[When genomic medicine becomes personal].

Next-generation sequencing is a powerful diagnostic tool, and even though it is still of limited use in clinical practice, genome sequencing will be increasingly applied. Transition to wider genetic screening methods as clinical exome or genome sequencing has the potential to detect more variants of unknown significance as well as secondary findings. It calls for close cooperation between the laboratory geneticist and the medical geneticist. Pre- and post-test genetic counselling must be offered systematically, and always when there is a high risk of finding germ line variants.

Genomic medicine for kidney disease.

Technologies such as next-generation sequencing and chromosomal microarray have advanced the understanding of the molecular pathogenesis of a variety of renal disorders. Genetic findings are increasingly used to inform the clinical management of many nephropathies, enabling targeted disease surveillance, choice of therapy, and family counselling. Genetic analysis has excellent diagnostic utility in paediatric nephrology, as illustrated by sequencing studies of patients with congenital anomalies of the kidney and urinary tract and steroid-resistant nephrotic syndrome. Although additional invest

Genomic medicine for cancer prognosis.

There is a heavy research emphasis on prognostic and predictive approaches based on genomic data, which has in turn challenged standard paradigms for the management of patients with malignant disease. This review will highlight the recent advances made in genomic medicine, specifically with regard to prognosis associated with thyroid cancer, cutaneous melanoma, and pancreatic adenocarcinoma. Although none of the markers reviewed have been incorporated into routine clinical practice, this review covers the most promising ones.

Genomic medicine for cancer diagnosis.

Genomic diagnostics in cancer has evolved since the completion of the Human Genome Project and the advancements made in diagnosis and therapy in chronic myelogenous leukemia. Among the diseases to achieve limited success or potentially benefit from diagnostic genetic testing are thyroid cancer, Burkitt's lymphoma, gastrointestinal stromal tumors, adrenocortical carcinoma, and colorectal cancer. With increased understanding of genomics, genetic tests should improve diagnosis and help guide medical and surgical management.

Genomic medicine in prenatal diagnosis.

Prenatal diagnostics has seen a rapid increase in the number of genetic disorders amenable to prenatal detection owing to advances in technology and research into the genetic etiology of many conditions. This article reviews the more traditional prenatal diagnostic techniques, such as amniocentesis and chorionic villus sampling for chromosome abnormalities and single gene disorders, and chromosome analysis of products of conception to determine the etiology of a spontaneous abortion, plus more recent advances such as rapid aneuploidy detection via fluorescence in situ hybridization and polymer

Genomic Medicine Sweden: Advancing precision medicine at the national level.

High-throughput sequencing has transformed clinical diagnostics of rare diseases (RD), cancer and infectious diseases by enabling the identification of disease-causing genetic alterations and facilitating individualised treatment and care. In response to these advances, Genomic Medicine Sweden (GMS) was established in 2017 as a national collaborative effort to accelerate implementation of genomics-based precision medicine within Sweden's regionally organized, publicly funded healthcare system. GMS brings together the seven university healthcare regions and their associated medical faculties, i

Applications of genomic medicine in endocrinology and post-genomic endocrine research.

In the mid 1980's, two advances revolutionized Medicine in a way that is comparable only to some of the most important events in the approximately 3,000 years of its history. The first was the introduction of the concept of "positional cloning", i.e. the idea that one can identify genes for human disease though knowing nothing or very little about their function. The second was the discovery of the method of polymerase chain reaction (PCR) which made DNA easier to work with for all biomedical researchers and clinicians. Fresh in the history of Endocrinology were the great discoveries of neuroe

[The mitochondrial genome in medicine].

Analysis of the mitochondrial genome contributed towards the elucidation of a number of deviations-deletions or point mutations in mtDNA which are considered the cause of some pathological conditions, usually neuro- and myopathies. The distribution of defective mtDNA in the affected organism is uneven not only at the tissue or organ level but there are also marked differences between individual cells. Maternal heredity assumed for the mitochondrial genome is apparent only in some families, frequently also autosomal dominant heredity is found.

[Cancer Genomic Medicine in Neuro-Oncology Practice].

Cancer genomic medicine is an integral component of neuro-oncology, enabling personalized treatments based on tumor-specific genomic alterations. In Japan, comprehensive genomic profiling (CGP) is performed using several tissue- and plasma-based platforms to detect genomic mutations, copy number alterations, gene fusions, tumor mutational burden, and other molecular biomarkers. Tissue-based assays are preferred for brain tumors because of the limited sensitivity of plasma testing. CGP results were interpreted by expert molecular tumor boards supported by the Center for Cancer Genomics and Adva

Advances of Genomic Medicine in Psoriatic Arthritis.

Psoriatic arthritis (PsA) is a common type of inflammatory arthritis found in up to 40% of patients with psoriasis. Although early diagnosis is important for reducing the risk of irreversible structural damage, there are no adequate screening tools for this purpose, and there are no clear markers of predisposition to the disease. Much evidence indicates that PsA disorder is complex and heterogeneous, where genetic and environmental factors converge to trigger inflammatory events and the development of the disease. Nevertheless, the etiologic events that underlie PsA are complex and not complet

Personalized genomic medicine: lessons from the exome.

While genomic sequencing methods are powerful tools in the discovery of the genetic underpinnings of human disease, incidentally-revealed novel genomic risk factors may be equally important, both scientifically, and as relates to direct patient care. We performed whole-exome sequencing on a child with VACTERL association who suffered severe post-surgical neonatal pulmonary hypertension, and identified a potential novel genetic risk factor for this complication: a heterozygous mutation in CPSI. Newborn screening results from this patient's monozygotic twin provided evidence that this mutation,

[Required Human and System Resources for Genomic Medicine].

Advances in genomic medicine have enabled the development of precise cancer therapies (precision cancer medicine) through multigene testing. Toward this end, we have developed a novel clinical sequencing system called PleSSision (Pathologist edited, Mitsubishi Space Software supervised clinical sequence system for personalized medicine) that performs amplicon exome sequencing targeting 160 cancer genes. Using this system, we have examined more than 600 cases over 3 years, and have identified druggable gene alteration in approximately 60% of the cases. Performing such clinical sequencing requir

Cancer Genomic Medicine for Urological Malignancies in Japan.

Cancer is a disease caused by genomic alterations, and identifying the profile of genetic changes in each cancer leads to the optimal choice of treatment, that is, precision medicine. In Japan, comprehensive genomic profiling tests were covered by insurance in June 2019, and cancer genomic medicine was fully implemented in clinical practice. In the field of urological malignancies, the approval of olaparib for metastatic castration-resistant prostate cancer with BRCA1/2 mutations has led to the practical application of cancer genomic medicine in many cases. However, various challenges exist in

Applying genomic and transcriptomic advances to mitochondrial medicine.

Next-generation sequencing (NGS) has increased our understanding of the molecular basis of many primary mitochondrial diseases (PMDs). Despite this progress, many patients with suspected PMD remain without a genetic diagnosis, which restricts their access to in-depth genetic counselling, reproductive options and clinical trials, in addition to hampering efforts to understand the underlying disease mechanisms. Although they represent a considerable improvement over their predecessors, current methods for sequencing the mitochondrial and nuclear genomes have important limitations, and molecular
