic processes, probably because of the lack of standardization of data collection methods. Therefore, 3D gait analysis is currently used as an assessment rather than a diagnostic tool. In this work, we aimed to determine

The integration of robotics and Electrical Stimulation (ES) in neurorehabilitation leverages robotics' precise task execution alongside ES-induced motor learning, muscle conditioning, and cardiovascular benefits. We propose a hybrid system for overground gait training, combining neuromuscular ES and a motorized exoskeleton. Different combination modalities are proposed: ES-motor cooperation for the swinging knee, synchronized but independent ES and motor assistance for hip movements and for the knee during stance, and ES-only for the non-actuated ankle. Twelve non-disabled subjects and eleven

KIF1A-associated neurological disorder (KAND) encompasses recessive and dominant variants with wide clinical variability. Several de novo variants in the KIF1A gene have been reported to cause a complicated form of hereditary spastic paraplegia (HSP), frequently accompanied by peripheral neuropathy, cerebellar ataxia, and cognitive impairment. Instrumented three-dimensional (3D) gait analysis is underutilized in this population, and comparative data between KAND and pure HSP remain limited. We performed a comprehensive instrumented 3D gait analysis in a 19-year-old woman with KAND caused by a

Functional neurological disorder (FND) is a condition defined by neurological symptoms that may be motor, sensory or cognitive in nature and can result in significant functional impairment, including total loss of mobility. There is evidence supporting the use of robotics in other neurological conditions such as stroke and spinal cord injury, but there is currently no published evidence for its use in the treatment of FND. This case using the Hocoma Lokomat robotic exoskeleton device represents the first known case of robot-assisted gait training in the treatment of FND. The aim of this case s

Falls are one of the most common and devastating effects of neurological diseases, especially in patients with stroke outcomes, Parkinson's Disease (PD), and Multiple Sclerosis (MS). To prevent negative outcomes and guide tailored rehabilitation, it is necessary to identify risk factors early. The current study aims to assess whether and how the risk of falling is related to spatiotemporal and kinematic parameters in stroke, PD, and MS. It also seeks to determine how these factors can help manage patients and identify more personalized and appropriate rehabilitation treatments. Ninety patients

The Head-Trunk System (HTS) is crucial for maintaining visual acuity and stabilizing the head during activities such as walking. However, no consensus exists on methods for measuring HTS movements during walking. Alterations in HTS function, particularly in head movement, are detectable even in the early stages of neurological disorders (PwND). Therefore, this study aimed to summarize the methods and tools used to evaluate head movements in healthy subjects (HS) and PwND during unperturbed linear gait. This systematic review followed the PRISMA Guidelines and the Cochrane Handbook; the protoco

BACKGROUND: Transcranial direct-current stimulation (tDCS) is an easy-to-apply, cheap, and safe technique capable of affecting cortical brain activity. However, its effectiveness has not been proven for many clinical applications.
OBJECTIVE: The aim of this systematic review was to determine whether the effect of different strategies for gait training in patients with neurological disorders can be enhanced by the combined application of tDCS compared to sham stimulation. Additionally, we attempted to record and analyze tDCS parameters to optimize its efficacy.

Assessing balance is crucial in neurological rehabilitation, yet while wearable sensors enable real-world monitoring, identifying reliable digital biomarkers remains challenging. This study utilized a high-fidelity instrumented walkway to determine which gait parameters best predict balance impairment, providing robust targets for future wearable applications. We analyzed 49 steady-state gait metrics from 140 individuals with diverse neurological conditions. Using statistical analysis and machine learning, we evaluated these parameters against objective force plate sway scores and clinical fal
