[Coronary artery bypass grafting using gastroepiploic artery].

Since March 1986, coronary artery bypass grafting (CABG) by utilizing the right gastroepiploic artery (GEA) has been performed in 60 patients during 3 year period. There were 52 males and 8 females, and age ranged from 34 to 73 year old with the mean of 56.2 year old. Triple vessel disease and the left main disease involved 90% of the patients. There were two patients under hemodialysis for chronic renal failure, one patient with idiopathic thrombocytopenic purpura, one patient with aneurysm of the abdominal aorta, and two patients with arteriosclerosis obliterance, preoperatively. Five patien

The right gastroepiploic artery graft.

This is the presentation of coronary artery bypass grafting with gastroepiploic artery (GEA) in both skeletonized GEA harvesting technique and anastomosis: After median sternotomy and harvesting internal thoracic artery, the median incision is extended less than one inch. Following laparotomy, dissection of GEA is started from two third distal of the great curvature of the stomach and ended above the pylorus. The GEA is passed into pericardial cavity through a small tunnel in the diaphragm. The most common site of the anastomosis is distal right coronary artery (postero-descending or atriovent

An aneurysm of the right gastroepiploic artery.

A palpable mass was unexpectedly found in the upper abdomen of a 67-year-old woman. Computed tomography (CT) and abdominal angiography both indicated an aneurysm of the right gastroepiploic artery. An aneurysmectomy was performed, and the aneurysm was found to be spherical with a diameter of 7.5 x 7.7 cm. The patient was eventually discharged in good condition. The histological findings of the aneurysmal wall revealed medial fibrosis and atherosclerosis. Gastroepiploic artery aneurysms are rare among splanchnic artery aneurysms and most such aneurysms are found to be ruptured. The present repo

Diaphragmatic hernia post coronary artery bypass with gastroepiploic artery.

An 86-year-old man, with a medical history of coronary artery bypass grafting with the right gastroepiploic artery 20 years prior, was admitted to our hospital for right-sided heart failure. Computed tomography findings revealed an intrapericardial diaphragmatic hernia of the transverse colon compressing the right ventricle. The hernia was successfully repaired, and the patient recovered without any complications. Diaphragmatic hernia is rare but may be a lethal complication following coronary artery bypass grafting. We should be aware of the possibility of this condition.

A case of a huge gastroepiploic arterial aneurysm.

An 85-year-old man complaining of vague abdominal discomfort was admitted to our hospital. A pulsatile 8 × 7-cm mass in the right upper abdomen was noticed on clinical examination. Computed tomography of the abdomen showed a huge arterial aneurysm in the right gastroepiploic artery, and the left gastroepiploic artery was meandering and expanding. An image diagnosis of gastroepiploic arterial aneurysm (GEAA) was made. Because of the huge size of the aneurysm and the predicted high risk of perforation, surgical intervention was planned. The aneurysm was identified in the greater curve and was fo

Arteriosclerosis of the gastroepiploic and internal thoracic arteries.

Arteriosclerosis of the right gastroepiploic artery (GEA) and the internal thoracic artery (ITA) were compared by pathological observation. Specimens were obtained from 35 patients who underwent coronary artery bypass grafting with simultaneous use of these two kinds of arterial grafts. Degree of arteriosclerosis was classified in five categories: 0, normal; 1, luminal narrowing less than 25%; 2, luminal narrowing between 25% and 50%; 3, luminal narrowing greater than 50%; and 4, overt atherosclerosis with ulceration or calcification. The number of arteries with degree 0, 1, 2, 3, and 4 was 16

Denervation of gastroepiploic artery graft can reduce vasospasm.

BACKGROUND: The right gastroepiploic artery is useful as an in situ arterial graft for coronary artery bypass grafting. However, the gastroepiploic artery is more likely to cause vasospasms compared with the internal thoracic artery. We hypothesized that the cause of the spasms is the stimulation of the periarterial sympathetic nerve, because the gastroepiploic artery is classified as a muscular artery. In this study, we examined whether the spasm is reduced by removing the periarterial sympathetic nerve. METHODS: Unused parts of the gastroepiploic artery were obtained from patients who under

An anomalous right gastroepiploic artery arising from the superior mesenteric artery.

During a routine dissection, the right gastroepiploic artery was found to arise from the superior mesenteric artery. The gastroduodenal artery ran in front of the common bile duct and descended along the posterior surface of the head of the pancreas (posterior superior pancreaticoduodenal artery). The enlarged pancreatic branch arising from the superior mesenteric artery mainly supplied the anterior surface of the head of the pancreas and then continued to become the right gastroepiploic artery. This route seemed to be formed due to the lack of connection between the posterior superior pancrea

Spasm of the gastroepiploic artery used for coronary artery bypass grafting.

The GEA, though similar to the internal mammary artery in size and flow characteristics, is different histologically as well as physiologically. When used as a free graft, the GEA is very vasospastic, which may influence its long-term patency. When it is necessary to use the GEA as a conduit, it may be preferable to use it in situ rather than as a pedicle graft.

[Clinicopathological study on atherosclerosis of the right gastroepiploic arteries].

We evaluated the extent of atherosclerosis of the right gastroepiploic arteries (RGEAs) used as a graft material in coronary bypass grafting for ischemic heart diseases, with analyzing the presence of arteriosclerotic lesions and the site of predilection and studying their relationship to clinical risk factors of ischemic heart disease. The subjects were 32 cases, 18 males and 14 females, ranging in age from 32 to 80 years (mean 61 years), in which gastrectomy was performed for early gastric cancer. RGEA 20 cm in length was taken from the junction of the gastroduodenal artery of the stomach an

[Upper abdominal complications after coronary artery bypass operations using right gastroepiploic artery].

We discussed upper abdominal complications after 161 coronary artery bypass graftings using right gastroepiploic artery (RGEA). We had one case of bleeding from RGEA branches (1), one case of pyrolus stenosis because of anterior stomach pressed by RGEA (2), three cases of gastric ulcer (3) (three cases with a past history of gastric ulcer, two case with IABP) and one case of gastric perforation (4) as early postoperative complications. As long-term postoperative complications, we had two cases of abdominal hernia (5) and two cases of gastric cancer (6). The cases of number 1, 2 and 3 are solva

A "new" intraabdominal artery. The pedicled right gastroepiploic artery for myocardial revascularization.

The right gastroepiploic artery (GEA) was used as a pedicled conduit for direct coronary artery revascularization in 20 patients presenting with more or less exhausted saphenous vein resources. The early angiographic patency of the GEA conduit appears to be satisfactory when it is connected to the right coronary artery system. A distinct disadvantage of GEA grafting is the necessity to enter the abdominal cavity, which may lead to probably rare and as yet unrecognized morbidity. Future abdominal surgery may injure the GEA conduit unless its topographic relations to the prepyloric antrum, liver

Pylorus-preserving pancreaticoduodenectomy after coronary artery bypass grafting using right gastroepiploic artery.

Coronary artery bypass grafting using right gastroepiploic artery and pylorus-preserving pancreaticoduodenectomy are both well known and commonly performed procedures independently. However, pylorus-preserving pancreaticoduodenectomy after coronary artery bypass grafting using right gastroepiploic artery has not been reported in the literature. We report the first case with operative demonstration of pylorus-preserving pancreaticoduodenectomy in a patient who had undergone coronary artery bypass grafting using an in situ right gastroepiploic artery graft.

Right Gastroepiploic Artery Transposition for a Common Hepatic Artery and Proper Hepatic Artery Aneurysm Repair.

Although hepatic artery aneurysms (HAAs) are uncommon, they are associated with risk of rupture. HAAs >2 cm in diameter require endovascular or open surgical repairs. For HAAs involving the proper hepatic artery or gastroduodenal artery, which is a collateral artery from the superior mesenteric artery, hepatic arterial reconstruction is especially important to avoid ischemic liver injury. In this study, right gastroepiploic artery transposition was performed in a 53-year-old man after a 4 cm common hepatic artery and proper hepatic artery aneurysm was identified. The patient was discharged wit
