Gallbladder neuroendocrine neoplasms: a review.

Gallbladder neuroendocrine neoplasms (GB-NENs) are rare, aggressive tumors that represent a unique subset of biliary tract malignancies. The etiology remains poorly understood, with hypotheses suggesting origins from metaplasia induced by chronic inflammation or differentiation from adenocarcinoma. Most GB-NENs present with vague or absent symptoms, leading to frequent incidental detection during cholecystectomy or at advanced stages with distant metastases. Diagnosis is challenging due to the lack of specific clinical and imaging features; thus, histopathological evaluation with immunohistoch

Gallbladder splenosis: a hereto unreported mimicker of a gallbladder neoplasm.

Splenosis is a completely benign entity that can mimic a primary mass or metastasis. Herein we report the first known case of gallbladder splenosis in a 49-year-old woman with an extensive surgical and medical history. Cross-sectional computed tomography and MRI demonstrated a single mass arising from the gallbladder fundus. Revelation of splenectomy, confirmed to be traumatic from medical records, raised the suspicion for splenosis. The diagnosis was confirmed with a Technecium-99m-labeled heat-damaged blood cell scan, considered the gold-standard for detection of ectopic splenic tissue.

[Ultrasonography aspects of primary gallbladder neoplasms].

Sixty six patients with primary gallbladder cancer were seen at the Hospital A. C. Camargo, São Paulo, Brazil, from January 1980 to December 1988 and who represent 0.14% of the total of 46,519 patients registered in the same period. Out of the 66 cases, 41 were submitted to an ultrasonographic exam. As to the other 25 patients, 20 had been submitted to a cholecystectomy at another hospital. This exam was not done in the other 5 patients for several reasons. The gallbladder cancer diagnosis was based on data obtained from surgery and confirmed by anatomopathological exam. Regarding the 20 cases

Incidental Gallbladder Neoplasms: A Growing Global Burden.

Background The increasing trend of laparoscopic procedures has made cholecystectomies one of the most common surgical specimens received for histopathological evaluation. This has also led to an increasing trend of finding incidental gallbladder malignancies for a presumed benign disease. The present study describes the histopathological spectrum of neoplastic lesions of the gallbladder along with the historadiological correlation with special emphasis on incidental gallbladder carcinomas (IGBC). Materials and methods All the cholecystectomies received over a span of two and a half years were

Intracholecystic Tubular Nonmucinous Neoplasm of the Gallbladder.

CONTEXT.—: Intracholecystic neoplasms (ICNs) of gallbladder are a challenging and relatively poorly understood family of tumors. Recently, a distinct category of ICNs, characterized by pedunculated polyps composed of lobules of back-to-back small tubular/acinar units lined by minimally mucinous or nonmucinous cells, has been found to be biologically, behaviorally, and molecularly different from other ICNs, and termed intracholecystic tubular nonmucinous neoplasm (ICTN). OBJECTIVE.—: To review the clinicopathologic characteristics of ICTN of the gallbladder. DATA SOURCES.—: This review is bas

A case of mucinous cystic neoplasm of the gallbladder.

The patient was a 70-year-old woman in whom examination revealed a high level of carbohydrate antigen 19-9. Abdominal ultrasonography and computed tomography (CT) revealed a multilocular cystic lesion compressing the gallbladder. CT indicated the presence of a multilocular cystic tumor (67 × 68 × 72 mm) in contact with the right hepatic lobe. Intraoperative findings indicated that the cyst diameter was 8.5 × 6.0 cm, and the cyst was continuous with the gallbladder. The gallbladder was resected along with the cyst. The cyst was multilocular and originated from the cystic duct and gallbladder wa

Cholescintigraphy: gallbladder nonvisualization secondary to neoplasm.

Whereas the diagnosis of acute cholecystitis is characterized by nonvisualization of the gallbladder with Tc-99m iminodiacetic acid derivatives, nonvisualization is not specific for acute cholecystitis. The first reported case of nonvisualization of the gallbladder due to neoplasm is added to an expanding list of causes of nonvisualization other than the more frequent causes: acute and chronic cholecystitis.

[Analysis of challenges in the surgical management of gallbladder neoplasms].

Gallbladder cancer is a highly malignant tumor of the biliary system. Radical resection remains the only treatment that can significantly improve patient prognosis. However, surgical management of gallbladder cancer in clinical practice still faces numerous challenges: intraoperative frozen section pathology and postoperative pathological examination are critical diagnostic tools, yet their accuracy is limited by medical technology and the inherent complexity of pathological diagnosis. Surgical strategies involving the extent of hepatectomy, lymph node dissection, and management of extrahepatic bi

Case 329: Intracholecystic Papillary Neoplasm of the Gallbladder.

A 45-year-old female patient who was previously healthy presented after several weeks of fullness in the right upper quadrant of the abdomen. The patient did not experience pain, nausea, vomiting, or jaundice, and had no contributory past medical or surgical history, including no history of malignancy. Upon examination, vital signs were within normal limits and the patient appeared well, with soft palpable fullness in the right upper quadrant. The abdomen was nontender and nondistended. Laboratory investigation revealed no abnormalities, with a normal complete blood cell count and normal serum

Three cases of intracystic papillary neoplasm of gallbladder.

Intracystic papillary neoplasm (ICPN) of gallbladder is a comparatively new concept and is described as pre-malignant lesions in Nakanuma et al. (In: Bosman et al. (eds) WHO Classification of Tumours of the Digestive System, World Health Organization of Tumours, IARC, Lyon, 2010). ICPN with high-grade intraepithelial neoplasia is understood to include intraepithelial carcinoma or noninvasive carcinoma. And lesions with invasive cancer components are classified as ICPN with an associated invasive carcinoma [1]. According to Adsay et al., more than half of patients diagnosed with ICPN have invas

Pathological characterization of intracholecystic papillary neoplasm: A recently proposed preinvasive neoplasm of gallbladder.

Intracholecystic papillary neoplasm (ICPN) is a recently proposed gallbladder neoplasm. Its prevalence and pathologies remain to be clarified. A total of 38 ICPN cases (28 ICPNs identified among 1904 cholecystectomies (1.5%) and in 100 surgically resected primary gallbladder neoplasms (28%) in Fukui Prefecture Saiseikai Hospital, Japan, and other 10 ICPNs) were examined pathologically and immunohistochemically. They were composed of 21 males and 17 females with a mean age of 75 years old, and presented intraluminal growth of papillary lesions with fine fibrovascular stalks. ICPNs were relative

Intramural gallbladder hematoma mimicking gallbladder neoplasm in a 55-year-old male patient.

Hemorrhage in the gallbladder (GB) is usually associated with cholecystitis, GB neoplasm, trauma, hemobilia, and cystic artery aneurysm. Our patient had not experienced any previous abdominal trauma, and GB hemorrhage was unlikely to result from cholecystitis or bleeding diathesis. A 55-year-old male was admitted because of right upper quadrant pain. Both prothrombin time and partial thromboplastin time were normal. Abdominal computed tomography, endoscopic ultrasound and magnetic resonance cholangiopancreatography were performed. Image studies revealed GB wall thickening and an intraluminal m

[Acute cholecystitis and gallbladder neoplasms: the experience of 2215 cases].

The authors have retrospectively analysed 2215 surgical procedures for gallbladder stones. The global incidence of gallbladder neoplasm has been 5%. This rate rises to 11% considering female patients over 60 years old. In this group, a gallbladder carcinoma was contemporary present in 17.4% of acute cholecystitis. Cholecystectomy may be suggested in every patients with gallstones, even asymptomatic, particularly if some risk factors are present.

Characterization of high-grade biliary intraepithelial neoplasm of the gallbladder in comparison with intracholecystic papillary neoplasm.

Biliary intraepithelial neoplasm (BilIN) is characterized by a microscopically identifiable preinvasive neoplasm of the biliary tract. In this study, the high-grade BilIN of gallbladder was examined pathologically and compared with the intracholecystic papillary neoplasm (ICPN) of gallbladder. Sixteen high-grade BilINs (height <0.5 cm) collected from 2297 cholecystectomies (0.7%) and another three cases (19 cases total) were examined and compared with 34 cases of ICPN (greatest diameter ≥1 cm and height ≥0.5 cm). High-grade BilINs were incidentally found in 11 cholecystectomies, and the remain

Incipient Intracholecystic Papillary Neoplasm of the Gallbladder Without Dysplasia.

Intracholecystic papillary neoplasm (ICPN) of the gallbladder is a rare tumor described as a mucosal exophytic neoplastic lesion that projects into the gallbladder lumen. In regards to the size, lesions that did not make the arbitrary 1cm cutoff are described as "incipient" ICPN. Not much is known about these incipient ICPNs, as they are often excluded in ICPN studies, given the attempted adherence to the traditional 1cm cutoff. We present the youngest reported case of incipient, non-mucinous gastric-pylorus type ICPN who underwent cholecystectomy. Resection with negative margin for ICPN appea

Xanthogranulomatous cholecystitis (XGC) is an infrequent type of chronic cholecystitis whose histologic features, in some cases, show a macroscopic appearance which may lead to suspicion of a gallbladder neoplasm with the consequent contraindication of the laparoscopic approach. The incidence of XGC was reviewed in a prospective series of 514 cholecystectomies performed in the authors' department from January 1991 to January 1996. Fourteen cases (2.7%) of XGC were identified with a male/female ratio of 1/1.8 and a mean age of 71 +/- 9 years. The most frequent form of presentation observed was
