Giant cell tumor of bone.

Giant cell tumor is a common benign bone tumor that possesses specific features including location at the end of long bone, a strong tendency toward local recurrence, and the rare capacity to metastasize to the lungs. Preferred treatment usually consists of extensive curettage and filling of the cavity with bone graft or cement. Debate still exists about the usefulness of local adjuvant treatment. Functional outcome is usually very good.

[Giant cell tumor of bone].

Giant cell tumor is a benign epiphyseo-metaphyseal bone tumor affecting the young patient. It is characterized by an extensive osteolysis, a high potential for recurrence, a risk of malignant transformation and pulmonary metastases. Curettage and cavity filling is the most common treatment, even in the case of a pathological fracture. A wide resection with prosthetic reconstruction must sometimes be considered. Better knowledge of the role of RANK-L in the pathophysiology of these tumors has led to clinical trials involving denosumab. Treatment with denosumab is suggested for inoperable lesion

Giant Cell Tumor of Bone Versus Tenosynovial Giant Cell Tumor - Similarities and Differences.

Giant cell tumor of bone (GCTB) and tenosynovial giant cell tumor (TGCT) share misleadingly similar names, soft texture and brown color macroscopically, osteoclast-like multinucleated giant cells microscopically and localisation in the musculoskeletal system. However, these two tumor types are biologically and clinically two distinct entities with different natural courses of progression and considerably different modes of surgical and medical treatment. In this article, we provide a detailed update on the similarities and the differences between both tumor types.GCTB is a locally aggressive o

Multicentric giant cell tumor of bone.

Giant cell tumor of bone accounts for 4% to 5% of primary bone tumors in the United States. Multicentric giant cell tumors occur in < 1% of all patients with giant cell tumors, and only 43 patients with multicentric giant cell tumor have been reported on in the literature. This series presents 3 additional cases of multicentric giant cell tumor, includes updated data for 2 patients previously reported on in the literature, and reviews 24 cases previously reported on in detail in the literature. The mechanism by which giant cell tumor involves multiple locations is not known. Multicentric giant

Denosumab-treated Giant Cell Tumor of Bone Exhibits Morphologic Overlap With Malignant Giant Cell Tumor of Bone.

Giant cell tumor (GCT) of bone is a locally aggressive benign neoplasm characterized by an abundance of osteoclastic giant cells that are induced by the neoplastic mononuclear cells; the latter express high levels of receptor activator of nuclear factor κ-B ligand (RANKL). Denosumab, a RANKL inhibitor, which is clinically used to treat GCT, leads to a marked alteration in the histologic appearance of the tumor with giant cell depletion and new bone deposition, leading to substantial histologic overlap with other primary tumors of bone. Most significantly, denosumab-treated GCT (tGCT) with abun

Imaging of giant cell tumor of bone.

Giant cell tumor (GCT) of bone is a benign but locally aggressive and destructive lesion generally occurring in skeletally mature individuals. Typically involving the epiphysiometaphyseal region of long bones, the most common sites include the distal femur, proximal tibia and distal radius. On radiographs, GCT demonstrates a lytic lesion centered in the epiphysis but involving the metaphysis and extending at least in part to the adjacent articular cortex. Most are eccentric, but become symmetric and centrally located with growth. Most cases show circumscribed borders or so-called geographical

Perigeniculate Giant Cell Tumor of Temporal Bone.

Giant cell tumors of bone (GCTBs) are benign osteolytic neoplasms that can be treated with either gross-total resection or subtotal resection with adjuvant radiotherapy. For the rare GCTB of the temporal bone, close proximity to critical structures can produce functional deficits and make gross-total resection difficult to achieve without significant morbidity. We present the case of a 28-year-old woman with progressive facial paresis, otalgia, neck pain, imbalance, and subjective hearing loss. She was found to have a facial nerve mass centered at the geniculate ganglion extending into the lab

Giant cell tumor of bone express p63.

p63 contributes to skeletal development and tumor formation; however, little is known regarding its activity in the context of bone and soft tissue neoplasms. The purpose of this study was to investigate p63 expression in giant cell tumor of bone and to determine whether it can be used to discriminate between other giant cell-rich tumors. Seventeen cases of giant cell tumor of bone were examined to determine the cell type expressing p63 and identify the isoforms present. Total RNA or cell protein was extracted from mononuclear- or giant cell-enriched fractions or intact giant cell tumor of bon

Giant cell tumor of the sphenoid bone.

OBJECTIVE AND IMPORTANCE: Giant cell tumor of the sphenoid bone is extremely unusual. This tumor is usually located in long bones and is rarely located in the cranial vault. It must be differentiated from reparative granuloma and the Brown tumor of hyperparathyroidism. This case report is only the second involving a patient in the first decade. CLINICAL PRESENTATION: A 9-year-old girl presented with cephalgia and diplopia. Radiographic imaging revealed an erosive mass in the clivus. INTERVENTION: The patient underwent an extended transphenoidal resection of the clivus. This was followed with

[Giant-cell tumor of bone in 2022].

Giant cell tumors of bone (GCTs) are rare mesenchymal tumors classified as intermediate in the WHO 2020 classification, i.e. neither completely benign nor definitely malignant, due to recurrence (frequent) and pulmonary metastases (rare). They involve the end of long bones as well as the axial bones of mature skeletons. They are made of mononuclear stromal tumor cells of (pre-) osteoblastic phenotype, mononuclear cells of the monocyte-macrophage lineage and osteoclast-like multinuclear giant cells responsible for tumor osteolysis. In 95% of cases, the stromal cells have a specific mutation in

Giant cell tumor of the hamate bone.

The authors present two cases of giant cell tumor of the hamate bone. The symptoms and signs were similar in both cases: pain, mild inflammatory changes, and localized swelling. The radiographic picture included lacunar osteolysis with interruption of the cortex in one case, intact cortex but more expansive lysis of the cancellous bone in the other. The findings of greatest interest were constituted by the extreme rareness of the localization, problems related to differential diagnosis, long-term follow-up in one case (20 years) and the different methods of treatment used.

Challenges of denosumab in giant cell tumor of bone, and other giant cell-rich tumors of bone.

PURPOSE OF REVIEW: Giant cell tumor of bone (GCTB) is an uncommon benign primary bone tumor, consisting of receptor activator of nuclear factor kappa-B (RANK) expressing reactive osteoclast-like giant cells and neoplastic spindle-shaped cells. Denosumab was approved by FDA in 2013 and by EMA in 2014 to treat adults and skeletally mature adolescents with unresectable GCTB or when resection is likely to result in severe morbidity. However, there is much discussion regarding the optimal applied treatment strategy. RECENT FINDINGS: Neoadjuvant treatment of GCTB with denosumab can effectively down

[Differential diagnosis of giant cell tumor of bone].

Osteoclast-like giant cells (GC) may dominate the histologic pattern not only in conventional giant-cell tumor (GCT)--originating as a radiologically pure lytic, possibly trabeculated lesion especially within the epiphyses of long tubular bones (LB) and pelvic bones of adults--but also in many tumor-like lesions as well as in various benign and malignant bone tumors which may simulate each other. Although the mononuclear cells as well as the amount of collagene fibres don't differ significantly, these lesions can be distinguished by substantial differences concerning their site, radiomorpholog

Giant cell tumor of the temporal bone (osteoclastoma).

A 49-year-old white female patient was referred to the ENT Department, Zentralkrankenhaus, Bremen, because of conductive hearing loss. Widespread tumor formation was found in the left temporal bone and was identified by biopsy to be an osteoclastoma. The tumor was extirpated surgically and reconstruction was performed. Periodic clinical follow-ups during a 36-month period after surgery have included control computed tomographic scans and have shown no evidence for recurrent disease. The specific pathology of the osteoclastoma is presented and different ways for treatment are discussed.

[Chromosome study on giant cell tumor of bone].

Chromosome study was made in 30 specimens of giant cell tumor of bone (GCT) and osteosarcoma (OS). In 15 tumors, karyotypes were satisfactorily made for chromosome analysis, including 13 GCTs, one OS and one cell-line of same. The karyotypes of all cells were recognized and described according to the international system for human cytogenetic nomenclature. Karyotype analysis showed that GCT differs from OS both in numerical chromosomal aberrations and in structural chromosomal aberrations. The number of chromosome of GCT concentrated on diploid or subdiploid, with the exception of some superdi

Endobronchial metastasis from giant cell tumor of bone.

A young woman presented with cough, dyspnea on exertion, and weight loss. A chest roentgenogram revealed collapse of the left lung. On doing fiberoptic bronchoscopy, a growth was found in the left main bronchus. Cytologic examination and sections from cell block revealed that it was a metastatic growth from a giant cell tumor (GCT) of the bone. To the best of our knowledge, this is the first report of endobronchial metastasis from a GCT of the bone.

Pheochromocytoma and multicentric giant cell tumor of bone.

Pheochromocytomasand paragangliomas (PPGLs) are rare catecholamine-secreting tumors that can affect systemic physiology, including bone metabolism. Although they are typically associated with genetic syndromes such as MEN2 and von Hippel-Lindau disease, PPGLs are not commonly linked to primary bone tumors. However, recent findings suggest a novel cancer syndrome involving both PPGLs and giant cell tumor of bone (GCTB) through H3F3A mutations. We report a rare case of a 53-year-old woman with multicentric GCTB, initially diagnosed at age 24, who subsequently developed bilateral pheochromocytoma

Malignant giant cell tumor of bone: malignant transformation of a benign giant cell tumor treated by surgery.

A case of a giant cell tumor of the right proximal femur that developed malignant change following surgical treatment ("secondary" malignant giant cell tumor) is presented. The patient had a cystic lesion that was initially interpreted as an aneurysmal bone cyst. A local recurrence six months later demonstrated the histological features of typical giant cell tumor. A further recurrence 22 months after the original treatment showed histological malignancy. Death was due to massive local recurrence and to extension of the malignancy to the pleura.
